Objektivierung von Effekten der stationären minimalinvasiven Wirbelsäulentherapie - Eine Korrelationsanalyse von klinischen und sonographischen Daten

  • Diese prospektiv durchgeführte Arbeit beschäftigt sich mit der sonographischen Darstellung des Pennation Angles (PA) des lumbalen Musculus multifidus (LMM) bei Patienten mit chronischem unspezifischem Rückenleiden und dessen Veränderungen im Rahmen des stationären minimalinvasiven Wirbelsäulentherapie Regimes (sMIWT) wie es von 2011 bis 2013 in der Klinik für Orthopädie und orthopädischer Chirurgie der Universitätsmedizin Greifswald durchgeführt wurde. In einer vorausgehenden Arbeit der Arbeitsgruppe konnte die Machbarkeit, Inter- sowie Intraraterreliabilität der Methode gezeigt werden. Insgesamt wurden 52 Patienten und 25 Mitglieder einer Vergleichsgruppe in die Studie eingeschlossen. Der PA wurde als Mittel von drei verschiedenen -Winkeln zwischen dem oberflächlichen Blatt der Fascia thoracolumbalis und der Muskelfiederung des LMM definiert. Es erfolgten insgesamt fünf sonografische Messungen des Ansatzwinkels mit Hilfe eines paravertebralen Longitudinalschnittes auf Höhe LWK 4. Die erste vor Beginn, folgende Messungen im Anschluss an die aus täglichen epiduralen bzw. facettengelenksnahen Infiltrationen mit Lokalanästhetika mit und ohne Zusatz von Corticosteroiden, intravenöser Analgesie und Physiotherapeutischen Maßnahmen bestehende Behandlung. Ebenfalls wurde zu jedem Zeitpunkt der Number-Associated-Score (NAS) erhoben sowie an Aufnahme und Entlassungstagen der Ostwestry-Disability-Index (ODI). Durchschnittlich nach 56 Tage erfolgte eine telefonische Reevaluation des ODI und NAS. Es konnte ein signifikanter Unterschied (p= 0,009) des PA zwischen Aufnahmetag der Patienten (167,17° 1,41°) und der rückengesunden Kontrollgruppe (168,8° 2,51°) in der Sonographie gezeigt werden. Im Verlauf der Behandlung erfolgte eine signifikante Veränderung des Ansatzwinkels auf 169,77° (± 1,96°). Der NAS verbesserte sich im Verlauf der Behandlung signifikant von einem Median von 6 1,84 bzw. auf einen Median von 2 1,88 Punkten sowie der ODI sich von 40,76 16,9 auf 26,42 16,54 Punkten verbesserte. Damit haben NAS und PA einen Korrelationskoeffizienten von -0,506 bei einem p-Wert von < 0,001. Zum Zeitpunkt der Reevaluation waren die Werte für ODI und NAS auf ihr Ausgangsniveau zurückgekehrt. Es ist nach dieser Arbeit nicht möglich Rückschlüsse anhand des PAs auf Lokalisation (Seite, Ausstrahlung) und Ausprägung der Schmerzen zu schließen. Im Studienvergleich erreicht das untersuchte sMIWT im Bezug auf NAS und ODI kurz und mittelfristig vergleichbare Werte. Die sonographisch ermittelten Winkeldifferenzen zwischen Patienten mit Rückenleiden zu Probanden ohne Beschwerden entsprechen ebenfalls Daten aus vorangegangenen Publikationen. Darüber hinaus konnte gezeigt werden, dass Rückenschmerzreduktion anhand des Pennation Angles des LMM in der Sonographie gut darstellbar ist und Schmerzveränderung mit PA-Veränderungen im Ultraschallbild korrelieren. Somit ist die sonographische Messung des PAs des LMM eine geeignete Methode die Therapieeffekte des sMIWT darzustellen.
  • Purpose: The purpose of this study was to describe, analyze, and compare ultrasonographic imaging of the pennation angle (PA) of the lumbar multifidus muscle (LMM) on patients with and without low back pain (LBP). Furthermore, changes of the PA of inpatient minimally invasive spine therapy (iMIWT) were examined. Methods: Over a period of eleven months fifty-two patients with an iMIWT were included. As a control group twenty-five patients scheduled for a total knee replacement (TKR) were used. The PA was measured sonographically with a paraspinal view at the level of lumbar vertebra four between the superficial portion of the fascia thoracolumbalis and the muscle fibres of the LMM. The PA was defined as the mean of three fibre angles for each side. The measurements were executed by two experienced examiners. The first investigation was performed one day before starting with iMIWT followed by daily ultrasonographic control during the four days of iMIWT. All measurements were performed 3 hours after facet joint or epidural infiltration (with or without supplement of corticosteroids) and at the end of physical therapy. Patients were grouped according to the therapeutic regime and the number of infiltrations. Number associated score (NAS) was prompted on each day. Additionally, the clinical outcome score Ostwestry-Disability-Index (ODI) was obtained at day one and five. These scores were re-evaluated at a mean of 56 days after discharge via a telefone interview. A feasibility and an intra-/interobserver study were performend before starting the clinical investigation. Results: Patients with LBP (n=56) had a mean PA of 167,17° ± 1,41° at the time of admission to the hospital. The PA decreased significantly to 169,77° ± 1,96° after five days of iMIWT. The average PA of the control group was statistically significant higher at day one 168,8° ± 2,51° compared to the study group. NAS and ODI improved significantly during the inpatient treatment (median NAS 6 ± 1,84 versus 2 ± 1,88; median ODI 40,76 ± 16,9 versus 26,42 ± 16,54). NAS and PA were negatively correlated (R = - 0,506). At a mean of 56 days after treatment NAS and ODI values returned to the level before facet joint infiltration. Subgroup analysis did not reveal any significant differences. Further, ultrasonographic findings did not correlate with pain location and characteristics. Conclusions: This is the first study which observed the differences in PA and clinical outcome scores of iMIWT over time. It has been demonstrated that reduction of low back pain correlates with changes of PA of the LMM. Ultrasonographic imaging is a reliable method to quantify these changes. Thus sonographic measurements are a convenient tool to evaluate the therapeutic effect of iMIWT.
