Interdisziplinäre Therapie der CMD mit Hilfe der orthopädischen Wirbelsäulenvermessung

  • Der vermutete Zusammenhang zwischen Kieferfehlstellung und Wirbelsäulenfehlstellung wird untersucht und Möglichkeiten der Wirbelsäulenkorrektur durch Bissänderungen dargestellt. 60 Patienten mit deutlicher Schmerzsymptomatik und einer messbaren Kiefergelenkabweichung in habitueller Okklusion und 20 Probanden einer Kontrollgruppe sind Untersuchungsgegenstand. Nach einer klinischen Funktionsanalyse erfolgt eine orthopädische Untersuchung und eine instrumentelle Funktionsanalyse mittels Condylograph der Firma Gamma. Die Vermessung der Wirbelsäule erfolgt mit dem formetric 3D®-Gerät der Firma Diers. Aus den Messdaten des 3D-Geräts werden die Werte der Lotabweichung und der Rotationsabweichung extrahiert. Die Lotabweichung misst den seitlichen Überhang der Wirbelsäulensenkrechten auf die Beckenhorizontale. Die Rotationsabweichung misst die Abweichung der Wirbelkörper in der Rotation gegeneinander. Die statistische Auswertung der Patienten zeigt in der Lotabweichung bei 73% und in der Rotation bei 72% eine deutlich verbesserte Wirbelsäulenstellung mit Bisskorrektur. Die simulierte Okklusionsstörung mittels einseitig eingebrachter Watterollen führt bei den Probanden bei der Lotabweichung zu einer Verschlechterung der Werte bei 70% und bei der Rotation zu einer Verschlechterung bei 85% der Probanden, Die Ergebnisse dieser Studie zeigen, dass Veränderungen der Bisslage einen gewissen Einfluss auf die Wirbelsäulenstatik bei Patienten mit CMD sowie gesunden Kontrollprobanden zu haben scheinen. Zur Sicherung dieses Zusammenhangs ist allerdings die Durchführung randomisierter kontrollierter Studien notwendig.
  • Objective of this study is to investigate the assumed correlation between temporomandibular malpositions and spinal malpositions and to demonstrate possibilities to correct spinal malpositions by means of occlusal corrections. A population of 60 patients with typical pain syndroms and a measurable deviation of the temporomandibular joint in habitual occlusion and 20 subjects of a study group are examined. After a clincial functional analysis and a basic orthopedic examination an instrumental functional analysis is executed with the condylographic device of the company Gamma, Austria. The static scan of the spine is carried out with the 3D device "formetric" of the company Diers International, Germany. From the data of the 3D scan the values of the trunk imbalance and the deviation in rotation were extracted. The trunk imbalance is measuring the lateral inclination of the spine in relation to the sacrum plane. The deviation in rotation is measuring the deviation of the vertebra in its rotation. The statistical analysis of the patient group shows a significant improvement of the spinal position upon occlusal correction in 73% of the trunk imbalance values and in 72% of the values of the deviation in rotation. The simulated occlusal dysfunktion by means of unilaterally positioned cotton rolls in the subject group leads to an impairment of the values of the trunk imbalance in 70% and of the values of the deviation in rotation in 85%. The results of this study demonstrate a certain influence of occlusal manipulations on the spinal position on TMJ patients as well as on healthy subjects. Further randomized controlled studies are necessary in order to substantiate this correlation.
