Background: The plasminogen activator system plays a key role in ovarian cancer (OC) tumor progression. The plasminogen activator inhibitor type 1 (PAI-1) and the recently identified PAI-1 RNA binding protein 1 (PAI-RBP1) are primary regulators of plasminogen activation and thus are putative biomarkers for OC progression. Methods: One hundred fifty six OC patients were analyzed to identify the presence of PAI-1 and PAI-RBP1 and subsequently correlated to clinicopathological parameters. Primary cells obtained from OC patient samples were applied in fluorescence microscopy analysis for examination of PAI-1 and PAI-RBP1 distribution. Results: PAI-1 and PAI-RBP1 have been found to be predictive markers for OC patients' outcome. PAI-1 levels significantly correlated with volume of ascites, FIGO staging, and lymph node status. PAI-RBP1 expression significantly correlated with age at first diagnosis, histological tumor type, presence of distant metastasis (pM), and recurrence. PAI-1 showed a trend toward association and PAI-RBP1 was significantly associated with progression-free survival. Notably, PAI-1 protein in recurrent OC tissues was exclusively localized in the nucleus. Conclusion: This study has shown that a combination of PAI-1 and PAI-RBP1 may represent novel prognostic factor for OC. Prospective trials are needed.
Osteoporose und Atherosklerose wurden lange als unabhängige Erkrankungen mit
gemeinsamen Risikofaktoren betrachtet, jedoch wurde in den letzten Jahren auch ein
unabhängiger Zusammenhang zwischen beiden Erkrankungen postuliert. Bisherige Studien
zur Assoziation zwischen Osteoporose und Atherosklerose sind oft auf postmenopausale
Frauen beschrankt und es gibt es nur wenige Studien, die die Knochengesundheit anhand von Osteosonographiemesswerten beurteilen. Ziel der vorliegenden Arbeit war zu untersuchen, ob es eine unabhängige Assoziation zwischen den Steosonographiemesswerten an der Ferse und der IMT der A. carotis, dem Vorhandensein von Plaques der A. carotis und dem ABI gibt. Grundlage für die Analyse bilden die Daten von SHIP-2 und SHIP-Trend. Multivariate logistische Regressionsmodelle zeigten einen von klassischen kardiovaskularen Risikofaktoren (Hypertonie, Dyslipidamie, Diabetes mellitus) unabhängigen Zusammenhang zwischen den Osteosonographiemesswerten und dem Vorkommen von Plaques der A. carotis bei Männern. Bei Frauen konnte jedoch keine entsprechende Assoziation detektiert werden. Darüber hinaus konnten keine Assoziationen zwischen den Steosonographiemesswerten und der IMT der CCA oder dem ABI in vollständig adjustierten Modellen detektiert werden. IMT und Plaques sind Marker für Atherosklerose, die sich sowohl in ihrer Pathogenese, im Risikofaktorprofil sowie in ihrer Vorhersagefähigkeit fur kardiovaskulare Ereignisse unterscheiden. Plaques stellen dabei den härteren Endpunkt der Atherosklerose dar, was erklären konnte, warum unabhängige Assoziationen zwischen den Osteosonographiemesswerten und Plaques beobachtet wurden aber nicht zur IMT.
Geschlechtsspezifische Unterschiede im Knochenstoffwechsel und der kardiovaskularen
Gesundheit sind denkbare Ursachen für die ausschließlich bei den Männern beobachteten
Assoziationen. Der pathophysiologische Zusammenhang zwischen Osteoporose und
Atherosklerose ist Gegenstand aktueller Forschung. Diskutiert werden u.a. Einflüsse von
Vitamin D, RANK/RANK-L und OPG, MGP, FGF-23 und Klotho, Fetuin A und Cathepsin K.
Während sich die vorliegende Studie durch die große Anzahl an Probanden beider
Geschlechter, einem breiten Altersbereich und einer umfassenden Berücksichtigung von
Confoundern auszeichnet, lasst das querschnittliche Studiendesign keine Ruckschlusse auf
Kausalität der beobachteten Assoziation zu. Weitere Limitationen stellen u.a. die Einnahme
von kardioprotektiver Medikation, die eingeschränkte Vergleichbarkeit mit früheren Studien
aufgrund unterschiedlicher Messmethoden und die Beschränkung der Analysen zum ABI auf
eine Subgruppe dar. Die beobachtete signifikante Assoziation zwischen sinkendem
QUS-basierten SI und dem Vorkommen von Plaques bei Männern weist darauf hin, dass
Männer, die klinische Zeichen einer Osteoporose aufweisen, ein erhöhtes Risiko für
atherosklerotische Erkrankungen, unabhängig von kardiovaskularen Risikofaktoren, wie Alter, BMI, Rauchen, Bewegungsmangel, Diabetes mellitus, Hypertonie und Dyslipidamie haben, sodass diese von einer frühzeitigen Evaluation des atherosklerotischen Risikos profitieren konnten. Insgesamt ist der Zusammenhang zwischen Osteoporose und Atherosklerose komplex und in nachfolgenden Studien wäre es von großem Interesse speziell das männliche Geschlecht in den Fokus zu rucken. Darüber hinaus ist die Betrachtung einzelner Signalstoffe von großem Interesse, um die gegenseitige Beeinflussung beider Erkrankungen tiefgreifender zu verstehen.
Enzalutamid ist seit 2012 ein zugelassenes Medikament für die Hormontherapie nach Docetaxel-Behandlung bei kastrationsresistenten Prostatakarzinom (CRPC). Als bekannter Wirkmechanismus gilt der Antagonismus am Androgenrezeptor (AR) und die Verhinderung der Translokation in den Nukleus, sowie die Inhibition der Transkription am AR.
In dieser Arbeit wurde der Wirkmechanismus von Enzalutamid auf zellulärer und molekularer Ebene präzisiert. In der Wachstumskinetik zeigt sich eine Wachstumshemmung sowohl der AR-positiven LNCaP Zellen, als auch der AR- negativen PC-3 Zellen. Zur Untersuchung des antiproliferativen Effektes von Enzalutamid betrachtet diese Arbeit AR-assoziierte Wachstumsfaktoren, den AR selbst und Östrogenrezeptoren. Als Einflussgrößen gelten die AR-assoziierten Proteine, wie Hitzeschockprotein (HSP) 27, HSP70, HSP90α/β und die Co-Chaperone HSP40 und HSP 70/90 organizing Protein (HOP). Hierbei zeigte vor allem HSP27 aber auch HSP90β eine deutliche Suppression der Protein-Expression in beiden Zelllinien. Die Untersuchung der Proteinexpression der Co-Chaperone, HSP40 in beiden Zelllinien und HOP in LNCaP Zellen, ergab ebenfalls eine deutliche Supression. Diese Proteine, insbesondere HSP27, haben aufgrund ihrer zellulären Funktion Einfluss auf die Proliferation der PC-Zellen, somit kann ein supprimierender Effekt unter Enzalutamid die Erklärung für das eingeschränkte Wachstum der LNCaP Zellen aber auch der hormonresistenten PC-3 Zellen sein. In LNCaP Zellen konnte der supprimierende Effekt von Enzalutamid auf die AR-Expression bestätigt werden. Als weiteren möglichen Proliferationsfaktor wurde in dieser Arbeit der Östrogenrezeptor (ER) mit seinen Isoformen ERα, ERβ1 und ERβ2 untersucht. Es zeigte sich dabei eine deutliche Suppression des ERβ1, welcher in einem möglichen Zusammenhang mit dem AR stehen kann. Die Suppression der ERβ1-Expression zeigte sich aber auch deutlich in AR-negativen PC-3 Zellen, was auch als antiproliferativer Effekt gewertet werden kann. Die Untersuchung der weiteren ER-Isoformen unter Einwirkung von Enzalutamid zeigten keine Effekte.
Das Resultat dieser Arbeit ist, dass Enzalutamid nicht nur ein Hormonrezeptorantagonist ist, sondern auch auf AR-unabhängige Signalkaskaden Einfluss hat. Enzalutamid zeigt im Zellmodell einen deutlich antiproliferativen Effekt, was mit seiner effektiven Wirkung bei CRPC korreliert und auch an den in dieser Arbeit betrachteten Faktoren liegen kann.
Background: Despite optimized medical therapy, severe idiopathic pulmonary arterial hypertension (IPAH) is a devastating disease with a poor outcome. Autoantibodies have been detected in IPAH that can contribute to worsening of the disease. Objectives: The objective of this prospective, open-label, single-arm, multicenter trial was to evaluate the safety and efficacy of immunoadsorption (IA) as an add-on to optimized medical treatment for patients with IPAH. Methods: A total of 10 IPAH patients received IA over 5 days. Their clinical parameters, including hemodynamics measured by right heart catheter, were assessed at baseline and after 3 and 6 months. The primary endpoint was the change in pulmonary vascular resistance (PVR). Secondary endpoints included the change in 6-min walking distance, quality of life, safety, and plasma levels of IgG and autoantibodies. Results: The evaluation of the 10 IPAH patients (75% female; 51 ± 12 years; 166 ± 10 cm; WHO functional class III; 53% on combination therapy) revealed that IA was a safe procedure that efficiently removed IgG and autoantibodies from the circulation. After 3 months, the mean PVR improved significantly by 13.2% (p = 0.03) and the cardiac index improved by 13.1%, but no significant changes were found in 6-min walking distance. The quality of life physical functioning subscale score significantly improved after 6 months. The serious adverse events in 3 patients were possibly related to IA and included pneumonia, temporary disturbance in attention, and thrombocytopenia. Conclusions: IA as an add-on to targeted medical treatment for IPAH is a safe procedure with beneficial effects on hemodynamics, especially in patients with high levels of autoantibodies. Larger-scale controlled studies are needed to assess its efficacy in IPAH and to identify responders.
The M protein of Streptococcus canis (SCM) is a virulence factor and serves as a surface-associated receptor with a particular affinity for mini-plasminogen, a cleavage product of the broad-spectrum serine protease plasmin. Here, we report that SCM has an additional high-affinity immunoglobulin G (IgG) binding activity. The ability of a particular S. canis isolate to bind to IgG significantly correlates with a scm-positive phenotype, suggesting a dominant role of SCM as an IgG receptor. Subsequent heterologous expression of SCM in non-IgG binding S. gordonii and Western Blot analysis with purified recombinant SCM proteins confirmed its IgG receptor function. As expected for a zoonotic agent, the SCM-IgG interaction is species-unspecific, with a particular affinity of SCM for IgGs derived from human, cats, dogs, horses, mice, and rabbits, but not from cows and goats. Similar to other streptococcal IgG-binding proteins, the interaction between SCM and IgG occurs via the conserved Fc domain and is, therefore, non-opsonic. Interestingly, the interaction between SCM and IgG-Fc on the bacterial surface specifically prevents opsonization by C1q, which might constitute another anti-phagocytic mechanism of SCM. Extensive binding analyses with a variety of different truncated SCM fragments defined a region of 52 amino acids located in the central part of the mature SCM protein which is important for IgG binding. This binding region is highly conserved among SCM proteins derived from different S. canis isolates but differs significantly from IgG-Fc receptors of S. pyogenes and S. dysgalactiae sub. equisimilis, respectively. In summary, we present an additional role of SCM in the pathogen-host interaction of S. canis. The detailed analysis of the SCM-IgG interaction should contribute to a better understanding of the complex roles of M proteins in streptococcal pathogenesis.
Whether mice are an appropriate model for S. aureus infection and vaccination studies is a matter of debate, because they are not considered as natural hosts of S. aureus. We previously identified a mouse-adapted S. aureus strain, which caused infections in laboratory mice. This raised the question whether laboratory mice are commonly colonized with S. aureus and whether this might impact on infection experiments. Publicly available health reports from commercial vendors revealed that S. aureus colonization is rather frequent, with rates as high as 21% among specific-pathogen-free mice. In animal facilities, S. aureus was readily transmitted from parents to offspring, which became persistently colonized. Among 99 murine S. aureus isolates from Charles River Laboratories half belonged to the lineage CC88 (54.5%), followed by CC15, CC5, CC188, and CC8. A comparison of human and murine S. aureus isolates revealed features of host adaptation. In detail, murine strains lacked hlb-converting phages and superantigen-encoding mobile genetic elements, and were frequently ampicillin-sensitive. Moreover, murine CC88 isolates coagulated mouse plasma faster than human CC88 isolates. Importantly, S. aureus colonization clearly primed the murine immune system, inducing a systemic IgG response specific for numerous S. aureus proteins, including several vaccine candidates. Phospholipase C emerged as a promising test antigen for monitoring S. aureus colonization in laboratory mice. In conclusion, laboratory mice are natural hosts of S. aureus and therefore, could provide better infection models than previously assumed. Pre-exposure to the bacteria is a possible confounder in S. aureus infection and vaccination studies and should be monitored.
Purpose
Due to the demographic change morbidity raises the demand for medical hospital services as well as a need for medical specialization, while economic and human resources are diminishing. Unlike other industries hospitals do not have sufficient data and adequate models to relate growing demands and increasing performance to growth in staff capacity and to increase in staff competences.
Method
Based on huge medical data sample covering the years from 2010 to 2014 with more than 150,000 operations of the Department for Anesthesiology at the University Hospital Muenster, Germany, comparisons are drawn between the development of medical services and the development of personnel capacity and expertise.
Results
The numbers of surgical operations increased by 21% and “skin incision to closure” time by 17%. Simultaneously, personnel capacity grew by 16% largely resting upon recruiting first-time employees. Expertise measured as “years of professional experience” dwindled from 10 years to 5.4 years on average and staff turnover accelerated.
Conclusion
Static benchmark data collected at fixed reference dates do not sufficiently reflect the nexus between capacity and competence and do not reflect the dynamic changes in a hospital’s requirements for expertise and specialization, at all. Staff turnover leads to a loss of experience, which jeopardizes patient safety and hampers medical specialization. In consequence of the dramatic shortage of medical specialists, drop-off rates must be reduced and retention rates must be increased. To that end, working conditions need to be fundamentally converted for a multigeneration, multicultural, and increasingly female workforce.
Introduction
Primary immunodeficiency disorders (PIDs) are a heterogeneous group of more than 200 rare diseases. Timely diagnosis is of uttermost importance. Therefore, we aimed to develop a diagnostic questionnaire with computerized pattern-recognition in order to support physicians to identify suspicious patient histories.
Materials and methods
Standardized interviews were conducted with guardians of children with PID. The questionnaire based on parental observations was developed using Colaizzis’ framework for content analysis. Answers from 64 PID patients and 62 controls were analyzed by data mining methods in order to make a diagnostic prediction. Performance was evaluated by k-fold stratified cross-validation.
Results
The diagnostic support tool achieved a diagnostic sensitivity of up to 98%. The analysis of 12 interviews revealed 26 main phenomena observed by parents in the pre-diagnostic period. The questions were systematically phrased and selected resulting in a 36-item questionnaire. This was answered by 126 patients with or without PID to evaluate prediction. Item analysis revealed significant questions.
Discussion
Our approach proved suitable for recognizing patterns and thus differentiates between observations of PID patients and control groups. These findings provide the basis for developing a tool supporting physicians to consider a PID with a questionnaire. These data support the notion that patient’s experience is a cornerstone in the diagnostic process.
For the normal development of pregnancy, a balance between immune tolerance and defense is crucial. However, the mechanisms mediating such a balance are not fully understood. CD83 is a transmembrane protein whose expression has been linked to anti-inflammatory functions of T and B cells. The soluble form of CD83, released by cleavage of the membrane-bound protein, has strong anti-inflammatory properties and was successfully tested in different mouse models. It is assumed that this molecule contributes to the establishment of immune tolerance. Therefore, we postulated that the expression of CD83 is crucial for immune tolerance during pregnancy in mice. Here, we demonstrated that the membrane-bound form of CD83 was upregulated in T and B cells during allogeneic murine pregnancies. An upregulation was also evident in the main splenic B cell subtypes: marginal zone, follicular zone, and transitional B cells. We also showed that there was an augmentation in the number of CD83+ cells toward the end of pregnancy within splenic B and CD4+ T cells, while CD83+ dendritic cells were reduced in spleen and inguinal lymph nodes of pregnant mice. Additionally, B lymphocytes in late-pregnancy presented a markedly higher sensitivity to LPS in terms of CD83 expression and sCD83 release. Progesterone induced a dosis-dependent upregulation of CD83 on T cells. Our data suggest that the regulation of CD83 expression represents a novel pathway of fetal tolerance and protection against inflammatory threats during pregnancy.

Mittels der vorliegenden Dissertation sollten, anhand eines größeren Patientinnenkollektivs, uterine Leiomyosarkome (LMS) umfassend charakterisiert werden um als Vergleichsmaterial zu regelhaften Leiomyomen (LM) für die Errechnung eines präoperativen LMS-Risiko-Scores zu dienen.
Zu diesem Zweck wurden insgesamt 240 LMS-Beratungsfälle des Deutschen klinischen Kompetenzzentrums für genitale Sarkome und Mischtumoren der Universitätsmedizin Greifswald (DKSM) aus dem Zeitraum August 2009 – September 2017 retro- und prospektiv erfasst und analysiert. Von diesen wurden 154 Fälle einer vorangegangenen Promotion (126) des DKSM entnommen. Letztere wurden mit erweiterten bzw. neu hinzugekommenen Kriterien aktualisiert. Um eine vergleichende Auswertung mit LM zu ermöglichen, war es erforderlich, die relevanten Kriterien in ein Schema zu integrieren, das simultan bei den LM-Analysen der anderen Arbeitsgruppen des DKSM zur Anwendung kam.
Das mediane Alter von 52 Jahren und die höchste Inzidenz zwischen 50 – 59 Jahren decken sich mit Angaben aus der Literatur. Der Anteil postmenopausaler Frauen lag mit 60,8 % über den ermittelten Werten anderer Autoren.
Die anamnestischen Angaben, wie eine Tamoxifenexposition, eine vorausgegangene medikamentöse Therapie mit Ulipristalacetat (UPA) und vorausgegangene Operationen wegen Varianten des LM und eines STUMP (glattmuskulärer Tumor mit unsicherem malignem Potential) spiegeln die Literaturangaben nur bedingt wider. Ein Therapieversagen von UPA bei symptomatischen LM scheint ein Hinweis auf ein uterines Sarkom zu sein. Allerdings wurden in dieser Untersuchung insgesamt nur 8 entsprechende Fälle gefunden. Eine vorausgegangene Operation wegen Varianten des LM bzw. eines STUMP fand sich nur in 2,1 % der Fälle.
Betreffs der Blutungsstörungen wurden bei 44 % der prämenopausalen Frauen intermenstruelle Zusatzblutungen und bei 45,2 % Blutungen in der Postmenopause beschrieben. Zudem wurden tumorbezogene Symptome ohne jegliche Blutungsstörungen, wie uterine oder Unterbauchschmerzen, ein vermehrter Harndrang sowie ein Druckgefühl oder eine Zunahme des Abdominalumfanges in 46,3 % der Fälle beobachtet.
In mehr als der Hälfte der Fälle (53,4 %) lag ein schnelles Tumorwachstum vor. Der Wert lag in der Postmenopause sogar bei 55,5 %. In 45,5 % wurde das LMS als weich bzw. auffällig weich beschrieben.
In 99,6 % aller Fälle wurde eine diagnostische Sonografie durchgeführt. Dabei zeigte sich in 82 % ein als auffällig bezeichneter Befund.
Der Durchmesser der beobachteten LMS betrug median 9 cm. Am häufigsten wiesen die LMS eine Größe zwischen 6 – 10 cm auf. Der Anteil der solitären LMS (keine zusätzlichen LM) lag bei 51,7 %. Der mittlere Durchmesser der solitären LMS betrug 10,3 cm.
Bei 73 der Fälle (30 %) wurde eine diagnostische Abrasio oder eine direkte Biopsie durchgeführt. Unter diesen wurde nur in 38,4 % primär die richtige Diagnose gestellt. Die Literaturangaben zur Stadienverteilung der LMS decken sich nicht mit eigenen Ergebnissen, nach denen sich 79,6 % der Fälle im Stadium I und nur 0,4 % im Stadium IV befanden. Ebenfalls widersprechen die eigenen Daten den Angaben bezüglich einer Lymphknotenbeteiligung. In insgesamt 80,4 % (n = 189 Fälle) wurde keine Lymphonodektomie (LNE) durchgeführt. Insgesamt 10,9 % der LMS mit einer LNE (n = 46 Fälle) wiesen positive Lymphknoten auf.
Die Literaturangaben zur Netz- und Zervixbeteiligung konnten bestätigt werden. Von 235 operierten Frauen fand nur in 21 Fällen (13,2 %) eine Netzresektion statt. In 4 dieser Fälle war das Netz vom Tumor befallen. In 15,1 % der Fälle der vorliegenden Arbeit wurde ein Zervixbefall im Sinne einer Infiltration durch einen korporalen Tumor bzw. einer primären Lokalisation in der Zervix gefunden.
In insgesamt 67,7 % aller Fälle wurde das LMS primär nicht erkannt und konsekutiv unter der falschen Indikation operiert. In 66,0 % bildete ein LM die Operationsindikation bei der Enddiagnose LMS. Dementsprechend wurden nur 35,7 % der LMS mittels totaler Hysterektomie ohne Uterus- und/oder Tumorverletzung operiert.
In 71,3 % der Fälle lagen Daten zur Mitosenzahl je 10 HPF vor. Der errechnete Mittelwert aller LMS betrug 20,7 M/10 HPF und speziell die der spindelzelligen LMS 21,7 M/10 HPF. Diese Zahlen decken sich mit den Angaben aus der Literatur. Das gilt gleichermaßen für die Expression des Ki67-Antigens (im Mittel 37,2 %). Die 5-Jahres- Überlebenswahrscheinlichkeit (Nomogramm Overall Survival Probability nach Zivanovic) lag bei 144 berechenbaren Fällen median bei 48 %. In den vorliegenden Fällen konnten in 8,3 % primäre Fernmetastasen nachgewiesen werden. Diese fanden sich mit 65,1 bzw. 25,3 % am häufigsten in der Lunge und/oder der Leber. Eine Ausbreitung in die Knochen sowie in die Knochen plus simultan in jegliche anderweitigen Strukturen lag nur in jeweils 6,0 % vor.
Die gewonnenen Ergebnisse wurden in der Zentrale des DKSM zur Berechnung des LMS-Risiko-Scores mittels Vergleich mit 670 prospektiv analysierten LMS aus 2 weiteren Promotionen verwendet. Der Score ist zwischenzeitlich publiziert worden.
