40-Year Longitudinal Caries Development in German Adolescents in the Light of New Caries Measures

This study assessed the 40-year longitudinal caries development in German adolescents in the light of the sixth National Oral Health Survey in Children (NOHSC, 2016) employing initial DMFT (IDMFT), Significant Caries Index (SiC) and Specific Affected Caries Index (SaC). On the basis of the current NOHSC (randomized cluster selection using school list or regional community school surveys, 55,956 12-year-old sixth-graders examined by 482 calibrated community/study dentists) DMFT, SiC, a novel IDMFT including initial lesions (IT) and the recently introduced SaC were calculated and also recalculated for national and international surveys from the last 4 decades. In 2016, 78.8% of children were caries-free (DMFT = 0), 65.5% including IT lesions. The mean DMFT was 0.44 (single components: DT = 0.14, MT = 0.02, FT = 0.29, IT = 0.52) showing a clear association with the school type as marker for the socio-economic status. The mean number of affected teeth in children with DMFT >0 was 2.07 (SaC) in comparison to almost 9 teeth in the 1970s. The current care index on the tooth level was 66.3%, leaving only 7.7% of children with restorative treatment needs. Longitudinally, a continuous caries decline of more than 80%, including the risk groups (SiC/SaC), to an internationally extremely low level was observed. In conclusion, the National Oral Health Surveys reveal a continuous caries decline to a very low caries level in 12-year-old 6th-graders in Germany even if IT lesions are included (IDMFT). In spite of proportional reductions in the risk groups (SiC/SaC), the polarized caries distribution according to socio-economic parameters reveals the need for targeted preventive programmes.
In the present work high density helicon plasma discharges are created and characterized as a promising concept towards the realization of plasma wakefield accelerators to build up electric fields in the order of GV/m to accelerate electrons to energies in the TeV range with proton driving bunches. For such a concept plasma sources are needed that are able to maintain discharges with plasma densities of n_e = 7E20 m^-3 over long distances with a low variation in plasma density. Measurements at the PROMETHEUS-A device are performed for variable parameters, like magnetic induction, RF heating power and filling gas pressure. A CO2 laser interferometer, a laser induced fluorescence (LIF) diagnostic and a reaction rate model are combined to give a full picture. It is shown that in most cases the plasma density is centrally peaked with a high density region +- 5 mm from the center. The peak plasma density increases with increasing filling gas pressure, RF heating power and magnetic induction, limited by the number of neutral particles in low pressure discharges, by the transferred heating power and the increasing recombination and electron quenching rates of argon ions in high filling pressure cases. The increase in plasma density with increasing magnetic induction correlates to the direct proportionality in the helicon dispersion relation. For all investigated operational parameters the time evolution of the helicon discharge shows the same characteristics and is reliably reproducable inside the error bars. The electron temperature is determined by combining the collisional radiative model with line ratio measurements of two spontaneously emitted LIF lines. The low electron temperature regime of 1.2 eV < T_e < 1.4 eV and the electron temperature profiles are consistent with helicon wave heating via collisional power dissipation. The maximum plasma density of n_e = (6 +- 1)E20 m^-3 is measured at high RF power of P_RF = 24 kW, p_0 = 9 Pa filling gas pressure and a magnetic induction of B = 105 mT with a maximum electron temperature at 1.4 eV. At these operational parameters the plasma density peaking time and width are determined to be 270E-6 s and 50E-6 s, respectively. This shows that specific plasma density requirements for the use of a wakefield accelerator are reachable and the duration of the peak plasma density is more than sufficient for a relativistic particle to pass a 1 km long plasma cell. Additionally time-resolved LIF profile measurements for neutral and singly ionized argon were conducted to complement the previously evaluated measurements. The time resolution of the LIF diagnostic was chosen in a way to adequately represent the evolution of densities and to allow full profile measurements over one day. A resolution of 200E-6 s was chosen. The time-resolved neutral and ion metastable densities show hollow profiles with high densities at the edges over the first ms indicating higher ionization levels and increasing electron quenching rates. The metastable densities are highly determined by electron temperature, RF heating power and filling neutral gas pressure and do not reflect the neutral argon evolution. To investigate the influence of neutral depletion on the density evolution and maximum plasma density, the argon neutral and ion ground state densities are determined. Both time-resolved density profiles show a hollow profile with highest densities at the edges over a longer time interval of 3-4 ms. The penetration depths (ionization mean-free paths) indicate increased ionization of neutral argon while dissipating inwards, corresponding well to the theoretical value of lambda = 20 mm. This results in a depletion of neutrals in the center of the discharge, leading to a limitation and a fast decrease of plasma density after the neutrals are partially ionized. The shown refilling effect of neutral argon is too slow to have an important impact. At operation parameters for highest plasma density, the calculated ground states also show a fast increase in density at the end of the discharge after the RF-heating is switched off. This indicates recombination effects to these atomic states and higher ionization levels than ArII in the helicon discharge.
Understanding the fundamental mechanisms in the extracellular matrix of cells (ECM) is crucial for the development of drugs and biomaterials. Therefore, an atomistic model of the extracellular matrix is a cost-efficient way to observe influences of drugs, test the effect of mutations or misfolds in proteins or study the properties of fibril or network-forming peptides.
With this thesis, a refined molecular model of an adhesion complex is proposed that contains collagen, fibronectin and the cell receptor integrin. During the building of the model, major new insights are given for each of these proteins and a powerful protein-folding algorithm is
developed.
This thesis describes experiments with clusters stored in an electrostatic ion trap called Multi-reflection time-of-flight (MR-ToF) analyzer. These devices are established as mass separators and analyzers with high resolving powers and fast processing times. The objective was to characterize an experiment that utilizes such analyzer for cluster research, to this end a laser-ablation ion source was combined with an MR-ToF analyzer.
In the first part, an experiment scheme that combines two operating modes, namely in-trap lift operation and mirror operation, is presented and characterized for the present setup. For ion capture in-trap lift switching was employed and exit-side mirror switching for ejection with higher information content. Measurements were performed with small lead clusters to illustrate individual advantages of both techniques and the gain of combining them with focus on the ions’ ToF ejection window.
In the second part, a recently introduced method of ion separation by transversal ejection of unwanted species inside the trap was studied for the present setup. The ejection is performed by appropriate pulses of the potentials of deflector electrodes located in the trap. The various parameters affecting the selection effectivity and resolving power are illustrated with tin-cluster measurements, with resolving powers of up to several tens of thousands.
The third part presents the experiment in detail, with the construction of each component and measurements for its various performance parameters. Because the heart of the setup is the MR-ToF analyzer the characterization focuses on the trap. In addition, cluster ions were mass selected in the MR-ToF device and photodissociated. The charged fragments were stored and mass analyzed in a proof-of principle MS/MS experiment where both MS steps were performed in the MR-ToF operation mode.
For a long time the apocryphal Ladder of Jacob was accessible only in arbitrarily selected translations. Without a critical edition and a comprehensive study of the whole textual segment, scholars were unable to evaluate its significance for Early Jewish and Christian literature. Since 2015/17, with the publication of a new critical edition and German translation (accompanied by a detailed introduction, footnote commentaries and appendices with related texts), a new approach to this important but hitherto widely unknown text has been made possible. This approach verifies the different layers or strata in the text, which are: a supposed Jewish apocalypse (mid-second century), a Christian expansion of the angels speech in light of the praeparatio evangelica tradition (fourth–seventh centuries), a Jewish mystical prayer (eleventh century) and the incorporation of this narrative block into the Tolkovaja Paleja together with a series of exegetical commentaries (end of the thirteenth century). In the light of the new approach, it can be said that the Ladder of Jacob is most of all an outstanding example of mutual relations between Jewish and Christian theology.
Hyperoxia is a well-known cause of cerebral white matter injury in preterm infants with male sex being an independent and critical risk factor for poor neurodevelopmental outcome. We investigated the underlying mechanisms behind such a sex dependent difference in oligodendrocyte progenitor cells (OPCs). Our findings demonstrate that oxidative stress severely affects cellular functions related to energy metabolism, stress response, and maturation in male derived oligodendrocyte progenitor cells (OPCs) whereas the female cells remain largely unaffected. This impairment of maturation is accompanied by the downregulation of nucleoporin and nuclear lamina proteins. We identify Nup133, which regulates OPC maturation as a major target protein affected by hyperoxia in male cells and that this differential response is mediated by an inverse Nup133 regulation in the male and female cells. It also regulates mitochondrial function and oxidative stress response through its downstream target Nuclear respiratory factor 1 (Nrf1). Additionally, the presence of 17-β estradiol and higher amounts of fetal zone steroids (precursors for maternal estrogen synthesis during fetal development) confer resistance to the female cells mediated by the estrogen receptor alpha (ERα) along with Nup133. Both Nup133 and ERα regulate mitochondrial function and oxidative stress response by transcriptional regulation of Nrf1. These findings establish prominent sex based differences and the molecular mechanisms involved in differential response of OPCs towards oxidative stress and the important role of Nup133 in mediating a severe negative outcome in the male cells.
Microbial cell factories have been largely exploited for the controlled production of recombinant proteins, including industrial enzymes and biopharmaceuticals. The advent of high-throughput ‘-omics’ techniques have boosted the design of these production systems due to their valuable contribution to the field of systems metabolic engineering, a discipline integrating metabolic engineering with systems and synthetic biology. In order to thrive, the field of systems metabolic engineering needs absolute proteomics data to be generated, as proteins are the central players in the complex metabolic and adaptational networks. Due to advent of mass spectrometry-based proteomics, a substantial amount of absolute proteomic data became available in the past decade. However, membrane proteins remained inaccessible to these efforts.
Nonetheless, comparative studies targeting the membrane proteome have been quite successful in characterizing physiological processes. Hence, label-free proteomics was used in a study (Quesada-Ganuza et al, 2019 – Article I) to identify and optimize PrsA in Bacillus subtilis, for improved yield of amylase. Amylase is one of the most relevant enzymes in the biotechnological sector. By employing a label-free mass spectrometry approach targeting the membrane proteome of this bacterium, relative changes in heterologous and native levels of PrsA could be quantified. The results of this study evidenced that each PrsA shows different relative abundancies, but with no relevant impact in the yield of amylase.
Even though relative protein quantification can already provide a good visualization of the physiological changes occurring between different conditions, they are not sufficient to understand how resources are allocated in the cell under certain physiological conditions. Therefore, a global method for absolute membrane protein quantification remains the biggest requirement for systems metabolic engineering.
Hence, with this work, we successfully developed a mass spectrometry-based approach enabling the absolute quantification of membrane proteins (Antelo-Varela et al, 2019 – Article II). This study was also performed in the Gram-positive model organism Bacillus subtilis, regarded as a prolific microbial cell factory. The method developed in this work combines the comprehensiveness of shotgun proteomics with the sensitivity and accuracy of targeted mass spectrometry. Fundamental to the method is that it relies on the application of a correction and an enrichment factor to calibrate absolute membrane protein abundances derived from shotgun mass spectrometry. This has permitted, for the first time reported, the calculation of absolute membrane protein abundances in a living organism.
The newly developed approach enabled to accurately quantify ~40% of the predicted proteome of this bacterium, offering a clear visualization of the physiological rearrangements occurring upon the onset of osmotic stress. In addition, this work also provides evidence for new membrane protein stoichiometries.
Overall, this study enabled the development of a straightforward methodology long-needed in the scientific and biotechnological community and, for the first time reported, providing absolute abundances of one of the most puzzling fractions of the cell – the membrane proteome.
The next step of the work summarized here was to implement the afore described method to a biotechnological relevant strain, as absolute membrane protein abundances are essential to understand the fundamental principles of protein secretion and production stress. Hence, this work was applied in a genome-reduced B. subtilis strain, ‘midiBacillus’, expressing the major staphylococcal antigen IsaA (Antelo-Varela et al, submitted – Article III). The employed absolute membrane protein quantification methodology enabled the analysis of physiological rearrangements occurring upon the induction of heterologous protein production. This work showed that, even though IsaA was successfully secreted into the growth medium, one of the main requirements for the biotechnological sector, it was still partly accumulated in the cell membrane of this bacterium. This led to an exacerbated physiological response where membrane proteins involved in the management of secretion stress were activated. In addition, this study also showed that a rearrangement of the cell’s translocation machinery occurs upon induction of production, where a ‘game’ of in- and decrease of transporters takes place.
Anticipating the impact of genetic and environmental insults, such as the ones caused by production stress, is essential for the field of systems metabolic engineering. Thus, the highly accurate and comprehensive dataset generated during this work can be implemented in predictive mathematical models, thereby contributing in the rational design of next-generation secretion systems.
Analyse der metabolischen Anpassung von Streptococcus pneumoniae an antimikrobielle Umwelteinflüsse

Das Gram-positive Bakterium Streptococcus pneumoniae ist ein humanspezifisches Pathogen des oberen Respirationstraktes. Der opportunistische Krankheitserreger kann jedoch mehrere Organe befallen und tiefer in den Körper vordringen, was zu lokalen Entzündungen wie Sinusitis und Otitis media oder zu lebensbedrohlichen Infektionen wie Pneumonie, Meningitis oder Sepsis führen kann. Für das Bakterium S. pneumoniae wurden bisher kaum Metabolom-Daten erhoben. Daher war das Ziel dieser Dissertation eine umfassende Charakterisierung des Metaboloms von S. pneumoniae. In dieser Dissertation wurden als analytische Methoden die Gaschromatografie (GC) und Flüssigkeitschromatografie (LC) jeweils gekoppelt mit Massenspektrometrie (MS) sowie die Kernspinresonanzspektroskopie (NMR) verwendet, um die Metaboliten zu analysieren. Es sind mehrere Analysetechniken erforderlich, um den Großteil des Metaboloms mit seinen chemisch verschiedenen Metaboliten zu erfassen. Artikel I fasst die Literatur zu Untersuchungen des Metabolismus von S. pneumoniae in den letzten Jahren zusammen. Um eine Momentaufnahme des biologischen Systems zum jeweiligen Zeitpunkt zu erhalten, ist neben dem reproduzierbaren Wachstum während der Kultivierung auch die exakte Probenahme zu beachten. Aus diesem Grund wurde in dieser Dissertation ein Probenahmeprotokoll für das Endometabolom von S. pneumoniae etabliert (Artikel II). Mithilfe des optimierten Protokolls wurde eine umfassende Metabolomanalyse in einem chemisch definierten Medium durchgeführt (Artikel II). Um S. pneumoniae in einer Umgebung ähnlich der im Wirt zu untersuchen, wurde in einem modifizierten Zellkulturmedium kultiviert. Intermediate zentraler Stoffwechselwege von S. pneumoniae wurden analysiert. Das intrazelluläre Stoffwechselprofil wies auf einen hohen glykolytischen Flux hin und bot Einblicke in den Peptidoglykan-Stoffwechsel. Darüber hinaus widerspiegelten die Ergebnisse die biochemische Abhängigkeit von S. pneumoniae von aus dem Wirt stammenden Nährstoffen. Ein umfassendes Verständnis der Stoffwechselwege von Pathogenen ist wichtig, um Erkenntnisse über die Anpassungsstrategien während einer Infektion zu gewinnen und so neue Angriffspunkte für Wirkstoffe zu identifizieren.
Die zunehmende Verbreitung von resistenten S. pneumoniae-Stämmen zwingt zur Suche nach neuen antibiotisch wirksamen Substanzen. Im Zuge dessen wurde in Artikel III die metabolische Reaktion von S. pneumoniae während des Wachstums unter dem Einfluss antibakterieller Substanzen mit dem Ziel der Identifizierung metabolischer Anpassungsprozesse untersucht. Dabei wurden Antibiotika mit unterschiedlichen Wirkmechanismen verwendet, wie die Beeinflussung der Zellwandbiosynthese (Cefotaxim, Teixobactin-Arg10), der Proteinbiosynthese (Azithromycin) sowie Nukleotidsynthese (Moxifloxacin). Es konnten keine Wirkmechanismus-spezifischen Marker-Metaboliten identifiziert werden. Jedes Antibiotikum verursachte weitreichende Veränderungen im gesamten Metabolom von S. pneumoniae. Die Nukleotid- und Zellwandsynthese waren am stärksten betroffen. Besonders vielversprechend sind Antibiotika mit zwei Wirkorten wie Teixobactin-Arg10 und Kombinationen aus zwei Antibiotika. In dieser Dissertation wurde das erste Mal das synthetisch hergestellte Teixobactin-Arg10 mittels einer der modernen OMICS-Techniken untersucht. Die vorliegende umfassende Metabolom-Studie bietet wertvolle Erkenntnisse für Forscher, die an der Identifizierung neuer antibakterieller Substanzen arbeiten.
Insgesamt tragen die Ergebnisse der Dissertation zu einem besseren Verständnis der bakteriellen Physiologie bei.
Uterine Leiomyosarkome (LMS) sind eine Rarität und dementsprechend ist für eine CT- und MRT-Bildgebung kaum Datenmaterial verfügbar.
Gynäkologen haben als „radiologische Laien“ große Probleme, vorliegende Bilder selbst zu interpretieren bzw. die physikalischen Befundbeschreibungen einem morphologischen Befund oder einer Diagnose zuzuordnen. Das Ziel dieser Arbeit bestand daher darin, erstmals anhand eines größeren Patientinnenkollektivs zusätzlich aus den Daten zur Indikation einer CT/MRT beim LMS Informationen zu gewinnen, die es auch dem nichtradiologischen Anwender ermöglichen aufgrund von typischen Befunden uterine LMS selbst besser zu erkennen.
Ausgewertet wurden 235 LMS - Beratungsfälle des Deutschen klinischen Kompetenzzentrums für genitale Sarkome und Mischtumore an der Universitätsmedizin Greifswald, bei denen eine CT- oder MRT- Untersuchung dokumentiert war, aus einem Zeitraum von August 2009 bis März 2017.
Eine präoperative CT (n = 69) bzw. MRT (n= 12) wurde nur selten angewendet. Die häufige klinische Fehldiagnose als LM (65,7 %) führte dazu, dass in 81,4 % bzw. 92,9 % der Fälle keine präoperative CT bzw. MRT und somit keine weiterführende bildgebende Diagnostik erfolgt ist. In der präoperativen CT bzw. MRT wurde in nur 14 % bzw. 18,2 % der Fälle der V. a. ein Sarkom gestellt. Diese Daten sind für den klinisch praktizierenden Arzt unbefriedigend. Die CT diente bei einem präoperativ bereits bekanntem LMS jedoch vorrangig der Ausbreitungsdiagnostik, mit der zu diesem Zeitpunkt in 17,6 % bereits Metastasen erkannt wurden.
Am häufigsten (17,5 %) wurde in der CT von den Radiologen eine nicht näher bezeichnete Raumforderung/tumoröse Läsion beschrieben. In 10,5 % der Fälle wurden ein auffälliges Myom bzw. ein „infizierter Uterus myomatosus“ oder ein „zentral nekrotisierendes Myom“ angeführt. Differentialdiagnosen zu einem in der CT-Untersuchung gesehenen Tumor lagen in 12,3 % der Fälle vor. Vermutet wurden dabei vom Ovar ausgehende Tumoren, wie Fibrome oder Granulosazelltumore. Aber auch ein Zervixkarzinom, ein Uterussarkom ohne nähere Bezeichnung, eine fokale Adenomyosis uteri, ein Endometriumkarzinom und ein Korpuskarzinom wurden als Differentialdiagnose in Betracht gezogen.
In den 12 Fällen der MRT lauteten die Befunde je einmal „V.a. Sarkom,“ „tumoröse Läsion bzw. Raumforderung“, „auffälliges Myom“ bzw. „unauffälliges Myom“. Der Verdacht auf ein anderes Malignom, in diesem Fall auf ein Ovarialkarzinom, ergab sich in einem Fall. Als Differentialdiagnose wurde in einem Fall ein Tumor, der vom Ovar ausgeht, wie z.B. ein Fibrom oder ein Granulosazelltumor, betrachtet.
Bei der Auswertung der Übereinstimmung der tatsächlichen Tumorgröße aus dem pathologisch-anatomischen Befund mit der Tumorgröße in der Bildgebung fällt auf, dass die Tumorgröße in der Hälfte der Fälle in der CT größer beschrieben wird, während sie in der MRT zu gleichen Anteilen (je 37,5 %) größer und kleiner als im pathologisch-anatomischen Befund angegeben wird. Eine Übereinstimmung hinsichtlich der zystischen Anteile im Tumor und dem pathologisch-anatomischen Befund lag nur in der CT vor. Hingegen zeigt sich in allen Fällen eine Übereinstimmung zwischen den in der Bildgebung beschriebenen zentralen Nekrosen und dem pathologisch-anatomischen Befund.
Aufgrund der postoperativen Zufallsdiagnose LMS und der konsekutiven Staging- bzw. Nachsorgeuntersuchungen wurde eine CT (136 Fälle) bzw. eine MRT (29 Fälle) postoperativ deutlich häufiger angewendet. Bei einer unmittelbar postoperativ durchgeführten CT lagen in nur 5,1 % der Fälle Metastasen vor, während bei einer CT, die über einen Monat nach der Operation stattgefunden hat, bereits bei 41,7 % der Patientinnen Metastasen mehrheitlich in der Lunge nachgewiesen werden konnten. In allen 34 (25 %) CT und 11 (37,9 %) MRT, die unter der Indikation „Rezidivdiagnostik“ durchgeführt wurden, zeigte sich in der Bildgebung tatsächlich ein Rezidiv. Pelvine Rezidive waren dabei in der CT mit 67,6 % und in der MRT mit 72,7 % die häufigste Lokalisation.
Zusammenfassend sprechen nach den eigenen Daten und der berücksichtigten Fachliteratur folgende Kriterien für ein LMS: MRT - irreguläre und schlecht definierte Tumorgrenzen, eine hohe Signalintensität in der T2W in Kombination mit hyperintensen Abschnitten in der T1W sowie Zeichen von Zysten und/oder Nekrosen. CT - große unregelmäßig begrenzte Tumoren mit unterschiedlichen Densitäten im Sinne von Nekrosen und/oder Zysten und/oder Einblutungen. Die Aussagekraft der CT hinsichtlich der Tumorentität ist sehr begrenzt.
Da die analysierten diagnostischen Aussagen zur CT- und MRT-Bildgebung für den Anwender unbefriedigend waren, sollte daher die interdisziplinäre Zusammenarbeit zwischen Radiologen und Gynäkologen verbessert werden. Voraussetzung dafür ist, dass dem Radiologen ausreichend anamnestisches und diagnostisches Material (einschließlich Laborwerte z.B.: LDH) mit gezielter Fragestellung zur Verfügung gestellt und ihm zusätzlich nach der Enddiagnose der genaue morphologische Befund übermittelt wird.
In phylogenetics, evolutionary relationships of different species are represented by phylogenetic trees.
In this thesis, we are mainly concerned with the reconstruction of ancestral sequences and the accuracy of this reconstruction given a rooted binary phylogenetic tree.
For example, we wish to estimate the DNA sequences of the ancestors given the observed DNA sequences of today living species.
In particular, we are interested in reconstructing the DNA sequence of the last common ancestor of all species under consideration. Note that this last common ancestor corresponds to the root of the tree.
There exist various methods for the reconstruction of ancestral sequences.
A widely used principle for ancestral sequence reconstruction is the principle of parsimony (Maximum Parsimony).
This principle means that the simplest explanation it the best.
Applied to the reconstruction of ancestral sequences this means that a sequence which requires the fewest evolutionary changes along the tree is reconstructed.
Thus, the number of changes is minimized, which explains the name of Maximum Parsimony.
Instead of estimating a whole DNA sequence, Maximum Parsimony considers each position in the sequence separately. Thus in the following, each sequence position is regarded separately, and we call a single position in a sequence state.
It can happen that the state of the last common ancestor is reconstructed unambiguously, for example as A. On the other hand, Maximum Parsimony might be indecisive between two DNA nucleotides, say for example A and C.
In this case, the last common ancestor will be reconstructed as {A,C}.
Therefore we consider, after an introduction and some preliminary definitions, the following question in Section 3: how many present-day species need to be in a certain state, for example A, such that the Maximum Parsimony estimate of the last common ancestor is also {A}?
The answer of this question depends on the tree topology as well as on the number of different states.
In Section 4, we provide a sufficient condition for Maximum Parsimony to recover the ancestral state at the root correctly from the observed states at the leaves.
The so-called reconstruction accuracy for the reconstruction of ancestral states is introduced in Section 5. The reconstruction accuracy is the probability that the true root state is indeed reconstructed and always takes two processes into account: on the one hand the approach to reconstruct ancestral states, and on the other hand the way how the states evolve along the edges of the tree. The latter is given by an evolutionary model.
In the present thesis, we focus on a simple symmetric model, the Neyman model.
The symmetry of the model means for example that a change from A to C is equally likely than a change from C to A.
Intuitively, one could expect that the reconstruction accuracy it the highest when all present-day species are taken into account. However, it has long been known that the reconstruction accuracy improves when some taxa are disregarded for the estimation.
Therefore, the question if there exits at least a lower bound for the reconstruction accuracy arises, i.e. if it is best to consider all today living species instead of just one for the reconstruction.
This is bad news for Maximum Parsimony as a criterion for ancestral state reconstruction, and therefore the question if there exists at least a lower bound for the reconstruction accuracy arises.
In Section 5, we start with considering ultrametric trees, which are trees where the expected number of substitutions from the root to each leaf is the same.
For such trees, we investigate a lower bound for the reconstruction accuracy, when the number of different states at the leaves of the tree is 3 or 4.
Subsequently in Section 6, in order to generalize this result, we introduce a new method for ancestral state reconstruction: the coin-toss method.
We obtain new results for the reconstruction accuracy of Maximum Parsimony by relating Maximum Parsimony to the coin-toss method.
Some of these results do not require the underlying tree to be ultrametric.
Then, in Section 7 we investigate the influence of specific tree topologies on the reconstruction accuracy of Maximum Parsimony. In particular, we consider balanced and imbalanced trees as the balance of a tree may have an influence on the reconstruction accuracy.
We end by introducing the Colless index in Section 8, an index which measures the degree of balance a rooted binary tree can have, and analyze its extremal properties.
Spinal cord injury (SCI) above mid-thoracic levels leads to autonomic dysfunction affecting both the cardiovascular system and thermoregulation. The renin-angiotensin system (RAS) which is a potent regulator of blood pressure, including its novel beneficial arm with the receptor Mas could be an interesting target in post-SCI hemodynamics. To test the hypothesis that hemodynamics, activity and diurnal patterns of those are more affected in the Mas deficient mice post-SCI we used a mouse model of SCI with complete transection of spinal cord at thoracic level 4 (T4-Tx) and performed telemetric monitoring of blood pressure (BP) and heart rate (HR). Our data revealed that hypothermia deteriorated physiological BP and HR control. Preserving normothermia by keeping mice at 30°C prevented severe hypotension and bradycardia post-SCI. Moreover, it facilitated rapid return of diurnal regulation of BP, HR and activity in wild type (WT) mice. In contrast, although Mas deficient mice had comparable reacquisition of diurnal HR rhythm, they showed delayed recovery of diurnal rhythmicity in BP and significantly lower nocturnal activity. Exposing mice with T4-Tx (kept in temperature-controlled cages) to 23°C room temperature for one hour at different time-points post-SCI, demonstrated their inability to maintain core body temperature, Mas deficient mice being significantly more impaired than WT littermates. We conclude that Mas deficient mice were more resistant to acute hypotension, delayed nocturnal recovery, lower activity and more severely impaired thermoregulation. The ambient temperature had significant effect on hemodynamics and, thus it should be taken into account when assessing cardiovascular parameters post-SCI in mice.
Die Stimulation des Vagusnervs ist eine vielversprechende therapeutische Methode der Neuromodulation zur Behandlung von akuten und chronischen Schmerzen, epileptischen Anfällen sowie Stimmungs-störungen und Angstzuständen. Dabei kann die Stimulation elektrisch mit Hilfe invasiver Elektroden sowie noninvasiv transkutan erfolgen. Als Ort für eine nicht-invasive Stimulation des Vagus-Nervs eignet sich insbesondere die Ohrmuschel, da dort die Haut unter anderem von vagalen Afferenzen innerviert ist (Peuker & Filler, 2002). In der folgenden Arbeit sollen zunächst kurz die Besonderheiten sowie die therapeutischen Anwendungen der Stimulation dieses Hirnnervs beschrieben werden. Auf Grundlage von vier Publikationen unserer Arbeitsgruppe wird die Vagusnervstimulation als möglicher Wirkmechanismus der Ohrakupunktur postuliert. Anschließend werden die Ergebnisse unserer experimentellen Studie zur Wirkung von tVNS auf die Wahrnehmung von wiederholten schmerzhaften Hitzereizen vorgestellt und diskutiert. Im letzten Teil der Arbeit wird auf der Grundlage der Ergebnisse zweier weiterer Untersuchungen zu den Effekten von Ohrakupunktur zur Reduktion von Prüfungsangst sowie prä-operativer Angst ein alternativer Wirkmechanismus der tVNS sowie dessen systematische Untersuchung in zukünftigen Studien vorgeschlagen.
Summary
The susceptibility of Candida albicans biofilms to a non‐thermal plasma treatment has been investigated in terms of growth, survival and cell viability by a series of in vitro experiments. For different time periods, the C. albicans strain SC5314 was treated with a microwave‐induced plasma torch (MiniMIP). The MiniMIP treatment had a strong effect (reduction factor (RF) = 2.97 after 50 s treatment) at a distance of 3 cm between the nozzle and the superior regions of the biofilms. In addition, a viability reduction of 77% after a 20 s plasma treatment and a metabolism reduction of 90% after a 40 s plasma treatment time were observed for C. albicans. After such a treatment, the biofilms revealed an altered morphology of their cells by atomic force microscopy (AFM). Additionally, fluorescence microscopy and confocal laser scanning microscopy (CLSM) analyses of plasma‐treated biofilms showed that an inactivation of cells mainly appeared on the bottom side of the biofilms. Thus, the plasma inactivation of the overgrown surface reveals a new possibility to combat biofilms.
Zielsetzung: Cutibacterium (C.) acnes ist Teil des anaeroben Haut-Mikrobioms. Besonders in der Schulterchirurgie kommt es postoperativ häufig zu Infektionen mit C. acnes auf Grund der schwierigen Erreichbarkeit des Erregers durch das Hautantiseptikum in den Haarfollikeln der Haut.
Basierend auf der Hypothese, dass eine verlängerte Einwirkzeit des Hautantiseptikums in talgdrüsenreichen Arealen wie der Schulter das Eindringen in die Haarfollikel begünstigt und dadurch die antiseptische Wirksamkeit verbessert wird, sollte die Effektivität der derzeit üblichen Einwirkzeit von 2,5 min mit der Einwirkzeit von 30 min verglichen werden. Aufgrund der unterschiedlichen Eigenschaften von PVP-Iod und Chlorhexidindigluconat (CHG) sollten zwei handelsübliche alkoholische Hautantiseptika mit Gehalt an PVP-Iod bzw. CHG miteinander verglichen werden.
Methode: Im cross-over-Design wurde auf der Schulter von 16 gesunden, freiwilligen Probanden die Einwirkzeit von 2,5 min mit einer Einwirkzeit von 30 min jeweils mit einem PVP-I-haltigen Hautantiseptikum und einem CHG-haltigen Hautantiseptikum verglichen werden. Die Probenentnahmen erfolgten vor, direkt nach und 3h nach erfolgter Hautantiseptik. Analysiert wurde sowohl die aerobe als auch die anaerobe Hautflora.
Ergebnisse: Die aerobe residente Hautflora wurde durch beide Antiseptika bereits bei der kurzen Einwirkzeit effektiv eradiziert.
Die anaerobe residente Flora wurde hinsichtlich der Sofortwerte durch das PVP-I-haltige Antiseptikum sowohl bei der Einwirkungszeit von 2,5 min als auch von 30 min effektiver reduziert als durch das CHG-haltige Antiseptikum. Die nach 3 h erhobenen Werte zeigten dagegen nur die Tendenz der höheren Wirksamkeit bei der verlängerten Einwirkzeit des PVP-I-haltigen Antiseptikums. Die verlängerte Einwirkzeit hatte bei beiden Antiseptika keinen negativen Einfluss auf die intakte Haut, d.h. keiner der Probanden klagte über Hautirritationen oder Beschwerden.
Schlussfolgerung: Die alkoholische Zubereitung mit PVP-Iod ist dem CHG basierten Alkohol an Wirksamkeit gegen die anaerobe Flora im Sofortwert überlegen. Die klinische Relevanz dieser Studie sollte in einer randomisierten klinischen Studie mit dem Endpunkt SSI verifiziert werden.
Introduction: Hearing and vision loss are highly prevalent in elderly adults, and thus frequently occur in conjunction with cognitive impairments. Studies have shown that hearing impairment is associated with a higher risk of dementia. However, evidence concerning the association between vision loss and dementia, as well as the co-occurrence of vision and hearing loss and dementia, has been inconclusive.
Objectives: To assess the association between: (i) either hearing or vision loss and the risk of dementia, as well as between; and (ii) the combination of both sensory impairments and the risk of dementia.
Methods: This case-control study was based on a 5-year data set that included patients aged 65 years and older who had initially been diagnosed with dementia diseases by one of 1,203 general practitioners in Germany between January 2013 and December 2017. In total, 61,354 identified dementia cases were matched to non-dementia controls, resulting in a sample size of 122,708 individuals. Hearing loss and vision loss were identified using the ICD-10 diagnoses documented in the general practitioners’ files prior to the initial dementia diagnosis. Multivariate logistic regression models were fitted to evaluate the associations between visual and/or hearing impairment and the risk of dementia and controlled for sociodemographic and clinical variables.
Results: Hearing impairment was documented in 11.2% of patients with a dementia diagnosis and 9.5% of patients without such a diagnosis. Some form of vision impairment was documented in 28.4% of patients diagnosed with dementia and 28.8% of controls. Visual impairment was not significantly associated with dementia (OR = 0.97, CI = 95% 0.97–1.02, p = 0.219). However, patients with hearing impairment were at a significantly higher risk of developing dementia (OR = 1.26, CI = 95% 1.15–1.38, p < 0.001), a finding that very likely led to the observed significant association of the combination of both visual and hearing impairments and the risk of dementia (OR = 1.14, CI = 95% 1.04–1.24, p = 0.005).
Discussion: This analysis adds important evidence that contributes to the limited body of knowledge about the association between hearing and/or vision loss and dementia. It further demonstrates that, of the two, only hearing impairment affects patients’ cognition and thus contributes to dementia risk.
Brain aging even in healthy older adults is characterized by a decline in cognitive functions including memory, learning and attention. Among others, memory is one of the major cognitive functions affected by aging. Understanding the mechanisms underlying age-related memory decline may help pave the road for novel treatment strategies. Here, we tried to elucidate the neural correlates associated with memory decline using structural and functional neuroimaging and neuromodulation with transcranial direct current stimulation (tDCS).
Over the course of three studies, we investigated 1) the influence of white matter integrity and grey matter volume on memory performance in healthy older adults, 2) the role of functional coupling within the memory network in predicting memory performance and the impact of tDCS in modulating retrieval performance in healthy older adults, 3) the effect of tDCS over the sensorimotor cortex on cognitive performance in young adults.
MRI was used to study associations of cognitive performance with white matter integrity and grey matter volume, and examine their causal relationship in the course of aging. White matter integrity was assessed by acquiring diffusion tensor imaging (DTI) and performing deterministic tractography based on constrained spherical deconvolution. Grey matter volume was estimated using fully automated segmentation. Both white matter integrity and grey matter volume were correlated with behavioral data of a verbal episodic memory task. Percentage of correct answers at retrieval was used to measure memory performance (Manuscript 1). In addition, anodal tDCS (atDCS) (1 mA, 20 min) was applied over CP5 (left temporoparietal cortex) to modulate memory formation in healthy older adults. Participants underwent resting-state fMRI before the stimulation. Functional connectivity analysis was performed to determine whether functional coupling within the memory network predicted initial memory performance, and to examine its association to tDCS-induced enhancement effect (Manuscript 2). Finally, atDCS (1 mA, 20 min) was applied over C3 (left sensorimotor cortex) to explore the effect of tDCS over the sensorimotor cortex on cognitive performance in young adults. During the stimulation, participants performed three tasks; gestural task, attentional load task and simple reaction time task (Manuscript 3).
Results showed that volumes of the left dentate gyrus (DG) and tractography-based fractional anisotropy (FA) of individual fornix pathways were positively related to memory retrieval in older adults. Brain-behavior associations were observed for correct rejections rather than hits of memory performance, indicating specificity of memory network functioning for detecting false associations. Thus, the data suggested a particular role of neural integrity that promotes successful memory retrieval in older adults. Subsequent mediation analysis showed that left DG volume mediated the effect of fornix FA on memory performance (48%), corrected for age, revealing a crucial role of hippocampal pathway microstructure in modulating memory performance in older adults (Manuscript 1). tDCS results showed that atDCS led to better retrieval performance and increasing learning curves, indicating that brain stimulation can induce plasticity of episodic memory processes in older adults. Combining tDCS and fMRI, hippocampo-temporoparietal functional connectivity was positively associated with initial memory performance in healthy older adults and was positively correlated with the magnitude of individual tDCS-induced enhancement, suggesting that individual tDCS responsiveness may be determined by intrinsic network coupling (Manuscript 2). Finally, our findings suggested that atDCS over left sensorimotor cortex reduced reaction times in the gestural-verbal integration task, specifically for incongruent pairs of gestures and verbal expressions, indicating the role of sensorimotor cortex in gestural-verbal integration in young adults (Manuscript 3).
The results of all three studies may help to elucidate age-related structural deterioration and functional coupling network underlying cognitive processes in healthy adults. Furthermore, these studies emphasized the importance of interventions like tDCS in modulating cognitive performance, specifically episodic verbal memory and gestural-verbal integration. By unveiling the specific role of brain structures and functional network coupling as well as the role of tDCS in modulating cognitive performance, our results contribute to a better understanding of brain-behavior associations, and may help to develop clinical interventional approaches, tailored for specific cognitive functions in aging.
Im Rahmen der vorliegenden Untersuchung konnten die Assoziationen zwischen 25(OH)D und der kardiopulmonalen Leistungsfähigkeit, sowie der Lungenfunktion und deren potentielle Mediation durch gesundheitsrelevante Einflussfaktoren in einer großen bevölkerungsbasierten Studienpopulation aufgedeckt und bezugnehmend auf die körperliche Leistungsfähigkeit in einer zweiten, unabhängigen, großen Kohorte reproduziert werden. Nicht nur das Erweitern untersuchter Variablen zur Beschreibung von körperlicher Leistungsfähigkeit und Lungenfunktion, im Vergleich zur bisherigen Erkenntnissen, sondern auch der methodische Einsatz der Spiroergometrie zur Datengewinnung stellt eine weitere Stärke vorliegender Studie dar. Komplettierend wurde zudem erstmals eine Mediatoranalyse durchgeführt, um die eventuelle Vermittlung der gefundenen Effekte durch Mediatoren zu überprüfen.
Allerdings sind in der Interpretation der Studienergebnisse Einschränkungen zu berücksichtigen, wie z. B. Ernährungsgewohnheiten (insbesondere vegane Ernährung), die 25(OH)D-Werte beeinflussen können109 und in der vorliegenden Untersuchung nicht berücksichtigt wurden. Trotz enormer Stärken sind die vorliegenden Untersuchungen durch ihr Querschnittsdesign limitiert, welches nicht geeignet ist eine Kausalität der Zusammenhänge aufzuzeigen. Auch wenn ein statistisch signifikanter Zusammenhang zwischen 25(OH)D und der körperlichen Leistungsfähigkeit, sowie der Lungenfunktion bei gesunden Probanden auf Grundlage der dargestellten Ergebnisse eindeutig ist, sind Längsschnittanalysen wünschenswert, um zu beweisen, dass 25(OH)D ursächlich mit der körperlichen Leistungsfähigkeit und Lungenfunktion in Verbindung steht. Zwar hat eine erst kürzlich erschienene französische Untersuchung gezeigt, dass Vitamin D Werte von Probanden, die im Zeitraum von November bis April unzulängliche Werte aufwiesen, durch eine zusätzliche Gabe von 80.000 internationalen Einheiten (IU) normwertig angehoben werden konnte, ohne dabei überdosiert zu sein110, jedoch fehlen Daten hinsichtlich einer Beeinflussung der körperlichen Leistungsfähigkeit, weswegen zukunftsorientiert anzumerken bleibt, dass weitere interventionelle Studien erforderlich sind, um zu untersuchen, inwieweit eine Vitamin-D-Ergänzung einer Verbesserung der allumfassenden körperlichen Leistungsfähigkeit zuträglich ist und somit zur Gesundheitsversorgung in der allgemeinen Bevölkerung beitragen kann.

In this thesis, we elaborate upon Bayesian changepoint analysis, whereby our focus is on three big topics: approximate sampling via MCMC, exact inference and uncertainty quantification. Besides, modeling matters are discussed in an ongoing fashion. Our findings are underpinned through several changepoint examples with a focus on a well-log drilling data.
Im Rahmen der vorliegenden Arbeit wurde bei 41 Patienten mit JME das EFHC1-Gen sequenziert und die identifizierten Varianten mit in mehreren vorherigen Studien beschriebenen Prädiktoren für das klinische Outcome korreliert. Insgesamt konnten drei Varianten - p.Phe229Leu, p.Arg182His und p.Arg294His - identifiziert werden.
Die Variante p.Phe229Leu, welche bei zwei Patienten auftrat, konnte mit einem milderen JME Subtyp assoziiert werden. Drei Patienten wiesen die Variante p.Arg182His auf und zeigten eine erhöhte Neigung zur Ausbildung einer photoparoxysmalen Reaktion. Eine Assoziation mit einem frühen Auftreten der Epilepsie (≤10. Lebensjahr) und einem Subtyp der JME, die sich aus einer kindlichen Absencenepilepsie entwickelt, konnte bei den zwei Patienten mit Variante p.Arg294His nachgewiesen werden, aufgrund der geringen Zahl an untersuchten Patienten jedoch ohne statistische Relevanz. In Zukunft könnte möglicherweise das Vorliegen einer dieser Varianten für klinische Fragestellungen hilfreich sein, um im Zusammenspiel mit klinischen Parametern eine Aussage über den Langzeitverlauf der JME und damit verbundene Therapieentscheidungen treffen zu können.
Nach aktuellen Bewertungen in der Human Gene Mutation Database sind p.Arg182His und p.Arg294His als benigne Varianten/Polymorphismen und die Variante p.Phe229Leu als wahrscheinlich pathogene Variante einzustufen. Somit muss eingeschätzt werden, dass die Varianten eher einen modifizierenden Einfluss auf den Krankheitsverlauf nehmen und nicht ursächlich für die JME sind.
Zusammenfassend sprechen die Ergebnisse dafür, dass es sich bei der JME um ein heterogenes Krankheitsbild handelt. Vorrangiges Ziel zukünftiger Studien sollte sein, weitere potenzielle Kandidatengene zu identifizieren und zu untersuchen.
Staphylococcus aureus is one of the commonly encountered bacteria of the human microbiome. Although mostly a seemingly harmless commensal microbe, S. aureus can act as an invasive pathogen with seriously devastating effects on its host’s health and wellbeing. A wide range of infections caused by this bacterium has been reported to affect diverse parts of the human body, including the skin, soft tissues and bones, as well as important organs like the heart, kidneys and lungs. Particularly, S. aureus is infamous for being a major causative agent of respiratory tract infections that may escalate up to necrotizing pneumonia. Due to its clinical relevance, this pathogen has been intensively studied for many years. Nonetheless, further research in this field is still needed, because of the high capacity of S. aureus to evolve drug resistance, its high genomic plasticity and adaptability and, not in the last place, the plethora of niches within the human body where it can thrive and survive. In this regard, there are still many uncertainties concerning the specific adaptations carried out by S. aureus during colonization and infection of the human body, the transition between both stages, and upon the invasion of different types of host cells. To shed more light on some of these adaptations, the research described in this thesis has employed in vitro models of infection that mimic particular conditions during the infectious process with special focus on the lung epithelium. The adaptations displayed by S. aureus were monitored using advanced proteomics. Furthermore, the analyses documented in this thesis included S. aureus strains with diverse backgrounds and epidemiology to take into account the genetic diversity encountered in this species.
Nachdem ab 2012 mehr als 200, vor allem kleinere, Untersuchungen, zum Thema Irisin veröffentlicht wurden, jedoch noch sehr große Diskrepanzen bezüglich der Ergebnisse herrschten, war Ziel meiner Arbeit, die Beziehung zwischen körperlicher Aktivität mittels kardiopulmonaler Parameter und Irisin-Spiegel erstmals in einer großen Population zu untersuchen. Hierzu wurden Daten von 740 Probanden der SHIP-Trend Kohorte herangezogen und geschlechtsspezifische Analysen zum Zusammenhang zwischen Irisin-Spiegeln und kardiopulmonaler Leistungsfähigkeit durchgeführt.
Die Ergebnisse weisen bei den Männern eine inverse Beziehung zwischen Irisin und Belastungskapazität gemessen mittels VO2,peak und maximaler Leistung auf. Demgegenüber steht eine positive Beziehung zwischen Irisin und VO2,peak bei Frauen, welche mögliche geschlechtsspezifische Unterschiede nahelegt. Darüber hinaus zeigten die Analysen erstmals einen speziellen jahreszeitlichen Verlauf des Irisin-Spiegels mit zwei Gipfeln in den Winter- (Dezember bis Februar) und in den Sommermonaten (Juli bis August) mit höheren Werten im Vergleich zu den restlichen Monaten.
Trotz diverser Studien zum Thema Irisin gibt es noch viele ungeklärte Aspekte in Hinblick auf die Sekretion und Wirkung. So ist momentan noch unklar, ob Irisin auch von Fettzellen gebildet werden kann. Auch die laborchemischen Detektionsmethoden für Irisin sind verbesserungswürdig, um eine verlässliche und standardisierte Quantifikation von Irisin zu gewährleisten. Zudem sollte in Zukunft neben der Entstehung und Detektion Irisins die Wirkung auf die menschlichen Körperzellen eingehend geklärt werden, um weitergehende Schlüsse aus dem propagierten „Adipositas-Heilmittel“ Irisin zu ziehen. Die von uns erstmals entdeckten jahreszeitlichen Schwankungen des Irisins sind ein weiterer Punkt, der momentan noch zu wenig erforscht ist, und in künftigen Studien weiter analysiert und standardisiert werden sollte, um letztendlich Gewissheit bezüglich der Wirkungsweise des Irisins und der möglichen klinischen Relevanz dieses Hormons zu erlangen.
Bildgebung zur Vorhersage und Verlaufsbetrachtung der motorischen Restitution nach Schlaganfall

In dieser kumulativen Dissertation wird erörtert, welche Rolle die Bildgebung mittels MRT bei der Beantwortung von wichtigen Fragen im Hinblick auf die Erholung nach einem Schlaganfallereignis spielt.
Da Schlaganfälle gerade in Deutschland noch immer die Hauptursache von längerfristigen Behinderungen und Einschränkungen darstellen, ist es von großem Interesse, zu ergründen, inwiefern ein einzelner Patient wieder volle Funktionsfähigkeit und somit eine Wiederintegration in den Alltag erlangen kann.
Ein frühzeitiger Einsatz bildgebender Verfahren kann die Vorhersage der motorischen Fähigkeiten eines Patienten wesentlich verbessern. Dieses Vorgehen ist jedoch mit zahlreichen Schwierigkeiten versehen, die im Übersichtsartikel [Horn et al., 2016a] erläutert werden und der somit Gründe liefert, weshalb eine solche Prädiktion nicht bereits in den Klinikalltag integriert werden konnte. Im Artikel werden strukturelle und funktionelle Bildgebungsparameter hinsichtlich ihrer Eignung als Biomarker für die Vorhersage der motorischen Fähigkeiten nach dem Schlaganfall bewertet. Begleitend dazu werden in der vorliegenden Arbeit Beispiele für eine solche Vorhersage anhand eines weiteren Artikels erläutert [Lindow et al., 2016].
Es zeigen sich deutliche Unterschiede in der Prädiktionskraft struktureller Parameter im Vergleich zu funktionellen Parametern.
Neben den zahlreichen Faktoren, die die Bildgebung und die Vorhersage daraus beeinflussen, fehlen auch grundsätzlich die Datenmengen, die ein strukturiertes Abarbeiten verschiedener Hypothesen ermöglichen. In der vorliegenden Arbeit wird beschrieben, weshalb zunächst bereits erforschte Parameter erhoben werden müssten, um Patienten diesbezüglich zu gruppieren. Erst dann können neue Bildgebungsparameter in den Vorhersage- oder Klassifizierungsprozess eingefügt werden. Ist das nicht der Fall, sind kleine Stichproben mit verschiedenen Vorhersage- und Outcome-Parametern nur schwer vergleichbar und die Forschung bewegt sich wenig voran in Richtung einer individuellen Vorhersage.
Es kann zudem durchaus vorteilhaft sein, eine Prädiktion nicht nur datengetrieben durchzuführen, sondern zusätzlich Modelle zu entwickeln, wieso gewisse Prädiktionen so gut funktionieren und auf welchem Wege gewisse Parameter diese Vorhersage ermöglichen.
Ein weitergehendes Verständnis und eine Verfeinerung dieser Prädiktion würde eventuell irgendwann dazu führen, dass man anhand diverser Messungen eines Patienten eine Art Simulation durchführen könnte, wie sich dessen Gehirn über die Zeit entwickeln wird und so individuelle Vorhersagen ermöglichen. Hier braucht es jedoch nicht nur entsprechende biophysikalische Modelle der Interaktion der verschiedenen hierarchischen Ebenen sondern auch grundlegende Forschung, die alle Faktoren bestimmt, die einen Einfluss auf die Bildgebungsergebnisse haben.
Das Verständnis für die zugrundeliegenden strukturellen und funktionellen Veränderungen während der Erholung ist wesentlich für eine verbesserte Vorhersage der Endzustände solcher Plastizitätsprozesse. Aus diesem Grund ist eine andere sinnvolle Herangehensweise an die Gesamtproblematik Schlaganfallforschung mittels longitudinaler Studien zu bewerkstelligen. Hierbei können während mehrfacher Messungen Prozesse abgebildet werden, die ausgehend von einem Initialzustand des geschädigten Netzwerks vonstattengehen. Auch dabei gilt es, sich auf bestimmte Patienten zu beschränken, damit so viele Faktoren wie möglich konstant gehalten werden können, die für eine gewisse Varianz zwischen Patienten verantwortlich sind. In einer weiteren hier beschriebenen Studie [Horn et al., 2016b] geschah dies durch eine Beschränkung auf eine Gruppe, die schon bestimmte motorische Fertigkeiten aufwies und dementsprechend lediglich eine mäßige Schädigung des motorischen Systems. Die Veränderungen der Gehirnaktivierung beschränkten sich auf eine spezifische Region, den ventralen prämotorischen Kortex, dessen Aktivität mit der Zeit zunahm. Die funktionellen Veränderungen dieses Areals, das vermehrt für Objektmanipulation aktiviert wird, konnten zudem mittels einer Analyse der Verbindungsstärken ergänzt werden. Zahlreiche weitere Untersuchungen sind notwendig, um zu verstehen, wie diese verschiedenen Ebenen der Reorganisation miteinander interagieren und welche Faktoren einen Einfluss auf diese Messungen haben. Dies muss bereits an Gesunden erforscht werden, um die pathologischen Prozesse in Patienten von weiteren Faktoren abzugrenzen, die einen Einfluss auf die Bildgebungsergebnisse
Biocatalytic Production of Amino Carbohydrates through Oxidoreductase and Transaminase Cascades

Plant-derived carbohydrates are an abundant renewable re- source. Transformation of carbohydrates into new products, in- cluding amine-functionalized building blocks for biomaterials applications, can lower reliance on fossil resources. Herein, bio- catalytic production routes to amino carbohydrates, including oligosaccharides, are demonstrated. In each case, two-step bio- catalysis was performed to functionalize d-galactose-contain- ing carbohydrates by employing the galactose oxidase from Fusarium graminearum or a pyranose dehydrogenase from
Agaricus bisporus followed by the w-transaminase from Chro- mobacterium violaceum (Cvi-w-TA). Formation of 6-amino-6- deoxy-d-galactose, 2-amino-2-deoxy-d-galactose, and 2-amino- 2-deoxy-6-aldo-d-galactose was confirmed by mass spectrome- try. The activity of Cvi-w-TA was highest towards 6-aldo-d-gal- actose, for which the highest yield of 6-amino-6-deoxy-d-galac- tose (67%) was achieved in reactions permitting simultaneous oxidation of d-galactose and transamination of the resulting 6- aldo-d-galactose.
Weltweit leiden 4,4% der Weltbevölkerung an einer Depression. Das variable Erscheinungsbild dieser Erkrankung erschwert deren Erforschung. Dass ein besseres Verständnis und die Entwicklung optimaler Therapiemöglichkeiten dieser Erkrankung sehr wichtig sind, zeigen die starke Einschränkung des gesamten Lebensalltags Betroffener (DALYs) und auch die hohe Suizidalität. Es existieren verschiedene ätiologische Theorien zur Depression, wie zum Beispiel die Monoaminmangel-Hypothese, die auf einen Mangel an Neurotransmittern im synaptischen Spalt und deren sekundären Effekte beruht. Neuroendokrine Theorien verweisen auf einen Hyperkortisolismus mit Störung der HPA-Achse und eine Stress-bezogene Ätiologie. Ein andauernder Hyperkortisolismus führt laut der Neutrophin-Hypothese zu Neurotoxizität, zu einer verminderten Neurogenese, Atrophie und Volumenminderung. Bildmorphologische Untersuchungen mittels PET-CT oder fMRT verweisen auf strukturelle und funktionelle Veränderungen bestimmter Hirnregionen bei depressiven Probanden. Auf zellulärer Ebene kann eine veränderte Zellaktivität mittels immunhistochemischer Methoden wie der C-Fos-Färbung dargestellt werden.
Für die vorliegende Untersuchung wurden Hirne von 8 männlichen Ratten untersucht, die zuvor einen Verhaltensversuch der Erlernten Hilflosigkeit zugeführt wurden. Dieser von M. Seligman entwickelte Verhaltensversuch gilt als tierexperimentelles Modell der Depressionsforschung. Durch Selektion wurden Tiere mit angeborener Erlernter Hilflosigkeit gezüchtet (congenitally Learned Helplessness, cLH) und von nicht-hilflosen Tieren unterschieden (congenitally Non Learned Helplessness, cNLH). Tiere mit Erlernter Hilflosigkeit zeigen ein mit dem Menschen vergleichbares depressives Verhalten.
Die Rattenhirne von je 4 cLH- und cNLH-Tieren wurden mit indirekter Immunhistochemie auf ihre C-Fos-Expression untersucht. Dabei wurden die Hirne in 40 μm dicke Scheiben geschnitten und nach Anwendung eines standardisierten Färbeprotokolls 11 Hirnregionen unter Rhodaminfilter mikroskopiert. In einem 200 x 200 μm großen Rahmen wurden die C-Fos-positiven Zellen ausgezählt. Im Vergleich der cLH und cNLH-Hirne ergab sich hierbei eine signifikant geringere C-Fos-Expression der cLH-Tiere in der hippocampalen Regionen CA1 und CA3, dem medialen und centralen Amygdalakern und der Substantia nigra. Kein signifikanter Unterschied fand sich in den hippocampalen Regionen CA2 und dem Gyrus dentatus, dem basolateralen und lateralen Amygdalakern, dem Nucleus accumbens und dem prälimbischen Kortex. Die signifikanten Ergebnisse verweisen auf eine Hypoaktivität bei Depression in bestimmten Hirnregionen. Eine Hypotrophie und Hypoaktivität bestimmter Regionen (z. B. dem Hippocampus) wurde bereits in weiteren Studien beschrieben, während in anderen Regionen (z.B. den Amygdalakernen) eher Hyperaktivität vorbeschrieben wurde. Zusammenfassend konnte gezeigt werden, dass in einigen Hirnregionen eine verminderte Aktivität in Zusammenhang mit der depressiven Erkrankung steht.
Long-term nationally representative caries data in the primary dentition are rare, but nonetheless central to assess needs in caries prevention and treatment. This study evaluated the prevalence and trends of caries levels in the primary dentition of 6- to 7-year-olds in Germany as a whole and its federal states individually. In 2016, employing a randomized cluster selection, 6- to 7-year-old first graders were included in the National German Oral Health Survey performed regularly since 1994/95. Children were examined by 482 calibrated dentists in all 17 German regions using the WHO criteria for the decayed, missing, and filled teeth (dmft) including the assessment of initial carious lesions (it). In total, 151,555 6- to 7-year-olds were examined. Caries prevalence in the primary dentition dropped from 65% in 1994 to 44% in 2016, while the mean caries experience dropped from 2.89 to 1.73 dmft (dt = 0.74, mt = 0.19, ft = 0.80). When initial lesions were included, the mean caries experience increased to idmft = 2.12 (it = 0.38). In 2016, 49.7% of the examined 6- to 7-year-olds were caries-free including initial lesions. The Care Index at the tooth level was 57.5%, and the Significant Caries Index was 4.84 dmft. Depending on the German region, the mean dmft varied considerably, ranging from 1.37 to 2.31. In conclusion, despite the overall caries decline in 6- to 7-year-olds in Germany, only minor caries reductions were observed over the last decade, with a still existing high proportion of untreated dental decay. This calls for more effective preventive and restorative efforts with focus on the primary dentition in Germany.
Changes in Interhemispheric Motor Connectivity Across the Lifespan: A Combined TMS and DTI Study

Age-related decline in interhemispheric connectivity between motor areas has been reported with both transcranial magnetic stimulation (TMS) and diffusion tensor imaging (DTI) measurements. However, not all studies were able to confirm these findings, and previous studies did not apply structural (DTI) and functional (TMS) measurements within each individual appropriately. Here, we investigated age dependency of the ipsilateral silent period (ISP) and integrity of fibers in the corpus callosum as operationalized by fractional anisotrophy (FA), using TMS and DTI, respectively, in 20 participants between 19 and 72 years of age. We found age-dependent increase for ISP, and decrease of FA, both indicating a decrease in interhemispheric inhibition, with a negative association between FA and ISP for the dominant hemisphere (r = −0.39, p = 0.043). Our findings suggest that aging leads to decline of interhemispheric motor connectivity, as evidenced in both structural and functional parameters, which should be taken into account when interpreting disease- or medication-related changes.
Transportproteine und metabolisierende Enzyme sind wesentliche Bestandteile der intestinalen Absorptionsbarriere und entscheidend für die Aufnahme, Verteilung, Metabolisierung und Exkretion von Nährstoffen, Arzneimitteln oder Xenobiatika. Es gibt Hinweise darauf, dass sowohl deren Expression als auch Funktion im Zusammenhang mit entzündlichen Prozessen beeinträchtigt sind. Um die Auswirkung von Colitis Ulcerosa auf das lokale Expressionsmuster klinisch relevanter intestinaler Transporter und Enzyme abschätzen zu können, wurde in der vorliegenden Arbeit u.a. deren Genexpression, Proteingehalt sowie mögliche krankheitsbezogene Regulationsmechanismen untersucht. Mit Biopsien aus entzündetem und nicht entzündetem Gewebe von 10 Colitis Ulcerosa-Patienten als auch mit gesundem Kolongewebe ohne Entzündungszeichen wurden mittels real-time quantitative PCR mRNA- (9 Enzyme, 15 Transporter, 9 Zytokine) und microRNA- (N = 54) Expressionsanalysen durchgeführt. Der Proteingehalt wurde durch validierte HPLC-MS/MS targeted proteomics Verfahren ermittelt. Die Genexpression folgender Enzyme und Transporter zeigten sich während intestinaler Entzündung signifikant reduziert: CYP2B6, CYP2C9, UGT1A1, UGT1A3, UGT2B7, UGT2B15, ABCB1, ABCG2, SLC16A1 und SLC22A3. Ein signifikanter Anstieg der mRNA-Level im entzündeten Gewebe von Colitis Ulcerosa-Patenten konnte für ABCC1, ABCC4, ORCTL2 und OATP2B1 nachgewiesen werden. Bezogen auf den Proteingehalt ließen sich die auf mRNA Ebene beobachteten Expressionsunterschiede nur für MCT1 bestätigen. Korrelationsanalysen demonstrierten den möglichen Einfluss von Zytokinen und microRNAs auf die Regulation intestinaler Enzym- und Transporterexpression. Insbesondere scheinen TNFα, IL17 A sowie miR-142-3p/5p, miR-146a-5p und miR 223-3p starken Einfluss auf krankheitsbezogene Expressionsmuster zu besitzen. Zusammenfassend kann gesagt werden, dass Colitis Ulcerosa mit komplexen Veränderungen in der intestinalen Expression von metabolisierenden Enzymen, Transportern, Zytokinen und microRNAs einhergeht, welche sowohl Auswirkungen auf die medikamentöse Therapie als auch auf die Pathogenese der Erkrankung selbst haben können.
Charakterisierung der Expression und Funktion metabolischer Enzyme im humanen intestinalen Gewebe

Bei der Arzneimittelentwicklung liegt der Fokus nicht nur auf der Wirksamkeit und Sicherheit einer pharmakologisch aktiven Substanz, sondern auch auf einer möglichst einfachen, idealerweise oralen Applikation. Um die benötigten Wirkstoffkonzentrationen im Zielorgan zu erreichen, wird die einzunehmende Dosis eines Medikaments in Abhängigkeit der präsystemischen Elimination ermittelt. Inzwischen ist bekannt, dass nicht ausschließlich der hepatische, sondern auch der intestinale Stoffwechsel die orale Bioverfügbarkeit eines Medikaments wesentlich beeinflussen kann. Arzneistoffe, die während der Darmpassage einer starken Metabolisierung unterliegen, sind zudem prädestiniert für unerwünschte Interaktionen mit anderen Substanzen, welche die entsprechenden Stoffwechselenzyme hemmen oder induzieren. Für die Abschätzung pharmakokinetischer Parameter eines neuen Wirkstoffs sind daher Kenntnisse zur Expression sowie Funktion klinisch relevanter intestinaler Stoffwechselenzyme von Bedeutung.
Bisher publizierte Daten basieren größtenteils auf der Genexpression, obwohl aufgrund posttranskriptionaler Prozesse nicht zwingend Aussagen zur resultierenden Proteinmenge getroffen werden können. Die verfügbaren Daten zum intestinalen Proteingehalt wurden mittels immunologischer Methoden erhoben, die erhebliche Limitationen in Bezug auf Spezifität, Reproduzierbarkeit und Robustheit aufweisen. Diese Aspekte finden bei den inzwischen etablierten LC-MS/MS-basierten Targeted-Proteomics-Methoden Berücksichtigung. Dazu werden die Proteine einer Messprobe enzymatisch gespalten, um entstehende proteospezifische Peptide zur Quantifizierung der Proteine von Interesse zu nutzen.
Ein Ziel der vorliegenden Arbeit bestand in der Entwicklung und Validierung einer entsprechenden Methode zur gleichzeitigen Bestimmung von CYP1A2, CYP2B6, CYP2C8, CYP2C9, CYP2C19, CYP2D6, CYP2E1, CYP3A4, CYP3A5, UGT1A1, UGT1A3, UGT2B7 sowie UGT2B15 in biologischen Matrices, welche die aktuell gültigen Leitlinien in Bezug auf Selektivität, Linearität, Richtigkeit, Präzision und Stabilität erfüllt. Bereits bei der ersten Anwendung der Methode zur Quantifizierung der Enzyme in kommerziell erhältlichen und selbst isolierten Mikrosomen zeigte sich, welchen erheblichen Einfluss die Probenvorbereitung auf die ermittelten Proteingehalte hat.
Diese Erkenntnis wurde im Rahmen eines internationalen Projektes bestätigt, bei dem humane Leberproben desselben Ursprungs in diversen Laboren mit den dort etablierten Methoden prozessiert worden sind. Bezogen auf die eingesetzte Gewebemenge ergaben sich bei der Messung der Mikrosomen 6 - 30-fach geringere Enzymgehalte als bei der Analyse des nicht-fraktionierten Gewebes, da die subzelluläre Aufspaltung einer Probe mit erheblichen Proteinverlusten einhergeht. Folglich wurden alle weiteren Untersuchungen zur absoluten Enzymquantifizierung unter Verwendung von filterbasierten Zentrifugaleinheiten (filter aided sample preparation; FASP) mit Gesamtgewebelysatproben durchgeführt. Sowohl die optimierte Probenaufarbeitung als auch die validierte Targeted-Proteomics-Methode fanden bei der Untersuchung der Darmsegmente von 9 Spendern Anwendung, wobei jeweils Gewebe aus dem Duodenum, oberen und unteren Jejunum, Ileum sowie Colon zur Verfügung stand. Von den 13 untersuchten Enzymen wurden in allen Dünndarmabschnitten nur CYP2C9, CYP2C19, CYP2D6, CYP3A4, CYP3A5, UGT1A1, UGT1A3 und UGT2B7 nachgewiesen, deren Gehalt im Jejunum am höchsten war. Im Colon wurde auf Proteinebene keines der Metabolisierungsenzyme detektiert. Die entsprechenden Genexpressionsdaten dieser 8 Enzyme korrelieren signifikant mit den ermittelten Proteinwerten. Korrespondierend zur fehlenden Nachweisbarkeit der übrigen 5 Enzyme auf Proteinebene waren die Gene CYP2B6, CYP2C8, CYP2E1 sowie UGT2B15 nur sehr geringfügig und CYP1A2 gar nicht exprimiert.
Zur Charakterisierung der metabolischen Aktivität der intestinalen Enzyme wurde eine weitere LC-MS/MS-basierte Methode entwickelt und validiert. Als Modellsubstrate fungierten Diclofenac (CYP2C9), Omeprazol (CYP2C19), Dextromethorphan (CYP2D6), Midazolam (CYP3A), Ezetimib (UGT1A) und Naloxon (UGT2B7). Die begrenzte Verfügbarkeit des intestinalen Gewebes sowie dessen sehr geringer mikrosomaler Proteingehalt stellten besondere Anforderungen an die Sensitivität der Methode. Ihre Eignung zur Charakterisierung der intestinalen Metabolisierungsaktivität wurde bei der Anwendung auf ein jejunales Mikrosomen-Gemisch gezeigt.
Die im Rahmen dieser Arbeit generierten Daten zur Expression klinisch bedeutsamer Metabolisierungsenzyme entlang des humanen Darms tragen zu einem besseren Verständnis des intestinalen First-Pass-Metabolismus bei. Diese Kenntnisse können sowohl bei der Entwicklung neuer Arzneistoffe als auch für die Erstellung von Physiologie-basierten pharmakokinetischen Modellen (PBPK-Modellen) nützlich sein, um die orale Bioverfügbarkeit sowie das Interaktionspotential pharmakologisch aktiver Substanzen abzuschätzen.
Das klarzellige Nierenzellkarzinom (ccRCC) ist eine von vielen Krebserkrankungen. Viele Patienten weisen eine Mutation im Von-Hippel-Lindau-Gen (VHL) auf und/ oder zeigen eine Überexpression des Enzyms Nicotinamid-N-Metyltransferase (NNMT).
Es wurden insgesamt fünf etablierte Zelllinien verwendet, die embryonale Nierenzelllinie HEK-293 und vier ccRCC-Zelllinien (Caki-1, Caki-2, 769-P, 786-O), welche sich in ihrer Expression der Proteine NNMT und VHL unterscheiden.
Zudem wurde eine stabile Zelllinie aus den Caki-2 Zellen generiert, die durch ein Doxycyclin induzierbares Tet-On-System NNMT vermehrt exprimiert (C2NNMTs).
Es wurden sowohl molekularbiologische als auch biochemische Methoden zur Analyse angewendet.
Die Zelllinien wurden für Transfektionsstudien zur Überexpression oder zum Knockdown von NNMT genutzt, um die Einflussnahme auf die Enzyme Nikotinamid-phosphoribosyltransferase (NAMPT), Sirtuin 1 (SIRT1), Methioninadenosyltransferase-2 β-Untereinheit (MAT2B) und Aldehydoxidase (AOX1) zu analysieren.
Da SAM (S-Adenosylmethionin) der Methyldonor von NNMT ist, wurde auch der Einfluss der Methioninkonzentration betrachtet. Viele der bisherigen publizierten Versuche wurden bei 100 µM Methionin durchgeführt, was jedoch nicht der humanen Serumkonzentration entspricht, welche bei 20 µM Methionin liegt.
Umfangreiche massenspektrometrische Analysen führten zur Identifizierung weiterer Proteine, welche durch die NNMT-Modulation beeinflusst wurden. Die Identifikation einer Vielzahl veränderter Targets verdeutlichte den Einfluss auf den Energiemetabolismus bis hin zur Apoptose. Es zeigten sich unterschiedliche Regulationen von Glykolyse-, Respirations-, Citratzyklus-, Pentosephosphatweg- und Lipidsyntheseproteinen. Insgesamt ergaben sich individuelle, zellspezifische Regulierungen, welche auf die Sirtuine zurückzuführen sind.
Weiterhin wurden Untersuchungen zur erhöhten Expression von NNMT unter Einfluss von Nikotinamid (NAM) sowie Interleukin-6 (IL-6) durchgeführt. Die Analysen zeigten, dass zwischen der Pseudohypoxie und der Erhöhung der NNMT-Expression ein Zusammenhang besteht, denn IL-6 phosphoryliert ERK (engl. Extracellular-signal Regulated Kinases) und STAT3 (engl. Signal transducer and activator of transcription 3), welche beide benötigt werden, um die Transkription des NNMT-Gens zu beeinflussen und die NNMT-Proteinexpression zu fördern.
Die Ergebnisse dieser Untersuchungen sollen dazu dienen, die biochemischen Zusammenhänge einer veränderten Expression von NNMT besser zu verstehen und damit neue diagnostische Ansätze zu ermöglichen.
Das Bovine Virale Diarrhoe Virus (BVDV) gehört zum Genus Pestivirus innerhalb der Familie der Flaviviridae. BVDV verursacht eine gefährliche Durchfallerkrankung bei Rindern,
besonders Kälbern, die Bovine Virusdiarrhoe (Mucosal Disease). BVDV gehört zu den einzelsträngigen RNA-Viren und ist eng verwandt mit dem Erreger der Klassischen Schweinepest sowie der Border Disease der Schafe. Wie andere Pestiviren, ist BVDV ein behülltes Virus, auf dessen Hülle drei Strukturproteine (Erns, E1 und E2) zu finden sind. In dieser Arbeit wurden die Glykoproteine E1 und E2 genauer charakterisiert, um Hinweise auf den Mechanismus der Knospung (Budding) von Pestiviren zu erhalten. Dazu ist die subzelluläre Lokalisation, die Topologie sowie der Retentionsmechanismus für E1 und E2 genauer untersucht worden. Die subzelluläre Lokalisation der Hüllproteine wurde mit Hilfe von Kolokalisationsanalysen mit spezifischen Kompartimentmarkerproteinen bestimmt. Sowohl als Einzelexpression als auch nach Expression als E1-E2-Vorläuferprotein zeigte sich eine Anreicherung der beiden Proteine vorwiegend im ER. Die Lokalisationsanalyse E2s in mit verschiedenen BVDV-Stämmen infizierten Zellen zeigte ein sehr ähnliches Bild wie die Einzelexpression. Die, die intrazelluläre Retention vermittelnden, Regionen wurden zunächst mit CD72-Fusionsproteinen eingegrenzt und zeigten, dass die Transmembrandomänen der Proteine für die Retention entscheidend waren.
Anhand verschiedenster Mutanten, welche Substitutionen, Insertionen und Deletionen im Bereich des C-Terminus von CD72-E1 bzw. E2 (ohne CD72-Fusionspartner) beinhalteten,
konnten einzelne Aminosäuren identifiziert werden, die essentiell für die Retention sind oder diese zumindest beeinflussen. Für E2 konnte das Arginin an Position 355 als wichtig für die Retention bestätigt und weiterführend untersucht werden. Mit Mutationen des Glutamins an Position 370 konnte außerdem der modulierende Effekt des cytoplasmatischen Rests auf die Retention E2s gezeigt werden. Die Retention von E1 ist unabhängig von der Länge der TMD und wird durch mehrere polare Aminosäuren (K174, R177, Q182) beeinflusst. Die Topologieuntersuchungen
für E1 und E2 nach Einzelexpression mit Tag-markierter Proteinvarienten zeigte, dass die reifen Proteine eine einspännige Transmembrandomäne mit dem N-Terminus auf der luminalen und dem C-Terminus auf der cytosolischen Seite des ERs besitzen.
Der hohe Anteil an inadäquat unter der Diagnose Leiomyom des Uterus operierten uterinen Sarkomen ist hauptsächlich der mangelnden Datenlage zu den uterinen Sarkomen und der schwierigen präoperativen Abgrenzung zu den regelhaften Leiomyomen oder dessen Varianten geschuldet. Die mit einer Morcellation eines uterinen Sarkoms einhergehende Prognoseverschlechterung sollte vermieden werden. Die Zielsetzung der vorliegenden Promotion bestand darin, Charakteristika von regelhaften Leiomyomen des Uterus an einem größeren Patientinnenkollektiv anamnestisch, klinisch und paraklinisch zu definieren. Darüber hinaus wurden die zufällig gefundenen Leiomyomvarianten gesondert betrachtet. Weiterhin erfolgte eine Analyse verschiedener Laborparameter, die laut Literatur zur präoperativen Abgrenzung von benignen und malignen Tumoren geeignet sein sollen. Hierfür wurden LDH-Werte, die neutrophilen Granulozyten/Lymphozyten-Ratio (NLR) und die Lymphozyten/Monozyten-Ratio (LMR) bei Patienten mit regelhaften Leiomyomen analysiert. Die Ergebnisse dieser Arbeit wurden anschließend mit der Promotion von Köller und Riechmann verglichen, die unter ähnlichen Erhebungs- und Auswertungsbedingungen Leiomyomdaten gesammelt haben. Die bereits abgeschlossene Promotion von Dennis und Lehr mit 3786 Patientinnen diente als Basis für die Formulierung der zu erhebenden Variablen. Da diese Arbeit im Rahmen der Promotions- und Forschungsgruppe für genitale Sarkome (PFGS) des Deutschen klinischen Kompetenzzentrums für genitale Sarkome und Mischtumore (DKSM) entstanden ist, wurden zum einen die hier gewonnen Daten dem DKSM zur weiteren Auswertung zur Verfügung gestellt, zum anderen wurden die innerhalb der PFGS erhobenen Leiomyosarkom- Daten wiederum mit den eigenen Leiomyomdaten verglichen. Zweck der Leiomyom- Charakterisierung war es also, mögliche Determinanten der präoperativen Differenzierung zwischen Leiomyomen und uterinen Sarkomen zu definieren.
Zunächst wurden im Rahmen dieser Arbeit von Mai 2011 bis November 2014 alle Patientinnen mit der Operationsdiagnose uterines Leiomyom und der Operationsmethode suprazervicale laparoskopische Hysterektomie, die in der gynäkologischen, ambulant operativen Einrichtung GynVelen operiert wurden, retrospektiv ausgewertet. Als die PFGS begann, die oben genannten Laborwerte bei Leiomyomen und uterinen Sarkomen zu analysieren, wurden je 50 Datensätze im Zeitraum von Februar bis November 2016 (GynVelen) und von Januar bis April 2016 (GynMünster) prospektiv ausgewertet. Insgesamt konnten 199 regelhafte Leiomyome, 96 zellreiche Leiomyome, 4 mitotisch aktive Leiomyome, 2 Lipoleiomyome, 1 Leiomyom mit bizarren Kernen und 1 glattmuskulärer Tumor mit unsicherem malignen Potential (STUMP) erfasst werden.
Signifikante Unterschiede zwischen den regelhaften Leiomyomen und dessen Varianten konnten nicht festgestellt werden. So waren die Patientinnen mit regelhaften Leiomyomen (n=199) im Mittel 44,9 Jahre alt und zeigten eine mittlere Tumorgröße von 4,53 cm, während diese Werte bei den zellreichen Leiomyomen bei 44,56 Jahre und 4,84cm lagen. Die regelhaften Leiomyome waren in 46,22 % der Fälle ein solitärer Tumor, während die zellreichen Leiomyome mit 53,12 % mehrfach als solitärer Tumor vorlagen. Die solitären, zellreichen Leiomyome waren mit im Mittel 1 cm signifikant (p=0,023) größer als die multiplen, zellreichen Leiomyome. Die regelhaften, solitären Leiomyome waren ebenfalls größer als die multiplen (4,99 cm zu 4,19 cm), hier konnte ebenso eine Signifikanz festgestellt werden (p=0,007).
Die Entität des zellreichen Leiomyoms trat in dem dieser Arbeit zugrunde liegendem Patientinnenkollektiv deutlich häufiger auf als in den vergleichbaren Gruppen anderer Promotionsarbeiten der PFGS. Dieses Ergebnis wurde dem zuständigen pathologischen Institut mitgeteilt sowie mögliche Ursachen anschließend erörtert.
Über den Referenzwert erhöhte LDH-Werte, eine erhöhte NLR sowie eine erniedrigte LMR kamen auch bei Leiomyomen vor. Der LDH-Wert lag bei den regelhaften Leiomyomen im Mittel bei 208,77 U/l. Es konnte eine signifikante, positive Korrelation zwischen dem LDH- Wert und der Tumorgröße bzw. dem Alter bei Operation festgestellt werden. Weiter unterschied sich der LDH-Wert bei einer Tumorgröße von < 5 cm und ≥ 5 cm mit einem p=0,03 signifikant. Bei den zellreichen Leiomyomen war der LDH-Wert mit im Mittel 203,22 U/l sogar etwas niedriger als bei den regelhaften Leiomyomen. Eine Signifikanz bestand jedoch weder im Vergleich mit den regelhaften Leiomyomen noch in Bezug des LDH-Werts auf die Tumorgröße bzw. das Alter.
Die NLR lag bei den regelhaften Leiomyomen im Mittel bei 2,2. 50 % der Leiomyome zeigten eine NLR von >2,05, 25 % eine NLR > 2,61 und 5 % > 4,56. Bei den zellreichen Leiomyomen lag die NLR im Mittel bei 2,3. Hier zeigten 50 % eine NLR von > 2,2, 25 % eine NLR von > 2,55 und 5 % eine NLR von > 4,56. Bei beiden Entitäten bestand eine positive Korrelation zwischen der NLR und der Tumorgröße (regelhafte Leiomyome, Korrelationskoeffizient: 0,292, p=0,022; zellreiche Leiomyome, Korrelationskoeffizient: 0,35, p=0,03), bei den zellreichen Leiomyomen konnte darüber hinaus ein hoch signifikanter Unterschied in der NLR bei Tumoren < 5 cm und ≥ 5 cm mit einem p=0,009 festgestellt werden.
Die LMR lag bei den regelhaften Leiomyomen im Mittel bei 4,8 und bei den zellreichen Leiomyomen bei 4,69. Es wurden jeweils negative Korrelationskoeffizienten bei dem Bezug von der LMR auf das Alter bei Operation bzw. der Tumorgröße festgestellt, diese waren jedoch nicht signifikant. Allein die LMR bei Patientinnen mit einem regelhaften Leiomyom unterschied sich signifikant zwischen Patientinnen mit einem Alter von < 35 Jahren und ≥ 35 Jahren (p=0,043).
In der Auswertung der Leiomyomdaten der vorliegenden Promotion und den Daten von Köller und Riechmann konnte ein signifikanter Unterschied (p<0,001) zwischen den Leiomyomen (Krichbaum, n=199; Köller und Riechmann, n=470) und den Leiomyosarkomen der PFGS des DKSM (n=271) bei den Variablen Alter, Tumorgröße, Solitärtumor, Zusatzblutung, Postmenopausestatus, schnelles Tumorwachstum und auffällige Sonographie festgestellt werden.
Diese Variablen scheinen also zur präoperativen Unterscheidung zwischen Leiomyomen und Leiomyosarkomen geeignet zu sein.
Im Ergebnis dieser und der Arbeit von Köller und Riechmann konnte vom DKSM in Zusammenarbeit mit dem Institut für Bioinformatik der Universitätsmedizin Greifswald ein zur Publikation eingereichter LMS-Risiko-Score entwickelt werden, der zur Entscheidung über eine geeignete Operationsmethode bei vermuteten LM verwendet werden kann.
Ein weiteres wesentliches Ergebnis dieser Arbeit ergibt sich aus den erhobenen Werten der neutrophilen Granulozyten/Lymphozyten-Ratio (NLR) und der Lymphozyten/Monozyten- Ratio (LMR). Die NLR lag bei den regelhaften Leiomyomen im Mittel bei 2,2, nur ca. 5 % wiesen eine NLR von ≥ 4,5 auf. Der in der Literatur diskutierte NLR-Grenzwert von ≥ 2,1 bzw. ≥ 2,12 wurde also regelhaft überschritten. Eine NLR von ≥ 4,5 wird somit als neuer Grenzwert zur Überprüfung vorgeschlagen. Die LMR lag bei den regelhaften Leiomyomen im Mittel bei 4,8, allerdings unterschritten immerhin noch 14,75% der ermittelten Werte den in der Literatur diskutierten Grenzwert von < 2,85. Da weniger als 5 % der regelhaften LM eine LMR von <2,3 aufwiesen, wird dieser Wert als neuer Grenzwert zur Überprüfung empfohlen.
The aim of the present dissertation was to investigate the biological and chemical potential of two European mushroom species: Fomitopsis betulina and Calvatia gigantea. For this purpose, different extracts of both fungi were tested for: antimicrobial, antifungal, cytotoxic, in vitro wound healing, and anti-adhesive properties. Bioassay-guided fractionation led to the isolation of bioactive compounds, altogether 20 compounds were isolated and identified. The compounds were obtained from the ethyl acetate extracts, they included triterpenes, sterols and aromatic compounds. The separated substances from both fungi were proved for biological activities, some of them showed antimicrobial and cytotoxic activities.
Due to a variety of plasma sources in terms of type of discharge, energy yield, working gas or geometric factors, it is recommended to standardize the study protocol by choosing a plasma source and easy access to rugged tumor surfaces as demonstrated by the CAP-plasma-jet. The intention of the trial shall be to optimize the plasma jet for tumor site capability and operating room implementation.
It makes sense to start clinical trials in plasma medicine with the treatment of head and neck squamous cell carcinoma patients of infected wounds and ulcerations.
CAP is able to reduce contamination of cancer ulcerations and the typical fetid odor that often accompanies head and neck cancer patients. The intention of the trial shall be to evaluate the efficiency of decontamination in head and neck cancer ulcerations in terms of pathogenic species, amount of reduction and reliability.

Procedure:
Plasma is applied for 1 minute per cm², spot area of 3 mm diameter distance between nozzle and tumor surface of 14 mm. 3 times/week with a break of 1 week followed by a repeated cycle for another week.
Conclusion:
The most important intention of the trial from the clinician’s point of view shall be to make CAP-treatment an effective and well-accepted addition to standard cancer therapy based upon EBM at least in palliative medicine.
Staphylococcus aureus has acquired resistance to antibiotics since their first use. The S. aureus protein NorA, an efflux pump belonging to the major facilitator superfamily (MFS), contributes to resistance to fluoroquinolones (e.g., ciprofloxacin), biocides, dyes, quaternary ammonium compounds, and antiseptics. Different compounds have been identified as potential efflux pump inhibitors (EPIs) of NorA that result in increased intracellular concentration of antibiotics, restoring their antibacterial activity and cell susceptibility. However, none of the currently known EPIs have been approved for clinical use, probably due to their toxicity profiles. In the present study, we screened approved drugs for possible efflux pump inhibition. By screening a compound library of approximately 1200 different drugs, we identified nilotinib, a tyrosine kinase inhibitor, as showing the best efflux pump inhibitory activity, with a fractional inhibitory concentration index of 0.1875, indicating synergism with ciprofloxacin, and a minimum effective concentration as low as 0.195 μM. Moreover, at 0.39 μM, nilotinib, in combination with 8 μg/mL of ciprofloxacin, led to a significant reduction in biofilm formation and preformed mature biofilms. This is the first description of an approved drug that can be used as an efflux pump inhibitor and to reduce biofilms formation at clinically achievable concentrations.
Determining the effect of a changing climate on tree growth will ultimately depend on our understanding of wood formation processes and how they can be affected by environmental conditions. In this context, monitoring intra-annual radial growth with high temporal resolution through point dendrometers has often been used. Another widespread approach is the microcoring method to follow xylem and phloem formation at the cellular level. Although both register the same biological process (secondary growth), given the limitations of each method, each delivers specific insights that can be combined to obtain a better picture of the process as a whole. To explore the potential of visualizing combined dendrometer and histological monitoring data and scrutinize intra-annual growth data on both dimensions (dendrometer → continuous; microcoring → discrete), we developed DevX (Dendrometer vs. Xylogenesis), a visualization application using the “Shiny” package in the R programming language. The interactive visualization allows the display of dendrometer curves and the overlay of commonly used growth model fits (Gompertz and Weibull) as well as the calculation of wood phenology estimates based on these fits (growth onset, growth cessation, and duration). Furthermore, the growth curves have interactive points to show the corresponding histological section, where the amount and development stage of the tissues at that particular time point can be observed. This allows to see the agreement of dendrometer derived phenology and the development status at the cellular level, and by this help disentangle shrinkage and swelling due to water uptake from actual radial growth. We present a case study with monitoring data for Acer pseudoplatanus L., Fagus sylvatica L., and Quercus robur L. trees growing in a mixed stand in northeastern Germany. The presented application is an example of the innovative and easy to access use of programming languages as basis for data visualization, and can be further used as a learning tool in the topic of wood formation and its ecology. Combining continuous dendrometer data with the discrete information from histological-sections provides a tool to identify active periods of wood formation from dendrometer series (calibrate) and explore monitoring datasets.
