Untersuchung des Einflusses von Mitomycin C auf die Wundheilung respiratorischer Epithelzellen in vitro

  • In den oberen Atemwegen stellt die Wundheilung und insbesondere die Narbenbildung, welche zu Luftwegsstenosen führt, ein dauerhaftes Problem für das Ergebnis einer Operation dar. Jene Stenosen erfordern einen weiteren operativen Eingriff, welcher durch die Verwendung eines Medikaments zur Verhinderung überschießender Narbenbildung verhindert werden könnte. Mitomycin C könnte eine Bereicherung diesbezüglich darstellen, da es leicht intraoperativ anzuwenden ist. Untersucht wurde der Effekt einer Verletzung des respiratorischen Epithels der Zelllinie S9 und IB3 und die Wundheilung unter Anwendung von 62,5 pg/µl Mitomycin C. Dabei stellte die Zelllinie S9 normales respiratorisches Epithel dar, während die Zellen der Zelllinie IB3 Patienten mit einer zystischen Fibrose repräsentierten. Die Analyse der Größenveränderung der Wundfläche verdeutlichte, dass sich mit Mitomycin C die Wunde deutlich langsamer schloß. Mitomycin C hemmte demnach die Wundheilung. Im Vergleich der Zelllinien zeigten die IB3-Zellen eine deutlich verzögerte Reaktion als die S9-Zellen. Mittels Proteomanalyse konnte die Wundheilung in S9 und IB3 dargestellt werden. Die klinischen Unterschiede zwischen den Zelllinien spiegelten sich in den ermittelten Daten wider. Anhand der Proteomanalyse wurden sowohl bei der Untersuchung des Wundeffekts als auch des Medikamenteneffekts bei der Zelllinie S9 insgesamt 51 in ihrer Anreicherung signifikant veränderte Proteine und bei der Zelllinie IB3 insgesamt 43 signifikant veränderte Proteine ermittelt. Dabei konnten 15 Proteine in beiden Zelllinien als signifikant verändert identifiziert werden. Diese Proteine beeinflussen den Zellzyklus, die Apoptose, die DNA-Replikation und Zellproliferation, die Proteinbiosynthese, den Proteintransport und die Proteinbindung, die Zellmigration, die Zellstabilisierung und die Aufrechterhaltung der Zellform. Auffällig ist die zeitlich spätere Expression identischer Proteine bei der Zelllinie IB3 im Vergleich mit S9. Dies bestätigt erneut eine verlangsamte Reaktion der IB3-Zellen, wie sie bereits bei der Wundschlusskinetik beobachtet wurde. Eine Verwundung resultierte bei der Zelllinie S9 in der Expressionsänderung von Proteinen des Zellzyklus, der Apoptose, der Stressantwort, der Proteinbiosynthese, der Proteinfaltung, der DNA-Transkription und -Replikation. Die IPA-Netzwerkanalyse inklusive der Erstellung einer TopToxListe der signifikant veränderten Proteine bestätigte das Auftreten von oxidativem Stress, einer mitochondrialen Dysfunktion und der Hypoxie-induzierten Signalweiterleitung infolge einer Verletzung des Epithelrasens. Die Zelllinie IB3 hingegen zeigte eine Expressionsänderung von Proteinen der Apoptose, des Zellzyklus, der Translationselongation, der Proteinbiosynthese, der Proteinfaltung und der Zellproliferation. Netzwerkeinteilung und TopToxListe offenbarten, dass durch Verwundung bei der Zelllinie IB3 im Vergleich zur Zelllinie S9 die Pro-Apoptose zusätzlich vorherrschend war. Die Proteomanalyse zeigte Unterschiede beim Zusatz von Mitomycin C im Vergleich einer Wundsetzung eines ungestört proliferierenden Monolayers, welches die Notwendigkeit belegt, für die Evaluation von Medikamenten in der postoperativen Phase auch standardisierte Wundmodelle zum Einsatz zu bringen. Bei der Untersuchung der Zelllinie S9 wurde die Expression von Proteinen der Stressantwort, Apoptose, DNA-Replikation, des Zellzyklus, der Antwort auf DNA-Schädigung und des mitotischen Zellzyklus verändert. Bei der Zelllinie IB3 wurde ebenfalls eine Expressionsänderung von Proteinen mit Funktion in der Apoptose, der Medikamentenantwort, Signaltransduktion, Stress, Antwort auf Hypoxie und DNA-Schädigung, Zellzyklus, der DNA-Replikation und RNA-Verarbeitung verzeichnet. Bei den Zelllinien S9 und IB3 wurde aufgrund der IPA-Netzwerkanalyse deutlich, dass Mitomycin C eine mitochondriale Dysfunktion, oxidativen Stress, p53-Signaltransduktion, Hypoxie-induzierte Signalweiterleitung und eine Beeinflussung des Zellzyklus bewirkte. Insgesamt kann die vorgelegte Arbeit damit eine detaillierte Analyse der beeinflussten und unbeeinflussten Wundheilung reproduzierbar darstellen. Das verwendete Wundmodell ist demnach geeignet für zukünftige Untersuchungen. Mitomycin könnte modellhaft als effektiver Wundheilungsinhibitor im Vergleich zu neu entwickelten Substanzen eingesetzt werden. Mitomycin C zeigte in vitro eine effektive Hemmung der Wundheilung, vermittelt durch seine mitochondriale Wirkung und Begünstigung der Apoptose. Durch das längere Offenhalten der Wunde könnten eine überschießende Narbenbildung und Restenosen reduziert werden. Die Praxistauglichkeit der Anwendung zur Optimierung des Wundheilungsergebnisses muss nun anhand von Untersuchungen in vivo gezeigt werden. Die Untersuchung weiterer Zellarten, z.B. humaner Fibroblasten, sollte sich anschließen.
  • In the upper respiratory tract, the healing of wounds and in particular scar formation, which leads to airway stenosis, represents a persistent problem for the result of an operation. Stenosis requires a further surgical procedure, which could be prevented by the use of a drug for the prevention of excessive scarring. Mitomycin C could be an enrichment in this regard, since it is easy to use intraoperatively. The effect of injury of the respiratory epithelial cell line S9 and IB3 as well as the healing of wounds using 62.5 pg/µl mitomycin C was examined. The cell line S9 represents normal respiratory epithelium, while the cells of the cell line IB3 represent patients with cystic fibrosis. The analysis of the change of the woundsize made it clear that the wound treated with mitomycin C closed significantly more slowly. Mitomycin C therefore inhibited wound healing. A comparison of the cell lines revealed that IB3 cells showed a significantly delayed response than S9 cells. Through proteomic analysis, wound healing in S9 and IB3 cells could be shown. The clinical differences between the cell lines were reflected in the determined data. Based on the proteomic analysis a total of 43 significantly altered proteins in the cell line S9 and 51 in cell line IB3 were identified both in the study of wound effect and drug effect. In this study, 15 proteins were significantly expressed in both cell lines. These proteins affect the cell cycle, apoptosis, DNA replication and cell proliferation, protein synthesis, protein transport, protein binding, cell migration, cell stabilization and maintenance of cell shape. Striking is the delayed expression of identical proteins in the cell line IB3 compared with S9. This again confirms a slower reaction of IB3 cells, which has already been observed in the wound healing kinetics. An injury resulted in cell line S9 in a change of expression of proteins of the cell cycle, apoptosis, stress response, protein biosynthesis, protein folding, DNA transcription and replication. The IPA network analysis confirmed the occurrence of oxidative stress, mitochondrial dysfunction and hypoxia-inducible factor signaling due to wounding. The cell line IB3, however, showed a change in expression of proteins of apoptosis, cell cycle, translation elongation, protein biosynthesis, protein folding and cell proliferation. Network analysis revealed that pro-apoptosis was also identified due to wounding in the IB3 cell line compared to the cell line S9. Proteome analysis showed differences in the addition of mitomycin C compared to the wound healing of an undisturbed proliferating monolayer. This demonstrates the need of standardized wound models to evaluate the use of drugs in the postoperative period. The examination of the cell line S9 showed an expression of proteins of stress response, apoptosis, DNA replication, cell cycle, regulator of DNA damage and the mitotic cell cycle. In the cell line IB3 a change in the expression of proteins which function in apoptosis, drug response, signal transduction, stress response to hypoxia and DNA damage, cell cycle, DNA replication and RNA processing was recorded. In cell lines S9 and IB3, the use of mitomycin C resulted in mitochondrial dysfunction, oxidative stress, p53-signaling, hypoxia-inducible factor signaling and influences the cell cycle according to the IPA network analysis. Overall, the work presented a detailed analysis of affected and unaffected wound healing in a reproducible way. The used wound model is therefore suitable for future investigations. Mitomycin C could be used as an effective wound healing inhibitor compared to newly developed substances. Mitomycin C in vitro showed an effective inhibition of wound healing mediated by its mitochondrial action and favoring apoptosis. Through the delay of wound healing, an excessive scarring could be reduced and restenosis could be prevented. The practicality of the application for optimizing wound healing results must be shown by investigations in vivo. The investigation of other cell types, for example human fibroblasts should follow.
