ABCC3-enthaltende inside-out Vesikel dargestellt werden.
  • Gadolinium ethoxybenzyl diethylenetriaminepentaacetic acid (Gd-EOB-DTPA) is a liver-specific magnetic resonance imaging (MRI) contrast agents. It is a widely used in clinical diagnostic use in focal liver lesions. Compared to other gadolinium-containing contrast agents Gd-EOB-DTPA is specifically taken up by hepatocytes. After intravenously bolus injection of Gd-EOB-DTPA is excreted up to 50% via the biliary and 50% via the kidneys. The mechanisms of hepatic uptake and biliary elimination are poorly understood. A further understanding is also needed to explain the large interindividual differences in liver accumulation of Gd-EOB-DTPA in patients and also to predict possible drug interactions. Therefore, the aim of this thesis was to explain the transport mechanisms of the contrast agent. Firstly in vitro experiments, the substrate affinity of Gd-EOB-DTPA would be examined in stable transfected cells. Secondly, an animal experiment was performed in wild-type and Abcc2 deficient-rats to investigate the mechanisms of hepatobiliary elimination of Gd-EOB-DTPA and to visualize the intestinal drug absorption using MRI procedure. These in vitro studies showed that Gd-EOB-DTPA is a substrate of the liver-specific transporters OATP1B1, OATP1B3 and NTCP, but not the ubiquitous OATP2B1. Therefore high liver specificity of the contrast agent can be explained. In vitro Gd-EOB-DTPA was taken up by all genetic variants of OATP1B1 with different Km values, the * 1b variant showed a significantly increased transport capacity compared to the wild type (WT). However the OATP1B1 * 5 (not significant) and 15 * (significant) showed a decreased uptake of Gd-EOB-DTPA. For OATP1B3, showed the variant p.233Ile a significantly lower substrate affinity in vitro. Gd-EOB-DTPA was taken up to the polymorphism p.112Ala/233Ile with a significantly lower transport activity compared to the WT. After intravenous administration of Gd-EOB-DTPA in wild-type and Abcc2-deficient rats has been shown that Abcc2 plays a important role in the hepatobiliary excretion of Gd-EOB-DTPA. At the same time it was shown that in Abcc2-deficient rat the elimination of the contrast agent is fully compensated by the kidneys. In Abcc2-deficient rats expression of ABCC3, which expresses in the basolateral membrane of the hepatocytes, is significantly increased. In vitro, a concentration-dependent uptake of Gd-EOB-DTPA was shown in ABCC2 or ABCC3-containing inside-out vesicles. ABCC3 could be a candidate for the return transport of Gd-EOB-DTPA from the hepatocytes into the blood. ABCC2 and ABCC3 are also expressed in the intestine. Since Gd-EOB-DTPA is a substrate of ABCC2 and ABCC3, it was investigated whether the contrast agent is also absorbed in the intestine and thus the absorption of drugs could be visualized. After oral administration of Gd-EOB-DTPA was absorbed (bioavailability 17%) in the intestine of rats. ABCC2 seems to be of great importance, because the bioavailability after oral administration of Gd-EOB-DTPA in Abcc2-deficient rats increased from 17% to 25%. In summary, this dissertation illustrate that Gd-EOB-DTPA is an in vitro substrate of the liver-specific transporters OATP1B1, OATP1B3 and NTCP. It is also a substrate OATP1A2, ABCC2 and ABCC3. The drug is therefore a good example of the complex interplay between intestinal absorption and hepatic and Effluxtransport, which for many substances already well established (eg, statins, Ezetmib). It was shown that the liver-specific uptake of Gd-EOB-DTPA is realized by OATP1B1 and OATP1B3, whereas ABCC2 represents the major efflux transporter of hepoatobiliären elimination. The work could further show that genetic variants would be a plausible explanation for the in clinical observed high interindividual variability of liver enrichment. Based on the in vitro and in vivo results of Gd-EOB-DTPA turns out, that Gd-EOB-DTPA is a new "probe drug" to visualize the absorption of drugs and to characterize the function of the transporter.
