Einfluss von Heparin auf die Proliferation, Zytokinsekretion und Invasion humaner endometrialer Karzinomzelllinien

  • Klinische Studien haben gezeigt, dass Heparin das Überleben von Karzinompatienten unabhängig von seiner antikoagulatorischen Wirkung verlängern konnte. Aufgrund seiner Eigenschaften wie die stark negative Ladung ist eine Interaktion mit vielen Molekülen möglich. Zytokine, zumeist positiv geladen, spielen eine wichtige Rolle im Tumormikromilieu. Sie können die Proliferation, die Invasion sowie das Überleben von Tumorzellen fördern. Die Tumorprogression wird bspw. durch die Zytokine IL-8, RANTES, MCP-1 und HGF verstärkt. Für diese Moleküle ist eine Bindung bzw. Interaktion mit Heparin beschrieben. Ebenfalls werden IL-8, RANTES und MCP-1 vermehrt von endometrialen Adenokarzinomzellen sezerniert, wohingegen HGF vorwiegend von Stromazellen freigesetzt wird. In der vorliegenden Arbeit wurde der Einfluss von Heparin auf die Spontanproliferation, Zytokinsekretion und Invasion von humanen endometrialen Adenokarzinomzelllinien untersucht. Die fünf unterschiedlich differenzierten humanen endometrialen Adenokarzinomzelllinien AN3 CA, ECC-1, HEC-1-A, KLE sowie RL95-2 wurden mit verschiedenen Dosen unfraktioniertem Heparin inkubiert. Es erfolgte die Bestimmung der relativen Zellzahl mit Hilfe des Vitalitätsassays CellTiterBlue®, der Zytokinsekretion mittels ELISA sowie der Zytokinexpression mithilfe der real-time RT-PCR. Ebenfalls wurde der Einfluss von verschiedenen niedermolekularen Heparinen und Fondaparinux auf die Zytokinsekretion mittels ELISA untersucht. Des Weiteren wurde das Invasionsverhalten der Zelllinien gegenüber Heparin und Zytokinen im Invasionsassay ermittelt. Der Einfluss von TNF alpha, IL-6 und Heparin auf die HGF-mRNA in endometrialen Stromazellen wurde mittels real time RT- PCR bestimmt. Die Experimente wurden meist sowohl unter Normoxie (21 % Sauerstoff) als auch unter Hypoxie (1 % Sauerstoff) durchgeführt. Heparin hat einen geringen Einfluss auf die Spontanproliferation, der abhängig von der Zelllinie, Heparinkonzentration sowie Dauer der Inkubationszeit war. Ebenfalls beeinflusste es die Zytokinsekretion auf Protein- und Genebene zelllinien- sowie zytokinspezifisch. Bei RL95-2, HEC-1-A und AN3 CA kam es zu einer dosisabhängigen Hochregulation der RANTES Sekretion, wohingegen bei KLE eine Abnahme der MCP-1 und IL-8-Sekretion schon bei der geringsten Dosis nachweisbar war. Diese Effekte waren sowohl unter Normoxie als auch Hypoxie gleichermaßen nachweisbar. Die niedermolekularen Heparine sowie Fondaparinux beeinflussten ebenfalls die Zytokinsekretion. In Abhängigkeit von der jeweiligen Zelllinie, der verwendeten Dosis und der Sauerstoffkonzentration zeigte sich eine Hoch-, eine Herunterregulation oder gar keine Beeinflussung des entsprechenden Zytokins. HGF erhöhte die Invasivität von KLE sowie unter Hypoxie von HEC-1-A. Die Kombination von HGF und Heparin erhöhte die Anzahl der invadierten Zellen bei HEC-1-A und RL95-2 im Vergleich zu unbehandelten Zellen. TNF alpha und IL-6 konnten die HGF-Sekretion in endometrialen Stromazellen teilweise erhöhen, wohingegen Heparin alleine keinen Einfluss hatte. Dahingegen konnte Heparin den durch TNF alpha und IL-6 erhöhten HGF mRNA-Gehalt in endometrialen Stromazellen auf das Ausgangsniveau wieder absenken. Heparin konnte sowohl die Spontanproliferation, die Zytokinsekretion und die Invasivität endometrialer Adenokarzinomzelllinien zelllinienspezifisch modulieren. Ebenfalls konnte es den mRNA-Gehalt endometrialer Stromazellen regulieren. Dadurch kann Heparin das Tumormikromilieu beeinflussen und so in wichtige Prozesse der Karzinogenese wie Wachstum sowie Metastasierung eingreifen. Da dem umliegende Gewebe wie karzinomassoziierte Fibroblasten und Makrophagen eine wesentliche Rolle in der Tumorprogression zukommt, müssen weitere Untersuchungen in Kokulturexperimenten erfolgen, um die Bedeutung von Heparin auf das Tumormikromilieu besser zu verstehen. Diese Beobachtung zeigt die Fähigkeit Heparins neben seiner Rolle als Antikoagulanz in wichtige Prozesse der Tumorinitiation und –progression einzugreifen und macht somit seine Anwendung in weiteren Gebieten neben der Antikoagulation vorstellbar.
  • Clinical studies have shown, that the anticoagulant heparin can improve the survival of cancer patients independent of its anticoagulatory activity. It is possible, that it can interact with many molecules because of the negative charge. Cytokines, which are positively charged, play an important role in the micro milieu of the tumor. They can increase proliferation, invasion and the survival of tumor cells. Progression of the tumor for instance is enhanced by different cytokines like RANTES, IL-8, MCP-1 and HGF. Interaction of these cytokines with heparin has been described. Furthermore endometrial cancer cells secret more IL-8 and MCP-1 compared with endometrial epithelial cells. HGF is secreted by endometrial stromal cells. In the following thesis the influence of heparin on the spontaneous proliferation, the cytokine secretion and the invasion of human endometrial adenocarcinoma cell lines was detected. The five human endometrial adenocarcinoma cell lines AN3 CA, ECC-1, HEC-1-A, RL95-2 and KLE (which are differently differentiated) were incubated with different doses of unfractionated heparin. The relative cell number was measured with the viability assay CellTiterBlue®, the cytokine secretion with the ELISA and the expression of the cytokines with real-time RT-PCR. The influence of different low molecular heparins and Fondaparinux on the cytokine secretion was detected also by ELISA. Furthermore the invasion activity for these cell lines was detected by the invasion assay. The influence of TNF alpha, IL-6 and heparin on HGF-mRNA in endometrial stroma cell was detected by real time RT-PCR. The experiments were mostly under normoxic (21 % oxygen) and hypoxic (1% oxygen) conditions. Heparin has minor influence on the spontaneous proliferation depending on the cell line, the dosage of heparin and the time of incubation. Furthermore it affects the cytokine secretion on the level of the protein as well as on the level of the gene. RANTES-Secretion of RL95-2, AN3 CA and HEC-1-A was increased by heparin in a dose dependent manner. In contrast the secretion of MCP-1 and IL-8 of KLE was decreased by every used dosage of heparin. The effects were detectable under normoxic as well as under hypoxic conditions. The low molecular heparin and Fondaparinux also have an influence on the cytokine secretion. There was an increase, a decrease or no difference on cytokine secretion depending on the cell line used, the dosage and the oxygen concentration. HGF enhanced the invasion of KLE and HEC-1-A. However, the invasion of latter was observed specifically under hypoxic conditions. The combination of heparin and HGFincreased the number of invasive cells of the cell line RL95-2 and HEC-1-A compared to untreated cells. TNF alpha and IL-6 increased the secretion of HGF in endometrial stroma cells. Heparin alone had no influence. The combination of heparin with TNF alpha or IL-6 reduced the content of HGF-mRNA to the base level. Heparin has an influence on the spontaneous proliferation, the cytokine secretion and the invasion of the endometrial adenocarcinoma cell lines, dependent on the cell line. Furthermore it can modulate the content of HGF-mRNA in endometrial stroma cell. Thus heparin can affect the micro milieu of the tumor and can interfere with important processes of carcinogenesis, tumor growth and metastasis. Because the surrounding mesh (like carcinoma associated macrophages and fibroblasts) plays an important role in tumor progression, further investigations like co-culture experiments are needed to gain a better insight into the role of heparin in the micro milieu of the tumor. These observations underline the capacity of heparin to interfere with important process of tumor initiation and progression beyond its role of anticoagulant. Thus utilization of heparin in fields other than anticoagulation is conceivable.
