Nachweis von Myelomzellen durch CDR III-klonspezifische real-time-PCR

  • Im Rahmen dieser Arbeit wurde zum Nachweis von Tumorzellen beim multiplen Myelom eine PCR-Methode getestet, die mit Hilfe der tumorklonspezifischen rekombinierten Immunglobulinsequenzen eine hochspezifische Gensequenz als Nachweisziel hat. Diese Methode kann zur Verlaufskontrolle, zur Therapieevaluation und zur Tumornachsorge genutzt werden. Zur Identifikation des Klons wurden VH-Familien-spezifische Primer für die höher konservierten Bereiche der Leader- und FR3 -Region sowie für die JH- und C-Region verwendet. Dieser PCR-Ansatz wurde im Vorfeld bei Patienten mit CLL bereits etabliert. Insgesamt konnte bei 20 von 26 untersuchten Myelompatienten das VDJ-Genrearrangement bestimmt und der jeweils beteiligten VH-Familie zugeordnet werden. Wegen der vermutlich zu stark mutierten Gensequenz war bei sechs Patienten die Identifikation des Myelomklons nicht möglich. Von 18 Myelomklonen wurden 17 erfolgreich sequenziert und analysiert. Bei einem Klon konnte trotz mehrerer Versuche keine eindeutige Sequenz bestimmt werden. Eine eindeutige Festlegung des beteiligten VH-Gens war aufgrund gleichrangiger Homologie im Genbankvergleich bei sieben Patienten nicht möglich. In den Sequenzanalysen wurde mit einer durchschnittlichen Mutationsrate von 0,092 eine hohe Anzahl somatischer Mutationen nachgewiesen, wodurch der Anteil der nicht analysierbaren Myelomklone erklärt werden kann. Nach Synthese und Kontrolle der Tumorklon-spezifischen ASO-Primer sowie Optimierung der Taqman-Sonden wurden bei zwei klinisch interessanten Patienten Verlaufskontrollen durchgeführt, bei denen sowohl Knochenmark als auch Blut untersucht wurde.
  • Proof of myeloma cells using a CDRIII-clone specific real-time-PCR Within this thesis a PCR method was tested to identify tumour cells for multiple myeloma. The method used tumour-clone specific recombined immunoglobulin-sequences in order to identify a highly specific genetic sequence. This method can be used as follow-up, to evaluate the therapy and for the after-treatment care. In order to identify the clone, VF-family-specific primers were used for the higher conserved sections of the leader- and FR3-region as well as the JH- and C-region. This PCR setting had been previously established for patient with CLL. In total the VDJ-genetic rearrangement could be determined and matched to the according VH-family in 20 of 26 myeloma patients. For 6 patients the identification of the myeloma clone was not possible due to a presumably extensively mutated genetic sequence. 17 of 18 myeloma clones were successfully sequenced and analysed. For one clone no exact sequence could be found, despite numerous tries. A clear record of the VH-gene was not possible for 7 patients due to similar homology in the comparison of genetic libraries. In the analysis of the sequences an average mutation rate of 0,092 showed a high number of somatic mutations, thus explaining the fraction of not analysable myeloma-clones. After synthesis and control of the tumour-clone specific ASO-primers as well as optimizing the Taqman-probe follow-ups were done for 2 clinically interesting patients during which both the bone marrow as well as the blood were examined.
