  • When the myocardium is reperfused after an ischaemic phase in which tissue is restricted in supply of oxygen and other nutrients complex signal transduction processes occur in the cells that damage (ischaemic-reperfusion damage) or protect the myocardium from further damage during reperfusion (cardioprotection). Felix et al. gave evidence of substances in the post-ischaemic effluent which are released during reperfusion and influence calcium metabolism in isolated adult cardiomyocytes and therefore reduce contractility of the cells (Felix et al. 2001). The aim of the study was to identify the signal transduction pathways leading to the negative inotropic effect on the isolated cardiomyocytes in order to draw conclusions on the identity of the mediators in the effluent. Experiments indicated that the negative inotropic mediators of the effluent are released from the arachidonic acid metabolism. Therefore in the present study the role of cyclooxygenase- and prostaglandin-metabolism was investigated in mediating the negative inotropic effect of the post-ischaemic effluent on the isolated cardiomyocytes. After 10 minutes of global stop-flow ischaemia adult rat hearts were reperfused and the coronary effluent was collected over a period of 30 seconds. The effect of the post-ischaemic effluent on the contractility (systolic cell shortening) and the calcium metabolism (calcium transient) of field-stimulated and laminated cardiomyocytes that were stained with fura-2AM was analysed by fluorescence microscopy. Effluent collected before the ischaemic phase was used as a control. The present study analysed the role of cyclooxygenase and prostaglandin receptors in the signaltransduction of the negative inotropic factors by using various inhibitors and antagonists. The expression of the cyclooxygenase-isoformes and the various prostaglandin receptors was analysed by Western Blot and immunofluorescence staining. Laminated and fura-2AM-stained cardiomyocytes expressed both isoformes of cyclooxygenase in adult cardiomyocytes. The expression of stress-induced cyclooxygenase-2 in laminated cardiomyocytes was increased in comparison to cardiomyocytes that were not laminated. Adult rat hearts basally expressed cyclooxygenase-2 at a lower level. 10 minutes of global ischaemia did not influence the expression in the hearts. Pre-incubation of cells with the cyclooxygenase-inhibitor indomethacin and the cyclooxygenase-2-inhibitors NS-398 and lumiracoxib inhibited the negative inotropic effect of the post-ischaemic effluent on calcium metabolism and contractility of the isolated cardiomyocytes. The cyclooxygenase-inhibitor SC-560 however did not influence the effect of the post-ischaemic effluent. The post-ischaemic effluent reduced calcium transients and beating frequency in neonatal cardiomyocytes. The reducing effect on both parameters was also blocked by the use of indomethacin, NS-398 and lumiracoxib. In isolated cardiomyocytes receptors of prostaglandin D (DP1 and DP2), E (EP1-EP4), I and thromboxanes were detected by Western Blot analysis. By the use of selective prostaglandin receptor-antagonists there was shown that the mechanism of signaltransduction downstream of cyclooxygenase-2 is probably mediated by the prostaglandin E-receptors EP2 and EP4. The effect of the post-ischaemic effluent is furthermore dependent on protein kinase A. The results of experiments with the protein kinase A-inhibitor Rp-cAMPS were confirmed by cAMP-analysis of lysates of isolated adult cardiomyocytes that were treated with the post-ischaemic effluent and that show an increased concentration of cAMP. The negative inotropic effect of the post-ischaemic effluent indicates a cardioprotective function of the post-ischaemically released substances. The effect of the effluent was blocked by the use of glibenclamide and HMR-1098 which are inhibitors of sarcolemmal and mitochondrial potassium (ATP)-channels. Furthermore the effect of the post-ischaemic effluent on cardiomyocytes was investigated when extracellular calcium is increased. In comparison to the control effluent the post-ischaemic effluent reduced the intracellular diastolic and systolic calcium increase. The mediators in the post-ischaemic effluent are probably substances released from arachidonic acid metabolism that were metabolised by cyclooxygenase-2 in isolated cardiomyocytes and act negative inotropic and cardioprotective by activating of sarcolemmal potassium (ATP)-channels.
