Deskription der postpartalen Sonomorphologie der Säuglingshüfte mit Analyse der Interobserver Variabilität in der Klassifizierung nach R. Graf

  • Die Hüftgelenksdysplasie wird seit Einführung der Hüftsonographie deutlich früher erkannt. Durch die sonographiegesteuerte Therapie ist sie bereits in den ersten Lebensmonaten gut behandelbar. Der Rückgang schwerer Dysplasieformen sowie eine Reduktion der Hüftoperationen im Kindesalter unterstreichen den Erfolg des generellen Screenings. In der hier vorliegenden retrospektiven, epidemiologischen Studie wurden die Ergebnisse der hüftsonographischen Untersuchungen an 288 Neugeborenen ausgewertet. Die Untersuchungen fanden im Zeitraum zwischen dem 01.06.2014 und 30.06.2015 statt und wurden in der ersten Lebenswoche, im Mittel am zweiten Lebenstag der Neugeborenen durchgeführt, sowie bei einem Teil der Säuglinge zur Verlaufskontrolle im Mittel nach 43 Tagen. Dysplastische Hüftveränderungen traten an der linken Hüfte doppelt so häufig wie an der rechten Hüfte auf. Bezüglich der Typisierung zeigte sich folgende Verteilung: (Typ Ia und Ib 34,1%, Typ IIa 61%, Typ IIc 2,1%, Typ D 2,3% und Typ IIIa 0,5%). Ein Anteil von 4,9% der Hüften wiesen den Hüfttyp IIc oder höher auf. Dies liegt über der für Deutschland angenommen Inzidenz von 2%-4%, entspricht aber der regionalen Inzidenz von ebenfalls 4,9%. Die Untersuchung der Interobserver-Varianzen, der gegenseitigen Kontrollen der Untersucher, wurde an den Erst- und Zweitsonogrammen durchgeführt und ergab eine hohe Korrelation und damit eine hohe Reliabilität. Die Ergebnisse der Kontrollen, die an den Erstsonogrammen durchgeführt wurden, wiesen eine höhere Korrelation auf, als jene, die an den Zweitsonogrammen erfolgten. Die Verlaufskontrollen zeigten eine rasche Befundverbesserung. Es konnte nachgewiesen werden, dass die tägliche Veränderung des α-Winkels unter Behandlung mittels Tübinger Hüftbeugeschiene größer war, als ohne Schienentherapie. Der Gesamtwinkel, die Summe aus α- und β-Winkel, blieb unter Ausreifung des Hüftgelenks konstant. Es kam jedoch zu einer Verlagerung der Grundlinie unter Therapie und konsekutiv zur Änderung der α- und β-Winkel zu Gunsten des α-Winkels. Für die richtige Winkelbestimmung und Typisierung ist die korrekte Einzeichnung der Grundlinie immens wichtig. Fehlerhaftes Positionieren der Grundlinie führt in den meisten Fällen zu einer positiveren Typisierung als es der reale pathologische Befund hergibt. Behandlungsbedürftige Hüftbefunde werden dadurch unter Umständen nicht erkannt. Aufgrund der hier ausgewerteten Befunde zeigt sich, dass ein zweizeitiges generelles Screening erforderlich ist, um alle Hüftgelenksdysplasien einer optimalen Therapie zuzuführen. Der erste Screeningzeitpunkt in der ersten Lebenswoche ermöglicht die frühzeitige typengerechte Therapie und nutzt das volle Wachstumspotential des Neu-geborenen in den ersten Lebenswochen. Dies verkürzt die Behandlungsdauer und reduziert die Anzahl schwerer Hüftdysplasien. Der zweite Zeitpunkt des generellen Screenings sollte zur U3 erfolgen, um initial kontrollbedürftige Hüftbefunde zu erfassen, Verschlechterungen der initial normalen Hüftbefunde zu erkennen und diese einer Behandlung zuzuführen. So würden einerseits Restdysplasien durch verspäteten Therapiebeginn vermindert und andererseits einer Übertherapie entgegengewirkt.
  • Since the general neonatal ultrasound hip screening has been implemented in Germany the developmental displacement of the hip is identified much earlier in lifetime. By the use of ultrasound guided therapy an effective treatment could be already realized during the first months of life. The decrease of the severe types of hip dysplasia and the reduction of the necessary surgical therapy during infancy emphasize the importance of the general neonatal hip ultrasound screening. The following epidemiological study evaluated retrospectively the results of the ultrasound hip screenings from 288 newborns performed between 01.06.2014 and 30.06.2015. All neonates were examined within the first week of life, on average on the second day of life. A section of the newborns received a follow-up sonographic check on average on the 43th day of life. The incidence of dysplasia on the left hip was twice as high as on the right hip. According to Reinhard Graf the classification of the hip survey resulted in: 34.1% type Ia and Ib, 61% type IIa, 2.1% type IIc, 2.3% type D and 0.5% type IIIa. Therefore, 4.9 % of the inspected hips showed a hip dysplasia of type IIc or more severe. This outcome was above the usual incidence in Germany (2% -4%), but corresponded to the regional incidence of 4.9%. The interobserver variability was determined by reciprocal control of the examiners within the first and second sonogram. As a result, consistent correlation was validated and consequently a high level of reliability was verified. The controls of the first sonograms assessed a higher value of correlation than the controls of the second sonograms. The outcome of follow up study revealed a rapid improvement of the hip dysplasia. The results proved that the daily alteration of the α-angle increased more under treatment by Tübinger Hüftbeugeschiene than without treatment. The total angle, the summation of the α- and β-angle, remained constant while during the maturation of the hip. However, the base line shifted under treatment and as a consequence the α- and β-angle varied whereby the α-angle increased particularly. Therefore, to mark the baseline on the accurate position is mandatory for determining the angle properly and classifying the type of dysplasia exactly. Additionally, an incorrect position of the baseline leads most commonly to a less severe classification of the hip than the actual results of the pathological analysis. Accordingly, hips demanding treatment could possibly not be identified under these circumstances. Based on these results it is evident that a dual sonographic hip screening is required in general to accomplish an optimal treatment for all kinds of hip dysplasia. Inspecting the hip during the first week of life facilitates an early treatment adjusted to the respective type of dysplasia. Because of the early point in time, the first screening is able to utilized the entire growth potential of the newborn. These preconditions reduce the duration of treatment and decrease of the total number of severe hip dysplasia. Between the fourth and sixth week of life the second screening of the hip should be performed corresponding to the regular examination “U3”. This follow up check enables to identify hips which need to be controlled regularly. Moreover, a decline of a hip, initially evaluated as regular, can be detected soon and treated appropriately. Therefore, the amount of permanent dysplasia caused by delayed initiation of therapy would decrease on the one hand; on the other hand, the dual sonographic hip screening is to counteract overtherapy.
