Retrospektive Auswertung postoperativer Ergebnisse nach Zyklokryokoagulation und Zyklophotokoagulation

  • In der Therapie des Glaukoms stellen zyklodestruktive Verfahren, wie die Zyklophotokoagulation und die Zyklokryokoagulation, wichtige Behandlungsmethoden dar. In der vorliegenden Dissertation wurden retrospektiv die Behandlungsergebnisse nach einer Zyklophotokoagulation den Ergebnissen nach einer Zyklokryokoagulation gegenübergestellt. In der Augenklinik des Universitätsklinikums Greifswald wurde im Zeitraum von Juli 2004 bis Juni 2008 bei 55 Patienten an insgesamt 57 Augen eine Zyklophotokoagulation durchgeführt. Bei 40 dieser Augen lag ein Sekundärglaukom, bei 16 Augen ein primär chronisches Offenwinkelglaukom und bei einem Auge ein kongenitales Glaukom vor. Der mittlere präoperative Augeninnendruck betrug 29,07 mmHg. Postoperativ lag der Augeninnendruck bei 17,44 mmHg (± 6,5 mmHg) und drei Jahre nach Entlassung bei 17,38 mmHg (± 6,1 mmHg). Präoperativ wurden durchschnittlich 2,45 Wirkstoffe zur Augeninnendrucksenkung benötigt, drei Jahre nach Entlassung 2,14. Der Visus lag präoperativ bei 0,239, postoperativ bei 0,231. 32% der Patienten benötigten präoperativ systemische Carboanhydrasehemmer, postoperativ waren es 11%. Als Komplikation nach einer Zyklophotokoagulation trat bei 8,8% der Patienten eine Chemosis auf, bei 1,8% eine Vorderkammereinblutung und bei 1,8% eine Phthisis bulbi. Für eine ausreichende Drucksenkung wurden im Durchschnitt 1,54 Sitzungen benötigt. Eine Zyklokryokoagulation wurde im Zeitraum von Januar 2000 bis Juni 2008 bei 45 Patienten an insgesamt 46 Augen durchgeführt. 39 dieser Augen waren an einem Sekundärglaukom erkrankt, hiervon zählten 20 zum Neovaskularisationsglaukom. Vier Augen hatten ein primär chronisches Offenwinkelglaukom und drei Augen ein kongenitales Glaukom. Der Augeninnendruck lag präoperativ im Durchschnitt bei 34,19 mmHg (± 10,3 mmHg), postoperativ bei 23,11 mmHg (± 9,1 mmHg) und drei Jahre nach Entlassung bei 18,17 mmHg (± 4,6 mmHg). Präoperativ benötigten die Patienten 2,17 augeninnendrucksenkende Wirkstoffe, drei Jahre nach Entlassung 2,00. Die Einnahme von systemischen Carboanhydrasehemmern wurde von 23,9% der Patienten präoperativ auf 17,4% postoperativ reduziert. Der Visus lag prä- und postoperativ bei 0,049. Als Komplikation trat nach der Zyklokryokoagulation bei 41% der Patienten eine Chemosis und bei 4,3% eine Phthisis bulbi auf. Für eine ausreichende Drucksenkung wurden wie in der Gruppe der Zyklophotokoagulation 1,54 Sitzungen benötigt. Beim Vergleich der beiden Behandlungsmethoden hinsichtlich der Senkung des Augeninnendruckes wird postoperativ und zum Zeitpunkt der Entlassung ein signifikant niedrigerer IOD in der Gruppe der Zyklophotokoagulationen gefunden. Allerdings ist auch der präoperative IOD in der Gruppe der Zyklophotokoagulationen signifikant niedriger als in der Gruppe der Zyklokryokoagulationen. Zu allen anderen Zeitpunkten gibt es keinen signifikanten Unterschied zwischen beiden Gruppen. Hinsichtlich der Medikamentenreduktion, der postoperativen Visusminderung und der Anzahl der benötigten Sitzungen unterscheiden sich beide Gruppen nicht signifikant. In der Gruppe der Zyklophotokoagulationen konnte die Einnahme von systemischen Carboanhydrasehemmern stärker reduziert werden als in der Gruppe der Zyklokryokoagulationen. Die Komplikationsrate ist in der Gruppe der Zyklokryokoagulationen höher. In der Gruppe der Zyklokryokoagulationen finden sich vor allem Patienten mit einem Neovaskularisationsglaukom oder anderen Sekundärglaukomen. Viele der postoperativen Komplikationen stehen sicherlich mit dem Glaukomtyp in Beziehung und sind nicht ausschließlich auf die Behandlungsmethode zurückzuführen. Insgesamt zeigten sich beide Behandlungsverfahren als effektive Methoden zur Senkung des Augeninnendruckes. Die Zyklophotokoagulation scheint einen etwas stärkeren drucksenkenden Effekt, bei geringerer Komplikationsrate, als die Zyklokryokoagulation zu haben.
  • For the therapy of the glaucoma cyclodestructive procedures, like cyclophotocoagulation and cyclocryocoagulation, are important treatment options. This dissertation compares in retrospect the treatment outcome after cyclophotocoagulation with the outcome after cyclocryocoagulation. In the ophthalmic clinic of the University Hospital of Greifswald during the period from July 2004 till June 2008 cyclophotocoagulation was performed on a total of 57 eyes at 55 patients. 49 of these eyes had a secondary glaucoma, 16 eyes a primary chronic open angle glaucoma and one eye had a congenital glaucoma. The mean intraocular pressure was 29,07 mmHg. Postsurgical the intraocular pressure was 17,44 mmHg (±6,5 mmHg) and 17,38 mmHg (±6,1 mmHg) three years after discharge. On average 2,45 agents for lowering the intraocular pressure were needed presurgical and 2,14 agents three years after discharge. Visual acuity was 0,239 presurgical and 0,231 postsurgical. Presurgical 32% of the patients needed carbonic anhydrase inhibitors, postsurgical it were 11%. As a complication after cyclophotocoagulation 8,8% of the patients had a chemosis, 1,8% a hyphema and 1,8% a phthisis bulbi. For a sufficient pressure lowering on average 1,54 sessions were needed. During the period from January 2000 till June 2008 a cyclocryocoagulation was performed on a total of 46 eyes at 45 patients. 39 of these eyes suffered from a secondary glaucoma with 20 of them being rated as neovascular glaucoma. Four eyes had a primary chronic open angle glaucoma and three eyes had a congenital glaucoma. On average the intraocular pressure was 34,19 mmHg (±10,3 mmHg) presurgical, 23,11 mmHg (±9,1 mmHg) postsurgical and 18,17 mmHg (±4,6 mmHg) three years after discharge. Presurgical the patients needed 2,17 agents for lowering the intraocular pressure and three years after discharge 2,00. The intake of systemic carbonic anhydrase inhibitors was reduced from presurgical 24,9% of the patients to 17,4% postsurgical. Visual acuity was 0,049 pre- and postsurgical. As a complication after cyclocryocoagulation 41% of the patients had a chemosis and 4,3% a phthisis bulbi. Like the group treated with cyclophotocoagulation 1,54 sessions were needed for a sufficient pressure lowering. During the comparison of both treatment methods concerning the reduction of the intraocular pressure there was a significant lower IOP (intraocular pressure) found postsurgical for the group of cyclophotocoagulation as well as at the time of discharge. But the presurgical IOP in the group of cyclophotocoagulation is also significantly lower than in the group of cyclocryocoagulation. At every other point in time there is no significant difference between both groups. Regarding the reduction of medication, the postsurgical visual acuity impairment and the number of sessions needed there are no significant differences between both groups. In the group of cyclophotocoagulation the intake of systemic carbonic anhydrase inhibitors could be reduced more than in the group of cyclocryocoagulation. The complication rate is higher in the group of cyclocryocoagulation. In the group of cyclocryocoagulation there are mainly patients with neovascular glaucoma or other secondary glaucomas. Many of the postsurgical complications are surely related with the type of the glaucoma and cannot solely be linked with the method of treatment. Overall both treatment procedures have shown themselves as efficient methods for lowering the intraocular pressure. Cyclophotocoagulation seems to have a slightly stronger pressure reducing effect at a lower complication rate than the cyclocryocoagulation.
