Metabolische Azidosen bei muttermilchernährten Frühgeborenen: Einfluss von Fortifiern

  • In der neonatologischen Intensivstation des Zentrums für Kinder- und Jugendmedizin in Greifswald war nach Einführen des Muttermilchsupplements FMS (Milupa®) für frühgeborene Kinder ein vermehrtes Auftreten metabolischer Azidosen aufgefallen. Dieser Eindruck sollte durch eine klinische Studie verifiziert oder falsifiziert werden. Durch die vorliegende prospektive, doppelblinde, randomisierte Studie konnte die Hypothese, dass das Muttermilchsupplement FMS für eine signifikant erhöhte Inzidenz von metabolischen Azidosen verantwortlich ist, verifiziert werden. Daraufhin wurde nach möglichen Erklärungen für diese metabolischen Azidosen gesucht. Die wahrscheinliche Ursache wurde in einer Imbalanz der Elektrolytzusammensetzung des Muttermilchsupplements gesehen. Um diese Ursächlichkeit zu beweisen und die erhöhte Inzidenz von metabolischen Azidosen zu beseitigen, wurde in Zusammenarbeit mit der Firma Milupa eine neue Rezeptur für das Muttermilchsupplement FMS entwickelt. Eine Folgestudie (nicht mehr Gegenstand dieser Dissertation) mit der neuen Rezeptur des Muttermilchsupplements FMS (FMS*) hatte zum Ergebnis, dass die Inzidenzen von metabolischen Azidosen unter Muttermilchsupplementierung mit FM 85 und FMS* keinen signifikanten Unterschied mehr aufwiesen. Den gewonnenen Ergebnissen unserer Studien zum Trotz werden noch viele Forschungsergebnisse notwendig sein, um die Ursachen für metabolische Azidosen von frühgeborenen Kindern unter der Supplementierung mit Fortifiern gänzlich zu erklären.
  • Objective A prospective, double-blind, randomized, cross-over-controlled trial was conducted to investigate the hypothesis that a new human milk fortifier increases the incidence of metabolic acidosis. Further it was intended to work out the possible mechanisms. Methods 17 infants with a birth weight of either < 1500g or >1500g combined with poor growth concerning percentile charts were enrolled and randomized into two groups. Group 1 received the fortifier FM 85 (Nestlé®), group 2 received FMS (Milupa®). Otherwise feeding and treatment algorithms were identical. Growth, biochemical indices of nutritional status, enteral intake, feeding tolerance and morbidity were assessed serially. Close-meshed controls of the acid base status were performed. In case of metabolic acidosis fortification was stopped and the second fortifier was introduced after normalization of the acid-base-status after a wash-out interval of few days. Two groups of historically matched controls according to gestational age and birth weight were studied retrospectively for comparison. Results In group 1 (n=7) only one case of acidosis occured. In group 2 (n=9) seven cases of acidosis were found, which were more severe in comparison to group 1 and prolonged even after changing fortification. Daily increase in weight was significantly higher in group 1. Neonates in group 2 needed higher rates of natrium, potassium and natrium bicarbonate supplementation. Data of historically matched controls corresponded to group 1 concerning acid-base-status, electrolyte supplementation and weight gain. Discussion The human milk fortifier FMS proved to be responsible for an increased incidence of metabolic acidosis. We detected the reason for the high incidence of acid base imbalances in group 2 as being the low electrolyte content of FMS. Compared to FM 85 FMS contains less natrium, potassium and chloride and these electrolytes are essential for the elimination of metabolic protons. Conclusions Supplementation of electrolytes has significant clinical impact on the acid base metabolism during long term feeding with fortified breast milk. The detection of clinical irregularities should lead to the thorough investigation of the treatment regimes even if the only change was a newly tested commercially available product.
