Untersuchung von Return-to-Play-Kriterien nach vorderer Kreuzbandrekonstruktion - Vergleich zwischen Semitendinosus- und Hamstringtransplantat-

  • Die Diskussion über die Rückkehr zum Sport nach vorderer Kreuzbandplastik wird in der Literatur aktuell kontrovers geführt. Der ideale Zeitpunkt und die Kriterien der Wiederaufnahme der sportlichen Aktivität nach Rekonstruktion des vorderen Kreuzbandes sind bis heute nicht eindeutig geklärt. Ziel dieser Arbeit war es zu untersuchen, ob sich die eigens entwickelte Testbatterie von Sprungtests als Kriterium hierfür eignet. Des Weiteren sollte das Outcome von Patienten nach Rekonstruktion des vorderen Kreuzbandes mittels entweder Semitendinosus- oder Hamstringtransplantat verglichen werden. Hierzu wurden insgesamt 97 Patienten untersucht. Die Studienteilnehmer wurden in zwei verschiedene Patientengruppen eingeteilt. In Gruppe 1 (n=45) wurden jene Patienten eingeteilt, die keine Knieverletzungen hatten. Es handelt sich hierbei um eine gesunde Kontrollgruppe. In Gruppe 2 (n=52) wurden alle Patienten erfasst, die nach Ruptur des vorderen Kreuzbandes eine Kreuzbandrekonstruktion erhalten hatten. Innerhalb der zweiten Gruppe erfolgte je nach erhaltenem Transplantat noch eine Zuordnung in zwei Untergruppen: Gruppe 2A (n=28) wurden dabei jene Patienten zugeordnet, die ein Semitendinosustransplantat erhielten, während Gruppe 2B (n=24) aus Patienten mit Hamstringtransplantat gebildet wurde. Die Patienten mit Kreuzbandrekonstruktion wurden im Rahmen der vorliegenden Studie das erste Mal in der 10. postoperativen Woche untersucht. Eine weitere Nachuntersuchung erfolgte nach einem mittlerem Follow-up von 13,6 Monaten (6-41 Monate) nach der Kreuzbandrekonstruktion. Zum Vergleich der beiden Gruppen wurden subjektive Beurteilungen des Knies, instrumentelle Stabilitätsmessungen (KT-1000-Arthrometer), Funktionsscores sowie Kraftmessungen und eine eigens entwickelte Testbatterie von Sprungtests herangezogen.
  • The discussion about the return to sport after anterior cruciate ligament plastic is currently controversial in the literature. The ideal time and the criteria of the resumption of sports activity after reconstruction of the anterior cruciate ligament are still not clear. The aim of this work was to investigate whether the specially developed test battery of jump tests is suitable as a criterion for this. Furthermore, the outcome of patients after reconstruction of the anterior cruciate ligament using either semitendinosus or hamstring grafts was compared. For this purpose, a total of 97 patients were examined. The study participants were divided into two different patient groups. In group 1 (n = 45) those patients were classified who had no knee injuries. It is a healthy control group. In group 2 (n = 52), all patients were recorded who had undergone cruciate ligament reconstruction after anterior cruciate rupture. Within the second group, assignment into two subgroups was still possible, depending on the transplant received: Group 2A (n = 28) was assigned to those patients who received a semitendinous transplant, while group 2B (n = 24) was formed from patients with hamstring transplantation. Patients with cruciate ligament reconstruction were examined for the first time in the 10th postoperative week in this study. Another follow-up was done after a mean follow-up of 13.6 months (6-41 months) after cruciate ligament reconstruction. For the comparison of the two groups subjective assessments of the knee, instrumental stability measurements (KT-1000-Arthrometer), functional scores as well as force measurements and a specially developed test battery of jump tests were used.
