Prävention aus vielen Händen in der Zahnarztpraxis am Beispiel der Darmkrebsvorsorgemotivation : Stellungnahmen von Patientinnen und Patienten zum Potenzial der Zahnärztlichen Praxis bei den Bemühungen, die Inanspruchnahme der Darmkrebsfrüherkennung zu steigern

  • Die zahnärztliche Praxis kann eine wichtige Rolle im Rahmen einer Motivationskampagne zum Thema Darmkrebsvorsorge übernehmen, denn „die regelmäßige Inanspruchnahme von Terminen zur Erhaltung der Zahngesundheit in der Bevölkerung stellt ein ideal geeignetes Portal zur Gesundheitsförderung und Prävention anderer populationsbezogener Erkrankungen dar.“ Patientinnen und Patienten im Wartezimmer einer ländlichen Zahnarztpraxis sind bereit, sich selbstständig mit dem Thema zu beschäftigen. Im ersten Moment empfindet zwar nur eine knappe Mehrheit, dass eine Beratung zur Darmkrebsvorsorge in die zahnärztliche Versorgung passt. Gegen Ende der Befragung wäre der Großteil jedoch dankbar für eine solche. Patientinnen und Patienten im Sprechzimmer der selbigen Praxis, reagieren im direkten Gespräch mit dem Zahnarzt aufgeschlossen, interessiert und positiv. Der Prozentsatz derer, die für Darmkrebsvorsorge keinen Platz in der zahnärztlichen Praxis sehen, nimmt immer mehr ab. 80 % der Patienten wäre dankbar für eine Beratung. Die vorliegende Studie mit je 50 ambulanten Patientinnen und Patienten einer Hauszahnarztpraxis zeigte, dass Zahnärzte als Gesundheitsexperten gesehen werden, die generell auch Erkrankungen außerhalb des Mundraumes betrachten sollten. Für den Großteil der 100 Patienten sind Beratungen zur Inanspruchnahme von Vorsorgedarmspiegelung nicht nur in den zahnärztlichen Alltag integrierbar, sie sollten dort auch zu den festen Aufgaben gehören. Wenngleich nach allgemeiner Ansicht der Befragten eine umfassende Beratung in fachärztliche Hände gehört, wurde durch die Befragung jedoch eindrucksvoll verdeutlicht, dass das Thema Darmkrebs kein Tabuthema ist und Frauen wie auch Männer es nicht als unangenehm empfinden, dies beim Zahnarzt zu thematisieren. Wie kann folglich eine Motivationskampagne in der Zahnarztpraxis umgesetzt werden? Oberste Priorität hat das direkte Gespräch mit dem Zahnarzt. Von einer Broschüre sind die Patienten ebenfalls nicht abgeneigt, möchten sie aber vom behandelnden Arzt erhalten. Zudem sollte diese eher einer kurzen Faktensammlung entsprechen. Die Besprechung der Krebsvorsorge mit dem Praxispersonal ist hingegen eher umstritten, wird aber nicht komplett abgelehnt. Die Illustration des Themas mit Filmen im Wartezimmer-TV oder auf Tablet-PCs finden die Patienten wünschenswert. Eine Beratung sollte ihrer Ansicht nach mindestens fünf Minuten in Anspruch nehmen. Die Integration der KRK-Screeningmotivation in den zahnärztlichen Alltag bedeutet dabei eine vom Wissenschaftsrat seit langen geforderten Verknüpfung innerhalb der Medizin. Jedoch wären hier auch Fragen der Aufwandsvergütung zu thematisieren. Dieses Konzept befördert die Krebsfrüherkennung und die Risikominimierung, die laut WHO und nach Auffassung der Onkologen wichtigsten Maßnahmen im Kampf gegen Krebs.
  • A dental clinic can play an important role in the course of a motivation campaign for colorectal cancer prevention, as “the regular dental visit to maintain tooth health is an ideal platform for health promotion and prevention of other population related diseases“. Patients in a waiting room of a dental clinic in a rural setting are willing to engage themselves with the topic of colorectal cancer prevention. In the first encounter, only a slight majority agrees that colorectal cancer prevention consultation should take place in a dental clinic. However, by the end of the survey a large share of the interviewed patients would be thankful for the consultation. Patients of the survey dental clinic were positively, open-minded and interested during the consultation with the dentist. The share of patients who think that colorectal cancer prevention consultation should not take place in a dental clinic is decreasing. 80 percent of the interviewed patients would be thankful for a consultation. The presented study of 50 ambulant male and 50 ambulant female patients of a dental clinic shows that dentists are viewed as health experts who should consider diseases beyond oral cavity. The majority of the 100 study participants view the consultation for precautionary colonoscopy not only as integrable but hold the opinion it should be an inherent part of the daily routine of a dental clinic. The study shows that despite the general consideration that a specialist should carry out an extensive consultation, colorectal cancer prevention is not a taboo topic. Neither female nor male patients conceive the topic as impropriate to discuss with their dentist. Given this finding, how can a colorectal cancer prevention motivation campaign be implemented in a dental clinic? First priority is the direct counselling by the dentist. Patients also welcome a brochure, which should be supplied directly by the dentist. The brochure should offer a good and short overview of the most important facts. The consultation with clinic staff is controversial, but not completely declined. Theming based on movies in the waiting room or on tablet pcs are welcomed. The consultation should last about five minutes. The integration of the colorectal-cancer-screening motivation in the daily routine of a dental clinic would meet the longstanding postulation of the German Council of Science and Humanities for a closer link within medicine. The concept addresses cancer prevention and risk reduction, which according to WHO and oncologists is the most important step in the fight against cancer.
