Zur Quantifizierung der funktionellen Okklusion – Entwicklung spezieller Analyseverfahren für den Einsatz digitaler Technologien in der Zahnmedizin

  • Die Okklusion ist in der Zahnmedizin der Kontakt zwischen den Zähnen des Ober- und Unterkiefers. Mit der Entwicklung der digitalen Zahnmedizin ist es wichtig die Okklusion digital zu erfassen und analysieren zu können. In dieser Arbeit werden verschiedene neu entwickelte Verfahren vorgestellt und mathematisch beschrieben, um die Okklusion in der Statik und Dynamik zu visualisieren und zu quantifizieren. Ein zweidimensionales Verfahren (GEDAS II, Greifswald Digital Analyzing System) ermöglicht den Einsatz in Studien (SHIP, Study of Health in Pomerania) und liefert quantitative Informationen über Zahnkontakte von Patienten. Detailliertere Informationen sind mit dreidimensionalen Scans von Zahnreihen oder Bissregistraten möglich. Es kann die Morphologie untersucht werden. Zur Unterstützung von digital erstelltem Zahnersatz bei der Herstellung werden virtuelle Artikulatoren benötigt. Hierfür wird eine Methode vorgestellt, welche die realdynamische Bewegung des Unterkiefers mit der Okklusion verbindet. Dazu werden 3D-Scans der Zahnreihen mit elektronischen Bewegungsaufzeichnungen gekoppelt. Mit dieser Methode (3D-VAS, 3D-Virtual Articulation System) ist es möglich nicht nur Kieferbewegungen sondern auch richtige Kaubewegungen zu registrieren. Im Ergebnis können Kontakte in der Dynamik betrachtet werden und es werden Ideen zur Einbindung in CAD/CAM-Prozesse vorgestellt. In der digitalen Welt lassen sich nicht nur die Kontakte, sondern auch der okklusale Freiraum erfassen. Anhand von fünf Fallbeispielen aus SHIP wird gezeigt, wie unterschiedlich die Okklusion bei verschiedenen Befunden und wie effektiv das Kauen auf der linken und rechten Seite sein kann. All diese Methoden zur Erfassung der oralen Physiologie sind ein Beitrag, um computergestützt die Qualität des Zahnersatzes zu verbessern. Und zum anderen hilft es Zahnärzten und Patienten in der Kommunikation, wenn die Patientensituation umfassender aufgezeichnet und visualisiert werden kann.
  • The dental occlusion is the contact between the teeth of the upper and lower jaw. The development of digital dentistry highlighted the importance to be able to capture and analyze the occlusion digitally. In this thesis, several newly developed methods are presented and described mathematically in order to visualize and quantify the occlusion in statics and dynamics. A two-dimensional method (GEDAS II, Greifswald Digital Analyzing System) offers the application in studies (SHIP, Study of Health in Pomerania) and provides quantitative information on tooth contacts of patients. To obtain more detailed information it is possible to use three-dimensional scans of teeth or bite registrations. Thus it is possible to study the morphology. To support the preparation of digitally created dentures virtual articulators are required. For this purpose a method is presented that combines the real dynamic movement of the mandible with the occlusion. Therefor 3D scans of the upper and lower jaws are coupled with electronic movement records. This method (3D-VAS, 3D Virtual Articulation System) enables the registration of jaw movements as well as real chewing. As a result the dental contacts can be viewed in the dynamics. Furthermore, ideas are presented, revealing the possibility of an integration into CAD/CAM processes. In the digital world it is not only possible to capture contacts but also the occlusal gap. Based on five case studies from SHIP it is shown how the occlusion differs according to the different findings and how effective chewing on the left and right side can be. All of these methods to detect the oral physiology are a contribution to improve the quality of the denture by computer-assisted systems. On the other hand it helps dentists and patients communicating with each other if the patients condition can be recorded and visualized more comprehensively.
