The thesis is about ideological change of political parties and the way parties gather information, learn by updating their beliefs and ultimately make "rational choices". Analyzing 1451 policy moves of 137 parties in 22 OECD-countries from 1950 to 2013 it is a story about rational learning, about emulating other parties abroad and chasing public opinion. Yet, the "internal life" of a party conditions the effects when activists have some influence over the formation of party policy. As volunteers facing a scarcity of time and resources, members of the party on the ground have a different information horizon, and may arrive at the opposite decision where to move than party elites which (can) rest their decision on a broader set of information resources. In some parties the party on the ground thus constitutes an "internal wall of resistance" to the strategy party elites would choose, if they were free from constraints.
For the normal development of pregnancy, a balance between immune tolerance and defense is crucial. However, the mechanisms mediating such a balance are not fully understood. CD83 is a transmembrane protein whose expression has been linked to anti-inflammatory functions of T and B cells. The soluble form of CD83, released by cleavage of the membrane-bound protein, has strong anti-inflammatory properties and was successfully tested in different mouse models. It is assumed that this molecule contributes to the establishment of immune tolerance. Therefore, we postulated that the expression of CD83 is crucial for immune tolerance during pregnancy in mice. Here, we demonstrated that the membrane-bound form of CD83 was upregulated in T and B cells during allogeneic murine pregnancies. An upregulation was also evident in the main splenic B cell subtypes: marginal zone, follicular zone, and transitional B cells. We also showed that there was an augmentation in the number of CD83+ cells toward the end of pregnancy within splenic B and CD4+ T cells, while CD83+ dendritic cells were reduced in spleen and inguinal lymph nodes of pregnant mice. Additionally, B lymphocytes in late-pregnancy presented a markedly higher sensitivity to LPS in terms of CD83 expression and sCD83 release. Progesterone induced a dosis-dependent upregulation of CD83 on T cells. Our data suggest that the regulation of CD83 expression represents a novel pathway of fetal tolerance and protection against inflammatory threats during pregnancy.
A New Kind of Permutation Entropy Used to Classify Sleep Stages from Invisible EEG Microstructure

The present doctoral dissertation comprises new studies on the fossil vertebrate assemblage recovered from the late Early Jurassic marine “Green Series” clay deposits of Grimmen and Dobbertin in north-eastern Germany that contribute to fill the gap of knowledge regarding its faunal composition and its relevance for understanding Early Jurassic vertebrate life. The investigations led to the recognition of wide range of vertebrate taxa, including basal gravisaurian sauropods, secondarily marine reptiles, a diverse fauna of leptolepid fishes, and a new genus and species of pycnodontiform fishes. In addition, a taxonomic revision of the Early Jurassic saurichthyid fish Saurorhynchus was performed, leading to the identification of two new, previously unnamed species. The results provide new insights into the taxonomic, systematic, and ecological diversity of Early Jurassic vertebrates, and hence add significant new data to our knowledge on Lower Jurassic vertebrate palaeobiodiversity patterns.
Numerous signalling pathways orchestrate the development, the functions, and the survival of cells, mostly in response to external stimuli. An overwhelming amount of data supports the concept of specific, spatio-temporal redox signalling pathways that affect the redox state of protein cysteinyl side chains and thus the biological function of these proteins. Glutaredoxins (Grxs) and thioredoxins (Trxs) catalyse reversible thiol-disulphide exchange reactions. The cytosolic Grx2 isoform Grx2c is essential for brain development and axonal outgrowth. A reversible dithiol-disulphide switch of CRMP2 has been identified as one of the major targets regulated by Grx2c. This CRMP2 redox switch is toggled in neuronal differentiation. Reduction of CRMP2 thiols induces profound conformational changes, modifying interactions and downstream elements of this redox switch. In [article I] and [manuscript V], we identified the Cys504 of CRMP2 to be the redox regulated residue. We used various in vitro assays with recombinant protein and molecular dynamics simulations to characterise the conformational change. The changes involve the solvent accessible surface area of at least one known phosphorylation site at the C-terminus of the protein. In [article III], we analysed the function of Grx2 and Trx1 in a model for perinatal asphyxia. Trx family proteins exhibit a very complex, cell-type and tissue specific expression pattern following hypoxia/ischemia and reoxygenation, especially Trx1 and Grx2. The results imply the clinical relevance for both proteins in perinatal asphyxia as well as many other neurological disorders. In agreement with the results presented in [articleI], Grx2 may be required for the re-establishment of neuronal integrity and connectivity. Cell shape, all forms of intracellular transport, and cell movement depend on the cytoskeleton, particularly on the fine tuned complex regulation of the dynamic re-arrangement of actin filaments and microtubules. In [article IV], we discuss the redox regulation of this dynamic cytoskeletal remodelling. Taking recent discoveries into account, we focus on redox signalling mechanisms, e.g. reversible thiol and methionyl switches. These switches are specifically controlled by enzymes such as Trx1 and Grx2c, for instance, and not the result of random modification by unspecific oxidants. Methionyl sulphoxidation of actin can be reversed by methionyl sulphoxide reductase (MsrA), promoting actin polymerisation. Human cells express two different Msr enzymes (MsrA and MsrB), that can reduce S- and R-methionyl sulphoxide, respectively. In the gram-positive Streptococcus pneumoniae, on the other hand, both Msr genes and thus enzymes were fused during evolution. In [article II], we characterised the surface-exposed thioredoxin family lipoproteins Etrx1 and 2 and regulators of this Msr (SpMsrAB). A loss of function of both Etrx proteins or SpMsrAB dramatically reduced pneumococcal virulence, enhanced the bacterial uptake by macrophages, and accelerated pneumococcal killing by H2O2 or free methionine sulphoxide. Identification and characterisation of components of this redox regulated system may contribute to the design of new antimicrobials. In [manuscript VI], we investigated the effects of Grx2c expression on cell morphology, migration, and invasion behaviour of cancer cells. Grx2c expressing cancer cells developed dramatic changes in phenotype, including alterations in cytoskeletal dynamics and significantly increased motility and invasiveness. We used quantitative proteomics and phopshoproteomic approaches to characterise the underlying mechanisms. Proteins and pathways regulating cytoskeletal dynamics, cell adhesion, and receptor-mediated signal transduction were detected to be specifically altered. We started a clinical pilot study with patients suffering from clear cell renal cell carcinoma (ccRCC). Grx2c was expressed with significantly higher frequency in ccRCC compared to healthy kidney tissue, associated with a strong trend for locally more advanced tumour stages and a clear tendency for a decreased cancer-specific survival, compared to patients without detectable Grx2c. These results were supported by data from "The Cancer Genome Atlas". In synopsis, the results presented and discussed in these articles and manuscripts, support the concept of specific redox signalling in different models and model organisms. They also demonstrate the importance of the specific redox control of signalling pathways that, in the case of errors or misinterpretations, contribute to pathophysiological alterations. The regulation of the CRMP2 redox switch by Grx2c, for instance, is physiologically essential for brain development, but might lead to cancer progression, if "switched on" in adult tissue. Identification of further interaction partners as well as the development of compounds modulating this redox switch and CRMP2s conformations, will be part of our future research.
Prüfungsangst stellt eine schwerwiegende und häufig auftretende psychische Störung dar. In der klinischen Praxis war die Abgrenzung klinisch relevanter Prüfungsangst von subklinischer Prüfungsaufregung lange Zeit schwierig und die psychische Störung wurde uneinheitlich als soziale oder als spezifische Phobie kodiert, weil es an eindeutigen Diagnosekriterien mangelte. In den vergangenen Jahrzehnten intensiver Beforschung des Themenkomplexes Prüfungsangst, insbesondere durch die Pädagogische Psychologie, wurden vielfältige Variablen mit Prüfungsangst in Verbindung gebracht und als direkte oder indirekte Prädiktoren diskutiert. Bislang fehlte es jedoch an der Integration dieser unterschiedlichen Erklärungsansätze in ein geeignetes Rahmenmodell. Zunächst wurde untersucht, ob sich das „Test Anxiety Inventory“ (TAI) eignet, klinisch unauffällige von klinisch relevanter Prüfungsangst abzugrenzen. Dazu wurden eine Stichprobe 47 prüfungsängstlicher Patienten einer Psychotherapieambulanz und eine Gruppe von 41 Studenten mit gesunden Ausmaßen an Prüfungsangst verglichen. Dabei wurde auch untersucht, mittels welcher Diagnose die Prüfungsangst der Patienten von den behandelnden Therapeuten kodiert wurde und ob sich objektivierbare Unterschiede zwischen unterschiedlich klassifizierten Patienten finden lassen. Im zweiten Schritt wurden in Anlehnung an das Prüfungsangstmodell von Zeidner und Matthews (2007) die wichtigsten Prüfungsangstprädiktoren hinsichtlich ihrer prädiktiven Validität für die Unterscheidung pathologischer und gesunder Prüfungsangstintensitäten analysiert. Im dritten und letzten Arbeitsschritt wurde eine Stichprobe von 22 Prüfungsangstpatienten im Längsschnittverlauf einer kognitiv-verhaltenstherapeutischen Behandlung unter realistischen Therapiebedingungen betrachtet. Ziel war es dabei zu überprüfen, ob die bisherigen Erkenntnisse auch für die Vorhersage des Therapieerfolgs bedeutsam sind. Ein Cut-Off-Wert von 80 Punkten im TAI scheint sich zur Unterscheidung klinischer und nicht-klinischer Ausmaße an Prüfungsangst zu eignen. Das Krankheitsbild der untersuchten Prüfungsangstpatienten zeigt sich sehr einheitlich und ist unabhängig von der vergebenen Störungsdiagnose des Therapeuten. Das Vorliegen einer komorbiden depressiven Erkrankung beeinflusst nicht die Schwere der Prüfungsangst. Selbst bei Beachtung des Einflusses der grundsätzlichen psychischen Belastung ist eine Unterscheidung pathologischer und nicht-pathologischer Prüfungsangst anhand der Konstrukte Lernzielorientierung, Fähigkeitsselbstkonzept, Selbstbeschuldigung, Elaboration im Lernen und Perfektionismus möglich. Diese Variablen mit der höchsten diskriminierenden Validität entspringen allen drei Erklärungsebenen des Prüfungsangstmodells von Zeidner und Matthews, welches sich offensichtlich zur Untersuchung der Bedeutung der unterschiedlichen Prüfungsangstprädiktoren eignet. Im Rahmen der psychotherapeutischen Behandlung der Prüfungsangst kam es zwar insgesamt zur Reduktion prüfungsängstlicher, depressiver und sozialängstlicher Symptome sowie der grundsätzlichen psychischen Belastung, jedoch haben die Patienten sehr unterschiedlich auf die Behandlung angesprochen. Nahezu 50 Prozent der behandelten Betroffenen weisen auch nach dem Therapieende noch immer klinisch relevante Werte an Prüfungsangst und nur unerhebliche Verbesserungen der anderen interessierenden Variablen auf. Die Bedeutung der Variablen Elaboration, Lernzielorientierung, Fähigkeitsselbstkonzept und Selbstbeschuldigung bestätigt sich auch in der Längsschnittanalyse. Der empfohlene Cut-Off-Wert im TAI sollte in repräsentativen Stichproben repliziert und das Instrument konventionell zur Diagnostik von Prüfungsangst verwendet werden um die Identifikation pathologischer Prüfungsangst zu erleichtern und dem Screening sowie der Differentialdiagnostik der Störung zu dienen. Schwere und Generalisierungsgrad sozialängstlicher Symptome sollten in der Prüfungsangstdiagnostik stärker beachtet werden. Die Möglichkeit, Prüfungsangst wie im DSM-5 als Sozialphobie mit dem Spezifikator „Nur in Leistungssituationen“ zu diagnostizieren, sollte zukünftig auch im ICD Anwendung finden um die Kodierung der Prüfungsangst zu vereinheitlichen. Parallel vorliegende psychische Erkrankungen sollten frühzeitig im Verlauf der Diagnostik in ihrer Bedeutung als Ursache oder Folge von Prüfungsangst identifiziert werden um entsprechende Ableitungen für den Behandlungsplan vornehmen zu können. Die klinische Forschung sollte sich stärker auf das Prüfungsangstmodell von Zeidner und Matthews und bei Replikation unserer Ergebnisse auf die zentralen Prüfungsangstprädiktoren Lernzielorientierung, Fähigkeitsselbstkonzept, Selbstbeschuldigung, Elaboration und Perfektionismus konzentrieren. Entsprechende Behandlungsansätze sollten gezielt auf ihren Therapieeffekt hin untersucht werden. Zudem sollte genau analysiert werden, welche weiteren Faktoren es gibt, die über das Therapieansprechen entscheiden.
Die Herzinsuffizienz mit eingeschränkter LVEF führt zu einer Reduktion des aortalen AIx in Ruhe. Wie sich die zentralen Parameter bei diesen Patienten unter Belastung verändern, ist bisher wenig untersucht. Probanden mit einer linksventrikulären Pumpfunktion ≤ 40 % und NYHA ≥ 2 und ohne Vorhofflimmern (CHF) wurden mit Kontrollprobanden ohne Herzinsuffizienz (Con) verglichen, die für Alter, Geschlecht und Nierenfunktion individuell gematcht wurden. Eine oszillometrische Pulswellenanalyse (Mobil-O-Graph®) wurde im Liegen, im Sitzen und über die Dauer von zehn Minuten nach einer spiroergometrischen Ausbelastung durchgeführt. In Einklang mit der Studienlage sahen wir einen niedrigeren AIx bei der CHF-Gruppe vs. der Con-Gruppe im Liegen in Ruhe (28,4 ± 3,4 % vs. 19,9 % ± 2,8; p < 0,05). In sitzender Position ließ sich dies jedoch nicht reproduzieren. Unmittelbar nach der Belastung war die Pulswellengeschwindigkeit in der Con-Gruppe signifikant höher als in der CHF-Gruppe (9,09 ± 0,25 m/s vs. 8,45 ± 0,29 m/s, p < 0,05). Der AIx stieg gegenüber dem Ruhewert bei der CHF-Gruppe signifikant an und war im Vergleich zur Con-Gruppe höher. Ursächlich waren Unterschiede in der Amplitude der reflektierten Welle, welche in der Kontrollgruppe nach der Belastung im Vergleich zur Ruhemessung abnahmen, in der CHF-Gruppe jedoch nicht. Die Belastungsintoleranz bei herzinsuffizienten Patienten konnte möglicherweise durch den erhöhten AIx erklärt werden. Ursächlich hierfür war ein Anstieg der Amplitude der reflektierten Welle. Die vorliegende Arbeit liefert eine Einsicht in die komplexen hämodynamischen Auswirkungen von körperlicher Belastung bei Patienten mit Herzinsuffizienz
Less invasive caries management techniques for treating cavitated carious primary teeth, which involve the concept of caries control by managing the activity of the biofilm, are becoming common. This study aimed to compare the clinical efficacy (minor/major failures) and survival rates (successful cases without any failures) of 3 carious lesion treatment approaches, the Hall Technique (HT), non-restorative caries treatment (NRCT), and conventional restorations (CR), for the management of occlusoproximal caries lesions (ICDAS 3-5) in primary molars. Results at 2.5 years are presented. A total of 169 children (3- to 8-year-olds) were enrolled in this secondary care-based, 3-arm parallel-group, randomised controlled trial. Participants were allocated to: HT (n = 52; sealing caries with stainless-steel crowns without caries removal), NRCT (n = 52; opening up the cavity and applying fluoride varnish), CR (n = 65; control arm, complete caries removal and compomer restoration). Statistical analyses were: non-parametric Kruskal-Wallis analysis of variance, Mann-Whitney U test and Kaplan-Meier survival analyses. One hundred and forty-two participants (84%; HT = 40/52; NRCT = 44/52; CR = 58/65) had follow-up data of 1-33 months (mean = 26). Overall, 25 (HT = 2, NRCT = 9, CR = 14) of 142 participants (17.6%) presented with at least 1 minor failure (reversible pulpitis, caries progression, or secondary caries; p = 0.013, CI = 0.012-0.018; Mann-Whitney U test). Ten (HT = 1, NRCT = 4, CR = 5) of 142 participants (7.04%) experienced at least 1 major failure (irreversible pulpitis, abscess, unrestorable tooth; p = 0.043, CI = 0.034-0.045). Independent comparisons between 2 samples found that NRCT-CR had no statistically significant difference in failures (p > 0.05), but for CR-HT (p = 0.037, CI = 0.030-0.040) and for NRCT-HT (p = 0.011, CI = 0.010-0.016; Kruskal-Wallis test) significant differences were observed. Cumulative survival rates were HT = 92.5%, NRCT = 70.5%, and CR = 67.2% (p = 0.012). NRCT and CR outcomes were comparable. HT performed better than NRCT and CR for all outcomes. In dieser Arbeit wird die Auswirkung des p75NTR-Knockouts auf die Dichte cholinerger Fasern und die Morphologie dendritischer Dornen im Hippocampus altersabhängig untersucht. Der p75-Knockout zeigt eine gesteigerte cholinerge hippocampale Faserdichte bei unveränderte Spinemorphologie. Dabei wird in einer getrennten Knockoutlinie nachgewiesen, dass die Expression des S-p75NTR, einer trunkierten Isoform des p75NTR, diese procholinergen Knockouteffekte altersabhängig verschwinden lässt.
Im deutschen Gesundheitswesen stellt MRSA nicht nur im stationären, sondern auch im ambulanten Bereich eine Behandlungserschwernis dar. Patienten mit chronischen Wunden sind bei MRSA-Infektionen besonders gefährdet. Deshalb wurden als Ausgangslage für die vorliegende Arbeit die Forschungsfragen formuliert, ob a) eine ambulante MRSA-Sanierung bei Patienten mit chronischen Wunden durchführbar ist und b) die damit erzielbaren Fallerlöse die Praxiskosten decken. Zur Beantwortung der Frage wurden a) Berichtsdaten der Kassenärztlichen Bundesvereinigung an das Bundesministerium für Gesundheit b) Abrechnungsdaten der Kassenärztlichen Vereinigung Mecklenburg-Vorpommern und c) die Daten der MRSA-fokussierten MEracL-Studie (MRSA eradication for chronic lesions) analysiert. Auf Grund der Ergebnisse konnten fünf Kernaussagen formuliert werden: 1) Die Durchführung einer ambulanten Sanierungsbehandlung ist möglich. 2) Das Vorhandensein einer chronischen Wunde ist ein MRSA-Risikofaktor, aber kein Hemmnis für die Durchführung einer ambulanten Sanierungsbehandlung. 3) Die Akzeptanz der Ärzte hinsichtlich der Durchführung einer ambulanten Sanierungsbehandlung ist regional unterschiedlich. 4) Die errechneten Fallerlöse decken nicht die realen Praxiskosten. 5) Das Budget für Leistungen im Zusammenhang mit MRSA sollte sektorenübergreifend geplant und eingesetzt werden. Die vorliegende Arbeit unterstreicht die Wichtigkeit einer gesundheitsökonomischen Analyse bei der Einführung neuer Behandlungsmethoden.
Die unterschiedlichen Behandlungsstrategien bei Verletzungen der Achillessehne werden in Fachkreisen intensiv und teils kontrovers diskutiert. Vor diesem Hintergrund wurde vorliegend eine Exploration der gegenwärtigen Versorgung von Patienten mit Achillessehnenschädigungen angestrebt. Die realisierte Methodik beinhaltete die Entwicklung eines standardisierten Fragebogens, der Items zur entsprechenden Exploration umfasste. Dieser Fragebogen wurde an Krankenhäuser in Deutschland (Fachabteilungen der Bereiche Chirurgie mit Schwerpunkt Unfallchirurgie, Chirurgie mit Schwerpunkt Orthopädie, Orthopädie mit Schwerpunkt Chirurgie, sowie Unfallchirurgie) geleitet. Es konnte ein Fragebogen-Rücklauf von N=112 erzielt werden. Darüber hinaus erfolgte eine Auswertung von Krankenblättern von Patienten mit Achillessehnenrupturen (N=70), die im Johanniter-Krankenhaus in Genthin-Stendal unfallchirurgisch versorgt wurden.
Das empirisch gewonnene Datenmaterial wies insbesondere auf eine multimodale Diagnostik bei einem Verdacht auf Achillessehnenruptur hin. Bestätigte Abrisse lagen weit mehrheitlich im mittleren Bereich der Achillessehne vor. Eindeutig dominierten männliche Patienten sowie sportbedingte Rupturen. Häufig waren bereits vor dem Trauma Probleme im Sehnenbereich wie etwa Tendopathien aufgetreten. Die Daten legten einen den Achillessehnenabriss tendenziell begünstigenden Einfluss des Faktors Diabetes nahe. Als Behandlungsmodus bei Achillessehnenverletzungen war weit überwiegend das offene operative Vorgehen gegeben, wobei unter den Nahttechniken insbesondere den Verfahren nach Bunnell und Kirchmayr/ Kessler Bedeutung zukam. Gewisse Potentiale eines primär konservativen, nicht als Regelbehandlung eingesetzten Vorgehens wurden nach den empirischen Daten bei partiellen Rupturen der Achillessehne, namentlich unter der Voraussetzung einer hohen Patienten-Compliance, erkannt. Grundsätzlich dominierte nach den vorliegenden empirischen Daten bei Achillessehnentherapien die operative Strategie, wobei im Vergleich zu früheren Berichten in der Fachliteratur eine gewisse Zunahme der Akzeptanz auch gegenüber einer konservativen bzw. primär noninvasiven Therapie erkennbar war. Unabhängig von der gewählten Behandlungsstrategie bei Achillessehnenverletzungen kann gemäß den empirischen Rückmeldungen die Schlussfolgerung gezogen werden, dass eine zügig eingeleitete und in der Folge sukzessive verstärkte Mobilisierung des Sehnenbereiches in der Rehabilitationsphase dieser Patienten einen hohen Nutzwert hat.
Zusammenfassung
Zielsetzung: Ziel dieser Studie war es einerseits zu ermitteln, ob Frauen mit hohen Testosteron-Konzentrationen oder niedrigen Sexualhormon-bindendes GlobulinKonzentrationen vermehrt kardiovaskuläre Krankheiten (KVK) entwickeln und andererseits mögliche Assoziationen von Androgenen und Gesamt- und KVKspezifischer Mortalität in der weiblichen Allgemeinbevölkerung zu untersuchen.
Studiendesign und Methodik: Aus der Study of Health in Pomerania (SHIP) wurden Daten von 2129 Frauen (mittleres Alter 49,0 Jahre) mit einem mittleren Follow-up von 10,9 Jahren verwendet. Die Assoziationen von Sexualhormon-Konzentrationen (totales Testosteron (TT), Sexualhormon-bindendes Globulin (SHBG), Androstendion (ASD), freiem Testosteron (fT), freiem Androgene Index (FAI)) und kardiovaskulären Krankheiten (KVK), sowie KVK-spezifischer und Gesamtmortalität wurden im Quer- und Längsschnittdesign mittels multivariabel-adjustierten Regressionsmodellen analysiert.
Ergebnisse: Die Ausgangsmessung zeigte eine KVK-Prävalenz von 17,8% (378 Frauen). Im Follow-up war die KVK-Inzidenz 50,9 pro 1000 Personenjahre. Im altersadjustierten Model zeigte sich eine inverse Assoziation von SHBG und prävalenten KVK (Relatives Risiko pro Standardabweichungsanstieg: 0,83; 95% Konfidenzintervall: 0,74-0,93), diese Assoziation verschwand jedoch im multivariabeladjustierten Modell. KVK-Inzidenz und den Sexualhormon-Konzentrationen waren sowohl im alters-, als auch im multivariabel-adjustierten Regressionsmodell nicht signifikant assoziierte. Für keines der untersuchten Sexualhormone konnte eine Assoziation mit der Gesamtmortalität aufgezeigt werden.
Fazit: Zusammenfassend erbrachte diese populationsbasierte Kohortenstudie keine konsistenten Assoziationen zwischen Androgenen und KVK-Inzidenz oder Mortalität.
Antibodies recognizing complexes of the chemokine platelet factor 4 (PF4/CXCL4) and polyanions (P) opsonize PF4-coated bacteria hereby mediating bacterial host defense. A subset of these antibodies may activate platelets after binding to PF4/heparin complexes, causing the prothrombotic adverse drug reaction heparin-induced thrombocytopenia (HIT). In autoimmune-HIT, anti-PF4/P-antibodies activate platelets in the absence of heparin. Here we show that antibodies with binding forces of approximately 60–100 pN activate platelets in the presence of polyanions, while a subset of antibodies from autoimmune-HIT patients with binding forces Z100 pN binds to PF4 alone in the absence of polyanions. These antibodies with high binding forces cluster PF4-molecules forming antigenic complexes which allow binding of polyanion-dependent anti-PF4/P-antibodies. The resulting immunocomplexes induce massive platelet activation in the absence of heparin. Antibody-mediated changes in endogenous proteins that trigger binding of otherwise non-pathogenic (or cofactor-dependent) antibodies may also be relevant in other antibody-mediated autoimmune disorders.
Gegenstand der hier vorgestellten Arbeit ist die Beschichtung von Wundauflagen mit Zinkoxid- und Silberhaltigen antibakteriellen Schichten. Die Aufbringung der Schichten erfolgt dabei auf den Wundauflagen mittels Atmosphärendruckplasma. Die Matrix der Schichten besteht aus Siliziumdioxid, in welcher die Wirkstoffe eingelagert sind. Auf diesem Weg hergestellten Wundauflagen wurden hinsichtlich ihrer antibakteriellen Wirkung und zytotoxischen Eigenschaften charakterisiert. Ziele waren ein minimaler Einsatz an Wirkstoffen und die Nutzung eines modernen Beschichtungsverfahrens. Der zytotoxische Einfluss der Wundauflagen wurde an 3D-Hautmodellen im Vergleich zu den am Markt befindlichen Produkten validiert.
Background: The association of polyomaviruses BK and JC with other opportunistic infections and graft-versus-host disease (GvHD) in allogeneic stem cell transplantation is controversially discussed. Methods: We conducted a retrospective study of 64 adult patients who received their first allogeneic stem cell transplantation between March 2010 and December 2014; the follow-up time was 2 years. Results: Acute leukemia was the most frequent underlying disease (45.3%), and conditioning included myeloablative (67.2%) and nonmyeloablative protocols (32.8%). All patients received 10 mg of alemtuzumab on day -2 (20 mg in case of mismatch) as GvHD prophylaxis. Twenty-seven patients (41.5%) developed cytomegalovirus (CMV) reactivation. BKPyV-associated hemorrhagic cystitis was diagnosed in 10 patients (15.6%). Other opportunistic infections caused by viruses or protozoa occurred rarely (<10%). There was no association of BKPyV or JCPyV with CMV reactivation, Epstein-Barr virus reactivation, human herpes virus 6, or parvovirus B19 infection requiring treatment. There was a significant correlation of BKPyV-associated hemorrhagic cystitis with toxoplasmosis (p = 0.013). Additionally, there was a significant link of simultaneous BKPyV and JCPyV viruria with toxoplasmosis (p = 0.047). BKPyV and JCPyV were not associated with GvHD, relapse, or death. Conclusion: We found no association of BKPyV or JCPyV with viral infections or GvHD. Only the correlation of both polyomaviruses with toxoplasmosis was significant. This is a novel and interesting finding.
Einleitung
Die Ausscheidung zahlreicher Medikamente über die Nieren erfordert bei Patienten mit CKD (chronic kidney disease) eine sorgfältige Dosisanpassung um Nebenwirkungen zu vermeiden. Ziel der Studie war es zu untersuchen, wie gut die verordnete Medikation bei Patienten mit CKD an die Nierenfunktion angepasst ist, welche Medikamente häufig fehlverordnet werden und Prädiktoren für Fehlverordnungen zu identifizieren.
Methode
In einer Querschnittstudie in 34 Hausarztpraxen in Vorpommern wurde die Medikation von Patienten mit einer CKD ≥ Stadium 3 auf Grundlage der Fachinformation und zusätzlich unter Berücksichtigung von Empfehlungen von Fachgesellschaften ausgewertet. Die verordneten Medikamente wurden mittels ATCCode erfasst. Es wurden kontraindizierte und überdosierte Verordnungen unterschieden. Prädiktive Faktoren für Fehlverordnungen wurden mittels logistischer Regressionsanalyse untersucht.
Ergebnisse
589 Patienten (Ø 78 Jahre, 63 % weiblich) mit einer CKD ≥ Stadium 3 aus 34 Hausarztpraxen wurden eingeschlossen. Sie konnten in 52 % dem CKD Stadium 3a, 37 % dem CKD Stadium 3b und 10 % CKD Stadium 4 und 5 zugeordnet werden. Insgesamt wurden in den Medikationsplänen 5102 Verordnungen erfasst (94,6 % Dauermedikationen, 5,4 % Bedarfsmedikationen). Im Mittel nahm jeder Patient fast 9 verschiedene Wirkstoffe ein (Ø=8,66; SD=3,6). 4,2 % aller Verordnungen waren nach Auswertung der Fachinformationen in der gegebenen Dosierung kontraindiziert (2,1 %) oder überdosiert (2,1 %). Bei 173 Patienten (29 %) war mindestens eine Verordnung betroffen. Der Anteil der Fehlverordnungen sank unter Berücksichtigung von neueren Empfehlungen auf 3,5 %. Metformin, Methotrexat und Kaliumpräparate wurden häufig trotz Kontraindikation verordnet. Ramipril, Sitagliptin und Simvastatin in Kombination mit Ezetimib wurden häufig überdosiert. Wichtigste Prädiktoren für Fehlverordnungen waren CKD Stadium ≥ 3b und die Anzahl der verordneten Dauermedikamente.
Schlussfolgerung
Der Anteil der unangepassten Verordnungen war in Bezug auf die Gesamtmenge aller Verordnungen gering, trotzdem war ein Viertel aller Patienten von mindestens einer unangepassten Verordnung betroffen. Unter zusätzlicher Berücksichtigung von Empfehlungen von Fachgesellschaften sank die Zahl der Fehlverordnungen. Zur klinischen Relevanz der Fehlverordnungen fehlen aussagekräftige Daten und hier besteht weiterer Forschungsbedarf. Für ein besseres Monitoring der Verordnungsqualität von Medikamenten bei CKD in der Hausarztpraxis wird ein Konsens benötigt, der auf Fachinformationen, Empfehlungen von Fachgesellschaften und klinischer Relevanz basiert. Wegen des hohen Aufwands des Einzelabgleichs sollten sich Maßnahmen zur Qualitätssicherung auf Patienten mit CKD Stadium ≥ 3b, Patienten mit Polypharmazie und auf problematische Wirkstoffe konzentrieren.
Assoziation zwischen Thyreotropin und dem Lipidprofil bei Kindern und Jugendlichen in Deutschland

Zielstellung:
Es gibt noch keine populationsbezogene Studie, die den Zusammenhang zwischen Thyreotropin (TSH) und den Lipidparametern Gesamtcholesterin, Low-Density-Lipoprotein-(LDL)-Cholesterin, High-Density-Lipoprotein-(HDL)-Cholesterin und Triglyzeriden bei Kindern und Jugendlichen unter-sucht hat, trotz der möglichen Auswirkungen eines erhöhten Lipidprofils auf die Entstehung von kardiovaskulären Erkrankungen.
Teilnehmer und Methoden:
Es wurden Eigenschaften und Laborparameter von insgesamt 6 622 drei- bis zehnjährigen Kindern und 6 134 elf- bis 17-jährigen Heranwachsenden der Studie zur Gesundheit von Kindern und Jugendlichen in Deutschland (KiGGS) analysiert. Primäre Expositionsvariable dieser Querschnittsstudie war der mit einer Elektroluminiszenz-Methode gemessene TSH-Spiegel im Serum. Primäre Outcomevariablen waren die Gesamtcholesterin-, LDL-Cholesterin-, HDL-Cholesterin- und Triglyzeridspiegel im Serum, die über enzymatische Farbanalysen bestimmt wurden. Die Variablen wurden mithilfe von altersspezifischen Grenzwerten kategorisiert.
Ergebnisse:
Signifikante positive Assoziationen lagen bei Kindern zwischen Serum-TSH-Spiegeln und allen Nicht-HDL-Cholesterinspiegeln (Gesamtcholesterin-, LDL-Cholesterin- und Triglyzeridspiegeln) vor (β = 0,90, 95 %-Konfidenzintervall [KI]: 0,53 – 1,27; β = 0,78, 95 %-KI: 0,44 – 1,13; und β = 0,90, 95 %-KI: 0,52 – 1,27), gleichermaßen bei Jugendlichen (β = 0,90, 95 %-KI: 0,47 – 1,32; β = 0,67, 95 %-KI: 0,29 – 1,05; und β = 0,92, 95 %-KI: 0,49 – 1,35; p < 0,05). Stratifizierte Modelle deuteten darauf hin, dass diese Zusammenhänge besonders stark bei übergewichtigen und adipösen Kindern waren. Kinder mit erhöhten TSH-Spiegeln hatten im Vergleich zu Kindern mit TSH-Spiegeln im Referenzbereich höhere Nicht-HDL-Cholesterinspiegel.
Fazit:
Wie schon bei Erwachsenen bekannt, zeigt diese Studie bei Kindern und Jugendlichen einen positiven Zusammenhang zwischen den Serumspiegeln von TSH und Nicht-HDL-Lipiden. Längsschnittstudien könnten klären, ob ein erhöhter Serum-TSH-Spiegel, der von einem unvorteilhaften Lipidprofil begleitet wird, im Erwachsenenalter fortdauert.
Es erfolgte die Auswertung nach Implantation einer Hüfttotalendoprothese bei geriatrischen Patienten mit einer Schenkelhalsfraktur. Es wurden retrospektiv Daten ausgewertet, welche den stationären Aufenthalt beschreiben. Zielkritieren waren u.a. Mobilität, Outcome in Abhängigkeit der ASA Klassifikation, Zusammenhang zwischen Letalität und ASA Klassifikation sowie die Erfassung von Komplikationen. Desweiteren wurde der modifizierte Harris Hip Score erhoben. Die Ergebnisse wurde mit aktueller Literatur verglichen.
Das Ziel dieser Studie war es, die Wirkung von kaltem Atmosphärendruckplasma und Natriumhypochlorit, allein oder in Kombination, auf einen Enterococcus faecalis Biofilm in Wurzelkanälen von extrahierten Zähnen zu vergleichen. Die antibakterielle Wirksamkeit wurde durch Ermittlung des Logarithmus der koloniebildenden Einheiten pro Milliliter bestimmt (log10CFU/ml). Zusätzlich wurden rasterelektronenmikroskopische Aufnahmen angefertigt, um den Behandlungseffekt auf den Biofilm zu visualisieren. Die adjuvante Behandlung mit NaOCl + Argon-O2-Plasma hat im Vergleich zur 12 min Monotherapie mit Natriumhypochlorit einen besseren, wenn auch statistisch nicht signifikanten Effekt auf die Keimreduktion in einem Enterococcus faecalis Biofilm. Die Behandlung mit Argon-O2-Plasma allein zeigt eine signifikante Verbesserung der Wirksamkeit bei einer Verlängerung der Behandlungszeit von 6 auf 12 min, blieb aber hinter der Keimreduktion nach Behandlung mit NaOCl + Argon-O2-Plasma zurück.
