Die Rolle des Immunsystems im Verlauf der akuten Pankreatitis im Mausmodell

  • Die Rolle des Immunsystems im Verlauf der Pankreatitis ist ein Zusammenspiel verschiedener Faktoren. Generell ist festzustellen das infiltrierende Leukozyten im Verlauf der Pankreatitis einen direkten Einfluss sowohl auf den pankreatischen Schaden als auch auf den systemischen Schaden haben. Es konnte gezeigt werden, dass die Pankreatitis in CD18 defizienten Mäusen zeitlich einen anderen Verlauf nimmt im Vergleich zu den Kontrolltieren. Mit der Verschiebung der Infiltration der Leukozyten verschiebt sich auch die Proteaseaktivierung, bzw. die Entstehung des pankreatischen Schadens. Neutrophile als auch Makrophagen/ Monozyten transmigrieren beide CD18 abhängig in das Pankreas und sind in den späteren Zeitpunkten für die Proteaseaktivierung in gleichem Maße verantwortlich. Die Depletion der Neutrophilen Granulozyten als auch die Depletion von Makrophagen/Monozyten vor Induktion der Pankreatitis führt in gleicher Weise zu einer Verminderung des Schweregrades der Erkrankung in den Tieren. Nicht nur in vivo konnte ein Effekt von Leukozyten auf den Verlauf der Pankreatitis gezeigt werden, auch in vitro zeigen Leukozyten einen direkten Effekt auf den Azinuszellschaden sowie auf die intrazelluläre Proteaseaktivierung. Makrophagen als auch Neutrophile zeigen diesen direkten Effekt auf Azinuszellen. Die Depletion von CD4 positiven T-Zellen führt zu einer verminderten proinflamatorischen Antwort in den Tieren welche in einem milderen Verlauf der Pankreatitis resultiert, da weniger Zellen des angeborenen Immunsystems in den Pankreas transmigrieren. Ein von Leukozyten vermittelter Mechanismus der zu einem erhöhten Azinuszellschaden als auch zu einem Anstieg der Proteaseaktivierung führt ist die Freisetzung von TNFα. In isolierten Azinuszellen konnte gezeigt werden das TNFα einen direkten Effekt auf das Überleben der Zellen hat und zu einer Cathepsin B vermittelten Aktivierung von Trypsinogen führt. Cathepsin B knockout Tiere reagieren nicht auf eine Stimulation durch TNFα mit einer Aktivierung von Trypsinogen oder einer Azinuszellnekrose. Dies stellt eine therapeutische Möglichkeit in Aussicht. Durch den Einsatz eines TNFα spezifischen Antikörpers konnte der Azinuszellschaden der durch Leukozyten vermittelt wird als auch die Cathepsin B vermittelte Proteaseaktivierung signifikant gesenkt werden. Dies stellt eine neue Möglichkeit der Therapie der chronischen Pankreatitis dar, da sie in gleicher Weise wie der Morbus Crohn oder die rheumatoide Arthritis als chronische Entzündungsreaktion mit Hilfe von TNFα inhibierenden Antikörpern behandelt werden könnte.
  • Introduction: Pancreatitis is an inflammatory disease. Transmigrating leukocytes contribute directly to pancreatic damage and intracellular protease activation. Not only neutrophils have a direct effect on the severity of the disease, also macrophages transmigrate in a high number into the pancreas and trigger pancreatic damage. The role of macrophages during pancreatitis were studied in C57Bl6 mice compared to macrophage depleted mice. Also the direct effects of leukocytes especially of macrophages/monocytes on acinar cells were studied, especially protease activation and necrosis were observed. In these study we want to clarify the role of macrophages in disease development and severity of the pancreatitis. Methods: CD-18 and TNFα deficient mice were generated by targeted disruption of the CD18 gene. Macrophage depletion was achieved by i.p injection of clodronate containing liposomes. Acute pancreatitis was induced by 8 hourly injections of Caerulein (50µg/kg i.p.). Local and systemic damage was recorded by histology and serum amylase and lipase as well as trypsin and elastase activity. CCK-induced trypsin and elastase activation in living acini was quantitated with fluorogenic substrates, as well as Cathepsin B and Caspase 3 activity, necrosis was measured by propidium iodide exclusion. Macrophages as well as leukocytes were isolated from spleen and co-incubated with living acinar cells and treated with an TNFα specific antibody. Living acinar cells were also stimulated with Interleukin 1 beta and Tumor necrosis factor α. Results: Wild type animals, as well as CD18 knockout, macrophage depleted and TNF-α knockout mice develop acute pancreatitis. The severity of pancreatitis depends on transmigrating leukocytes, CD18-/- animals and macrophage depleted animals shows significant lower pancreatic damage and lower serum amylase and lipase, as well as intrapancreatic protease activation 8h after induction of pancreatitis. Also in living acinar cells leukocytes have a direct effect on intracellular protease activation, showed by Trypsin activity and the activity of Cathepsin B. Also acinar cell necrosis depends on leukocytes. Stimulation with TNFα results in an increased Caspase 3 activity and leads to a necrotic cell death with a Cathepsin B mediated Trypsin activation. Treatment of acinar cells with a TNFα Antibody resultes in a significant lower Protease activation and necrosis ratio in presence of activated leukocytes. Conclusion: Genetic deletion of the CD-18 gene reduces protease activation 8hr and also decreases severity. Depletion of circulating macrophages reduces also intrapancreatic protease activation cascade and the severity of disease. Also in vitro Leukocytes and macropahges mediate intracellular protease activation. A critical key Mediator in this process is TNFα, which leads to an increased cell death and may lead to pancreatic necrosis and protease activation. In contrast IL 1 beta shows no effects on acinar cell necrosis or protease activation. Macrophages produce and secrete high amounts of TNFα and may play an important role for development of pancreatitis. Anti-TNFα therapy is well established in Chron’s disease and rheumatoid arthritis and may also be a potent therapeutic opinion for chronic pancreatitis patients.
