• Es sind bereits viele Scores zur präoperativen Diagnostik von Ovarialtumoren entwickelt worden. Die vorliegende Arbeit stellt einen neuentwickelten Score zur präoperativen Einschätzung derselben für Frauen im Alter von über 45 Jahren dar. Es werden die Daten von 330 Patientinnen ausgewertet, die aufgrund eines Ovarialtumors in Greifswald operiert wurden. Ausgewertet und auf Signifikanz geprüft werden: Alter, MP-Status, Hormontherapie, Blutungsstörungen, Beschwerden, bimanueller Tastbefund, Tumormarker CA125, sonographische Merkmale. Die im Zusammenspiel aussagekräftigsten (signifikanten) Merkmale werden in den Score aufgenommen. Dazu gehören : Blutungstörungen, bimanueller Tastbefund, CA 125, die sonographischen Merkmale, ausgeschlossen ist die sonographische Größe. Der Maximalwert des Scores beträgt 20 Punkte. Der Cut off ist bei 5 festgesetzt. 0-1 Punkte- mit hoher Sicherheit, 2-4 Punkte- wahrscheinlich benigner Tumor: Rat zur LSK; 5-7 Punkte- eher, ab größer 7 Punkten sicher maligner Tumor: Rat zur Laparotomie. Der Score erreicht eine Spezifität von 96,0%, eine Sensitivität von 93,1%, einen positiv prädiktiven Vorhersage wert von 91% und ein neg. präd. Vorhersage wert von 97%. Eine Kontrollgruppe von 68 Patientinnen erreicht eine Sensitivität von 89% und eine Spezifität von 96%.
• Many scores for preoperative diagnosis of ovarian tumors have already been developed. This study presents a newly developed score for preoperative assessment of ovarian tumors for women over 45 years old. Data from 330 patients (most postmenopausal), who underwent surgery because of ovarian tumor in Greifswald, are evaluated. The followig aspects were tested for their significance: age, menopausal status, hormone therapy, irregular bleedings, pain, bimanual palpation findings, Tumormarker CA 125 and sonographic characteristics. The characteristics, that when grouped together, were the most predictive markers were retained. Those were: irregular bleedings, bimanual palpation findings, CA125 and the sonographic findings except for size. The maximum score possible is 20 points. Scores above 5 are considered predictive of malignancy. 0-1: high assurance benign, 2-4: probably benign; 5-7: probable malignancy present, >7 malignancy present Hence, 0-4 points indicates laparoscopic exploratory surgery; > 5 points indicates laparotomy recommended. The Score has a specificity of 96% and a sensitivity of 93%, with a positiv predictive value of 91% and a negative predictive value of 97%. When the score was applied to a control group of 68 patients, it had a sensitivity of 89% and a specificity of 96%.
