The geometric arena here is a smooth manifold of dimension n equipped with a Riemannian or pseudo-Riemannian metric and an affine connection. Field theories following from a variational principle are considered on this basis. In this context, all invariants which are quadratic in the curvature are determined. The work derives several manifestly covariant formulas for the Euler-Lagrange derivatives or the field equations. Some of these field theories can be interpreted as gravitational theories alternatively to Einstein´s general relativity theory. The work also touches the difficult problem to define and to calculate energy and momentum of a gravitational field.
Das Ziel der Arbeit ist eine Erhebung gesundheitlicher Beschwerden von Greifswalder Studenten und dessen Darstellung und Erklärung aus existenzanalytischer Perspektive. Da gesundheitliche Beschwerden multifaktoriell bestimmt sind, werden Parameter des sozioökonomischen Status sowie das Element Sinnerfahrung als bedeutsame Prädiktoren untersucht.
Effectiveness of Varenicline as an Aid to Smoking Cessation in Primary Care: An Observational Study

Aims: Although varenicline is commonly prescribed in primary care, information on smoking-related comorbidities and the effectiveness of varenicline in this context in Germany is scarce. This study assessed the efficacy and safety of varenicline in a large sample of patients seeking smoking cessation treatment through their general practitioners. The frequency of comorbidities was also evaluated. Methods: This was a 12-week, prospective, observational, non-comparative phase IV trial conducted in Germany. Abstinence rates at week 12 were evaluated by verbal reporting using the nicotine use inventory. Results: Overall, 1,391 subjects were enrolled; 1,177 received study medication and were evaluated for effectiveness and safety. At the end of the study, 71.1% (95% confidence interval 68.5–73.7) of subjects were abstinent. There were a total of 205 all-causality adverse events; 2.2% were classified as serious or severe. There were no fatal adverse events. At inclusion, 66.7% of participants had at least 1 concurrent comorbidity, with chronic obstructive pulmonary disease (35.5%), hypertension (29.6%) and depression (10.4%) being the most commonly reported. Conclusion: These real-world data indicate that varenicline is an effective and well-tolerated smoking cessation treatment when used in the primary care setting including patients with smoking-related comorbidities.
Zur Pneumatisation des Siebbeinkomplexes beim Schimpansen, Pan troglodytes (Primates, Catarrhini)

Zur Vermessung und morphologischen Analyse des Siebbeinlabyrinthes und des Sinus frontalis beim Schimpansen (Pan troglodytes), welcher zu den Primaten gehört, standen Computertomographiedatensätze von 21 Köpfen unterschiedlichen postnatalen Alters und beiderlei Geschlechts zur Verfügung. Die Datensätze wurden mittels der 3D Rekonstruktionssoftware WinSurf® 4.1.0 ausgewertet. Um die gewonnenen Volumina in Bezug zu schädelmorphologischen Eigenschaften setzen zu können sind verschiedene Längen-, Breiten-, und Höhenmaße des Schädels ermittelt worden. Des Weiteren wurde der Schädelbasisknickungswinkel bestimmt und das Gesichtsschädelvolumen berechnet.
Streptococcus pneumoniae (pneumococci) are Gram-positive cocci and commensals of the human upper respiratory tract. Pneumococcal pathogenesis requires adherence to host cells and dissemination through cellular barriers and to evade host defense mechanisms. The Pneumococcal surface protein C (PspC) is an important virulence factor which has a crucial role in pneumococcal adhesion to host cells and immune evasion by manipulating the host complement system. To elucidate the pneumococcal adherence and uptake mechanism via factor H glycosaminoglycans (dermatan sulfate and heparin) were employed as competitive inhibitors in infection experiments with epithelial cells or human polymorphonuclear leukocytes (PMNs). Glycosaminoglycans significantly inhibited the FH mediated pneumococcal adherence and subsequent invasion to host epithelial cells. Furthermore, the short consensus repeats of FH which promotes the adhesion of pneumococci to host cells were identified by blocking experiments with domain mapped antibodies for specific regions of FH. Moreover, this study indicates that FH acts as adhesion molecule via cellular receptors recognized as integrin CR3 on human PMNs. Binding of Factor H loaded pneumococci to integrins CR3 was assessed by flow cytometry. Pneumococci coated with Factor H showed a significantly increased association with PMNs. This interaction was blocked by anti-CR3 antibodies and Pra1. This project further aims to study mechanisms of pneumococcal endocytosis by host cells, their intracellular fate, and the pathogen induced host cell signal transduction cascades including the calcium signaling upon pneumococcal infection of host cells via the PspC-hpIgR interaction. To assess now the role of protein tyrosine kinases (PTKs) during pneumococcal infection via PspC, cell culture infections were performed in presence of pharmacological inhibitors of PTKs and MAPKs or by employing genetic interference techniques. Blocking the function of Src or ER1/2 and JNK and genetic-knock down of Src and FAK reduced significantly internalization of pneumococci. These data indicated the importance of a coordinated signaling between Src PTKs, ERK1/2, and JNK during PspC-pIgR-mediated uptake of pneumococci by host epithelial cells. The impact of host cells intracellular calcium concentrations on pneumococcal PspC-hpIgR mediated internalization was studied. Intracellular calcium measurement of epithelial cells performed in the presence of pneumococci suggested a calcium influx in host epithelial cells and importantly this calcium influx was PspC- hpIgR specific as pspC-deficient pneumococci were unable to mediate calcium mobilization in host cells. The increase in intracellular calcium [Ca2+]i was dependent on phospholipase C as pretreatment of cells with a phospholipase C-specific inhibitor abolished the increase in [Ca2+]i. Furthermore, role of host intracellular calcium concentrations during pneumococcal internalization was demonstrated by employing specific pharmacological inhibitors and calcium chelators in epithelial cell culture infection assays. The results revealed that elevated host cells calcium concentrations diminished pneumococcal internalization while lower calcium concentration in host epithelial cells promoted pneumococcal uptake. This study further demonstrates that dynamin, clathrin and caveolin play a key role during pneumococcal endocytosis into host cells via PspC-hpIgR. The use of specific pharmacological inhibitors or genetic interference approaches against dynamin, clathrin and caveolin in epithelial cell culture infection assays significantly blocked pneumococcal uptake. Furthermore, confocal microscopy revealed that pneumococci co-localize with clathrin. At later stages of the infection the pathogen is sorted to early, late and recycling endosomes as indicated by co-localization of pneumococci with endosomal markers such as Rab5, Rab4, Rab 7, and Lamp1. In order to get further insights into PspC-hpIgR mediated uptake mechanisms, a chimeric PspC was constructed and expressed heterologously on the surface of Lactococcus lactis. Immunofluorescence staining, immunoblot and flow cytometric analysis of L. lactis confirmed the expression of PspC on the bacterial surface. Moreover the ability of recombinant lactococci expressing PspC to adhere to and to invade pIgR-expressing epithelial cells confirmed the functional activity of PspC when exposed on the lactococcal surface. PspC expressing lactococci confirmed the specificity of PspC-hpIgR mediated endocytosis in host epithelial cells as PspC deficient lactococci were not taken up by these host cells. Confocal microscopic analysis demonstrated that only PspC expressing lactococci were sorted to early, late and recycling endosomes, similar to the intracellular fate of S. pneumoniae.
Der Stellenwert der Lymphonodektomie beim Endometriumkarzinom im Stadium 1 nach UICC ist eine noch immer kontrovers diskutierte Frage. In der vorliegenden Arbeit wurde untersucht, inwiefern bei Patientinnen, welche aufgrund bestimmter histopathologischer Parameter der Gruppe der „low risk“ Endometriumkarzinome zugeordnet werden können, auf eine Lymphonodektomie verzichtet werden kann. Außerdem sollte das notwendige Ausmaß der Lymphonodektomie (pelvin vs. pelvin und paraaortal) erforscht werden. Informationsquellen für diese retrospektive Analyse waren zum einen das Archiv der Klinik und Poliklinik für Frauenheilkunde und Geburtshilfe der Universitätsmedizin Greifswald und das Archiv der Klinik für Strahlentherapie der Universitätsmedizin Greifswald. Komplettiert wurden die Informationen durch Fragebögen an die betreuenden niedergelassenen Gynäkologen zu Themen des weiteren Überlebens. Die statistische Auswertung erfolgte mittels SAS® Enterprise Version 4. In dieser Arbeit konnte kein Überlebensvorteil für die Durchführung einer Lymphonodektomie bei Patientinnen mit niedrigem Risiko eines Lymphknotenbefalls („low risk“) ermittelt werden. Auch das Ausmaß der Lymphonodektomie hatte an dem in dieser Arbeit untersuchtem Kollektiv keinen Einfluss auf das Überleben, weder für Patientinnen mit dem prognostisch günstigeren Typ I Endometriumkarzinom noch für Patientinnen mit hohem Risiko für Lymphknotenbefall („high risk“). Etablierte Prognosefaktoren für einen ungünstigen Verlauf, wie Tumorgröße, Myometriuminvasion, das Vorliegen einer „high risk“ Konstellation und Blut- und Lymphgefäßbefall, konnten in dieser Arbeit zumindest im Trend bestätigt werden. Ein besonderes Augenmerk richtete sich dabei auf einen relativ neuen Prognoseparameter, die „Tumorfreie Distanz“. Aufgrund der geringen Anzahl an erreichten Endpunkten konnte jedoch kein statistisch signifikanter Zusammenhang zwischen der Tumorfreien Distanz und der Prognose des Endometriumkarzinoms in einem niedrigen Tumorstadium gefunden werden.
In dieser Arbeit wird die Hypothese getestet, dass durch H2S-induzierte Hibernation eine Reduktion des Infarktvolumens nach Schlaganfall, sowie eine Verbesserung der funktionellen Erholung erreicht wird. Hierzu wird im Post-Akut-Tiermodell des Schlaganfalls an alten Tieren eine 48-stündige Ganzkörper-Hypothermie induziert. An diesen Tieren wird dann die funktionelle Erholung nach einem Schlaganfall ermittelt sowie das Infarktvolumen gemessen. Daraus könnten sich neue Behandlungsstrategien für Schlaganfallpatienten ableiten lassen. Wir können demonstrieren, dass gealterte Ratten nach einem Schlaganfall eine 50%ige Reduktion der Infarktgröße aufweisen, wenn ihre Körpertemperatur für 48 Stunden mittels H2S gesenkt wird ohne offensichtliche zusätzliche neurologische Defizite oder physiologische Nebenwirkungen zu produzieren. Gleichzeitig verbessert sich die Leistung der Tiere in Versuchen, die komplex-motorische Funktionen erfordern. Des weiteren zeigt sich in unseren Experimenten, dass die Anzahl der Makrophagen-ähnlichen Zellen, sowie die mRNA Expression der Marker für Inflammation und Apoptose NF kappa B, grp78 und caspase 12 in den Versuchstieren deutlich niedriger ist als in den Kontrolltieren und dass bei den Hypothermie-Tieren der Infarktkern deutlich weniger apoptotische Zellen aufweist als in den Kontrollen. Unsere Ergebnisse zeigen, dass die Hypothermie signifikantes Potential bei der Behandlung des Schlaganfalls im Nager hat und weitere Untersuchungen zu diesem Thema in klinischen Studien durchgeführt werden sollten.
Im modernen hygienischen Selbstverständnis westlicher und ostasiatischer Kulturen spielt die Reduktion des Schweißgeruchs eine zentrale Rolle. Dies wird durch eine Verringerung der Schweißsekretion, die Bekämpfung schweißgeruchverursachender Bakterien und die Maskierung des Restgeruchs erreicht. Ein Präventivkonzept auf der Basis der Reduktion von apokrinen Sekreten konnte bisher nicht etabliert werden. Dies ist darauf zurückzuführen, dass die Stoffwechselwege, auf denen Schweißgeruchpräkursoren entstehen und sekretiert werden, nicht umfassend untersucht wurden. Vorarbeiten zu dieser Arbeit lieferten jedoch bereits Hinweise darauf, dass das Transportprotein ABCC11 eine bedeutende Komponente in der Entstehung von Schweißgeruch darstellt. Für den kaukasischen Körpergeruch ist u.a. das schwefelhaltige Geruchsmolekül 3-Methyl-3-sulfanylhexanol charakteristisch. In der vorliegenden Arbeit wurde durch Messung des ABCC11-vermittelten Transports in isolierten Membranvesikeln gezeigt, dass der postulierte Glutathionylpräkursor von 3-Methyl-3-sulfanylhexanol (SG3MHol) ein Transportsubstrat darstellt, während für das final auf der Haut nachweisbare Cysteinyl-Glycyl-Konjugat (CG3MHol) kein ABCC11-vermittelter Transport festgestellt werden konnte. Dies legt nahe, dass in den apokrinen sekretorischen Vesikeln SG3MHol zu CG3MHol umgewandelt wird. Diese Reaktion wird durch γ-Glutamyltransferasen katalysiert. Sowohl auf mRNA- als auch auf Proteinebene konnte die Expression von γ-Glutamyltransferase 1 (GGT1) in der apokrinen Schweißdrüse nachgewiesen werden. Immunfluoreszenzmikroskopische Analysen gaben darüber hinaus den Hinweis auf eine Lokalisation in apikalen Vesikeln sekretorischer Zellen. Durch rekombinantes humanes GGT1 konnte die Abspaltung der γ-peptidisch gebundenen Glutaminsäure des SG3MHol in vitro katalysiert werden. Der Nachweis des entstehenden CG3MHol wurde durch massenspektrometrische Verfahren erbracht. Auf diese Weise konnten sowohl die Frage nach der Rolle des ABCC11-vermittelten Transports, als auch der intravesikulären Deglutamylierung durch GGT1 beantwortet werden. Für die Entstehung des Glutathionylkonjugats von 3-Methylhexanol (3MHol) wurde mit GSTM5 eine Glutathion-S-Transferase identifiziert, die auf mRNA- und Proteinebene abundant in apokrinen Schweißdrüsen exprimiert wird. Für die Konjugation mit GSH wurde mit 3-Methylhex-2-enal (3MHal) eine α, β-ungesättige Carbonylverbindung untersucht. Das aus der Konjugation hervorgehende aldehydische Glutathionylkonjugat SG3MHal wurde durch NMR-Spektroskopie nachgewiesen, allerdings entstand es unter in vitro Bedingungen auch in Abwesenheit Glutathion-transferierender Enzyme. Vor einem Transport durch ABCC11 muss SG3MHal jedoch noch zum alkoholischen SG3MHol reduziert werden. Hinweise auf das beteiligte Enzym Aldo-Keto-Reduktase 1 B10 (AKR1B10) konnten wiederum aus mRNA-Expressionsdaten entnommen werden. Auf Proteinebene konnte AKR1B10 ebenfalls nachgewiesen werden. Dieses Enzym ist unter anderem in der Lage aldehydische Glutathionylkonjugate zu reduzieren. Für die Reduktion von SG3MHal zu SG3MHol durch AKR1B10 konnten in vitro erste Anhaltspunkte gefunden und durch massenspektrometrische Verfahren bestätigt werden. In der vorliegenden Arbeit wurde erstmalig ein potentieller Entstehungsweg von schwefelhaltigen Schweißgeruchpräkursoren postuliert und durch experimentelle Befunde gestützt. Die beteiligten enzymatischen Umsetzungs- und Transportvorgänge wurden in vitro nachgestellt und die Entstehung der Schweißgeruchpräkursoren damit in die Nähe von Detoxifizierungsprozessen gerückt. Der in dieser Arbeit beschriebene trunkierte Mercapturatweg der apokrinen Schweißdrüse stellt damit einen ersten Ansatz dar, die Entstehung sekretierter Geruchsvorläufermoleküle zu erklären. Dies liefert einen Angriffspunkt zur kosmetischen Reduktion von Schweißgeruch an seiner Wurzel. Für die beteiligten Schlüsselenzyme konnten bereits erste Inhibitoren identifiziert werden.
Gesundheit und Lebensqualität im Alter stellen angesichts steigender Kosten im Gesundheitssystem und immer wiederkehrender Debatten über mangelnde Versorgung in Seniorenheimen und Desintegration der älteren Generation eine aktuelle relevante Thematik dar. Vor allem die gesundheitspolitische Bedeutung wird im Zusammenhang mit der steigenden gesellschaftlichen Alterung noch weiter zunehmen. Bei der bisherigen Forschung auf diesem Gebiet lassen sich vor allem Evaluationen der Qualität in der Pflege sowie Studien zu Gesundheit und sozialen Netzwerken mit älteren, aber selbständigen Menschen finden. Studien mit Bewohnern von Seniorenheimen sind vor allem aufgrund der Befragungsschwierigkeiten rar. Noch in weiten Teilen unbeachtet ist auch der Einfluss des Kohärenzgefühls, einem Kernstück des Konzeptes der Salutogenese, sowie die subjektive Bewertung von Gesundheit und Lebenszufriedenheit durch institutionalisiert lebende ältere Menschen. Bei dieser Studie, in der ausschließlich in Seniorenheimen lebende Personen befragt wurden, wurde hypothetisch angenommen, dass das Kohärenzgefühl und die sozialen Netzwerke, in Form von sozialer Integration und sozialer Unterstützung, miteinander korreliert sind und sowohl die Gesundheit als auch die Lebenszufriedenheit positiv beeinflussen. Weiterhin wurde davon ausgegangen, dass das Kohärenzgefühl als Mediatorvariable zwischen den sozialen Netzwerken auf der einen und Gesundheit und Lebenszufriedenheit auf der anderen Seite fungiert. Schließlich wurde auch die Existenz eines signifikanten Einflusses der subjektiv wahrgenommenen Gesundheit auf Wohlbefinden und Lebenszufriedenheit postuliert. Unter diesen Annahmen wurde mit Hilfe eines Fragebogens aus mehreren modifizierten Messinstrumenten eine Befragung mit 190 Personen zwischen 65 und 102 Jahren aus 20 verschiedenen Seniorenheimen in Einzel-Interviews durchgeführt. Die Auswertung der erhobenen Daten zeigte einen starken Zusammenhang zwischen sozialen Netzwerken und dem Kohärenzgefühl sowie einen hohen Einfluss des Kohärenzgefühls auf die subjektiv wahrgenommene Gesundheit und die Lebenszufriedenheit. Ein direkter Einfluss von sozialer Integration und sozialer Unterstützung konnte nicht nachgewiesen werden, ein Zusammenhang bestand jedoch über das Kohärenzgefühl als Mediatorvariable. Weiterhin konnte ein positiver Einfluss der subjektiv wahrgenommenen Gesundheit auf die Lebenszufriedenheit erhoben werden, während ein Zusammenhang zwischen Kohärenzgefühl und objektiver Gesundheit nicht messbar war.
