Charakterisierung kardiotroper Antikörper und antigener Strukturen bei Patienten mit dilatativer Kardiomyopathie

  • Grundlagen - Die Immunadsorption (IA) entfernt kardiale Autoantikörper aus dem Blut und stellt einen neuartigen Ansatz zur Therapie von Patienten mit dilatativer Kardiomyopathie (DCM) dar. Ziel dieser Arbeit ist es, die Bedeutung kardiotroper Antikörper für die kardiale Dysfunktion bei Patienten mit DCM zu untersuchen. Die kardiotropen Antikörper werden IgG-subklassenspezifisch charakterisiert und die potenzielle Bedeutung von Myosin als spezifischem Antigen der Autoantikörper untersucht. Methoden und Ergebnisse - Vor der IA wurden aus dem Plasma von n=45 Patienten die Antikörper isoliert und deren Effekt auf Rattenkardiomyozyten getestet. Nach der In-vitro-Testung wurde bei allen Patienten die IA durchgeführt. Die Antikörper von n=29 Patienten (Kardiodepressive Gruppe) hatten einen kardiodepressiven Effekt auf die Herzmuskelzellen (Ca++-Transient: -16,5 ±1.9%; Kontraktilität: -21,2 ±1.8%; p<0.001 vs. Kontrollgruppe), während die Antikörper von n=16 Patienten (Nicht-kardiodepressive Gruppe) keinen signifikanten Effekt zeigten. Im ersten Zyklus der IA stieg der Herzindex (CI) in der Kardiodepressiven Gruppe von 2,2 ±0,1 auf 2,9 ±0,1 l/min/m² (p<0.001). In der nicht-kardiodepressiven Gruppe traten unter der gesamten Dauer der IA (3-4 Monate) keine signifikanten Veränderungen der hämo-dynamischen Parameter auf. Die Auswurffraktion (LVEF) war nach Abschluss der Immun-adsorption nur in der kardiodepressiven Gruppe signifikant von 20,8 ±1,1 auf 30,5 ±1,1% angestiegen (p<0.001). Der Vergleich von Gesamt-IgG, isolierter IgG-3-Subklasse sowie IgG-3-freien-IgG von n=9 Patienten der kardiodepressiven Gruppe zeigte, dass sowohl Gesamt-IgG als auch die IgG-3-Subklasse einen kardiodepressiven Effekt haben (Ca++-Transient: Gesamt-IgG -12,3 ±0,3% / IgG-3: -12,1 ±0,3%; Kontraktilität: Gesamt-IgG -15,1 ±0,5% / IgG-3: -15,0 ±0,7%; p<0.001 vs. Kontrollgruppe). Das IgG-3-freie-IgG hatte keinen signifikanten Effekt. Die Inkubation der Antikörper von n=6 Patienten mit Myosin hatte keinen Einfluss auf den kardiodepressiven Effekt (Ca++-Transient: Antikörper -12,6 ±0,4%/ Antikörper + Myosin: -12,3 ±0,3%; Kontraktilität: Antikörper: -15,4 ±0,6%/ Antikörper + Myosin: -15,9 ±0,8%; p<0.001 vs. Kontrollgruppe). Schlussfolgerungen - Antikörper mit kardiodepressiver Wirkung wurden bei einer Untergruppe (64,5%) von DCM-Patienten identifiziert. Durch Testung auf kardiodepressiv wirkende Antikörper lässt sich der akute und prolongierte hämodynamische Effekt der IA prognostizieren. An der kardiodepressiven Wirkung sind Antikörper der IgG-3-Subklasse beteiligt, diese sollten bei der IA effektiv eliminiert werden. Spezifische Anti-Myosin-Antikörper scheinen nicht an der kardiodepressiven Wirkung beteiligt zu sein. Die Immunadsorption eliminiert kardiodepressiv wirkende Antikörper und weist auf eine funktionelle Bedeutung der humoralen Autoimmunität für die kardiale Dysfunktion bei Patienten mit DCM hin.
  • Background - Immunoadsorption (IA) which removes circulating cardiac autoantibodies represents an additional therapeutic approach in dilated cardiomyopathy (DCM). The role of the autoantibodies in cardiac dysfunction remains to be elucidated. The relation of antibody subclass specificity and specific antigenic structures (myosin) to the haemodynamic dysfunction in DCM is poorly understood. Methods and Results - Before IA antibodies were purified in vitro from plasma of 45 DCM patients. The antibodies were superperfused to adult rat cardiomyocytes. Changes in calcium transient and systolic cell shortening were detected. After in vitro analysis, IA was performed in all patients. Antibodies from 29 patients (Cardiodepressant group) induced a reduction of calcium transient (-16.5 ±1.9%) and a simultaneous reduction of systolic cell shortening (-21.2 ±1.8%) on cardiomyocytes (p<0.001 vs controls). Antibodies from 16 patients (Non-cardiodepressant group) did not significantly influence calcium transient or cell shortening. During the first course of IA the Cardiodepressant group showed an acute increase in cardiac index (CI) from 2.2 ±0.1 to 2.9 ±0.1 l/min/m² (p<0.001). In the Non-cardiodepressant group haemodynamics did not significantly change throughout the three months of IA. After three months the left ventricular ejection fraction (LVEF) increased significantly only in the Cardiodepressant group from 20.8 ±1.1 to 30.5 ±1.1% (p<0.001). Antibodies from 9 patients of the Cardiodepressant group were further processed to isolate the immunoglobulin subclass IgG-3. The effect of the antibodies, isolated IgG-3 and antibodies free of IgG-3 was analyzed. The antibodies and isolated IgG-3 induced a reduction of calcium transient (Antibodies: -12.3 ±0.3% / IgG-3: -12.1 ±0.3%; p<0.001 vs controls) and systolic cell shortening (Antibodies: -15.1 ±0.5% / IgG-3: -15.0 ±0.7%; p<0.001 vs controls). Antibodies free of IgG-3 had no significant effect compared to controls. Antibodies from 6 patients were incubated with myosin in the manner of a neutralization assay. A significantly different effect between antibodies and antibodies + myosin on the reduction of calcium transient (Antibodies: -12.6 ±0.4% / Antibodies + Myosin: -12.3 ±0.3%; p<0.001 vs. controls) and systolic cell shortening (Antibodies: -15.4 ±0.6% / Antibodies + Myosin: -15.9 ±0.8%; p<0.001 vs controls) could not be identified. Conclusions - In a subgroup of DCM patients (64.5%) cardiac autoantibodies contribute to cardiac dysfunction. Evidence for cardiodepressant antibodies predicts acute and prolonged haemodynamic benefit during IA. The cardiodepressant effect may be induced by antibodies belonging to the immunoglobulin subclass IgG-3. Myosin could not be identified as specific antigen. Removal of antibodies of the IgG-3 subclass may represent an essential mechanism of immunoadsorption in patients with DCM.
