Inflammasome activation and formation of ASC specks in patients with juvenile idiopathic arthritis

Objective
The formation of large intracellular protein aggregates of the inflammasome adaptor ASC is a hallmark of inflammasome activation and characteristic of autoinflammation. Inflammasome activated cells release the highly proinflammatory cytokine IL-1β in addition to ASC specks into the extracellular space. Autoinflammatory activity has been demonstrated in systemic JIA, however minimal data exist on the role of inflammasomes in other JIA subtypes. We therefore investigated, if pyroptotic cells are present in the circulation of oligo- and poly-articular JIA.
Methods
Peripheral blood of JIA patients (n = 46) was investigated for ASC speck formation, a key step in inflammasome activation, by flow cytometry and immunofluorescence. Free ASC and proinflammatory cytokine levels were determined by ELISA and multiplex assay.
Results
Oligo-articular JIA patients showed a significantly increased proportion of ASC speck+ monocytes compared to poly-articular JIA patients. In serum free ASC alone is not sufficient to assess inflammasome activity and does not correlate with ASC speck+ monocytes. Compared to control several cytokines were significantly elevated in samples of JIA patients. JIA serum containing antinuclear antibodies, incubated with ASC specks boosts a secondary inflammation by IL-1β production in macrophages.
Conclusion
For the first time, we detect ex vivo inflammasome activation by ASC speck formation in oligo- and poly-articular JIA patients. Most notably, inflammasome activation was significantly higher in oligo- compared to poly-articular JIA patients. This data suggests that inflammasome derived autoinflammation may have a greater influence in the previously thought autoimmune oligo-articular JIA patients.
Bei der juvenilen idiopathischen Arthritis (JIA) handelt es sich um eine multifaktorielle, polygene Autoimmunerkrankung, in deren Pathogenese T-Zellen eine wichtige Rolle spielen. Das cytotoxische T-Lymphozyten-Antigen-4 (CTLA-4) als Oberflächenmolekül auf aktivierten T-Zellen hat negativ-regulatorische Wirkung und supprimiert eine überschießende Immunantwort. Assoziationen verschiedener Single Nucleotid Polymorphisms (SNPs) im Gen des CTLA-4 zu unterschiedlichen Autoimmunerkrankungen sind vorbeschrieben. Ziel dieser Arbeit war es den Einfluss zweier ausgewählter Polymorphismen des CTLA-4 Gens (-318 Promotor C/T und +49 Exon 1 A/G) auf die Ausprägung der JIA und ihrer Unterformen zu untersuchen. 844 DNA-Proben von JIA-Patienten, sowie insgesamt 662 Kontrollen wurden genotypisiert. Die Kontrollgruppe setzte sich aus 500 lokalen Individuen der Region Vorpommern und 162 Personen aus ganz Deutschland zusammen. Die Genotypisierung erfolgte mittels Allelspezifischer Duplex-PCR und anschließender Gelelektrophorese. Beim Vergleich der JIA-Patienten mit der Kontrollgruppe konnte für die Psoriasisarthritis eine Assoziation für den -318 C/T SNP nachgewiesen werden. Für den +49 A/G SNP war keine Assoziation zu finden. Die Allelverteilung der JIA-Patienten zeigte eine nicht signifikante Häufung des A+49-Allels bei Psoriasisarthritis und Enthesitis-assoziierter Arthritis sowie einen signifikanten Zusammenhang des T-318-Allels bei Psoriasisarthritis. Bei Betrachtung der Allel- und Genotypverteilung der untersuchten SNPs in verschiedenen Regionen, Ländern und Ethnien in Daten der Literatur fiel eine große Variabilität, vor allem des +49-Exonpolymorphismuses auf. Weiterhin konnte ein vorbeschriebenes Linkage Disequilibrium, im Sinne einer genetische Kopplung, für die beiden untersuchten Polymorphismen bestätigt werden. Zur funktionellen und klinischen Bedeutung der von uns gefundenen Assoziation zwischen dem -318 C/T SNP und Psoriarisarthritis sind noch weitere Untersuchungen mit größeren Fallzahlen notwendig. Wichtig ist aufgrund der breiten Variabilität der Genotypverteilung in verschiedenen Ethnien, Ländern und Regionen adäquat passende Patienten- und Kontrollgruppen zu wählen. Aufgrund des Linkage Disequilibriums der beiden untersuchten Genorte empfehlen sich für weitere Studien Haplotypanalysen.
