Diagnostisches und therapeutisches Procedere der Choledocholithiasis im Rahmen der elektiven Gallenblasenchirurgie

  • Die präoperative ERC ist ein anerkanntes diagnostisches und therapeutisches Verfahren, wenn der Verdacht einer Choledocholithiasis besteht. Der Verdacht auf das Vorliegen einer Choledocholithiasis ergibt sich sowohl aus der klinischen Symptomatik als auch aus den sonographischen und laborchemischen Befunden. In der vorliegenden Arbeit wurden 11,8 Prozent der 298 Patienten präoperativ einer ERC unterzogen (n=36). Von diesen 36 Patienten wiesen 27 Patienten eine Choledocholithiasis auf. Die Rate der diagnostischen ERC betrug 25 Prozent. Bei radiologisch bewiesener Choledocholithiasis ist die ERC und Konkrementextraktion im Rahmen des therapeutischen Splittings Mittel der Wahl. Aus der Analyse der Daten und im Abgleich mit der Literatur ergibt sich folgendes: Gallengangssteine können nur durch aufwendige Verfahren bewiesen werden. Der Verdacht auf das Vorliegen einer Choledocholithiasis ergibt sich aus anamnestischen Daten, Alter und Geschlecht der Patienten, der Elevation der sogenannten Cholestaseparameter in Verbindung mit der sonografisch nachgewiesenen Dilatation des Ductus choledochus und wenn möglich dem direkten sonographischen Steinnachweis. Die Erhöhung der Vorhersagewahrscheinlichkeit kann durch die Kombination der verschiedenen pathologischen Befunde erreicht werden. Dennoch besteht die Gefahr, eine asymptomatische Choleocholithiasis zu übersehen. Dieser Gefahr kann durch die Anwendung der intraoperativen Cholangiographie begegnet werden. Stellen sich bei dieser Untersuchung Gallengangsteine dar, können diese im Rahmen einer postoperativen ERC bestätigt und entfernt werden. Diagnostizierte Gallengangssteine sind auch asymptomatisch eine Behandlungsindikation. Die Therapie sollte so minimal-invasiv wie möglich erfolgen
  • The praeoperative ERC is a recognized diagnostic and therapeutic procedure, if the suspicion of a choledocholithiasis exists. The suspicion on being present a common bile duct stones results both from the clinical symptomatology and from the ultrasonic and laboratory-chemical findings. In the available work 11.8 per cent of the 298 patients praeoperativ were submitted of a ERC (n=36). From these 36 patients 27 patients exhibited stones in the common bile duct. The rate of the diagnostic ERC amounted to 25 per cent. With radiologically proven common bile duct stones is the ERC and removal of stones in the context of the therapeutic splitting means of the choice. From the analysis of the data and in the alignment with the literature the following results: Bile duct stones can be proven only by complex procedures. The suspicion on being present a choledocholithiasis results from anamnestischen data, age and sex of the patients, the elevation of the laboratory-chemical findings in connection with the sonografisch proven dilatation of the ductus choledochus and if possible the direct ultrasonic stone proof. The increase of the probability of forecast can be achieved by the combination of the different pathological findings. The danger exists nevertheless to survey a asymptomatische choleocholithiasis. Can be met to this danger by the application of the intraoperativen cholangiographie. Places itself when this investigation bile duct stones, can be confirmed and removed these in the context of an postoperativ ERC. Diagnosed bile duct stones are also asymptomatisch a treatment indication. The therapy should take place as minimal invasiv as possible
