  • Purpose: Until today, no conclusive evaluation concerning the significance of EEG regarding the course of focal epilepsy has been performed. Therefore, we attempted to evaluate the prognostic relevance of the EEG in cryptogenic focal epilepsy. Methods: 32 patients with cryptogenic focal epilepsy, 16 of them pharmacoresistant, 16 free of seizures since the administration of anti-epileptic medication was begun, were examined prospectively from 1985-1992. We analysed 166 EEGs (ten-twenty), taken in intervals and including hyperventilation and photostimulation. The following factors were examined and statistically evaluated: backround activity, wave paroxysms, focal slowing and epileptiform discharges. The epileptiform discharges were measured semi-quantitatively regarding the duration and amplitude of the respective first peak potential. Results: 22 women and 10 men were examined. In the course of their affliction patients in both groups showed EEGs that were partly pathological, partly inconspicuous. Only the confrontation of all 166 EEGs and their analysis in the course of affliction showed statistical relationships between changes in the EEGs and the course of epilepsy. Significant: epileptiform discharges and their localization (p= .0001), duration (p= .003) and density (p= .0004) as well as the fact the patterns were influenced by hyperventilation. In patients that suffered seizures: short duration and high density of epileptiform discharges, localized on the right hemisphere and extratemporally, and appearing prominently in base EEGs. In seizure-free patients: longer duration and low density of epileptiform discharges, localized on the left hemishpere and temporally, with higher incidence under hyperventilation. Conclusion: For patients with cryptogenic focal epilepsy and normal EEG in the course of their epilepsy a prognosis cannot be predicted. When epileptiform discharges appear, their localization, amplitude, duration and density may lead to a prognosis.
