Over the past 10 years, the crisis of sepsis has remained a great challenge. According to data from 2016, the sepsis-related mortality rate remains high. In addition, sepsis consumes extensive medical resources in intensive care units, and anti-inflammatory agents fail to improve sepsis-associated hyperinflammation and symptoms of immunosuppression. The specific immune mechanism of sepsis remains to be elucidated. Reactive oxygen species (ROS) are triggered by energy metabolism and respiratory dysfunction in sepsis, which not only cause oxidative damage to tissues and organelles, but also directly and indirectly promote NOD-, LRR-, and pyrin domain-containing protein 3 (NLRP3) inflammasome activation. NLRP3 inflammasomes enlarge the inflammatory response and trigger apoptosis of immune cells to exacerbate sepsis progression. Inhibiting the negative effects of ROS and NLRP3 inflammasomes therefore provides the possibility of reversing the excessive inflammation during sepsis. In this review, we describe the interaction of ROS and NLRP3 inflammasomes during sepsis, provide prevention strategies, and identify fields that need further study.
Die Sepsis ist trotz Einführung neuer diagnostischer und therapeutischer Prinzipien nach wie vor ein Krankheitsbild mit sehr hoher Letalität. Eine hämodynamische Stabilisierung mit Hilfe adäquater Volumengabe und des Einsatzes potenter Vasokonstriktoren gilt als Basistherapie von Patienten mit septischem Schock. Letzteres birgt jedoch die Gefahr einer weiteren Verschlechterung der intestinalen Mikrozirkulation, deren Störung als ein kardinaler Mechanismus für die Entwicklung eines Multiorganversagens gilt. Ziel der Arbeit war es, im Rahmen einer experimentellen Endotoxinämie den Einfluss des synthetischen Vasopressin - Analogons Desmopressin, selektiver Agonist des Vasopressin V2-Rezeptors mit vasodilatatorischen Eigenschaften, auf die intestinale Mikrozirkulation mittels Intravitalmikroskopie zu evaluieren. Hierzu untersuchten wir in den submukösen Venolen der Darmwand von Ratten die Leukozytenadhärenz sowie die Dichten funktioneller, dysfunktioneller und nicht-funktioneller Kapillaren in der Lamina muscularis longitudinalis et circularis und im Stratum mucosae. Zusätzlich wurden pro- und antiinflammatorische Zytokine aus Serumproben mittels Durchflusszytometrie (FACS) bestimmt sowie die Auswirkungen der Therapie auf Körpertemperatur, Blutgase und Laktatkonzentration untersucht. Während der Versuche erfolgte eine konstante Messung des mittleren arteriellen Blutdrucks und der Herzfrequenz als makrohämodynamische Parameter. Die Desmopressin-Applikation führte zu einer signifikanten Verbesserung der Mikrozirkulation in allen untersuchten Darmschichten. In der Lamina muscularis longitudinalis verdreifachte sich die Dichte funktionell perfundierter Kapillaren im Vergleich zu den unbehandelten septischen Tieren (Kapillardichte 35.5 ± 4.81 cm-1 in der LPS-Gruppe vs. 105 ± 4.8 cm-1 in der mit Desmopressin behandelten septischen Gruppe; Mittelwerte ± SEM, je Gruppe n=10, P<0.001). Die Zahl adhärenter Leukozyten konnte in den submukösen Venolen 1. Ordnung signifikant um 22% reduziert werden (259 ± 26 n/mm2 in der LPS-Gruppe vs. 203 ± 17 n/mm2 in der Therapiegruppe; Mittelwerte ± SEM, je Gruppe n=10, P<0.05). Die Serumspiegel der proinflammatorischen Zytokine sanken unter der Therapie mit Desmopressin ebenfalls signifikant. Es konnte beispielsweise eine 39-prozentige Reduktion der TNF-α-Konzentration erreicht werden (429 ± 119 pg/ml in der LPS-Gruppe vs. 262 ± 69 pg/ml in der Therapiegruppe; Mittelwert ± SEM, je Gruppe n=10, P<0.05). Die makrohämodynamischen Parameter wurden von der Substanz nicht beeinflusst. Aufgrund der positiven Effekte des Desmopressins auf intestinale Mikrozirkulation, Leukozytenadhärenz und systemische Inflammation und der fehlenden negativen Beeinflussung der systemischen Hämodynamik erscheint es sinnvoll, in weiterführenden tierexperimentellen und klinischen Studien den Nutzen des Medikamentes in der Sepsistherapie, z.B. als möglicher Kombinationspartner bekannter Vasokonstriktoren, zu evaluieren.
Inflammatory Joint Disease Is a Risk Factor for Streptococcal Sepsis and Septic Arthritis in Mice

Septic arthritis is a medical emergency associated with high morbidity and mortality, yet hardly any novel advances exist for its clinical management. Despite septic arthritis being a global health burden, experimental data uncovering its etiopathogenesis remain scarce. In particular, any interplay between septic arthritis and preceding joint diseases are unknown as is the contribution of the synovial membrane to the onset of inflammation. Using C57BL/6 mice as a model to study sepsis, we discovered that Group A Streptococcus (GAS) – an important pathogen causing septic arthritis - was able to invade the articular microenvironment. Bacterial invasion resulted in the infiltration of immune cells and detrimental inflammation. In vitro infected fibroblast-like synoviocytes induced the expression of chemokines (Ccl2, Cxcl2), inflammatory cytokines (Tnf, Il6), and integrin ligands (ICAM-1, VCAM-1). Apart from orchestrating immune cell attraction and retention, synoviocytes also upregulated mediators impacting on bone remodeling (Rankl) and cartilage integrity (Mmp13). Using collagen-induced arthritis in DBA/1 × B10.Q F1 mice, we could show that an inflammatory joint disease exacerbated subsequent septic arthritis which was associated with an excessive release of cytokines and eicosanoids. Importantly, the severity of joint inflammation controlled the extent of bone erosions during septic arthritis. In order to ameliorate septic arthritis, our results suggest that targeting synoviocytes might be a promising approach when treating patients with inflammatory joint disease for sepsis.
Unabhängig vom medizinischen Fortschritt stellt die Sepsis auch im 21. Jahrhundert ein Krankheitsbild mit hoher Mortalitätsrate, progredienter Inzidenz und zunehmender volkswirtschaftlicher Bedeutung dar. Ein zentraler therapeutischer Faktor ist der Erhalt mikro- und makrozirkulatorischer Hämodynamik. In vorangegangenen Arbeiten konnte gezeigt werden, dass die Modulation von Tyrosinkinasen und Tyrosin-Phosphatasen die Mikrozirkulation in septischen Zuständen positiv beeinflussen kann.
Wir untersuchten die Auswirkungen des Tyrosine receptor kinase B -Agonisten Brain-derived neurotrophic factor (BDNF) auf die intestinale Mikrozirkulation und Leukozyten-Endothel-Interaktion unter experimenteller Endotoxinämie mittels intravitaler Fluoreszenzmikroskopie. Hierzu wurden die funktionelle, dysfunktionelle und nicht-funktionelle Kapillardichte der Lamina muscularis longitudinalis, circularis und des Stratums mucosae sowie die Leukozytenadhärenz der Venolen der submukösen Darmwand bestimmt. Ergänzend erfolgte eine Messung der hämodynamischen Parameter Herzfrequenz und mittlerer arterieller Blutdruck, sowie von Körpertemperatur, Blutgasen und Laktatkonzentration. Eine Bestimmung von Zyto- bzw. Chemokinen erfolgte mittels Fluorescent Bead Immunoassay.
Die intravenöse Applikation von BDNF führte unter Endotoxinämie zu einer signifikant erhöhten konstanten Leukozytenadhärenz in den Venolen dritten Grades der Darmwand und zu einer tendenziellen Zunahme um ca. 33 % in den Venolen ersten Grades. Die funktionelle Kapillardichte zeigte sich hingegen nach Behandlung der Endotoxinämie in den Laminae musculares longitudinalis und circularis tendenziell reduziert. Die Dichte nicht-funktioneller Kapillaren verhielt sich konkordant nach Behandlung mit BDNF tendenziell erhöht. Die Behandlung mit BDNF führte zu keiner Beeinträchtigung der Hämodynamik, jedoch zu einer signifikanten Basenabweichung unter Endotoxinämie. Eine Veränderung der Zytokinspiegel wurde nach BDNF-Applikation nicht verzeichnet.
Die Hinweise auf eine Verschlechterung der intestinalen Kapillarperfusion und der Nachweis einer verstärkten Leukozytenaktivierung nach BDNF-Applikation in systemischer Inflammation identifizieren die Blockade der durch endogenem BDNF vermittelten Kaskaden als mögliches therapeutisches Ziel. Es scheint daher sinnvoll, in weiterführenden tierexperimentellen und ggf. klinischen Studien den Nutzen von BDNF-Antagonisten in der Therapie systemisch-inflammatorischer Zustände wie der Sepsis zu evaluieren
Background
Sepsis is one of the leading causes of preventable deaths in hospitals. This study presents the evaluation of a quality collaborative, which aimed to decrease sepsis-related hospital mortality.
Methods
The German Quality Network Sepsis (GQNS) offers quality reporting based on claims data, peer reviews, and support for establishing continuous quality management and staff education. This study evaluates the effects of participating in the GQNS during the intervention period (April 2016–June 2018) in comparison to a retrospective baseline (January 2014–March 2016). The primary outcome was all-cause risk-adjusted hospital mortality among cases with sepsis. Sepsis was identified by International Classification of Diseases (ICD) codes in claims data. A controlled time series analysis was conducted to analyze changes from the baseline to the intervention period comparing GQNS hospitals with the population of all German hospitals assessed via the national diagnosis-related groups (DRGs)-statistics. Tests were conducted using piecewise hierarchical models. Implementation processes and barriers were assessed by surveys of local leaders of quality improvement teams.
Results
Seventy-four hospitals participated, of which 17 were university hospitals and 18 were tertiary care facilities. Observed mortality was 43.5% during baseline period and 42.7% during intervention period. Interrupted time-series analyses did not show effects on course or level of risk-adjusted mortality of cases with sepsis compared to the national DRG-statistics after the beginning of the intervention period (p = 0.632 and p = 0.512, respectively). There was no significant mortality decrease in the subgroups of patients with septic shock or ventilation >24 h or predefined subgroups of hospitals. A standardized survey among 49 local quality improvement leaders in autumn of 2018 revealed that most hospitals did not succeed in implementing a continuous quality management program or relevant measures to improve early recognition and treatment of sepsis. Barriers perceived most commonly were lack of time (77.6%), staff shortage (59.2%), and lack of participation of relevant departments (38.8%).
Conclusion
As long as hospital-wide sepsis quality improvement efforts will not become a high priority for the hospital leadership by assuring adequate resources and involvement of all pertinent stakeholders, voluntary initiatives to improve the quality of sepsis care will remain prone to failure.
Im Rahmen einer Sepsis oder experimentellen Endotoxinämie zeigt sich eine Aktivierung des Gerinnungs-Fibrinolyse-Systems, des Kontakt- und Komplementsystems. Es kommt es zu einer erhöhten Leukozyten-Endothelinteraktion, einer endothelialen Dysfunktion und einem Capillary-leakage-Syndrom. Bei Sepsis-Patienten kann eine verminderte Konzentration des funktionellen C1-Esterase-Inhibitors festgestellt werden. Durch exogene Zufuhr konnte in klinischen und tierexperimentellen Untersuchungen eine Verminderung des capillary leakage und eine hämodynamische Stabilisierung erreicht werden. Ziel unserer Untersuchungen war die Bestimmung des Einflusses des C1-INH auf hämodynamische, biochemische und intravitalmikroskopische Parameter der Mikrozirkulation unter Endotoxinämie. Die Untersuchungen wurden an der Darmwand (Submukosa-Muskularis-Mukosa)und am Mesenterium durchgeführt. Die Hämodynamik blieb durch den C1-INH im Wesentlichen unbeeinflusst. Es zeigte sich eine Reduktion der funktionellen Kapillardichte der Mukosa des Darmes durch die Endotoxinämie. Die funktionelle Kapillardichte war in der Tunica muscularis circularis im Sinne eines „mismatch“ erhöht. Somit zeigte sich eine typische septische Mikrozirkulationsstörung. Die Gabe von C1-INH konnte die Mukosadurchblutung ohne Einfluss auf dieses „mismatch“ signifikant erhöhen. Die Endotoxinämie führte zu einer vermehrten Leukozytenadhärenz. C1-INH konnte diese Endotoxinwirkung signifikant vermindern. Nach unseren Erkenntnissen ist dies der erste intravitalmikroskopische Nachweis einer verminderten Leukoyzten-Endothel-Interaktion durch C1-INH in postkapillären Venolen der Darmwand. Die zur Quantifizierung des Capillary-leakage-Syndroms gemessene Plasmaextravasation konnte durch C1-INH ebenfalls reduziert werden. Als biochemischen Parameter einer proinflammatorischen Immunantwort zeigte sich ein erhöhter TNF-alpha-Spiegel in beiden Verumgruppen. Im Gegensatz dazu ergab sich durch die Applikation von C1-INH ein Anstieg der Konzentration des antiinflammatorischen Interleukin-10. Zusammenschauend könnten diese C1-INH-Wirkungen, insbesondere eines verminderten capillary leakage, entweder auf die verminderte Leukozytenadhärenz oder die beobachtete Immunmodulation zurückzuführen sein. Nach Analyse der Literatur scheint der von uns gefundene erhöhte Interleukin-10-Spiegel in der Frühphase der Sepsis einen positiven Einfluss aufzuweisen. Weitere tierexperimentelle Arbeiten sollten Adhäsionsmoleküle sowie Parameter des Kontakt- und Komplementsystems erfassen.
Infections are often caused by pathobionts, endogenous bacteria that belong to the microbiota. Trauma and surgical intervention can allow bacteria to overcome host defences, ultimately leading to sepsis if left untreated. One of the main defence strategies of the immune system is the production of highly specific antibodies. In the present proof-of-concept study, plasma antibodies against 9 major pathogens were measured in sepsis patients, as an example of severe systemic infections. The binding of plasma antibodies to bacterial extracellular proteins was quantified using a semi-automated immunoblot assay. Comparison of the pathogen-specific antibody levels before and after infection showed an increase in plasma IgG in 20 out of 37 tested patients. This host-directed approach extended the results of pathogen-oriented microbiological and PCR diagnostics: a specific antibody response to additional bacteria was frequently observed, indicating unrecognised poly-microbial invasion. This might explain some cases of failed, seemingly targeted antibiotic treatment.
Although antigen-specific priming of antibody responses is impaired during sepsis, there is nevertheless a strong increase in IgM and IgG serum concentrations. Using colon ascendens stent peritonitis (CASP), a mouse model of polymicrobial abdominal sepsis, we observed substantial increases in IgM as well as IgG of all subclasses, starting at day 3 and peaking 2 weeks after sepsis induction. The dominant source of antibody-secreting cells was by far the spleen, with a minor contribution of the mesenteric lymph nodes. Remarkably, sepsis induction in splenectomized mice did not change the dynamics of the serum IgM/IgG reaction, indicating that the marginal zone B cells, which almost exclusively reside in the spleen, are dispensable in such a setting. Hence, in systemic bacterial infection, the function of the spleen as dominant niche of antibody-producing cells can be compensated by extra-splenic B cell populations as well as other lymphoid organs. Depletion of CD4+ T cells did not affect the IgM response, while it impaired IgG generation of all subclasses with the exception of IgG3. Taken together, our data demonstrate that the robust class-switched antibody response in sepsis encompasses both T cell-dependent and -independent components.
