Der Einfluss von Antibiotika und Antikoagulantien auf die Gefäßkontraktilität In Vitro -eine experimentelle Studie

  • Patienten mit Sepsis haben eine schlechte Mikrozirkulation sowie kardiovaskuläre Depression. Zur Therapie müssen unbedingt und schnellstmöglich Antibiotika gegeben werden, die oft kombiniert wird mit Antikoagulantien-Therapie. Nun stellt sich die Frage, welchen direkten Effekt Antibiotika und Antikoagulantien auf den Gefäßtonus beim septischen Patienten haben. Als ersten Schritt dieser Forschung wurden an gesunden Ratten-Aortenringen Antibiotika und Antikoagulantien durch Messung der isometrischen Kontraktion in vitro getestet. Alle Medikamente wurden hinsichtlich ihrer Relaxationsfähigkeit bei Vorkontraktion mit 20/40 mM KCl oder Phenylephrin (5x10-7M oder 5x10-8M) getestet, sowie die Phenylephrin Dosis-Wirkungs-Kurve nach 30 Minuten bzw. 20 Stunden Inkubation (bei 37°C) konstruiert. Zudem wurde die Wirkung des Medikaments in hohen Konzentrationen auf den Ruhezustand der Ringe erprobt. Bei den Antibiotika konnte festgestellt werden, dass Linezolid möglicherweise ein K+-Kanal Aktivator ist, da es bei einer Präkontraktion von 40mM KCl und darauffolgender kumulativer Gabe von Linezolid signifikant weniger stark relaxiert, als bei 20mM KCl Vorkontraktion (P=0,02). Weiterhin könnten sowohl Linezolid als auch Daptomycin intrazelluläre Mechanismen beeinflussen, da sie bei der Inkubation von 30 Minuten (pEC50: 7.47±0.06(Lin)/7.45±0.08(Dap) vs 6.96±0.05(Kon)) und 20 Stunden (pEC50: 7.2±0.04 (Lin)/ 7.45±0.04 (Dap) vs 6.89±0.05(Kon)) und folgender Phenylephrin-Dosis-Wirkungs-Kurve beide eine Linksverschiebung der Kurve im Vergleich zur Kontrolle bewirken (P<0,05). Zudem ist ihre erreichte Maximalspannung in beiden Versuchsreihen signifikant höher als die der Kontrolle. Bei den Antikoagulantien wurde ermittelt, dass bei Enoxaparin (P=0,00002), Tinzaparin (P=0,0002) und Danaparoid (P=0,005) möglicherweise eine K+-Kanal Aktivierung vorliegen könnte, da sie alle bei 20mM KCl Vorkontraktion stärker relaxieren als bei 40mM KCl Präkontraktion. Zudem konnte bei Tinzaparin (P=0,002) und Enoxaparin (P=0,01) nach Vorkontraktion mit Phenylephrin eine signifikante Relaxation im Vergleich zur Kontrolle festgestellt werden, was eventuell hinweist auf eine α-adrenerge Stimulation. Nach 30 Minuten Inkubation fanden wir heraus, dass Argatroban und Danaparoid eine Linksverschiebung der Phenylephrin-Dosis-Wirkungs-Kurve gegenüber der Kontrolle bewirken (pEC50: 7.17±0.08(Arg)/7.12±0.05(Dan) vs 6.96±0.05(Kon)), während Enoxaparin (pEC50: 6.77±0.03) eine Rechtsverschiebung zeigte (bei allen P<0,05). Bei der 20-stündigen Inkubation hingegen zeigten alle Antikoagulantien im Vergleich zur Kontrolle eine erhöhte Sensitivität gegenüber Phenylephrin (pEC50: 7.37±0.05(Arg)/7.13±0.04(Dan)/7.1±0.05(Eno)/ 7.06±0.04(Tin) vs 6.89±0.05(Kon)). Möglicherweise beeinflussen sie also intrazelluläre Mechanismen. Die Maximalspannung war, im Vergleich zur Kontrolle, bei Enoxaparin und Argatroban erhöht. Diese Ergebnisse deuten darauf hin, dass einige der getesteten Substanzen möglicherweise eine K+-Kanal Aktivierung, eine α-adrenerge Stimulation oder eine intrazelluläre Modulation bewirken. Ob diese Ergebnisse eine Bedeutung für den Menschen haben und ob sie bedeutsam bei Sepsis sind, muss noch untersucht werden.
  • Patients with sepsis have a poor micro-circulation and cardiovascular depression. For the therapy must necessarily be given antibiotics as soon as possible, often combined with anticoagulant therapy. Now is the question which direct effect some antibiotics and anticoagulants have on vascular tone in septic patients. As a first step of this research we investigated the effect of antibiotics and anticoagulants in healthy rat aortic rings tested by measuring the isometric contraction in vitro. All drugs were tested for their relaxation at precontraction with 20/40 mM KCl or phenylephrine (5x10-7 or 5x10-8M) and the phenylephrine dose-response curve after 30 minutes and 20 hours of incubation (at 37 ° C) was designed . In addition, the effect of the drugs in high concentrations on the resting rings was tested. Within the antibiotics it was found that linezolid may be a K + channel activator, as it relaxes in a 40 mM KCl precontraction with cumulative administration of linezolid significantly less than with 20 mM KCl precontraction (P = 0.02). Both linezolid and daptomycin could affect intracellular mechanisms, as they during the incubation of 30 minutes (pEC50: 7.47 ± 0.06 (Lin) / 7.45 ± 0.08 (Dap) vs. 6.96 ± 0.05 (Con)) or 20 hours (pEC50: 7.2 ± 0.04 (Lin) / 7:45 ± 0.04 (Dap) vs. 6.89 ± 0.05 (Con)) and the following phenylephrine dose-response curve both showed a leftward shift of the curve compared to the effect to the control (P <0.05). Also their reached maximum tesion in both trials was significantly higher than that of the control. At the anticoagulants it was found that enoxaparin (P = 0.00002), tinzaparin (P = 0.0002) and danaparoid (P = 0.005) may possibly give a K + channel activation, as they all relax more at 20 mM KCl precontraction than in 40 mM KCl precontraction. Furthermore, it was determined that in tinzaparin (P = 0.002) and enoxaparin (P = 0.01) after precontraction with phenylephrine was shown a significant relaxation compared to the control, suggesting a possible α-adrenergic stimulation. After 30 minutes of incubation, we found that the effect of argatroban and danaparoid showed a leftward shift of the phenylephrine dose-response curve compared with the control (pEC50: 7.17 ± 0.08 (Arg) / 7.12 ± 0.05 (Dan) vs 6.96 ± 0.05 (Con)) while enoxaparin (pEC50: 6.77 ± 0.03) showed a right shift (all P <0.05). In the 20-hour incubation, however, all anticoagulants showed, compared to control, an increased sensitivity to phenylephrine (pEC50: 7.37 ± 0.05 (Arg) / 7.13 ± 0.04 (Dan) / 7.1 ± 0.05 (Eno) / 7.06 ± 0.04 (Tin) vs. 6.89 ± 0.05 (Con)). They may therefore affect intracellular mechanisms. The maximum tension was increased compared to control, in enoxaparin and argatroban. These results suggest that some of the substances tested may cause a K + channel activation, α-adrenergic stimulation or intracellular modulation. Whether these results have relevance to humans and whether they are important in sepsis, needs to be investigated.
