Untersuchungen zur Expression und Funktion pH-regulatorischer Transporter beim Glioblastoma multiforme

  • Glioblastome gehören zu den aggressivsten Gehirntumoren. Trotz intensiver Forschung an neuen Therapieoptionen liegt die mittlere Überlebenszeit bei nur 12 bis 15 Monaten. Wie andere solide Tumoren weist das Glioblastom Abweichungen im extra- (pHe) und intrazellulären (pHi) pH im Vergleich zu gesundem Gewebe auf. In Tumoren liegt der pHi im alkalischen Bereich und das extrazelluläre Milieu ist saurer. Diese pH-Unterschiede begünstigen die Tumorentstehung und -progression und sind an der Ausprägung typischer Merkmale von Glioblastomen wie der hohen Infiltrierungsrate, der unkontrollierten Proliferation und den Resistenzmechanismen beteiligt. Die Aufrechterhaltung des pHi wird u.a. durch pH-regulatorische Transporter wie den Natrium-Protonenaustauschern (NHE), den Monocarboxylattransportern (MCT) und dem Natriumabhängigen Chlorid-Bicarbonat-Austauscher (NDCBE) gewährleistet. Diese Transporter sind wichtige Regulatoren des pHi in gesunden, wie auch in malignen Zellen und werden in vielen Tumoren verstärkt exprimiert und/oder weisen eine erhöhte Aktivität auf. Ziel dieser Arbeit war es daher, in humanen Gliomproben die Expression der Transporter NHE1, NHE5, MCT1, MCT4, MCT5 und NDCBE zu untersuchen. Des Weiteren sollte die Regulation dieser Transporter durch antitumorale Substanzen (Zytostatika, NSAIDs) in den humanen Glioblastomzelllinien LN18 und U87MG betrachtet werden. Auf mRNA-Ebene konnte eine erhöhte Expression von NHE1, MCT1, MCT4 und MCT5 in humanem Glioblastomgewebe im Vergleich zu nicht-malignem Gehirngewebe nachgewiesen werden. Der Transporter MCT4 zeigten zudem einen Anstieg im mRNA-Gehalt der Grad-IV-Glioblastome verglichen mit Hirntumoren der Grade I-III. Für NHE1, NHE5 und MCT5 wurden des Weiteren interindividuelle Expressionsunterschiede detektiert. Patienten, die zum Zeitpunkt der Diagnosestellung ein Alter oberhalb des Medians aufwiesen, zeigten eine erhöhte Expression von NHE1, NHE5 und MCT5. Zudem lag eine erhöhte NHE5- und MCT5-Expression bei denjenigen Patienten vor, deren Überlebenszeit kürzer als die mediane Überlebenszeit war. Als antitumorale Wirkstoffe zur Beeinflussung der Transporterexpression und der Transporteraktivität in vitro wurden Zytostatika und als neue Therapieoption nichtsteroidale Antiphlogistika (NSAIDs) betrachtet. Als einziges Zytostatikum zeigte Teniposid Effekte auf die Expression der pH-regulatorischen Transporter und auf die Transportaktivität von NHE1 und MCT1. Zudem führte Teniposid zu einer verminderten Phosphorylierung von NHE1, was die reduzierte Transportaktivität erklären könnte. Die Proteinexpression der möglicherweise durch alternatives Splicen entstandenen 52 kDa großen NHE1-Variante 2 wurde durch Teniposid begünstigt. Diese NHE1-Variante zeigte zudem eine vermehrte mitochondriale Lokalisation. Des Weiteren konnte in Transportstudien mit Laktat eine Beeinflussung der MCT1-Aktivität durch Teniposid beobachtet werden. Ob eine direkte Interaktion von Teniposid mit MCT1 vorliegt, muss in weiteren Untersuchungen geklärt werden. Durch Teniposid wurde zudem der pHi erniedrigt und die Zellviabilität gesenkt. Das NSAID Diclofenac reduzierte ebenfalls die Zellviabilität und verminderte die NHE1-Transportaktivität unabhängig von der NHE1-Phosphorylierung. Unter Celecoxib kam es zu einer veränderten intrazellulären Lokalisation von NHE1- sowie LAMP2-positiven Signalen. Die Akkumulation dieser Strukturen in Kernnähe und die reduzierte Zellviabilität könnten auf Autophagozytose unter Celecoxib hinweisen. Trotz bekannter anti-tumoraler Wirkung von Ibuprofen in verschiedenen In-vivo- und In-vitro-Studien, erhöhte dieses NSAID interessanterweise die Zellviabilität der Glioblastomzellen. Die Transporterexpression und -aktivität blieb jedoch unbeeinflusst. Auf Grund dieser Befunde wurde zur weiteren Untersuchung von Ibuprofen ein intrakranielles Maus-Glioblastommodell unter Verwendung der murinen Glioblastomzellen GL261-GFP am Institut für Pharmakologie etabliert. In diesem In-vivo-Glioblastommodell konnte ein vermindertes Tumorwachstum bei den Ibuprofen-behandelten Tieren im Vergleich zu den Kontrolltieren festgestellt werden. Die Diskrepanz der In-vitro- und In-vivo-Daten könnte u.a. auf die in der Literatur beschriebene anti-angiogenetischen Wirkung von Ibuprofen zurückgeführt werden und zeigt die Wichtigkeit von tierexperimentellen Untersuchungen bei der Aufklärung von Medikamentenwirkungen. Die Ergebnisse dieser Arbeit liefern erste Hinweise, dass pH-regulatorischen Transporter wie NHEs und MCTs in humanen Glioblastomen eine Bedeutung bei der Tumorprogression und beim Ansprechen auf antitumorale Substanzen haben können. Die durch Teniposid und den NSAIDs modulierte Transporterexpression und –aktivität könnte eine Bedeutung für die antitumorale Wirksamkeit der Substanzen haben. Eine detaillierte Aufklärung dieser Mechanismen kann neue Erkenntnisse über die Interaktionen von bekannten Substanzen mit neuen Zielstrukturen liefern.
  • Glioblastoma are amongst the most aggressive brain tumors. Despite intensive research for new therapeutical options the mean survival time is still between 12 to 15 months. Like in other solid tumors, changes in the intercellular (pHi) and extracellular (pHe) pH compared to non-malignant tissue can also be observed in glioblastoma. In tumors the pHi is in the alkaline range while the pHe is more acidic. These differences in pH facilitate the development and progression of tumors and increase uncontrolled proliferation and evolution of resistances as well as infiltration of surrounding tissue. PH-regulatory transporters like the sodium-proton exchanger (NHE), the monocarboxylate transporters (MCT) and sodium-dependent chloride-bicarbonate exchanger (NDCBE) are responsible for the maintenance of the pHi. These transporters are important regulators of the pHi in both healthy and malignant cells and show higher expression and activity in tumors. Aim of this doctoral thesis is the analysis of the expression of the transporters NHE1, NHE5, MCT1, MCT4, MCT5 and NDCBE in human glioblastoma samples. Furthermore the influence of anti-tumoral compounds (cytostatics, non-steroidal anti-inflammatory drugs - NSAIDs) on the regulation of these transporters in human glioblastoma cell lines LN18 and U87MG is to be researched. On mRNA-level an increased expression of NHE1, MCT1, MCT4 and MCT5 in human glioblastoma tissue compared to non-malignant tissue could be observed. Moreover MCT4 expression in grade-IV-glioblastoma was higher than in grade-I-III-glioma. Furthermore, interindividual differences in expression of NHE1, NHE4 and MCT5 were detected. Patients older than the median age showed an increased expression of NHE1, NHE5 and MCT5, an increased expression of NHE5 and MCT5 was also observed in patients with shorter survival time than the median survival time. For the research on the impact of anti-tumoral compounds on the expression and activity of the transportes, cyctostatics and NSAIDs were selected. Teniposide was the only cytostatic showing effects on the expression of pH-regulatory transporters and on the activity of NHE1 and MCT1. Moreover teniposide led to a decreased phosphorylation of NHE1, a fact that could explain the reduced transport activity. Furthermore protein expression of the 52-kDa-NHE1 variant 2, possibly formed by alternative splicing, was raised by teniposide. This NHE1-variant was located in the mitochondria to a great extent. In transport studies with lactate an influence of teniposide on the activity of MCT1 could be observed. Whether this is caused by direct interaction of teniposide with MCT1 remains a subject of further investigation. Additionally teniposide lowered pHi and cell viability. The NSAID diclofenac also reduced cell viability and NHE1 activity independent of NHE1-phosphorylation. After treatment with celecoxibe an altered intracellular localization of NHE1- and LAMP2-positive signals was observed. The accumulation of these structures in the proximity of the nucleus and the reduced cell viability could indicate the induction of autophagocytosis by celecoxibe. Despite known anti-tumoral effect of ibuprofen in several in vivo and in vitro studies, interestingly this NSAID increased the viability of glioma cells. Expression and activity of the transporters were not influenced. Based on this results an intercranial murine glioma model using the murine glioblastoma cells GL261-GFP was established at the institute of pharmacology for further studies of ibuprofen. In this glioma model an diminished tumor size was detected in ibuprofen treated mice compared to control mice. The discrepancy between in vitro and in vivo findings could be explained by the anti angiogenic effect of ibuprofen which is described in literature. The results of this doctoral thesis show first directions that in human glioblastomas pH regulatory transporters like NHEs and MCTs are distinguished for tumor progression and reaction to anti tumoral drugs. The influence of teniposid and the NSAIDs on transporter expression and transporter activity could be important for the anti tumoral potency of this drugs.
