CT-Volumetrie der Harnblase - ein möglicher Indikator für Alkoholintoxikation polytraumatisierter Patienten?

  • Einleitung: Akuter und chronischer Alkoholkonsum sind bezüglich Ursache, Diagnostik und Therapie des polytraumatisierten Patienten ein bedeutender Faktor. Die Studie untersuchte den bisher nur in der post mortem Diagnostik nachgewiesenen Zusammenhang zwischen Harnblasenvolumen (HBV) und Blutalkohol in einem in vivo-Kollektiv, sowie die Genauigkeit der US-Formel gegenüber der 3D-Volumetrie in der CT zur Volumenberechnung. Material und Methoden: Die Methodenbetrachtung zur Volumenmessung erfolgte mittels Phantomkörperberechnung. CT-Untersuchungen, Laborparameter, toxikologische Befunde und Anamnese von 1242 polytraumatisierten Patienten wurden retrospektiv betrachtet, in die finale Studienpopulation wurden 831 Patienten aufgenommen. Das Verhältnis von Harnblasenvolumen zu Hämatokrit bzw. Hämoglobin dienten als Surrogatparameter. U-Test, ROC-Analysen, Cutoff-Wert-Bestimmung und der Spearmans Korrelationskoeffizient kamen zur statistischen Analyse und Auswertung zur Anwendung. Ergebnisse: Es zeigte sich eine signifikant geringere mittlere Abweichung der 3D-Volumetrie von 6,79 ± 2,3 % zum Realvolumen im Vergleich zur US-Volumetrie mit 21,2 ± 7,9 % in der Modellbetrachtung. Es existierte ein signifikanter Unterschied (US: p < 0,01; 3D: p < 0,01) zwischen Patienten mit pos. und neg. toxikologischem Befund. Positive toxikologische Befunde korrelieren signifikant positiv mit dem HBV (US: r = 0,352; 3D: r = 0,405; p ≤ 0,01). Berechnungen mittels 3D-Volumetrie des HBV polytraumatisierter Patienten ergab eine Sensitivität/Spezifität von 50,9 % / 76,3 % bei einem Cutoff von 416,3 ml hinsichtlich einer Alkoholintoxikation. Die Sensitivität konnte auf 64,2 % gesteigert werden, wenn ein Index aus HBV und Serumkreatinin berechnet wird. Hierfür liegt der Cutoff-Wert bei 4,2 ml*l/μmol. Diskussion: Die CT-Volumetrie ist die verlässlichere und genauere Messmethode zur Bestimmung des HBV. Der Zusammenhang zwischen einem gesteigertem HBV und einem positivem Ethanolkonsum ist signifikant und sollte als suspektes radiologisches Zeichen für eine Ethanolintoxikation Beachtung finden.
  • Introduction: Acute and chronic consumption of alcohol are important coefficient in relation to cause, diagnostic investigation and therapy of the polytraumatized patients. The purpose of the study was to analyses the accuracy of radiological measured urinary bladder volume (UBV) by ultrasonic formula (ultrasonic-volumetry (US)) and 3D-volumetry as well as to investigate the relationship between intoxication and calculated UBV in an in vivo population. Materials and methods: The evaluation of volume measurement accuracy was performed by the use of a spheric phantom. Computer tomography scans, laboritory values, toxicology reports and clinical history of 1242 polytraumatized patients were retrospectively reviewed, the final study population included 831 patients. The data was analysed using Mann-Whitney-U-test, ROC-analysis and Spearmen’s rho test. Sensitivity and specificity were calculated for individual cutoff values. Results: There is a significant higher accuracy for the 3D-volumetry (6,79 ± 2,3 %) in comparison to the ultrasonic formula (21,2 ± 7,9 ) in relation to the real volumes of the phantom. Positive toxicology reports correlate significant with UBV (US: r = 0,352; 3D: r = 0,405; p ≤ 0,01). UBV higher than 416,3 ml indicates positive toxicology results with a sensitivity/specificity of 50,9 % / 76,3 %. The use of a quotient of UBV and creatinine increases the sensitivity up to 64,2 % with an cutoff-value of 4,2 ml*l/μmol. Conclusions: 3D-volumetry of UBV provides significant higher accuracy compared to ultrasound based analysis. The relationship between blood alcohol and UBV is significant and should be used as a suspect parameter and radiological sign for alcohol intoxication.
