Langzeitergebnisse und beeinflussende Faktoren nach der Laserinduzierten Thermotherapie (LITT) hepatischer Metastasen kolorektaler Karzinome

  • Zweck: Bestimmung Langzeitüberleben nach LITT hepatischer Metastasen kolorektaler Karzinome und das Outcome beeinflussende Faktoren. Besonders hervorzuheben ist die einzigartige Vergleichbarkeit auf Grund eines durchführenden Interventionalisten. Abschließend wurde hinterfragt, ob eine LITT weiterhin eine palliative Therapieoption ist, oder eventuell parallel zum derzeitigen Goldstandard der operativen Resektion eingesetzt werden könnte. Material und Methoden: Diese retrospektive Studie umfasste 119 Patienten. Die Behandlungen via LITT fanden von 07/97 bis 10/13 statt. 95 Patienten waren männlich, 24 Patienten weiblich. Der Hauptanteil der Patienten war zwischen 61 und 70 Jahre alt. Betrachtet wurden das Langzeitüberleben nach LITT in Monaten, sowie beeinflussende Faktoren darauf. Hierbei wurden in beeinflussbare Faktoren (Applikatorenzahl, Leistung, Zeit der Leistung und Gesamtmenge der LITT), sowie unbeeinflussbare Faktoren (Alter, Geschlecht, Komorbiditäten nach ASA und synchrone/metachrone Metastasen) unterschieden. Zum Zeitpunkt der Datenabfrage waren 110 Patienten (92,4%) verstorben. Zur statistischen Auswertung der Überlebenszeitanalyse wurde das kaplan- Meier- Verfahren genutzt. Ergebnisse: Die Mittlere Überlebenszeit betrug 36 Monate (Median 27 Monate), die 1-, 3- und 5- Jahresüberlebensrate (JÜR) betrug 80,67%, 33,61% und 11,76%. Statistisch signifikant waren Einflüsse auf die Mittlere Überlebenszeit bei den unterschiedlichen Komorbiditäten nach ASA, zwischen Patienten mit und ohne extrahepatische Metastasen, zwischen den unterschiedlichen Wattzahlen und hinsichtlich der Gesamtmenge der Behandlungen via LITT. Die höchste Mittlere Überlebenszeit zeigten Patienten der ASA- Klasse 1 (66,3 Monate), gefolgt von ASA 2 (38,3 Monate), ASA 3 (219 Monate) und ASA 4 (19,3 Monate). Die höchste 1 JÜR zeigte sich bei Patienten mit ASA 2 (87,5%), gefolgt von ASA 1 (76,5%), ASA 3 (66,7%) und ASA 4 (33,33%). Die 3- JÜR zeigte sich wie folgt: ASA 1 58,8%, ASA 2(31,3%, ASA 3 19% und ASA 4 33,33%). Die 5- JÜR zeigte sich in ASA 1 mit 35,3%, ASA 2 mit 17,9%, ASA 3 mit 4,76% und ASA 4 mit 0%. Patienten ohne extrahepatische Metastasen überlebten im Mittel 56,55 Monate nach LIT (Median 33 Monate). Der Anteil Überlebender bei Abfrage betrug 24%. Die 1-, 3- und 5- JÜR waren 80%, 48% und 24%. Patienten mit extrahepatischen Metastasen überlebten im Mittel nach LITT 32,67 Monate (Median 24 Monate). Der Anteil der Überlebenden betrug 6,7%. Die 1-, 3- und 5- JÜR waren 75,5%, 24,4% und 14,8%. Unterschiede im Outcome bestanden auch hinsichtlich der Wattzahl. Patienten, bei denen Leistungen unter 14 Watt eingesetzt wurden lebten nach LITT im Mittel 13,36 Monate. Die 1-, 3- und 5- JÜR betrug 50%, 7,1% und 0%. Patienten bei denen Leistungen von 14 Watt verwendet wurden lebten nach LITT im Mittel 40,26 Monate (Median 28 Monate). Hier betrug die 1-, 3- und 5- JÜR 77,8%, 28,9% und 15,5%. Patienten mit Leistungen über 14 Watt lebten im Mittel 37,33 Monate (Median 30 Monate). Hier lagen die 1-, 3- und 5- JÜR bei 88,7%, 41,5% und 127%. Patienten mit über 5 LITT´s lebten mit 56 Monaten Überlebenszeit im Median am längsten nach LITT. Die 1-, 3- und 5- JÜR betrug 75%, 75% und 50%. Patienten mit 5 LITT´s lebten im Median nach LITT 37 Monate. Hier lag die 1-, 3- und 5- JÜR bei 85,7%, 57,1% und 0%. Patienten mit 4 Ablationen lebten im Median 16 Monate nach LITT und die 1-, 3-und 5- JÜR lag bei 53,8%, 23,1% und 0%. Bei 3 Interventionen lebten Behandelte nach Therapie im Median 29 Monate und die 1-, 3- und 5- JÜR lag bei 92,9%, 28,6% und 7,1%. Bei 2 LITT´s überlebten Patienten im Median26 Monate und die 1-, 3- und 5- JÜR betrug 85,3%, 26,5% und 13,7%. Bei 1 LITT betrug die mediane Überlebenszeit 27 Monate, die 1-, 3-und 5- JÜR 78,7%, 34% und 16,8%. Die anderen untersuchten Ergebnisse zeigen Trends, waren jedoch nicht statistisch signifikant. Schlussfolgerung: Weniger schwere Komorbiditäten bedingen eine längere Überlebenszeit nach LITT. Sind zusätzliche extrahepatische Metastasen vorhanden, so verkürzt sich die Überlebenszeit nach LITT. Angewendete Leistungen unter 14 Watt führen ebenfalls zu kürzeren Überlebenszeiten. Es sollten also nur Leistungen von größer oder gleich 14 Watt angewendet werden. Eine multifokale hepatische Metastasierung ist keine Kontraindikation für eine LITT, Re-Ablationen sind möglich- auch ohne Verkürzung der Überlebenszeit. Die Durchführung durch nur einen Behandler stellt keinen Überlebenszeitvorteil dar, generiert jedoch eine einzigartige Vergleichbarkeit der Ergebnisse. Das Outcome der LITT in dieser Studie ist vergleichbar mit in der Literaturrecherche zu findenden Vorstudien. Sie kann als Alternative zur operativen Therapie angewendet werden.
  • Longterm survival after LITT of hepatic metastases of colorectal carcinomas and the influence of different factors. Unique is the comparability caused by the treatment made only by one therapist. One oft he questions was, if LITT only should be an palliative therapy, or better be used equal to surgery in the future. Material and methods: At this retrospective study data of 119 (95 males and 24 females, mean age between 61 and 70) patients with hepatic metastases of colorectal carcinomas and previous therapy with Laser Induced Thermotherapy (LITT) were evaluated concerning mean survival time and potential influencing factors. At the time of request 110 of 119 patients were dead. I distinguished between varaibale (number and power of applikators, time of power and total number of LITT) and non- variable factors (age, sex, comorbidities by ASA classification and synchronous/ metachronous metastases). For statistical evaluation I used the Kaplan- Meier- Method. Results: The median survival time amounted to 27 months (95% CI 29,8 months to 42,5 months). Collectively the range in survival time was from 2 up to 162 months. The overall- survival rates at 1-, 3- and 5 years were 80,67%, 33,61 % and 11,76 %. Statistically significant were the influence of the median survival time caused by different Komorbidities classified by ASA, between patients with or without extrahepatic metastases, between different power and between different total numbers of interventions. The longest median survival time could be found in the group of patients with comorbidities of ASA class 1 (66,3 months), followed by ASA 2 (38,3 months), ASA 3( 21,9 months) and ASA 4 (19,3 months). The highest 1- year overall survival rate was by ASA 2 with 87,5%, followed by ASA 1(76,5%), ASA 3 (66,7%) and ASA 4 (33,33%). The 3- year overall survival rates are the following: ASA 1 58,8%, ASA 2 31,3%, ASA 3 19% and ASA 4 33,33%. The 5- year overall survival rates are the following: ASA 1 35,3%, ASA 2 17,9%, ASA 3 4,76% and ASA 4 0%. The mean survival time for patients without extrahepatic metastases amounted 56,66 months (median survival time 33 months). The share of surviving was about 24%. The 1-, 3- and 5- year overall survival rates were 80%, 48% and 24%. The mean survival time for patients with extrahepatic metastases amounted 32,67 months after LITT (median survival time 24 months). The share of surviving was about 6,7%. The 1-, 3- and 5- year overall survival rates were 75,5%, 24,4% and 14,8%. In the group of patients treated with a power lower than 14 Watt the mean survival time was 13,36 months (median survival time 28 months). Here the 1-, 3- and 5- year overall survival rates accounted for 50%, 7,1% and 0%. Patients treated with a power of 14 Watt had a mean survival time of 40,26 months (median survival time 28 months). Here the 1-, 3- and 5- year overall survival rates accounted for 77,8%, 28,9% and 15,5%. Patients treated with a power over 14 Watt had a mean survival time of 37,33 months (median survival time of 30 months). Here the 1-, 3- and 5- year survival time accounted for 88,7%, 41,5% and 12,7%. Patients with a total number of more than 5 LITT interventions had the longest median survival time of 56 months. The 1-, 3- and 5- year overall survival rates acounted for 75%, 75% and 50%. Patients with a total number of 5 LITT´s had a median survival time of 37 month after first LITT. Here I found a 1-, 3- and 5- year overall survival rate of 85,7%, 57,1% and 0%. Patients with 4 ablations lived after LITT in the median 16 month and the 1-,3- and 5- year overall survival rates were 53,8%, 23,1% and 0%. For patients with 3 LITT´s a median survival time of 29 month after treatment was found. The 1-, 3- and 5- year overall survival rates were 92,9%, 28,6% and 7,1%. Patients with 2 LITT´s lived in the median after LITT 26 months and the 1-, 3- and 5- year overall survival rates were 85,3%, 26,5% and 13,7%. The treatment with 1 LITT accounted a median survival time of 27 month after LITT and the 1-, 3- and 5- year overall survival rates has been detected with 78,7%, 34% and 16,8%. The evaluation of the other survey results showed trends, but were not statistically significant. Conclusion: Less severe complications determine for a longer mean survival time after first LITT treatment. Ancillary extrahepatic metastases reduce the mean survival time of patients. Further a power less than 14 Watt reduces the mean survival time, too, and should not be used. The existance of multifocal hepatic metastases are not a contraindication of LITT. Re- ablation is possible and doesn´t reduce the survival time. The implementation of LITT made by only one therapist does not mean a benefit for patients outcome, but means a unique comparability of the results. The long term survival rate of patients in this study is at least equivalent to other studies. So, the LITT can be used as an alternative to surgery.
