Einfluß der Applikation von unterschiedlich lange gelagerten Erythrozytenkonzentraten auf die Mikrozirkulation und den Sauerstoffverbrauch bei Patienten im septischen Schock

  • Die Störungen des Sauerstofftransports und die daraus resultierenden Oxygenierungsstörungen der Gewebe mit Entwicklung von Organdysfunktionen bis hin zum Organversagen sind in der Sepsis die Hauptursachen für die hohe Mortalität. Zum Therapieziel gehört daher die Stabilisierung der Hämodynamik und das Aufrechterhalten bzw. Wiederherstellen eines suffizienten Sauerstofftransports. In diesem Behandlungsregime nimmt die Bluttransfusion einen festen Platz ein. Ihre Effektivität wird jedoch möglicherweise stark von der Qualität der Erythrozyten beeinflusst. Die Ursache liegt in der Abnahme der Funktionsfähigkeit der Erythrozyten im Laufe der Lagerung der Blutkonserven. Ziel der vorliegenden Pilotstudie war es daher, den Effekt der Lagerungsdauer von Erythrozytenkonzentraten anhand der Messung systemischer Sauerstofftransport-parameter als auch regionaler Parameter der Gewebeoxygenierung zu beurteilen. Zu diesem Zweck wurden 13 Patienten doppelblind in zwei Behandlungsgruppen randomisiert. Den Patienten der Gruppe 1 wurden lang gelagerte Erythrozyten-konzentraten (> 28 – 35 Tage), den Patienten der Gruppe 2 kurz gelagerte Erythrozyten-konzentrate (< 5 Tage) ab einem Hämoglobinwert von 7,0 mmol/l (11,3 g/dl) transfundiert. Vor Beginn der Transfusion, unmittelbar nach deren Beendigung und bis 7 Stunden danach wurden zu festgelegten Messpunkten Hämoglobin und Hämatokrit, hämodynamische Parameter (HF, MAP, CI, ZVD, PCWP), systemische Sauerstofftransportparameter (DO2, VO2, SmvO2), die Laktatkonzentration und tonometrische Daten (pHi, CO2gap) bestimmt. Die Bluttransfusion erreichte eine Steigerung der Hämoglobinkonzentration, des Hämatokrits und der DO2 in beiden Behandlungsgruppen. Der Verlauf des VO2 konnte jedoch nicht beeinflusst werden. Dies scheint Indiz dafür, dass sich die Patienten trotz erhöhter Laktatwerte und eines hohen SOFAscores nicht in einer oxygen-supply-dependency befanden. Das Alter der transfundierten Erythrozyten-konzentrate kann möglicherweise einen Einfluß auf die Oxygenierung im Splanchnikusgebiet ausüben. Diese Hypothese konnte die vorliegende Pilotstudie aufgrund der stark streuenden Werte des kleinen Patientenkollektivs jedoch nicht bestätigen.
  • The main reasons for the high mortality in septic patients are the disturbances in oxygen transport and in reason of that the impairment of the tissue oxygenation with the development of organ dysfunction. Therefore is the stabilisation of the hemodynamic and the restitution and the maintenance of a sufficient oxygen transport an important aim of the therapy. In this therapy concept has the transfusion of red blood cells (RBC) a stable place. But likely their effectiveness is depended from the quality of the RBC’s. This is reasoned in the impairment of structurally and functionally changes of the RBC during the storage. The aim of this pilot study was to judge the influence of the storage time of RBC’s with the measuring of systemic parameter for the oxygen transport and measuring of regional parameter for the tissue oxygenation. Hence, 13 patients were randomize double-blind in 2 therapy groups. The patients of the therapy group 1 were transfused with long storaged RBC (>28-35 days), the patients of the group 2 were transfused with short storaged RBC (< 5 days) when the concentration of hemoglobin was < 7,0 mmol/l (11,3 g/dl). Before the beginning, immediately after the transfusion and till 7 hours later were measured hemoglobin and hematocrit, hemodynamic parameter (HF, MAP, CI, ZVD, PCWP), parameter for the systemic oxygen transport (DO2, VO2, SmvO2), lactate, pHi and CO2gap. The transfusion was able to increase the concentration of hemoglobin, of the hematocrit and of DO2 in both therapy groups. But there were no influence of the VO2 during the whole investigation time. This should be a sign for the absence of a oxygen-supply-dependency. The hypothese that the age of transfused RBC is able to influence the tissue oxygenation we couldn’t prove.
