Leberfettquantifizierung mit Hilfe der Proton-Density-Fett-Fraktion im Hochfeld-MRT

  • Zielstellung: In dieser Machbarkeitsstudie soll gezeigt werden, dass die Leberfettquantifizierung am 7 Tesla-MRT durch Bestimmung der Proton-Density Fettfraktion (PDFF) möglich ist. Zusätzlich werden zwei Rekonstruktionsverfahren zur Berechnung der PDFF verglichen. Material und Methoden: In einem 7 Tesla Kleintier-MRT wurde die PDFF von 15 leptindefizienten ob/ob-Mäusen und 8 Kontrolltieren bestimmt. Die PDFF wurde mit zwei anerkannten Rekonstruktionsverfahren, der magnitudenbasierten Rekonstruktion und der komplexen Rekonstruktionstechnik bestimmt. Die ermittelten Fettgehalte beider Rekonstruktionen wurden miteinander verglichen (Passing- Bablok-Regressionsanalyse und Bland-Altman-Analyse) und dem Goldstandard, der Histopathologie, gegenübergestellt (lineare Regression). Ergebnisse: Die PDFFs beider Rekonstruktionsverfahren lieferten unterschiedliche Ergebnisse (mittlere Abweichung 5,7 ± 3,82%). Visuell waren die PDFF-Karten der komplexen Daten durch störende Artefakte überlagert, welche möglicherweise zu einer Fehleinschätzung des Fettgehaltes führen. Es besteht ein linearer Zusammenhang zwischen der PDFF und der Histopathologie für die magnitudenbasierte Rekonstruktion (R2= 0,806) und auch für die komplexe Rekonstruktion (R2= 0,770). Schlussfolgerungen: Die Leberfettquantifizierung ist mit Bestimmung der PDFF am 7 Tesla Hochfeld-MRT möglich. Die PDFF berechnet aus den Magnitudendaten ist der PDFF berechnet aus den komplexen Daten vorzuziehen, da diese subjektiv weniger anfällig für Artefakte ist.
  • Purpose: Purpose of this study was to investigate the feasibility of assessing the proton-density fat fraction (PDFF) in a 7 Tesla MR system and in addition to compare the accuracy of liver fat quantification of two approved techniques ( complex-based and magnitude-based fitting) in this high-field MR system. Material and methods: In a 7 Tesla animal scanner 15 leptin-deficient ob/ob mice and 8 controls were examined and PDFF was calculated using the complex and the magnitude based fitting technique. Differences between the techniques were compared using Passing-Bablok regression analysis and Bland-Altman analysis. Linear regression was used for comparison with the clinical gold standard: histopathology. Results: The mean bias between the two techniques demonstrated divergent results (5.7% ± 3.82%). Subjectively there were artifacts to be seen in the PDFF-maps from the complex fitting leading to a possible misinterpretation of the fat content. A linear correlation can be shown for both techniques with histopathology (magnitude based : R2=0.806 ; complex based: R2=0.770). Conclusion: Liver fat quantification using the PDFF in a high field MR system is principally feasible. The magnitude based PDFF appears to be more accurate as it is less prone to artifacts.
