Untersuchung des Knochenstoffwechsels mittels biochemischer Formations- und Resorptionsmarker bei Sporttauchern

  • Bei Berufstauchern und anderen in Überdruckatmosphäre arbeitenden Berufsgruppen stellen dysbarische Osteonekrosen eine ernst zu nehmende Komplikation dar. Es liegen Untersuchungen vor, die belegen, dass es auch innerhalb der Gruppe der Sporttaucher zum Auftreten dieser Komplikation kommt. Sporttauchen ist als Freizeittauchen ohne Dekompressionsphasen definiert (Nullzeittauchen). Bei 15 Probanden im Alter von 21 - 48 Jahren wurde der Knochenstoffwechsel während eines fünftägigen Sporttauchurlaubes, mittels biochemischer Knochenmarker analysiert. Die circadiane Serumkonzentration des carboxyterminalen Telopeptids des Typ-I-Kollagens zeigte einen signifikanten Anstieg (p = 0,0001) um 100% nach einem stattgefundenen Tauchgang. Die Analyse des Pyridinium- Quervernetzungspeptid (PYD) in proteingebundener und freier Form im Serum bestätigte dieses Ergebnis ebenfalls signifikant. Die circadiane Konzentration des carboxyterminalen Prokollagen Typ-I-Propeptids (CICP) zeigte rund 20 h nach absolvierten Tauchgängen ein signifikantesAbsinken um 29% (p = 0,04). Die Analyse der knochenspezifischen alkalischen Phosphatase (BAP) im Serum ergab ein nicht signifikantes Absinken um 16%. Weder die Konzentrationsverläufe der Serummarker, noch die der CrossLaps (U-CL/Cr) sowie der Pyridinium crosslinks, Pyridinoline (U-PYD/Cr) und Deoxypyridinoline (U-DPD/Cr) im Urin zeigten ein akkumulierendes Verhalten während der Testperiode. Aufgrund der gewonnenen Daten ist auch bei moderatem Nullzeittauchen mit einem deutlich gesteigerten Knochenabbau und einer Reduktion des Knochenaufbaus zu rechnen. Um das natürliche osteogene Potential zu fördern und damit einem durch das Tauchen ausgelösten Knochenmassenverlust entgegen zu wirken, wird für Tauchsportler ein regelmäßiges Krafttraining empfohlen.
  • Dysbaric osteonecrosis is a serious complication in compressed air work and in commercial diving. Studies can be found that give evidence, that recreational divers also can be affected by this complication. Recreational scuba diving is defined as pleasure diving to a depth of 130 feet without decompression stops. In this study, the bone turnover of 15 scuba divers, aged 21 - 48 years, was investigated during 5 days of recreational diving by using biochemical markers of bone turnover. The circadian serum concentration of C-terminal telopeptide of type I collagen (CTx) showed an immediate and signifcant (p = 0,0001) rise of 100%, right after a diving session. Serum concentrations of free and peptide-bound pyridinoline (Pyr) significantly confirmed this result. The circadian concentration of the carboxyterminal propeptide of type I procollagen (CICP) in serum showed a significant decrease of 29% (p = 0,04), approximately 20 h after a diving session. Bone alkaline phosphatase (BAP) concentration in serum showed a non significant decrease of 16%. Neither the concentration of the serum markers, nor the urinary CrossLaps (U-CL/Cr) and urinary pyridinium crosslinks, pyridinoline (PYD) and deoxypyridinoline (DPD), showed signs of accumulation over the testing period. The results suggest that recreational diving too does affect bone metabolism, leading to an increase of bone resorption and to a reduction of bone formation. Therefore weight training on regular bases is recommended for recreational divers, supporting the natural osteoplastic potential in order to limit the bone mass loss due to diving.
