Anwendungsbeobachtung zur Ermittlung der Anzahl erforderlicher Händedesinfektionen auf zwei Nichtrisikostationen unter Zugrundelegung der WHO-Indikationen zur Händedesinfektion

  • Es handelt sich um eine Anwendungsbeobachtung zur Ermittlung der Anzahl erforderlicher Händedesinfektionen im Stationsalltag anhand der von der WHO definierten Indikationen für die Händedesinfektion. Bisher liegen Werte zur Häufigkeit der HD nur in der Intensivpflege vor, nicht aber in sogenannten „Nichtrisikobereichen“, sodass die Bewertung von Verbrauchsdaten nicht möglich ist. Erstmalig wurden mit einer Beobachtungsstudie für zwei stationäre Nichtrisikobereiche Richtwerte zur Anzahl erforderlicher HD und zusätzlich die Compliance ermittelt. Es wurden 2424 Indikationen zur HD dokumentiert. Eine Pflegekraft kommt im Durchschnitt auf 70 (MKG) bzw. 161 (Nephrologie) Indikationen zur HD pro Arbeitsschicht. Beim ärztlichen Personal wurden durchschnittlich 16 (Nephrologie) bzw. 17 (MKG) Indikationen pro Arbeitsschicht beobachtet. Im Mittel ist die Compliance der HD mit 61% auf der MKG-chirurgischen Station und mit 65% auf der nephrologischen Station > 60%. Mit einem Wert von 66,7% weist das ärztliche Personal der Nephrologie die höchste Compliance auf, gefolgt vom ärztlichen Personal der MKG-Chirurgie mit einer Compliance von 66,1%, dem Pflegepersonal der Nephrologie mit 64,7% und der MKG-chirurgischen Station mit 59,9%. Nach Auswertung der Apothekenverbrauchswerte konnte gezeigt werden, dass die Compliance durch den Hawthorne-Effekt beeinflusst wurde. Trotz im Klinikum ausreichend vorhandener Anzahl von Desinfektionsspendern und Piktogrammen zur richtigen Durchführung der HD sowie geschultem Personal ist die Compliance mit durchschnittlich 60% auf beiden Stationen ähnlich hoch wie die Compliance auf Intensivstationen mit durchschnittlich 59% (Scheithauer et al. 2009). Es wird offensichtlich, dass die Compliance auch in Nichtrisikobereichen weiter verbessert werden muss.
  • It is an observational study to determine the number of required hand disinfections in everyday ward routine based on the indications for hand disinfection defined by the WHO. So far, frequency values of hand desinfection are only available about intensive care units, but not in so-called "non-risk areas", so that the evaluation of consumption data is not possible. For the first time, an observational study for two normal wards evaluated the number of required hand desinfectins and additionally determined the compliance. 2424 indications for hand disinfection were documented. A nurse has an average of 70 (maxillofacial surgery) and 161 (nephrology) indications for hand desinfection in a work shift. An average of 16 (nephrology) and 17 (maxillofacial surgery) indications were observed in a doctor’s work shift. On average, the compliance of hand disinfection is 61% in the maxillofacial surgery ward and 65% on the nephrological ward. With a compliance rate of 66.7% does the medical staff of the nephrological ward show the highest compliance rate, followed by the medical staff of the maxillofacial surgery ward with a compliance rate of 66.1%, the nurses of the nephrological ward with 64.7% and the nurses of the maxillofacial surgery ward with 59.9%. After evaluating the pharmacy consumption values, it could be shown that compliance was influenced by the Hawthorne effect. Despite a sufficient number of disinfectant dispensers in the clinic and pictograms for the correct implementation of the hand disinfection as well as trained staff is the compliance rate with an average of 60% on both normal wards similar to the compliance rate on intensive care units with an average of 59% (Scheithauer et al. 2009). It gets obvious that compliance has to be further improved.
