Prüfung der mikrobioziden Wirksamkeit von Octenidindihydrochlorid im Prüfkörpertest

  • Hintergrund: Es sollte die mikrobiozide Wirksamkeit von Octenidin-Wundgelen in niedrigeren als den handelsüblichen Konzentrationen (0,005 % - 0,1 %) ermittelt werden. Da niedrigere Konzentrationen eine geringere Zytotoxizität aufweisen, kann so die Anwendbarkeit von Octenidin in der Therapie chronischer Wunden abgeschätzt werden. Zum Vergleich wurde ein Wundgel mit 0,02 % Polihexanidgehalt geprüft. Material und Methoden: Für die Studie wurde ein praxisnaher Prüfkörpertest (Phase 2 / Stufe 2) etabliert. Als Prüforganismen fanden S. aureus, S. epidermidis, E. faecium, P. aeruginosa, S. marcescens und C. albicans Verwendung. Die Testungen erfolgten ohne Belastung sowie mit einer dem physiologischen Wundsekret nahekommenden Belastung (MEM + 10 % FBS). Es wurden Einwirkzeiten zwischen 5 min – 24 h gewählt. Ergebnisse und Schlussfolgerung: Im praxisnahen Test zeigte sich, dass höhere Konzentrationen und längere Einwirkzeiten des antiseptischen Wirkstoffs notwendig sind, als aus Ergebnissen des quantitativen Suspensionstests hervorgeht. Octenidin 0,02 % erreichte unter wundsekretähnlicher Belastung nach 24 h eine ausreichende antiseptische Wirksamkeit (Reduktionsfaktor > 3 lg) gegen alle Testorganismen und übertraf Polihexanid 0,02 % deutlich an Effektivität (keine ausreichende Erregerreduktion von C. albicans, E. faecium). Mit dieser niedrigen Octenidinkonzentration wäre die Gewebeverträglichkeit bei der Wundbehandlung erheblich verbessert. Produkte auf Basis von 0,02 % Octenidin könnten somit in der Zukunft eine Alternative zur Behandlung chronischer Wunden darstellen.
  • Background: The antimicrobial efficacy of an octenidine-wound gel in lower than the commercially available concentrations (0.005 % - 0.1 %) should be determined. Because lower concentrations show a lower cytotoxicity, the benefit of octenidine in the treatment of chronic wounds can be estimated by this investigation. For comparison, a wound gel with 0.02 % polihexanide was tested. Material and methods: A practical disc carrier test (phase 2/step 2) was established for this study. The used test organisms were S. aureus, S. epidermidis, E. faecium, P. aeruginosa, S. marcescens and C. albicans. The tests were performed without challenge and with MEM + 10 % FBS, simulating wound fluid. Exposure times between 5 min - 24 h were checked. Results and conclusion: The practical test demonstrated that higher concentrations of antiseptics and longer exposure times are required than those determined in the quantitative suspension test. Octenidine 0.02 % reached (with wound fluid simulation) after 24 h exposure time a sufficient antiseptic efficacy (> 3 lg-reduction) against all tested organisms. Octenidine 0.02 % obviously exceeded polihexanide 0.02 % in efficacy (no sufficient reduction of C. albicans, E. faecium). With the concentration of 0.02 % the tissue compatibility increased significantly. Therefore, products with octenidine 0.02 % could be a reliable alternative for the treatment of chronic wounds in future.
