Hirnentwicklung Frühgeborener: Einfluss peripartaler Pathologien

  • Ziel: Beurteilung der Hirnreifung bei Frühgeborenen am Termin mittels MRT Bildgebung und Korrelation zu peripartalen pathologischen Einflüssen. Methode: Auswertung von 160 MRTs in Bezug auf Gyrierung/Sulcierung und Myelinisierung anhand des v. d. Knaap Scores und McArdle Scores durch zwei für die Krankengeschichte verblindete Auswerter. Korrelierung zu prospektiv festgelegten systemischen und neurogischen Pathologien sowie zu Einflüssen der neonatologischen Intensivmedizin. Ergebnisse: Die Hirnentwicklung der 37-42 Schwangerschaftswoche ist unabhängig vom Geburtsalter. Langzeitige Beatmungstherapien (Intubation, CPAP) verzögern die Myelinisierung sowie die Reifung des Cortex in bestimmten Hirnregionen. Diskussion:Nicht alleine das Alter bei Geburt sondern auch intensivmedizinische Einflüsse und peripartale Pathologien beeinflussen die Hirnentwicklung Frühgeborener signifikant. Insbesondere respiratorische Probleme haben einen ungünstigen Einfluss auf die Hirnreifung.
  • Objective: To assess brain maturation of premature born infants at term in correlation with pathological events and intensive care conditions. Study design:160 infants, 1,5-T whole-body system, extremity coil, T1 and T2 weighted, spin-echo images obtained in all three planes. Independent review by two investigators blinded for gestational age and clinical history, regarding the development of myelination and prominent sulci/gyri using a modified van der Knaap Score. Correlation to defined clinical findings like brain pathologies (IVH, PVL) and postnatal clinical history (pathologies and exposition to intensive care). Exclusion criteria: major cerebral malformation, chromosomal/metabolic disorders. Results: Progressive evolution of gyration and sulcation was measured between gestational age 37 to 42 for the whole cortex and for the fronto – central and temporo – occipital subregions. Impaired myelination was correlated with the following conditions: Severe IVH (p = 0,023), pneumothorax (p = 0,020) and duration of mechanical ventilation (p = 0,017). Delayed sulcation and gyration scores were correlated with the duration of need for respiratory support (p = 0,017). Conclusion: This MRI study of prematurely born infants correlates defined clinical influences and pathologies on the development of the brain at near term. Alterations of myelination and gyration / sulcation could be shown for brain pathologies like severe IVH and prolonged respiratory therapies.
