Alcohol-related somatic disorders are highly prevalent among general hospital inpatients. Alcohol problem drinking can be differentiated into alcohol use disorders (alcohol dependence and alcohol abuse) and three subtypes of drinking above recommended levels (at-risk drinking only [AR], heavy episodic drinking only [HE], at-risk and heavy episodic drinking [ARHE]). The aims of this study were threefold. First, proportions of alcohol problem drinking among general hospital inpatients in a region of north-eastern Germany were estimated (study 1). Second, among individuals with alcohol problem drinking the association between beverage preference and alcohol-related diseases was tested (study 2). Third, subtype differences regarding demographics, alcohol-related variables, motivation to change drinking behaviors, and the risk to develop short-term alcohol dependence among the three subtypes of drinking above recommended levels were analyzed (study 3). The data presented in this dissertation are based on the randomized controlled trial “Early Intervention at General Hospitals”, which is part of the Research Collaboration Early Substance Use Intervention (EARLINT). Study 1 includes a sample of consecutively admitted general hospital inpatients between 18 and 64 years old (n = 14,332). The study adopted a two-stage-sampling approach: (a) screening and (b) ascertainment of alcohol problem drinking. Those who were identified with alcohol problem drinking were asked for written consent for further study participation. This included further baseline assessment, the consent to use routine treatment diagnoses and participation in a follow-up interview 12 months after hospitalization. For study 2, routine treatment diagnoses were provided by hospital physicians for a total of 1,011 men with problem drinking. These diagnoses were classified into three categories according to their alcohol-attributable fractions (AAF): diseases totally attributable to alcohol by definition (AAF=1), diseases partially attributable to alcohol (AAF<1) and diseases with no empirical relationship to alcohol or with a possibly protective effect associated with alcohol (AAF=0). Study 3 was restricted to study participants with drinking above recommended levels (n=425). Study 1: Among all general hospital inpatients, 8.9% were identified with current problem drinking in the following descending order: 5.3% exhibited alcohol use disorders and 3.6% drinking above recommended levels. Higher proportions of problem drinking were found at rural sites compared to urban sites (13.7 vs. 7.5%, p<.001). Study 2: Because of the low proportion of women with alcohol problem drinking the following analyses were restricted to males. Multinomial regression analyses revealed different risks for alcohol-related diseases in relation to beverage preference while controlling for alcohol-associated and demographic confounders. Compared to all other groups, spirits only drinkers had the highest risk for having a disease with AAF>0; e.g., beer only drinkers had lower odds of having a disease with AAF<1 (odds ratio, OR=0.50, 95% confidence interval, CI: 0.27-0.92). Study 3: Men with alcohol use disorder were excluded from the following analysis. At baseline, multinomial logistic regression revealed differences between individuals with AR, HE and ARHE while controlling for age. ARHE was associated with higher odds of having a more severe alcohol problem (OR=2.06, CI: 1.23-3.45), using formal help (OR=2.21, CI: 1.02-4.79), and having a disease with AAF=1 (OR=3.43, CI: 1.58-7.43), compared with AR. In addition, individuals with ARHE had higher odds of taking action to change drinking behaviors (i.e., beginning to implement change) than individuals with HE (OR=2.29, CI: 1.21-4.34) or AR (OR=2.11, CI: 1.15-3.86). At follow-up, individuals with ARHE had higher odds of having alcohol dependence according to the DSM-IV (OR=4.73, CI: 1.01–22.20) compared to individuals with AR. In addition to alcohol use disorders, drinking above recommended levels is a common problem among general hospital inpatients. Thus, the implementation of systematic alcohol screening and brief interventions should be considered. These data suggest an association between beverage preference and alcohol-related diseases. Among hospitalized problem drinkers, spirits only drinkers had the greatest risk of having diseases with AAF>0. Of the three subtypes of drinking above recommended levels, ARHE seems to be particularly problematic because there appears to be an indication of a subclinical diagnosis. To provide adequate intervention, clinical practice should distinguish between the three groups of drinking above recommended levels. Brief alcohol intervention should be tailored to the individual’s motivation to change and to the type of alcohol problem drinking. The effectiveness of such a procedure remains to be evaluated in further studies.
Hintergrund: Alkoholassoziierte Morbidität und Mortalität ist ein bedeutender Kostenfaktor im Gesundheitswesen. Daher sind genaue Kenntnisse über den Zusammenhang zwischen Alkoholkonsum und der Inanspruchnahme medizinischer Leistungen von Relevanz für alle Entscheidungsträger im Gesundheitswesen. Studien aus Japan, Kanada, Spanien und den USA zeigen, dass Personen mit riskantem Alkoholkonsum und Personen, die derzeit abstinent leben, seltener medizinische Hilfe in Anspruch nehmen als leichte Alkoholkonsumenten. Ziel dieser Arbeit ist es, diesen beschriebenen U-förmigen oder umgekehrt linearen Zusammenhang in der bundesdeutschen Allgemeinbevölkerung zu bestätigen. Außerdem werden zwei Erklärungsansätze geprüft, welche dem Befund einer höheren Inanspruchnahme medizinischer Leistungen bei abstinent lebenden Personen zugrunde liegen könnten. Methode: Die Daten wurden im Rahmen der Study of Health in Pomerania (SHIP) und des Bundesgesundheitssurvey 1998 (BGS) erhoben und basieren auf Zufallsstichproben der erwachsenen Allgemeinbevölkerung. Die Studienregion der SHIP umfasst die Region Nordost-Vorpommern und erreichte mit N = 4.310 eine Ausschöpfungsquote von 69%. Der BGS umfasste das gesamte Bundesgebiet; N = 7.124 nahmen an der Untersuchung teil (Ausschöpfung 60%). In beiden Querschnittsstudien wurden mittels Fragebögen, computergestützten Interviews und medizinischen Untersuchungen Daten erhoben, welche mit Hilfe multivariabler statistischer Verfahren ausgewertet wurden. Ergebnisse: In der erwachsenen Allgemeinbevölkerung bestand eine höhere Nachfrage medizinischer Leistungen bei abstinent lebenden Personen als bei Konsumenten mit moderatem Alkoholkonsum. Darüber hinaus fand sich ein umgekehrter Zusammenhang zwischen Alkoholkonsum und der Inanspruchnahme medizinischer Leistungen. Zwei Hypothesen für die höhere Inanspruchnahme abstinent lebender Personen wurden untersucht. (1) Ehemalige Risikokonsumenten, die derzeit keinen Alkohol trinken, nahmen mehr ambulante Leistungen wahr als andere Abstinente. (2) Abstinent lebende Personen und Alkoholkonsumenten unterschieden sich hinsichtlich sozialer, lebensstil- und gesundheitsbezogener Risikofaktoren, welche mit einem schlechteren Gesundheitszustand und höherer Inanspruchnahme in Verbindung stehen. Die Berücksichtigung dieser Merkmale als konfundierende Variablen im statistischen Modell verdeutlichte, dass insbesondere alkoholassoziierte Erkrankungen, welche bei Abstinenten häufiger auftraten, für den Befund einer höheren Inanspruchnahme abstinenter Personen verantwortlich sind. Diskussion: Angesichts der vorliegenden Ergebnisse muss davon ausgegangen werden, dass die in internationalen Studien gefundene höhere Inanspruchnahme medizinischer Leistungen auf unberücksichtigte konfundierende Variablen sowie auf eine Fehlklassifikation der ehemaligen Risikokonsumenten zurückzuführen ist. Dieser Befund ist auch für die wissenschaftliche Evidenz zum protektiven Effekte des moderaten Alkoholkonsum von Relevanz, weil in den meisten Studien Konfundierung und Fehlklassifikation bei der Datenanalyse nicht ausreichen Berücksichtigung gefunden haben.
Background: Cardiovascular disease (CVD) remains the major cause of mortality and morbidity worldwide and produces large productivity loss. The majority of CVD mortality could be prevented with changes in modifiable risk factors including tobacco use, physical inactivity, unhealthy diet and harmful use of alcohol. Successful behavioral prevention of CVD requires the identification of relevant target behaviors and reach of populations at risk. Presenteeism i.e. attending work while ill is discussed as a work-related risk factor for CVD. However, little is known about the interplay of presenteeism with established health risk behaviors. The first aim of this dissertation was to examine the association of presenteeism with health behaviors (study 1). The second aim was to examine factors that can enhance the public health impact of CVD prevention efforts. Therefore, the effect of recruitment strategy used on reach (study 2) and of communication channel used on intervention usage (study 3) was examined. Methods: Study 1 comprised data from 710 Australian employees aged 18 years and older who completed an online-survey. Linear regression analysis was used to examine the association of health behaviors (physical activity, work and non-work-related sitting time, sleep duration and sleep quality) with presenteeism. For study 2 individuals aged 40-65 years were invited to a two-stage cardio-preventive program including an on-site health screening and a cardiovascular examination program (CEP) using face-to-face recruitment in general practices (n = 671) and job centers (n = 1,049), and mail invitations from a health insurance company (n = 894). Recruitment strategies were compared regarding three aspects of reach: (1) participation rate, (2) participants’ characteristics i.e. socio-demographics, self-reported health and CVD risk factors, and (3) predictors of program participation. Study 3 compromised 16,948 users (aged 18 years and older) of the feely available physical activity promotion program 10,000 Steps. Users were grouped based on which platform (website, app) they logged their physical activity: Web-only, App-only, or Web-and-app. Groups were compared on socio-demographics, engagement parameters and logged physical activity. Non-usage attrition i.e. discontinued program usage over the first three months was examined using Kaplan-Meier survival curves. A Cox regression model was used to determine predictors of non-usage attrition. Results: Analyses from study 1 revealed that presenteeism was associated with poor sleep quality and suboptimal sleep duration after controlling for socio-demographics, work and health-related variables. Engaging in three health risk behaviors was associated with higher presenteeism compared with engaging in none or one. Study 2 showed screening participation rates of 56.0%, 32.8%, 23.5% for general practices, job centers and the health insurance company, respectively. Participation rate for the CEP among eligible individuals was 80.3%, 65.5%, and 96.1%, respectively. Job center clients showed the lowest socio-economic status and the most adverse CVD risk pattern. Whereas being female predicted screening participation across all strategies, higher age predicted screening participation only within individuals recruited via the health insurance company. Within general practices and job centers CEP participants were less likely to be smokers than non-participants. Study 3 revealed that engagement with the program was highest for Web-and-app users. Cox regression showed that user group predicted non-usage attrition: Web-and-app users (hazard ratio = 0.86; P < .001) and App-only users (hazard ratio = 0.63; P < .001) showed a reduced attrition risk compared to Web-only users. Further, older age, being male, being non-Australian, higher program engagement and higher number of steps logged were associated with reduced non-usage attrition risk. Conclusion: The results of this dissertation have three implications for designing CVD behavioral interventions with a high public health impact. First, employees suffering from presenteeism may require interventions addressing health risk behaviors including suboptimal sleep behaviors. Second, implementing prevention efforts in job centers may be especially useful to reduce health inequalities induced by social gradient. Third, the population impact of web-based interventions may be increased when using mobile delivery channels.
This study aims to analyze psychometric properties and validity of the Compulsive Internet Use Scale (CIUS) and the Internet Addiction Test (IAT) and, second, to determine a threshold for the CIUS which matches the IAT cut-off for detecting problematic Internet use. A total of 292 subjects with problematic or pathological gambling (237 men, 55 women) aged 14-63 years and with private Internet use for at least 1 h per working or weekend day were recruited via different recruitment channels. Results include that both scales were internally consistent (Cronbach's α = 0.9) and had satisfactory convergent validity (r = 0.75; 95% CI 0.70-0.80). The correlation with duration of private Internet use per week was significantly higher for the CIUS (r = 0.54) compared to the IAT (r = 0.40). Among all participants, 25.3% were classified as problematic Internet users based on the IAT with a cut-off ≥40. The highest proportion of congruent classified cases results from a CIUS cut-off ≥18 (sensitivity 79.7%, specificity 79.4%). However, a higher cut-off (≥21) seems to be more appropriate for prevalence estimation of problematic Internet use.
Abstract
Aims
To demonstrate the attitudes of general practitioners (GPs), nurses, persons with dementia, and caregiver towards suitable tasks and qualification needs for and the acceptance and impact of advanced nursing roles in German dementia primary care.
Design
Observational study using a questionnaire survey with 225 GPs, 232 nurses, 211 persons with dementia, and 197 caregivers, conducted between December 2017–August 2018.
Methods
A questionnaire was generated that includes specific assessment, prescription, and monitoring tasks of advanced nursing roles in dementia primary care as well as qualification requirements for and the acceptance and the impact of advanced nursing roles. Data were analysed using descriptive statistics. Group differences were assessed using the Fisher's exact test.
Results
Advanced nursing roles were highly appreciated across all groups. Assessment and monitoring tasks were rated as highly suitable, and prescription authorities as moderately suitable. Nurses felt less confident in assessment and monitoring, but more confident in prescribing as practitioners expected. Patients and caregivers would appreciate a takeover of tasks by nurses; nurses and practitioners preferred a delegation. A dementia‐specific qualification was rated as best suitable for advanced nursing roles, followed by ‘no specific qualification’ if medical tasks that only can be carried out by practitioners were delegated and an academic degree if tasks were substituted. Advanced nursing roles were rated as beneficial, strengthening the confidence in nursing care and improving the cooperation between professionals and the treatment. Practitioners assumed that advanced nursing roles would improve job satisfaction of nurses, which was not confirmed by nurses.
Conclusion
There is an extended consensus towards the enlargement of advanced nursing roles, represented by high endorsement, acceptance, and willingness to reorganize tasks.
Impact
Results debunk the common notion that German practitioners would be reluctant towards advanced nursing roles and a takeover of current practitioner tasks, supporting the implementation of advanced nursing roles in Germany.
ZUSAMMENFASSUNG HINTERGRUND: In dieser Arbeit werden subjektiv wahrgenommene Arbeitsbedingungen im Zusammenhang mit psychischen Störungen untersucht. Die theoretischen Grundlagen beruhen auf den Annahmen des Anforderungs-Kontroll-Modells. ZIEL: Zum einen wird das „Normative und Subjektive Verfahren der Arbeitsanalyse" (NUSA) validiert. Zum anderen werden Hypothesen zum Zusammenhang von subjektiven Arbeitsbedingungen und psychischen Störungen in einer einwohnermeldeamtsbasierten Stichprobe mit international standardisierten Erhebungsinstrumenten geprüft. METHODE: Die Untersuchung ist Teil der Studie „Transitions in Alcohol Consumption and Smoking" (TACOS). Durch das vollstandardisierte „Münchener Composite International Diagnostic Interview" (M-CIDI) wurden Daten zur klassifikatorischen Diagnostik psychischer Störungen und die Gesamtwerte des Five-Item Mental Health Screening Test (MHI-5), des Alcohol Use Disorders Identification Test (AUDIT) und des Fragerström Test for Nicotine Dependence (FTND) als abhängige Variablen genutzt. ERGEBNISSE: Als Ergebnis der Validierung des NUSA wurden die Dimensionen Arbeitsanforderungen und Handlungsspielraum und die Skala der physikalischen Arbeitsbedingungen repliziert. Zur Überprüfung des Zusammenhangs von Arbeitsbedingungen und psychischen Störungen ließen sich bei den Männern die Annahmen des Anforderungs-Kontroll-Modells nur für den MHI-Gesamtwert und für die Nikotinabhängigkeit und ihre Schwere, jedoch für keine weitere Störung empirisch validieren. Diesbezüglich fanden sich bei Frauen empirische Beweise für die Annahmen des Anforderungs-Kontroll-Modells für die abhängigen Variablen Schwere der Nikotinabhängigkeit, Angststörungen und die Komorbidität psychischer Störungen. DISKUSSION: Das NUSA kann durch die Validierung als tätigkeitsunabhängige Beschreibung von gesundheitsrelevanten Arbeitsbedingungen universell verwendet werden. Die Analysen in dieser Arbeit lassen die Annahmen des Anforderungs-Kontroll-Modells nur begrenzt bestätigen. Es ist auf die über Arbeitsbedingungen hinausgehende multifaktorielle Bedingtheit psychischer Störungen hinzuweisen.
Die Förderung der Intention zur Tabakabstinenz in der Hausarztpraxis – Merkmale von Hausärzten

Hintergrund: Das Tabakrauchen ist in den Industrienationen der bedeutendste vermeidbare Faktor unter den Gesundheitsrisiken. Daher ist die Senkung der Prävalenz ein wichtiges Ziel der Präventivmedizin. Verschieden Studien haben gezeigt, dass kurze und proakive Interventionen, wie eine Beratung oder der Ratschlag eines Hausarztes, die Wahrscheinlichkeit eines Rauchstopps erhöhen. Hausärzte erreichen einen großen Anteil der Raucher in ihrer Praxis. Obwohl kosteneffektive Interventionen verfügbar sind und deren Anwendung in den Leitlinien empfohlen wird, erfolgen sowohl ein systematisches Screening bezüglich des Rauchstatus als auch ein Beratungsangebot zum Rauchen im Praxisalltag bislang unzureichend. Das Ziel dieser Studie war es zu untersuchen 1) in welchem Umfang Raucher mit einem systematischen Beratungsangebot erreicht werden können, (2) welches Beratungsverhalten bei Hausärzten nach einer Schulung erzielt werden kann, wenn organisatorische Unterstützung stattfindet, (3) ob zwischen dem Rauchstatus des Hausarztes und der Wirksamkeit einer Kurzberatung ein Zusammenhang besteht. Methode: Die Daten wurden im Rahmen der Studie Pro GP „Proactive interventions for smoking cessation in General medical Practices“ erhoben. In dieser Studie wurde mit einem quasi experimentellen Design die Wirksamkeit von Interventionen zur Tabakabstinenz untersucht. Dazu wurde eine Auswahl von 39 Hausarztpraxen zufällig gezogen, 34 Praxen nahmen teil. Die Hausärzte erhielten eine Schulung von bis zu zwei Stunden in ihrer Praxis. Alle Patienten, die die Praxis im Verlauf einer Woche aufsuchten, wurden nach ihrem Rauchstatus gefragt (N=3434). Die Einschlusskriterien der Studie, aktuell Raucher und zwischen 18 und 70 Jahre alt erfüllten n=551 Patienten, 81,8% nahmen an der Studie teil. Mit einem Dokumentationsbogen, ausgefüllt durch eine Studienassistentin, wurden tabakrauchbezogenen Angaben des Patienten an den Hausarzt weitergeleitet. Der Hausarzt wurde darauf hingewiesen, für jeden Patienten nach der Beratung eine Kurzdokumentation auszufüllen. Die Patienten wurden 6, 12, 18 und 24 Monate nach der Kurzberatung wiederbefragt. Die Daten im Querschnitt wurden deskriptiv analysiert und eine logistische Regression gerechnet. Im Längsschnitt fand aufgrund der hierarchischen Struktur der Daten eine Multilevel Analyse Anwendung. Ergebnisse: Die Prävalenz des Rauchens unter den Hausärzten lag bei 24,6%. Weiterhin zeigten die Daten, dass 45,9% (n=17) der Hausärzte regelmäßig den Rauchstatus der Patienten bei Erstvorstellung in der Praxis erheben. Häufige genannte Barriere gegen die Beratung von Rauchern war neben Zeitknappheit, das vermutetes mangelndes Interesse auf Seiten der Patienten. In 96% (n=433) der Fälle konnte der Hausarzt das Thema Rauchen ansprechen und in 87% (n=396) dokumentierten die Hausärzte, dass ein Beratungsgespräch zum Rauchen stattgefunden hat. Eine logistische Multilevel- Regression hat gezeigt, dass durch die Kurzberatung, durchgeführt von einem nichtrauchenden Hausarzt bei den Patienten eine erhöhte Wahrscheinlichkeit für eine 24 Stunden- Nikotin- Abstinenz besteht. Eine schwächere Assoziation wurde für das Maß der 4- Wochen- prolongierten Tabakabstinenz gefunden. Diskussion: Trotz der berichteten und wahrgenommenen Barrieren wurde die Beratung von Rauchern durch die Hausärzte umgesetzt. Mit maßgeschneiderten Interventionskonzepten können nahezu alle rauchenden Patienten erreicht werden. Dass Hausärzte selbst mit dem Rauchen aufhören ist eine wichtige Voraussetzung, um ein persönliches und für die Patienten effektives Kurzberatungsangebot zu implementieren. Es sind spezielle Angebote, z. B. von Berufsverbänden erforderlich, damit sich die Motivation unter Ärzten, mit dem Rauchen aufzuhören, erhöht.
