Impfeinstellung und -verhalten unter niedergelassenen Kinder- und Jugendmedizinern mit und ohne Schwerpunkt-/Zusatzbezeichnung Homöopathie – deskriptive Querschnitts-Studie für Deutschland –

  • Hintergrund: Die zunehmende Impfkritik in bestimmten Bevölkerungskreisen wird in Zusammenhang mit der zunehmenden Verbreitung von Komplementärmedizin gebracht. Homöopathie ist einer der stärksten Vertreter unter der wachsenden Komplementär-Medizin. Besonders in der Pädiatrie, wo die meisten Impfungen durchgeführt werden, wird Homöopathie vermehrt eingesetzt. Eine Umfrage aus dem Jahre 1996 zeigte erstmals, dass entgegen der öffentlichen Meinung homöopathische Ärzte in Deutschland keine entschiedenen Impfgegner sind, aber durchaus kritischer und individueller beim Impfen vorgehen. Methodik: In der vorliegenden Arbeit werden Impfeinstellung und Impfverhalten unter niedergelassenen Pädiatern in ganz Deutschland untersucht. Hierbei werden Pädiater mit und ohne Zusatzbezeichnung Homöopathie gegenübergestellt. Mittels einer repräsentativen Querschnittuntersuchung wurden im Jahre 2006/2007 50 % (N=3019) aller niedergelassenen Pädiater aus ganz Deutschland per Post (Zufallsstichprobe) zum Thema Impfen befragt. Ergänzend wurden zeitgleich 245 homöopathisch orientierte Pädiater in die anonymisierte Befragung eingeschlossen. Das Instrument wurde zuvor in einer Pilotstudie unter 200 deutschen Pädiatern evaluiert. Die Auswahl der Pädiater erfolgte über Adressverzeichnisse der Kassenärztlichen Vereinigungen und Ärztekammern der verschiedenen Bundesländer, sowie zusätzlich über den Deutschen Zentralverein für homöopathische Ärzte (DZVhÄ) und der Karl und Veronica Carstens (KVC)-Stiftung um eine möglichst große Vergleichsgruppe an homöopathischen Pädiatern zu erhalten. Die Rückläufe der Zufallsziehung, der zusätzlich homöopathischen Pädiater und der Pilotstudie wurden für die Auswertung gepoolt. Für einen Vergleich wurden mittels 6 (Teil)-Fragen des Erhebungsinstrumentes Pädiater mit und ohne Schwerpunkt-/Zusatzbezeichnung Homöopathie differenziert. Letztere wurden zusätzlich unterschieden in Pädiater, die Homöopathie oder eine andere alternative/komplementäre Medizin anwenden oder rein konventionelle Medizin betreiben. Das Impfverhalten wurde im Hinblick auf das Einhalten der STIKO-Empfehlungen anhand von 3 Fragen des Erhebungsinstrumentes als Mittelwert-Score operationalisiert:1. Impfen Sie nach dem empfohlenen Impfkalender der STIKO? 2. Wie würden Sie eigene minderjährige Kinder Impfen? 3. Ab welchem Alter empfehlen Sie in der Regel die erste Durchführung von Impfungen? Um die Impfeinstellung zu operationalisieren, wurden die skalierten Positionierungen zu 8 vorgegebenen Meinungen zum Impfen zu einem Mittelwert-Score verrechnet. Der Einfluss folgender relevanter Variablen auf Impfeinstellung und Impfverhalten wurde mit einer multivariablen linearen Regression untersucht: Alter, Geschlecht, Ort der Niederlassung, Anteil an Privatpatienten und Zusatzbezeichnung Homöopathie. Für einen Vergleich mit den Studienergebnissen von 1996 wurde ein Impfkritiker-Index erstellt. Ergebnisse und Diskussion: Mit insgesamt 3464 angeschriebenen niedergelassenen Pädiatern (über 50 % aller niedergelassenen Pädiater aus jedem Bundesland) handelt es sich um die bisher größte bundesweite Studie zum Thema Impfen. Die Responserate ist mit insgesamt ca. 67 % überdurchschnittlich hoch, was ein großes Interesse zum Impfthema vermuten lässt. Vor dem Hintergrund einer weitestgehend vollständigen und aktuellen Praxis-Adress-Datenbasis kann diese Studie als deutschlandweit repräsentativ angesehen werden. Die Ergebnisse dieser Arbeit sind trotz leichter methodisch bedingter Einschränkungen in der Konstruktion der Stichprobe einmalig und aussagekräftig. Pädiater mit und ohne Zusatzbezeichnung Homöopathie unterscheiden sich im Hinblick auf ihre Impfeinstellung und ihr Impfverhalten (orientiert an den STIKO-Empfehlungen) signifikant. Die überwiegend positive, aber kritische Haltung zum Impfen unter homöopathischen Pädiatern ähnelt den Ergebnissen von P. Lehrke von 1996. Das Vorurteil „Homöopathen seien Impfgegner“ konnte für Kinderärzte aussagekräftig widerlegt werden. Mit ca. 70 % hat die Mehrheit der homöopathisch arbeitenden Pädiater in Deutschland eine positive Einstellung zum Impfen. Sie sind aber auch mehrheitlich individuelle Impfkritiker. Die stärkste Impfkritik ist unter rein klassischen Homöopathen zu finden, welche ca. 47 % der Pädiater mit Zusatzbezeichnung Homöopathie ausmachen. Die Zusatzbezeichnung Homöopathie hat von allen relevanten Variablen den größten Einfluss auf eine impfkritische Einstellung und entsprechendes Verhalten. Abweichungen von den STIKO-Empfehlungen betreffen aber überwiegend spätere Impfungen (überwiegend 1–3 Monate), sowie neuere Impfungen wie gegen Pneumokokken, Varizellen und Meningokokken. Als häufigster Grund für Abweichungen von den STIKO-Richtlinien wurde der Wunsch der Patienten angegeben. Da impfkritische Eltern eher Pädiater mit komplementärmedizinischem Zusatzangebot auswählen, beeinflussen sie auch die Erfahrungen und das Impfverhalten des Arztes. Es kann davon ausgegangen werden, dass sich das Patientenklientel von Homöopathen und Nicht-Homöopathen systematisch unterscheidet. Unterschiedliche Erfahrungen durch den Umgang mit Ungeimpften und den Möglichkeiten homöopathischer Behandlungen könnten die Veränderungen der Impfeinstellung hin zu mehr Impfkritik im Lebenslauf von Homöopathen erklären. Homöopathen geben mit Abstand am häufigsten (über 60 %) eine Veränderung in ihrer Einstellung zum Impfen im Lebenslauf an, Nicht-Homöopathen in nur ca. 10–22 %. Bezüglich der Impfentscheidung der Patienten hat wiederum die persönliche ärztliche Empfehlung als wichtigste Vertrauensperson einen großen Stellenwert. Erfahrungen und Einstellung beeinflussen sich wechselseitig. Basierend auf der Zufallsstichprobe haben ca. 10 % aller niedergelassenen Pädiater Deutschlands die Zusatzbezeichnung Homöopathie. Es gibt Hinweise, dass die Zusatzbezeichnung Homöopathie auf Grund der Patientennachfrage zugenommen hat. Das kann die häufige Anwendung von Homöopathie unter Pädiatern ohne Zusatzbezeichnung Homöopathie (ca. 40 %) erklären. Entsprechend dem marktwirtschaftlichen Prinzip des Gesundheitssystems wird die Homöopathie trotz wissenschaftlich strittiger Studienlage zunehmend von den Kassen vergütet, u.a. um konkurrenzfähig zu bleiben und Kosten vor allem im Bereich chronischer Krankheiten zu reduzieren. Ausblick: Ein Arzt sollte sich der Wirkung seiner Impf-Einstellung auf seine Patienten bewusst sein und bei seiner Impfberatung die Erwartungen und auch die Impfkritik seiner Patienten berücksichtigen. Die Komplexität und Vielseitigkeit des Impfthemas sollte den Impfentscheidern (Patienten) nicht vorenthalten werden sondern eine eigenverantwortliche Impfentscheidung (wie gesetzlich vorgesehen) fördern. Eine fundierte, neutrale und individualisierte Impf-Beratung sollte daher fester Bestandteil der medizinischen Ausbildung sein und in der Umsetzung auch entsprechend honoriert werden, unabhängig von der Impfentscheidung der Patienten. Damit können Verunsicherungen durch zT. emotionalisierende und polarisierende Informations-Medien (Internet, soziales Umfeld, Hebammen) vorgebeugt werden. Bei zunehmender Verbreitung von Komplementärmedizin und speziell der Homöopathie sollten das Thema Komplementärmedizin in der medizinischen Ausbildung fest integriert werden und entsprechend den Möglichkeiten zur Heilungsunterstützung in der Forschung und klinischen Anwendung den entsprechenden Stellenwert bekommen.
  • Background: The increasing criticism of vaccination in certain sections of the population is being linked to the increasing spread of complementary medicine. Homeopathy is one of the strongest representatives among the growing complementary medicine. Especially in paediatrics, where most vaccinations are carried out, homeopathy is increasingly used. A survey from 1996 showed for the first time that, contrary to public opinion, homeopathic doctors in Germany are not decisive opponents of vaccination, but take a more critical and individual approach to vaccination. Methods: In the present work, vaccination attitude and vaccination behaviour among registered paediatricians throughout Germany are investigated. Pediatricians with and without an additional qualification in homeopathy are compared. By means of a representative cross-sectional study, 50 % (N=3019) of all registered paediatricians from all over Germany were surveyed by post (random sample) on the subject of vaccination in 2006/2007. In addition, 245 homeopathically oriented paediatricians were included in the anonymized survey at the same time. The instrument was previously evaluated in a pilot study among 200 German pediatricians. The selection of the paediatricians was made via address lists of the Associations of Statutory Health Insurance for Physicians (KVn)and State Medical Board of Registration(ÄK) of everyfederal province of Germany, as well as additionally via the German Central Association for Homeopathic Doctors (DZVhÄ) and the Karl und Veronica Carstens-Stiftung (KVC- endowment) in order to obtain the largest possible comparison group of homeopathic paediatricians. The responses of the random drawing, the additional homeopathic paediatricians and the pilot study were pooled for evaluation. For a comparison, homeopathy was differentiated by means of 6 (partial) questions of the survey instrument paediatricians with and without a focus/additional designation. The latter were additionally differentiated into paediatricians who use homeopathy or another alternative/complementary medicine or who only use conventional medicine. To evalute whether the vaccination behaviour follows there comondations of the STIKO, the vaccination behaviour was operationalised as an average value score on the basis of 3 questions of the survey instrument:1.Do you vaccinate according to the recommended vaccination calendar of STIKO? 2. How would you vaccinate your own children? 3. At which age do you usually recommend the first vaccinations? In order to operationalize the vaccination setting, the scaled positions to 8 given opinions for vaccination were calculated to an average value score. The influence of the following relevant variables on vaccination settings and behaviour was investigated with a multivariable linear regression: Age, sex, place of establishment, proportion of private patients and additional title homeopathy. In order to compare the results of the 1996 study, a vaccination critic index was compiled. Results and discussion: With a total of 3464 registered paediatricians (over 50% of all registered paediatricians from each federal state), this is the largest nationwide study on vaccination to date. The response rate is above average with a total of approx. 67%, which suggests a great interest in the subject of vaccination. Against the background of a largely complete and up-to-date practice address database, this study can be regarded as representative throughout Germany. The results of this work are unique and meaningful despite slight methodological limitations in the construction of the sample. Paediatricians with and without the additional designation homeopathy differ significantly in terms of their vaccination attitude and vaccination behaviour (based on the recommendations of the STIKO. The predominantly positive but critical attitude towards vaccination among homeopathic paediatricians is similar to the results of P. Lehrke from 1996. With about 70%, the majority of homeopathic paediatricians in Germany have a positive attitude towards vaccination. But most of them are also individual critics of vaccination. The strongest criticism of vaccination can be found among purely classical homeopaths, which account for approx. 47 % of pediatrists with the additional designation homeopathy. Of all relevant variables, the additional designation homeopathy has the greatest influence on a vaccination-critical attitude and corresponding behaviour. However, deviations from the STIKO recommendations mainly concern later vaccinations (mainly 1-3 months), as well as newer vaccinations against Pneumococcus, Varicella and Meningococcus. The most frequent reason for deviations from the STIKO guidelines was the wish of the patients. Since parents critical of vaccination tend to choose paediatricians with additional complementary medical services, they also influence the experience and vaccination behaviour of the doctor. It can be assumed that the patient clientele systematically differs from homeopaths and non-homoeopaths. Different experiences from dealing with the unvaccinated and the possibilities of homeopathic treatments could explain the changes in the vaccination attitude towards more criticism of vaccination in the lives of homeopaths. Homoeopaths are by far the most frequent (over 60%) to report a change in their attitude to vaccination in their CV, non-homoeopaths in only about 10-22%. With regard to the patients' decision on vaccination, personal medical advice is of great importance as the most important person of trust. Experience and attitude influence each other. Based on the random sample, approximately 10% of all registered paediatricians in Germany have the additional designation of homeopathy. There are indications that the additional designation homeopathy has increased due to patient demand. This can explain the frequent use of homeopathy among paediatricians without additional designation homeopathy (approx. 40 %). In accordance with the market economy principle of the health system, homeopathy is increasingly being remunerated by the health insurance funds despite scientifically controversial studies, among other things in order to remain competitive and reduce costs, especially in the area of chronic diseases. Outlook: A physician should be aware of the effect of his vaccination attitude on his patients and take into account the expectations and also the vaccination criticism of his patients during his vaccination consultation. The complexity and versatility of the vaccination topic should not be withheld from the vaccination decision-makers (patients) but should promote a vaccination decision on their own responsibility (as provided for by law). A well-founded, neutral and individualized vaccination consultation should therefore be an integral part of medical training and should also be rewarded accordingly during implementation, irrespective of the patient's vaccination decision. In this way, uncertainties caused by emotionalising and polarising information media (internet, social environment, midwives) can be prevented. With the increasing spread of complementary medicine and especially homeopathy, the topic of complementary medicine should be firmly integrated into medical training and should be given the appropriate status in research and clinical application according to the possibilities for healing support.
