Untersuchung zum Einfluss von Dehydroepiandrosteron auf die intestinale Mikrozirkulation bei experimenteller Endotoxinämie

  • Untersuchung zum Einfluss von Dehydroepiandrosteron auf die intestinale Mikrozirkulation bei experimenteller Endotoxinämie Abstrakt Einleitung: Sepsis und septischer Schock stellen die Haupttodesursachen auf chirurgischen Intensivstationen dar. Das Ziel dieser Studie war es den Einfluss von Dehydroepiandrosteron (DHEA) auf die intestinale Mikrozirkulation bei experimenteller Endotoxinämie mittels Intravital-Mikroskopie (IVM) zu untersuchen. Methodik: Die Studie wurde mit 40 männlichen Lewis 1A Ratten durchgeführt. Die Tiere wurden in 4 Gruppen unterteilt: 1. Kontrollgruppe, 2. DHEA-Gruppe, 3. LPS-Gruppe, 4. DHEA+LPS-Gruppe. Die DHEA-Gabe erfolgte als pre-treatment (28 mg/kgKG i.m.). Endotoxinämie wurde mit 5 mg/kgKG LPS von E. coli induziert. Klinische Parameter (mittlerer arterieller Blutdruck, Herzfrequenz, Körpertemperatur) wurden gemessen. Bei allen Tieren erfolgte IVM der Leukozytenadhärenz in Venolen 1° und 3° in submukösen Schichten. Die funktionelle Kapillardichte (FCD) der longitudinalen und circulären Muskelschicht sowie der intestinalen Mukosa wurde gemessen. Blutentnahmen erfolgten zur Messung der Konzentrationen von TNF-alpha, IFN-gamma, IL-1 alpha, GM-CSF, IL-4 und des Laktats. Ergebnisse: 90 Minuten nach Endotoxinexposition sank die FCD signifikant in der nicht-therapierten LPS-Gruppe im Vergleich zur Kontrollgruppe. DHEA-Applikation verbesserte die FCD signifikant. Die Leukozyten-Endothel-Interaktion war in der nicht-therapierten LPS-Gruppe signifikant erhöht, im Vergleich zur Gruppe 1, jedoch signifikant reduziert in der DHEA-behandelten LPS-Gruppe. Konzentrationen proinflammatorischer Zytokine (TNF-alpha, IL-1 alpha) sowie die Laktatkonzentration waren in den nicht-therapierten LPS-Gruppen zum Zeitpunkt t=165 Minuten erhöht. DHEA-Behandlung reduzierte diese Konzentrationen signifikant. Schlussfolgerung: DHEA-Gabe verbessert die intestinale FCD, senkt die Leukozytenadhärenz und die Konzentration von TNF-alpha, IL-1 alpha sowie Laktat bei experimenteller Endotoxinämie.
  • Impact of dehydroepiandrosterone on the intestinal micocirculation in experimental endotoxemia Abstract: Introduction: Sepsis and septic shock represent the main cause of death in surgical intensive care units. Aim of this study was to investigate the impact of dehydroepiandrosterone (DHEA) on the intestinal microcirculation in experimental endotoxemia by intravital microscopy (IVM). Methods: The study was performed in 40 Lewis 1A rats. The animals were diveded into 4 groups: 1. Control group, 2. DHEA group, 3. LPS group, 4. DHEA+LPS group. DHEA was given as pre-treatment (28 mg/kg i.m.). Endotoxemia was induced by administration of 5 mg/kg LPS. Clinical parameters (mean arterial pressure, heart rate, body temperature) were measured. All animals underwent IVM studies of leukocyte adhesion in the intestinal submucosal layer (venules 1° and 3°). Functional capillary density (FCD) of the longitudinal and the circular muscle layers as well as the intestinal mucosa was measured. Blood samples were taken to analyze the release of the cytokines TNF-alpha, IFN-gamma, IL-1 alpha, GM-CSF, IL-4 and the lactate concentration. Results: 90 minutes after endotoxin challenge FCD decreased significantly in untreated LPS animals compared to control animals. Administration of DHEA resulted in a significant restoration of FCD. Leukocyte endothelial interactions were significantly increased in the untreated LPS-group compared to group 1 and significantly decreased in DHEA treated LPS animals. Proinflammatory cytokine levels (TNF-alpha, IL-1 alpha) and the lactate concentration were increased in untreated LPS animals at t=165 minutes. DHEA treatment reduced these changes significantly. Conclusion: Administration of DHEA improved intestinal FCD, decreased leukocyte adherence and levels of TNF-alpha, IL-1 alpha and lactate in experimental endotoxemia.
