Expression, Lokalisation und Funktion von Transportproteinen in der humanen Plazenta

  • Ziel der Arbeit war es, ein Probenkollektiv zu etablieren und die Expression, Lokalisation und Funktion pharmakologisch und physiologisch relevanter Membrantransporter systematisch zu untersuchen. Dafür wurde aus 70 humanen Plazenten mRNA, Protein und genomische DNA isoliert. Im Anschluss konnte die mRNA-Expression der Membrantransporter ABCB1 (P-gp), ABCC1 (MRP1), ABCC2 (MRP2), ABCC3 (MRP3), (ABCC5), ABCG2 (BCRP), SLC10A6 (SOAT), SLC22A4 (OCTN1), SLC22A5 (OCTN2), SLC22A11 (OAT4) und SLCO2B1 (OATP2B1) bestimmt und hinsichtlich ihrer Assoziation mit dem Gestationsalter und dem Geschlecht analysiert werden. Dabei zeigte sich eine signifikante gestationsabhängige Expressionsabnahme für das P-gp, BCRP, OATP2B1 und OAT4, darüber hinaus war die Expression von MRP1 in Plazenten männlichen Feten im Vergleich zu weiblichen Feten deutlich erhöht. Da fast alle Organe, mit Ausnahme der Leber, Niere und Gehirn, auf eine Carnitinaufnahme aus dem Blut angewiesen sind, spielen Carnitintransporter wie das SLC22A4 (OCTN1) und SLC22A5 (OCTN2) eine entscheidende Rolle: In der Plazenta sind sie bei der Versorgung des Fetus mit L-Carnitin unabdingbar. Auf mRNA-Ebene konnte in diesem Zusammenhang zunächst keine gestationsabhängige Expressionsänderung gezeigt werden, obwohl der Fetus zum Ende der Schwangerschaft einen deutlich erhöhten Carnitinbedarf hat. Die daraufhin durchgeführte Bestimmung der korrespondierenden OCTN2-Proteinexpression, die interessanter Weise nicht mit den mRNA-Werten korrelierte, zeigte tatsächlich eine erhöhte Transporterexpression zum Ende der Schwangerschaft hin. Für das OCTN2 wurden weiterhin Untersuchungen zum Einfluss eines bekannten Promotorpolymorphismus, dem -207G>C, der in anderen Studien mit der OCTN2 Expression assoziiert werden konnte, durchgeführt. Hier zeigte sich, dass diese genetische Variante zwar die mRNA Expression des Transporters beeinflusst, sich dieser Effekt aber wiederum nicht auf die Proteinebene übersetzt, so dass anzunehmen ist, dass die plazentare OCTN2 Expression auch von nicht transkriptionellen Faktoren (z.B. miRNA) bestimmt wird. Mit Blick auf die anderen untersuchten Transporter zeigte sich in vielen Fällen eine Korrelation in der Expression bestimmter Efflux- und Aufnahmetransporter. Ein Beispiel für ein solches Transporterpaar war dabei das BCRP und OATP2B1,das in der Folge weiter untersucht wurde, wobei insbesondere ein mögliches funktionelles Zusammenspiel im Mittelpunkt stand. Aufbauend auf der apikalen Expression des BCRP und der basalen des OATP2B1, wurde ein, auf MDCKII-Zellen basiertes, in vitro Zellsystem verwendet, um die Bedeutung dieser Transporter für den transzellulären Transport von Substanzen wie E1S und DHEAS zu zeigen. In der Tat war ein gerichteter Transporter dieser Substanzen nur zu beobachten, wenn beide Transporter in den Zellen exprimiert wurden. Diese Ergebnisse sind bezüglich der Hormonsynthese durch die Plazenta von Bedeutung, da diese während der Schwangerschaft einen zentralen Syntheseort darstellt, aber einige Vorläufermoleküle nicht selber synthetisieren kann. Zusammengefasst bietet die vorliegende Arbeit ein umfassendes Bild über die Regulation wichtiger physiologischer und auch pharmakologischer Membrantransporter im letzten Drittel der Schwangerschaft. Darüber hinaus konnte für ausgewählte Transporter das Zusammenspiel (OATP2B1 und ABCG2) und die Regulation (OCTN2) näher charakterisiert werden.
  • Based on a sample libary that was created within this work the present study systematically investigated the expression, localization and function of membrane transporters of pharmacological and physiological relevance For this, mRNA, protein and genomic DNA was isolated from 70 human placentas and mRNA expression of the following membrane transporters was determined: ABCB1 (P-gp), ABCC1 (MRP1), ABCC2 (MRP2), ABCC3 (MRP3), (ABCC5), ABCG2 (BCRP), SLC10A6 (SOAT), SLC22A4 (OCTN1), SLC22A5 (OCTN2), SLC22A11 (OAT4) and SLCO2B1 (OATP2B1). The results were analyzed for association with gestational age and sex of the fetus respectively and a statistically significant decrease of P-gp, BCRP, OATP2B1 and OAT4 with gestational age was detected, whereas expression of MRP1 in placentas of male fetuses was significantly increased comparing to female fetuses. Since almost all organs but liver, kidney and brain rely on carnitine uptake from the blood, carnitine transporters such as the SLC22A4 (OCTN1) and SLC22A5 (OCTN2) play a crucial role: In the placenta they are of high relevance for the supply of the fetus with essential L-carnitine. However, the present study could not demonstrate any changes of gene expression in terms of variations in mRNA level, although the fetus has a significantly increased carnitine requirement at the end of pregnancy. In addition the corresponding OCTN2 protein expression actually showed an increased transporter expression at the end of pregnancy which does not correlate with the mRNA expression. Concerning OCTN2, further investigations were performed on the influence of a the promoter polymorphism -207G>C, which in other studies was associated with the OCTN2 expression. I was able to demonstrate an effect of this SNP on the mRNA expression whereas a lack association with the protein expression was observed. The results indicate that placental OCTN2 expression is determined by non-transcriptional factors (eg, miRNAs). For the other transporters investigated, correlation in the expression of certain efflux and uptake transporters was observed in many cases. As an example of such a corresponding transporter pair, BCRP and OATP2B1 were investigated to show a possible functional interaction. Based on the apical expression of BCRP and the basal expression of OATP2B1 an in vitro MDCKII cell system was used to show the relevance of these transporters for the trans cellular transport of substances such as E1S and DHEAS. In fact, a directive transport of these substances was observed exclusively when both transporters were expressed in the cells. These results are of importance since allthough the placenta represents a central site of hormone synthesis during pregnancy, it cannot synthesize some precursor molecules for hormonal production by itself. In summary, I was able to deliver a comprehensive insight to the regulation of relevant physiological and pharmacological membrane transporters in the last trimester of pregnancy. Moreover, I could characterize the interaction (OATP2B1 and ABCG2) and regulation (OCTN2) for selected transmembrane transporters.
