Okklusales Verteilungsmuster bei festsitzenden und herausnehmbaren prothetischen Versorgungen mit dem „Greifswald Digital Analyzing System“ GEDAS – Erste orientierende Auswertung von bevölkerungsrepräsentativen Daten im Rahmen der „Study of Health in Pomerania 1"

  • Der demografische Wandel in der Bundesrepublik Deutschland führt zu wachsenden Anforderungen an die Quantität und insbesondere an die Qualität des Zahnersatzes im Hinblick auf Haltbarkeit, Ästhetik und Funktionalität. Zur Beurteilung der Funktionalität des prothetischen Zahnersatzes werden in dieser Dissertation okklusale Kontaktpunkte im Bereich der Prämolaren und Molaren nach Anzahl, Größe und Lokalisation analysiert und mit vorhandenen Okklusionskonzepten sowie naturgesunden Zähnen verglichen. Zugrunde liegen die Daten der Study of Health in Pomerania 1 (SHIP 1), bestehend aus einem Interview, einer zahnmedizinischen Untersuchung und einem Bissregistrat in habitueller Interkuspidation. Dieses Bissregistrat wird nach dem Einscannen mit dem Programm GEDAS II im Hinblick auf die Kontaktpunkte der Okklusionsfläche untersucht und in deskriptiven Statistiken ausgewertet. Es zeigt sich, dass weder bei prothetischen Versorgungen noch bei naturgesunden Zähnen alle Kauflächen okklusale Kontakte besitzen. Im Bezug auf Kontaktpunkthäufigkeit, Kontaktpunktanzahl, Häufigkeit der tripodischen Abstützung und Häufigkeit von mehr als drei Kontakten haben Kronen und naturgesunde Zähne die meisten Übereinstimmungen. Bei den Kriterien Kontaktpunkthäufigkeit und tripodische Abstützung gleichen Brückenglieder stärker dem naturgesunden Zahn, bei den Kriterien Kontaktpunktanzahl und Abstützung mit mehr als drei Kontaktpunkten liegt die größte Übereinstimmung bei Totalprothesen. Teilprothesen haben deutlich seltener und weniger Kontakte als festsitzende Versorgungen oder Totalprothesen. Die Lokalisation der okklusionstragenden Kontaktpunkte prothetischer Restaurationen entspricht größten Teils denen der natürlichen Zähne. Auffällig ist, dass sich bei Totalprothesen die häufigsten okklusionstragenden Kontaktpunkte des Oberkiefers und des Unterkiefers im gleichen Areal der Kaufläche befinden, was einem Kopfbiss entspricht. Verglichen mit den verschiedenen Okklusionskonzepten wird deutlich, dass die registrierte Häufigkeit von Kontaktpunkten auf prothetischen Versorgungen und auf naturgesunden Zähnen nicht der Theorie, dass alle Zähne okklusale Kontakte aufweisen, entspricht. Die durchschnittliche Kontaktpunktanzahl von naturgesunden Zähnen und festsitzenden wie herausnehmbaren Versorgungen ist, vor allem im Bereich der Molaren, deutlich geringer als in den Okklusionsschemata der ABC-Kontakte, der maximalen Interkuspidation und der idealisierten Okklusion nach Payne. Eine deutliche Übereinstimmung der prothetisch versorgten Zähne mit naturgesunden Zähnen und den Okklusionskonzepten zeigt sich bei der Betrachtung von Zähnen mit mindestens drei okklusalen Kontakten. Hier entsprechen die Häufigkeit und die Lokalisation der Kontaktpunkte in okklusionstragenden Höckerbereichen den Konzepten der biologischen und der harmonischen Okklusion.
  • The demographic changes in the Federal Republic of Germany are increasing the requirements of the quantity and especially of the quality of dental prosthetics in relation to functionality, aesthetics and durability. This dissertation evaluates the functionality of artificial dentition by analyzing registered occlusal contact points of premolar and molar teeth in number, size and position and comparing these to theoretical occlusal concepts and natural teeth. The data are based on the Study of Health in Pomerania 1 (SHIP 1) comprising an interview, a dental examination and a bite register in habitual intercuspation. After scanning, the bite register is examined with GEDAS II application according to dental contact points of the occlusal surface and evaluated in descriptive statistics. The results indicate that neither dental restorations nor natural teeth have occlusal contact at all times. Regarding the frequency and number of occlusal contacts as well as the tripodization and support with more than three contacts, the crown is mostly equal to healthy teeth. In the criteria of frequency of occlusal contact points and tripodization the pontic is more similar to natural teeth; in regard of number and support of more than three occlusal contacts the complete dentures rather agree to natural teeth. Partial prostheses have obviously rare and less occlusal contact points than fixed dental restorations and complete replacements. The localization of occlusion bearing contact points of prosthetic restorations corresponds largely with those of natural teeth. It is notable that on complete dentures the most occlusion bearing contacts in the maxilla and mandible are found in the same location of the occlusal surface, complying with an edge-to-edge bite. The comparison of the different occlusal concepts shows that the registered frequency of occlusal contact points on prosthetic restorations and natural teeth does not comply with the theory that all teeth have occlusal contacts. Particularly in the molar area the average number of contact points of natural teeth and fixed as well as removable dentures is clearly less than in occlusal concepts of ABC-contacts, maximal intercuspation and idealized occlusion of Payne. A strong correlation of dental prostheses with natural teeth and occlusal concepts appears in the consideration of teeth with at least three occlusal contacts. In this case frequency and localization of contact points in occlusion bearing cusps comply with the concepts of biological and harmonic occlusion.
