Wirkungen von CO2-Gas auf die Mikrozirkulation der Haut bei Chronischer Venöser Insuffizienz

  • Ziel dieser Arbeit war die Untersuchung der Wirkung der lokalen Applikation von Kohlendioxid auf die Mikrozirkulation trophisch geschädigter Haut von an einer Chronischen Venösen Insuffizienz erkrankten Probanden. Es ist bekannt, dass eine Chronische Venöse Insuffizienz (CVI) zu pathomorphologischen Veränderungen der Mikrozirkulation der Haut wie Dilatation, Verzweigung, Torsion, glomerulumartiger Form und Rarefizierung der Kapillaren führt. Diesen folgen Störungen der Trophik der Haut wie Purpura jaune d’ocre, Atrophie blanche und zuletzt Ulzeration. Mit den pathomorphologischen Veränderungen einher geht ein verringerter Sauerstoffpartialdruck des Gewebes, die nutritive Versorgung ist reduziert. Die nutritive Situation könnte also durch eine erhöhte Perfusion mit konsekutiv größerem Sauerstoffangebot verbessert werden. Die vasodilatatorische Wirkung von Kohlensäure ist bekannt und wird mit der Anwendung von kohlesäurereichen Bädern bei der peripheren arteriellen Verschlusskrankheit genutzt. Vasodilatation führt zur Erhöhung der Perfusion (Hagen-Poiseuille-Gesetz). Es stellt sich die Frage, ob die lokale Anwendung von Kohlensäure auch bei der Chronischen Venösen Insuffizienz zu einer verbesserten Perfusion führt. Um dies zu klären, wurde Kohlensäure in eine bereits auf dem Markt befindliche Wundauflage in Form einer Tablette eingearbeitet, die, nach Aktivierung durch eine Flüssigkeit, Kohlensäure freisetzen sollte. Es wurde jeweils die modifizierte sowie die reguläre Wundauflage an 19 Probanden bezüglich der durch sie ausgelösten Veränderungen im Laser-Doppler-Flux (LDF) untersucht. Der LDF stellt ein Maß für die Gewebeperfusion dar, das über die Reflexion von Licht an sich bewegenden Zellen (Blutzellen) gewonnen wird. Durch die Bewegung der Zellen wird das Licht in veränderter Frequenz reflektiert (Doppler-Effekt), hierbei wirken sich sowohl die Menge als auch die Durchschnittsgeschwindigkeit der Zellen auf das detektierte reflektierte Licht bzw. den Laser-Doppler-Flux aus. Das Laserlicht dringt nur sehr oberflächlich in die Haut ein (wenige Millimeter) und erfasst so die nutritiven und die thermoregulativen Gefäße. Der Laser-Doppler-Flux lässt sich in Frequenzbänder teilen, die physiologische Funktionen wie die Atmung, den Herzschlag sowie neurogene oder myogene Aktivität darstellen. Die Wundauflagen wurden über zwei Stunden auf Haut mit trophischen Störungen durch CVI angewendet. Während der gesamten Zeit wurde kontinuierlich der Laser-Doppler-Flux gemessen. Der Einfluss von Temperatur auf die Hautdurchblutung wurde über deren Konstanthaltung möglichst gering gehalten, so dass die aufgezeichneten Veränderungen im LDF hauptsächlich denen im nutritiven Kapillarbett entsprachen. Das aufgezeichnete LDF-Signal wurde bezüglich der Energie im myogenen Frequenzband (Vasomotionsband) weiter untersucht. Zusammen mit dem Laser-Doppler-Flux-Signal ließ sich so die Aussage über die Perfusion spezifizieren. Da mittels Laser-Doppler-Fluxmetrie nur ein indirekter Nachweis einer Gefäßdilatation über das Laser-Doppler-Flux-Signal und die Berechnung der Energie im myogenen Frequenzband erbracht werden kann, wurde eine Untersuchung der Reaktion der Nagelfalzkapillaren gesunder Probanden auf Kohlendioxid mittels Kapillarmikroskopie angeschlossen, die eine Messung des Gefäßdurchmessers ermöglichte.
  • Introduction/Background: In the tissue of the lower leg of patients with chronic venous insuffiency (CVI) the cutaneous capillary bed has underwent changes in morphology such as rarefication, dilatation, formation of contorted or glomerulus-like shaped capillaries, which lead to trophic skin changes like edema, hyperpigmentation, induration, atrophy blanche and finally ulceration. Whereas healing of venous ulcers is accompanied by an improvement of nutritive blood flow. Methods and Patients: To investigate the effects of CO2-gas on the microcirculation of the skin a study with 19 patients (13 female, 6 male)was performed, age between 43 and 77 years (average age 60,0 years) affected by CVI CEAP stage 4 and 5. Atrophy of the skin was found in 89.47% of the patients, hyperpigmentation in all while hypopigmentation was found in 52.63% additionally. Capillaritis alba occured only in 21.05% of the patients, induration was asessed in 42.11% and in 5.26% in terms of dermatosclerosis. 31.58% had had one or more (by example reoccurring) ulcers before (CEAP stage 5 or Widmer stage 3a) of whom 15.79% were triggered by a trauma. Patients with an active ulcer did not participate. Other diseases with influence on the microcirculation such as heart insuffiency, serious lung diseases, peripheral arterial occlusive disease or other severe diseases were excluded in the patients. CO2-exposition was given by the prototype of a semiocclusive wound dressing consisting of polyacrylate, which, activated by a solution of natriumbicarbonate 3.4%, released over a time period of about two hours CO2-gas. In a first session a semiocclusive CO2-emitting wound dressing, in a second session the same but only semiocclusive dressing was applied on trophicly affected skin of the lower leg. During the two hours the Laser Doppler Flux (LDF) was measured in 15 min units (total of 7), and continuously the temperature in this area. Since a part of the blood flow measured by LDF consists of the cutaneous temperature regulating plexus, temperature was kept constant in the room and in the measurement area to avoid changes in the flow of this plexus and to record mostly changes in the nutritive segment of the capillaries. The LDF signal was analyzed with the following methods: 1. analysis of trend 2. wavelet packet transformation (Haar filter) and reordering of the frequencies to the natural order, calculation of the energy rate of the vasomotion frequency band. An examination of the nailfold capillaries of 5 healty persons by capillary microscopy followed focusing on changes in diameter of the capillaries and in blood flow velocity. Results: Laser Doppler Flux as a gauge of skin perfusion increased under the application of a semiocclusive CO2-emitting wound dressing significantly. That means perfusion has increased significantly under the application of the CO2-gas emitting wound dressing over the time period of two hours. The frequency analysis of the vasomotion frequency bands (0.06 Hz to 0.16 Hz) shows that the median energy rate of this band is mostly significant lower for the CO2-emitting wound dressing. By capillary microscopy a significant increase of the diameter of the capillaries and a significant decrease of the blood flow velocity could be observed. Conclusion: In this study the hypothesis could be verified that CO2 gas (released out of the prototype of a wound dressing) would lead to a distinct increase of LDF as a gauge for skin perfusion in patients with advanced chronic venous insufficiency. Simultaneously the vasomotion is performed at a lower energy rate hence with a lower amplitude. We concluded from this results that the capillary bed must be constantly widened under the application of CO2-gas probably trying to give a better supply of O2 to the tissue. A prove of this conclusion was given by capillary microscopy.
