Effizienz des Toxoplasmosescreenings während der Schwangerschaft im Vergleich zum Rötelnscreening. Eine Untersuchung im Rahmen des Survey of Neonates in Pomerania (SNIP)

  • Ziel der Dissertation war die Analyse der Effizienz des derzeitigen Toxoplasmose-Screenings als individuelle Gesundheitsleistung (IGeL) während der Schwangerschaft im Vergleich zum Röteln-Screening als standardisierte Kassenleistung. Im Rahmen der Survey of Neonates in Pommerania (SNiP), einer prospektiven, populationsbasierten Studie wurden im Zeitraum zwischen Mai 2002 und November 2008 detaillierte Informationen zur Prävalenz und Inzidenz neonataler Morbiditätsfaktoren gesammelt. Davon wurden insgesamt 5403 schwangere Frauen in unsere Analyse aufgenommen. Wir stellten fest, dass das Röteln-Screening als Teil der Mutterschaftsrichtlinien eine um 22,8% höhere Teilnahme aufweist als das Toxoplasmose-Screening, eine individuelle Gesundheitsleistung, die von der Mutter selbst getragen werden muss. In unserer Untersuchung nehmen Mütter mit einer besseren Bildungsanamnese, im Angestelltenverhältnis arbeitend und mit einem höherem Einkommen vermehrt das Toxoplasmose-Screening war. Beim Röteln-Screening zeigt sich, dass die Teilnahme am Screening unabhängig vom Erwerbsgrad und vom Einkommen der Mutter ist. Mehr als doppelt so viele Frauen weisen im Röteln-Screening (88,11%) gegenüber dem Toxoplasmose-Screening (34,36%) einen positiven Immunitätsstatus auf und sind somit gegenüber einer möglichen Infektion in der Schwangerschaft geschützt. Deutlich sind die Unterschiede zwischen Toxoplasmose (39,61%) und Röteln (8,94%). Die bei Toxoplasmose im Vergleich zu Röteln fehlende Präventionsmöglichkeit durch eine Impfung und die Schwierigkeiten der Diagnostik und der Therapie machen angesichts der Schwere und Chronizität der konnatalen Toxoplasmose eine Konzentration auf allgemeine Hygiene-Präventionsmaßnahmen und Früherkennungsmethoden erforderlich. Da nach unseren Daten derzeit ein großer Teil der Schwangeren nicht an einem Toxoplasmose-Screening (25,75%) teilnimmt und nur 43,49% an einem zweiten Screening teilnehmen, dies jedoch eine frühe Diagnosestellung und Therapieeinleitung ermöglichen könnte, erscheint eine Aufnahme des Screenings in die Mutterschaftsrichtlinien, d.h. die Aufnahme als generelle Kassenleistung sinnvoll.
  • The objective of this retrospective dissertation was to analyse the efficiency of toxoplasmosis-screening as an individualized medical healthcare service during pregnancy in comparison to a rubella-screening as a standard healthcare service. Data was collected in the context of the survey of neonates in pommerania (SNiP), a prospective population-based study in north-east Germany providing detailed information about neonatal health, morbidity and mortality during May 2002 and November 2008. We included 5403 pregnant woman during this period in our analysis. We conclude that the rubella-screening, as part of the so called “mother-guidelines”, has a 22,8% higher attendance in comparison to the toxoplasmosis-screening, as an individualized healthcare service. Furthermore, we observed in our analysis a higher attendance for the toxoplasmosis-screening of mothers who have a higher graduation level, are employed and have a higher income. In contrast the attendance of the rubella-screening is independent of the mother’s graduation level and income. More than twice the numbers of the women have a positive immune status at the rubella-screening (88,11%) in comparison to the toxoplasmosis-screening (34,36%). As a consequence they are protected against an infection during pregnancy. Women who screened negatively are vulnerable for an primary infection during pregnancy. There are obviously differences between toxoplasmosis (39,61%) and rubella (8,94%) immune status. Compared to rubella, there is a lack of possibilities to prevent a toxoplasmosis infection. In addition to difficulties in diagnosis and therapy there is no vaccine for toxoplasmosis. So, there are many reasons to concentrate on an early detection and on hygienic-prevention to avoid this severe and chronic disease. As our data show that there is a lack of women who do not screen for toxoplasmosis (25,75%) and only 43,49% make a second screening it would be sensible to include the toxoplasmosis-screening during pregnancy into the “mother guidelines” so they become a standard medical healthcare service and therefore a screening for toxoplasmosis is able to make an early diagnosis and therapy initiation.
