Zentrische Kondylenpositionsanalyse - Übersicht und klinische Bewertung nach Untersuchungen mittels computergestützter Messsysteme

  • Zusammenfassung Ziel: Die Bestimmung der zentrischen Kondylenposition ist ein wesentlicher Bestandteil bei verschiedenen komplexen zahnärztlichen Versorgungs-maßnahmen. Dazu werden in der wissenschaftlichen Literatur verschiedene Methoden beschrieben und in der vorliegenden Arbeit diskutiert. Hauptziele eigener Untersuchungen waren die Erfassung von möglichen Vorteilen und Grenzen entsprechender computergestützter Verfahren und die Bewertung möglicher Fehler in der Prozesskette von der Registrierung bis zur Inkorporation eines Therapiemittels. Dazu sollten die Untersuchungen dem Praxisalltag entsprechend an behandlungsbedürftigen Patienten durchgeführt und die Ergebnisse mit denen, aus Studien an stomatognath gesunden, vollbezahnten Probanden verglichen werden. Material und Methoden: Die Untersuchungen erfolgten in 2 Schritten (Teile A und B) an 21 Patienten. Bei allen Patienten stimmte die zentrische Relation nicht mit der habituellen Okklusion überein. Es wurde im Teil A eine relative, direkte, modellunabhängige Kondylenpostionsanalyse nach Bestimmung der zentrischen Kondylenposition mittels verschiedener Methoden durchgeführt. Die Messungen erfolgten mit dem Software Modul „EPA-Elektronische UK-Positionsanalyse der UK-Lage“ des ARCUSdigma II (Fa.KaVo, Biberach, Germany). Als Referenzregistrat wurde ein Zentrikregistrat auf Kunststoffplattenbasis nach unforcierter einhändiger Führung (modifiziert nach LAURITZEN) genutzt. Zum Vergleich dienten Registrate die mittels der Module: „Geführte Zentrik“ und „Gotischer Bogen“ des ARCUSdigma II und nach einer IPR-Vermessung hergestellt wurden. Im Teil B wurde die relative, direkte, modellunabhängige Kondylenpositionsanalyse nach laborseitiger identischer Umsetzung der Registrate und Herstellung von „Positionierungsschienen“ durchgeführt. Alle Messungen wurden dreimal mit gesteigerter Kaukraft wiederholt. Vorab wurde mit dem Modul „Elektromyographie“ des ARCUSdigma II der Muskeltonus überprüft und zur Patienteninstruktion genutzt. Die Messergebnisse wurden deskriptiv, grafisch, metrisch, tabellarisch und statistisch ausgewertet. Ergebnisse und Schlussfolgerungen: Im Ergebnis zeigten sich im Teil A der Untersuchungen Abweichungen zum Referenzregistrat nur bei der IPR-Vermessung in der transversalen Achse und am linken Kiefergelenk in der Vertikalen für die Messung mittels des Moduls “Gotischer Bogen“. Alle anderen Messungen zeigen statistisch eine symmetrische Verteilung. Für den Teil B zeigten sich signifikante Abweichungen bei jeder angewandten Methode an mindestens einem Kiefergelenk. Der Vergleich beider Untersuchungsreihen zueinander ergab in mindestens einer Raumachse signifikante Abweichungen bei allen überprüften Methoden. Es ist davon auszugehen, dass sich im Zuge der Prozesskette die Bedingungen zur Bewertung der zentrischen Kondylenposition ändern können. Diese sind als behandlungsrelevant anzusehen. Die verwendete Messmethodik lässt aber keine zuverlässigen Aussagen zur Reliabilität einzelner Verfahren zu. Vorteile computergestützter Methoden liegen u.a. in der Visualisierung, Echtzeitkontrolle und Kombination verschiedener diagnostischer und therapeutischer Module. Aufwand (Kosten, Zeit, Technik) und patientenindividuelle Bedingungen begrenzen die Möglichkeiten dieser Systeme. Entscheidend für den erfolgreichen Einsatz eines Verfahrens zur Bestimmung der zentrischen Kondylenposition sind neben subjektiven Faktoren der klinische Ausgangszustand und der Stand der erforderlichen Vorbehandlungen. Die direkte, modellunabhängige elektronische Kondylenpositionsanalyse unterstützt dazu die Qualitätssicherung bei der Bestimmung der zentrischen Kondylenposition.
  • Abstract Objective: The determination of the centric condylar positioning is an essential component of various complex dental treatments. The present dissertation discusses the various methods described in the scientific literature. Main objectives of the own examinations are the capturing of possible advantages and constraints of respective computer-based procedures and the assessment a possible faults in the process chain starting with the registration and up to the incorporation of a prosthetic restoration or a splint. Therefore the examinations should be compared between the everyday life practise of treatment-destitute patients carried out and the results of those from studies of healthy, fully dentate subjects. Material and methods: The examinations took place in 2 steps (parts A and B) with 21 patients. With all patients the centric relation did not conform to the habitual occlusion. A relative, direct centric condylar positioning analysis independent of model was carried out in the part A after a determination of the centric condylar positioning by means of different methods. The measurements were carried out with the software module „EPA - electronic position analysis of LJ position“ of the ARCUSdigma II (KaVo, Biberach, Germany). As a reference registration a centric registration on acrylic wafer was used after unforced one-handed guidance (modified after LAURITZEN). For a comparison registrations that served by means of the software modules: "hand-guided centric relation" and centric relation via „gothic arch“ of the ARCUSdigma II and after an IPR measurement were produced. In the part B the relative, direct condylar positioning analysis independent of models was carried out after lab-sided identical conversion of the registration and the manufacturing of "positioning splints”. All measurements were repeated three times with increased clenching force. Upfront the muscle tone was reviewed using the software modul "EMG analysis”" of the ARCUSdigma II and this review was used to instruct patients. The measuring results were evaluated descriptive, graphically, metrical, in tabular form and statistically. Results and conclusions: As a result variances appeared in part A of the examinations to the reference registration only with the IPR measurement in the transversal axis and in the left temporomandibular joint in the verticals for the measurement by means of the module “gothic arch“. All the other measurements show statistically a symmetrical distribution. For part B significant variances appeared with every applied method of at least one temporomandibular joint. The comparison of both examination series to each other proved significant variances with all reviewed methods in at least one space axis. It is to be assumed that in the course of the process chain the conditions can change for the assessment centric condylar positioning. These are to be considered as relevant for treatment. However, the used measuring methodology does not allow reliable statements to the reliability of single procedures. Major advantages of computer-based methods are inter alia the visualisation, real time controls and combination of different diagnostic and therapeutic modules. Expenditure (Cost, time, technology) and patient-individual conditions are limitative factors to these computer-based methods. Critical to the successful application of a procedure for the determination of the centric condylar positioning are in addition to subjective factors the initial clinical condition as well as the necessary pre-treatments. The direct electronic condylar positioning analysis independent of model supports the quality assurance with the determination of the centric condylar positioning.
