Zytokine im Urin Frühgeborener bei einer Late-onset Sepsis

  • Late-onset Sepsis (LOS) betrifft bis zu 25% der Neugeborenen mit sehr niedrigem Geburtsgewicht (VLBWI; GG < 1500 Gramm) ab dem 4. Lebenstag. LOS ist eine häufige Ursache neonataler Morbidität und Mortalität sowie von verlängerten Krankenhausaufenthalten. Die frühe Diagnose und rechtzeitige Behandlung sind für die Vermeidung von schweren und lebensbedrohlichen Komplikationen entscheidend. Aufgrund der geringen Spezifität früher Sepsissymptome ist eine frühzeitige Diagnostik schwierig. Diese prospektive Studie untersuchte die Eignung pro- und antiinflammatorischer Zytokine ((Interleukin-(IL)-1-Rezeptorantagonist-(RA), IL-1α, IL-1β, IL-18, Makrophagen-Kolonie-stimulierender Faktor (M-CSF), IL-6, IL-8, IL-10, Interferon-gamma-induziertes Protein 10 (IP-10) und Granulozyten-Makrophagen-Kolonie-stimulierender Faktor (GM-CSF)) im Urin von VLBWI (Gestationsalter < 32 Schwangerschaftswochen und/oder GG < 1500 g) für die frühzeitige Diagnostik einer LOS. Der verwendete Multiplex-Assay erlaubt die schnelle und sensitive Messung multipler Zytokine in einem geringen Probenvolumen. In die Studie wurden 165 VLBWI, die auf der Neugeborenenintensivstation in Greifswald oder Kaunas (Litauen) zwischen Oktober 2007 und September 2009 behandelt wurden, eingeschlossen. Vom 4. bis zum 18. Lebenstag wurden täglich Urinproben gesammelt. Anhand eines Sepsis-Scores wurden die Frühgeborenen in eine Sepsis-Gruppe (n = 39), Nicht-Sepsis-Gruppe (n = 86) und nicht-klassifizierte Gruppe (n = 40) eingeteilt. In die Analyse gingen die Daten von n = 32 Frühgeborenen der Sepsis-Gruppe und n = 8 Frühgeborenen der Nicht-Sepsis-Gruppe ein. Alle untersuchten Zytokine (außer IL-1α und GM-CSF) traten in der Sepsis-Gruppe in signifikant höherer Konzentration auf. Es gab eine deutliche Korrelation zwischen der I/T (Immature to Total Neutrophil) Ratio und der IL-1β-Konzentration im Urin. Weiter korrelierten IL-1β und IL-6 mit dem CRP-(C-reaktives Protein)-Wert. Für IL-1RA (p = 0,002) und IL-18 (p = 0,019) lagen signifikante Unterschiede zwischen der Basiskonzentration und der Konzentration am Sepsistag vor. IL-6 (p = 0,03) und IL-1RA (p = 0,03) zeigten signifikant höhere Konzentrationsanstiege in der Sepsis-Gruppe verglichen mit der Nicht-Sepsis-Gruppe. Die ROC-(Receiver Operating Characteristic)-Analyse identifizierte IL-6 (AUC-(Area under the ROC-curve)-Wert 0,72) als das Zytokin, das eine LOS in den drei Tagen vor und am Tag der klinischen Diagnosestellung am sichersten detektiert. Weitere Untersuchungen müssen klären, ob eine kombinierte Bestimmung von Zytokinen im Urin Frühgeborener es ermöglicht, erstens eine LOS vor Beginn der klinischen Symptomatik zu identifizieren und zweitens somit eine frühere Antibiotikatherapie das Outcome von LOS bei VLBWI verbessern kann.
  • Late-onset sepsis (LOS) affects up to 25% of newborn infants with very low birth weight (VLBWI; BWT<1500 g) from the fourth day of life. LOS is a common cause of neonatal morbidity and mortality, as well as prolonged hospital stays. Early diagnosis and timely treatment are crucial to avoid serious and life-threatening complications. Due to the low specificity of early neonatal sepsis symptoms, an early diagnosis is difficult. This prospective study examined the suitability of pro- and anti-inflammatory cytokines (interleukin [IL]-1 receptor antagonist [RA], IL-1α, IL-1β, IL-18, macrophage colony-stimulating factor [M-CSF], IL-6, IL-8, IL-10, interferon gamma-induced protein 10 [IP-10], and granulocyte-macrophage colony-stimulating factor [GM-CSF]) in the urine of VLBWIs (gestational age <32 weeks and/or gestational weight <1500 g) for the early diagnosis of LOS. The employed multiplex assay allows the rapid and sensitive measurement of multiple cytokines in a small sample volume. The study enrolled 165 VLBWIs treated in the neonatal intensive care units in Greifswald (Germany) and Kaunas (Lithuania) between October 2007 and September 2009. Urine samples were collected daily from the 4th to 18th days after birth. With the aid of a sepsis score, the premature infants were divided into a sepsis group (n = 39), a non-sepsis group (n = 86) and a non-classified group (n = 40). Data were received for analysis from 32 preterm infants of the sepsis group and 8 preterm infants of the non-sepsis group. All examined cytokines (except IL-1α and GM-CSF) occurred in significantly higher concentrations in the sepsis group. There was a significant correlation between I/ T (immature to total neutrophil) ratio and IL-1β concentration in the urine. Furthermore, IL-1β and IL-6 correlated with the CRP (C-reactive protein) value. IL-1RA (p = 0.002) and IL-18 (p = 0.019) exhibited significant differences between the base concentration and the concentration on the clinical sepsis day (day 0). IL-6 (p = 0.03) and IL-1RA (p = 0.03) showed significantly higher rises in concentration in the sepsis group compared with the non-sepsis group. The ROC (receiver operating characteristic) analysis identified IL-6 (AUC [area under the ROC curve] value 0.72) as the cytokine for safest detection of LOS on the three days before and on the day of the clinical diagnosis. Further investigation is needed to clarify whether a combined determination of cytokines in the urine of neonates allows, firstly, the identification of LOS before the clinical symptoms and, secondly, if an earlier antibiotic therapy consequently can improve the outcome of LOS with VLBWIs.
