Background:
Epilepsy development during the course of multiple sclerosis (MS) is considered to be the result of cortical pathology. However, no long-term data exist on whether epilepsy in MS also leads to increasing disability over time.
Objective:
To examine if epilepsy leads to more rapid disease progression.
Methods:
We analyzed the data of 31,052 patients on the German Multiple Sclerosis Register in a case–control study.
Results:
Secondary progressive disease course (odds ratio (OR) = 2.23), age (OR = 1.12 per 10 years), and disability (OR = 1.29 per Expanded Disability Status Scale (EDSS) point) were associated with the 5-year prevalence of epilepsy. Patients who developed epilepsy during the course of the disease had a higher EDSS score at disease onset compared to matched control patients (EDSS 2.0 vs 1.5), progressed faster in each dimension, and consequently showed higher disability (EDSS 4.4 vs 3.4) and lower employment status (40% vs 65%) at final follow-up. After 15 years of MS, 64% of patients without compared to 54% of patients with epilepsy were not severely limited in walking distance.
Conclusion:
This work highlights the association of epilepsy on disability progression in MS, and the need for additional data to further clarify the underlying mechanisms.
EGFR is overexpressed in the majority of clear cell renal cell carcinomas (CCRCCs). Although EGFR deregulation was found to be of great significance in CCRCC biology, the EGFR
overexpression is not associated with EGFR-targeted therapy responsiveness. Moreover, the prognostic role of EGFR expression remains controversial. In the present study, we evaluated the role
played by EGFR overexpression in CCRCC and its prognostic significance associated with different
immunohistochemical localization patterns. In our study, the Total Score (TS) related to membranouscytoplasmic EGFR expression showed a significant correlation with grade, pathologic stage (pT),
and Stage, Size, Grade, and Necrosis (SSIGN) score, and a negative correlation with nuclear EGFR
expression. No significant correlations were shown between nuclear EGFR and clinic-pathological
features. Additionally, a correlation between SGLT1 expression levels and pT was described. Multivariate analysis identifies pT and SSIGN score as independent prognostic factors for CCRCC. A
significantly increased survival rate was found in the case of positive expression of nuclear EGFR
and SGLT1. Based on our findings, SGLT1 and nuclear EGFR overexpression defines a subgroup of
CCRCC patients with good prognosis. Membranous-cytoplasmic EGFR expression was shown to be
a poor prognostic factor and could define a CCRCC subgroup with poor prognosis that should be
responsive to anti-EGFR therapies
Background: Interstitial lung disease (ILD) is associated with high rates of comorbidities and non-infectious lung disease mortality. Against this background, we aimed to evaluate the prognostic capacity of lung function and cardiopulmonary exercise testing (CPET) in patients with ILD. Materials and Methods: A total of 183 patients with diverse ILD entities were included in this monocentric analysis. Prediction models were determined using Cox regression models with age, sex, body mass index (BMI), and all parameters from pulmonary function testing and CPET. Kaplan–Meier curves were plotted for selected variables. Results: The median follow-up period was 3.0 ± 2.5 years. Arterial hypertension (57%) and pulmonary hypertension (38%) were the leading comorbidities. The Charlson comorbidity index score was 2 ± 2 points. The 3-year and 5-year survival rates were 68% and 50%, respectively. VO2peak (mL/kg/min or %pred.) was identified as a significant prognostic parameter in patients with ILD. The cut-off value for discriminating mortality was 61%. Conclusion: The present analyses consistently revealed the high prognostic power of VO2peak %pred. and other parameters evaluating breathing efficacy (VÉ/VCO2 @AT und VÉ/VCO2 slope) in ILD patients. VO2peak %pred., in contrast to the established prognostic values FVC %pred., DLCO/KCO %pred., and GAP, showed an even higher prognostic ability in all statistical models.
Background: Following acute pulmonary embolism (PE), a relevant number of patients experience decreased exercise capacity which can be associated with disturbed pulmonary perfusion. Cardiopulmonary exercise testing (CPET) shows several patterns typical for disturbed pulmonary perfusion. Research question: We aimed to examine whether CPET can also provide prognostic information in chronic thromboembolic pulmonary hypertension (CTEPH). Study Design and Methods: We performed a multicenter retrospective chart review in Germany between 2002 and 2020. Patients with CTEPH were included if they had ≥6 months of follow-up and complete CPET and hemodynamic data. Symptom-limited CPET was performed using a cycle ergometer (ramp or Jones protocol). The association of anthropometric data, comorbidities, symptoms, lung function, and echocardiographic, hemodynamic, and CPET parameters with survival was examined. Mortality prediction models were calculated by Cox regression with backward selection. Results: 345 patients (1532 person-years) were included; 138 underwent surgical treatment (pulmonary endarterectomy or balloon pulmonary angioplasty) and 207 received only non-surgical treatment. During follow-up (median 3.5 years), 78 patients died. The death rate per 1000 person-years was 24.9 and 74.2 in the surgical and non-surgical groups, respectively (p < 0.001). In age- and sex-adjusted Cox regression analyses, CPET parameters including peak oxygen uptake (VO2peak, reflecting cardiopulmonary exercise capacity) were prognostic in the non-surgical group but not in the surgical group. In mortality prediction models, age, sex, VO2peak (% predicted), and carbon monoxide transfer coefficient (% predicted) showed significant prognostic relevance in both the overall cohort and the non-surgical group. In the non-surgical group, Kaplan–Meier analysis showed that patients with VO2peak below 53.4% predicted (threshold identified by receiver operating characteristic analysis) had increased mortality (p = 0.007). Interpretation: The additional measurement of cardiopulmonary exercise capacity by CPET allows a more precise prognostic evaluation in patients with CTEPH. CPET might therefore be helpful for risk-adapted treatment of CTEPH.
Das Peniskarzinom ist ein sehr seltener Tumor des älteren Mannes, mit geringer Studienlage. Hauptkriterium des Überlebens ist die Metastasierung. Untersuchungen zu Vorhersageparametern der Metastasierung gibt es, sind aber in der klinischen Anwendung sehr unsicher. Anhand von 30 invasiven Plattenepithelkarzinomen und 4 CIS (Carcinoma in situ) sind in der vorliegenden Arbeit die Metastasensuppressorgenprodukte (MSG) KAI1/ CD82, nm23-H1 sowie die Effektorcaspasen 3 und 6, hinsichtlich ihrer häufig beschriebenen Zusammenhänge sowohl mit dem Metastasenrisiko als auch dem Überleben untersucht worden. Zur Anwendung kamen die Immunhistochemie (Streptavidin-Biotin-Methode) zum Proteinnachweis sowie die TUNEL(Terminal deoxynucleotide transferase dUTP Nick End Labeling)-Methode zur Bestätigung des spezifischen Apoptosenachweises mit Caspaseantikörpern. Die Immunreaktion der untersuchten Proteine im Tumor wurde als negativ, mittel bzw. stark bewertet und Beziehungen zu klinikopathologischen Parametern mittels Fisher Exact Test, Chi-Quadrat Test und Kendall Tau Test untersucht. Überlebensanalysen wurden durch die Kaplan Meier Methode graphisch dargestellt und unter Verwendung des Log Rank Testes auf Signifikanz geprüft. Mit der multivariaten Analyse wurde der Einfluss verschiedener unabhängiger Parameter auf die Metastasierung und das Überleben untersucht. Eine verminderte oder fehlende KAI1/ CD82 sowie nm23-H1 Expression im Primärtumor zeigte einen hoch signifikanten Zusammenhang mit Lymphknotenmetastasen (p =<0,001). Darüber hinaus war eine starke Expression beider Proteine mit einem statistisch signifikanten längeren Überleben verbunden (p = 0,007 bzw. 0,0013). Die multivariaten Analysen betätigten einzig das Protein nm23-H1 als unabhängige Variable hinsichtlich Metastasierung und Überleben. Die untersuchten Caspasen wiesen keine Beziehungen zwischen Immunreaktivität, der Metastasierung und dem Überleben auf. Anhand der Ergebnisse der vorliegenden Arbeit ist ein klinischer Einsatz der beiden MSG zur Charakterisierung des Metastasierungsverhaltens und als Prognosekriterium denkbar. Jedoch sind weiterführende Untersuchungen für die allgemeine Anwendbarkeit von entscheidender Bedeutung.
Es besteht bei Patienten mit chronischer Herzinsuffizienz eine umgekehrte proportionale Beziehung zwischen der unter Belastung gemessenen Atemeffizienz, ausgedrückt als VE/VCO2 slope, und der in Ruhe ermittelten Atemeffizienz, gemessen als endexspiratorischer Kohlendioxidpartialdruck. Die Ruheatemeffizienz chronisch Herzinsuffizienter unterscheidet sich signifikant von der bei gesunden Personen gemessenen Atemeffizienz. Im Gegensatz zu einem während einer Belastungsuntersuchung gewonnenen Parameter lässt sich die Ruheatemeffizienz jederzeit unter körperlicher Schonung des Patienten messen. Sie ist nicht von der Motivation des Patienten oder der Erfahrung des Untersuchers abhängig und kann nichtinvasiv innerhalb weniger Minuten mit nur minimalem Aufwand ermittelt werden. Es konnte gezeigt werden, dass die Atemeffizienz in Ruhe ein Parameter zur nichtinvasiven Beurteilung der Prognose bei Patienten mit chronischer Herzinsuffizienz ist.
Fragestellung: Besteht eine Korrelation zwischen dem MIB-1 Markierungsindex und der 5-Jahres-Überlebensrate bei Patientinnen mit einem Karzinom der Zervix uteri? Gibt es eine Beziehung zwischen dem MIB-1 Index und dem Alter der Patientinnen, FIGO-Stadium, LK-Status, Lymph- und Blutgefäßeinbrüchen, Differenzierungsgrad, Tumorvolumen und Invasionstiefe? Ferner wurden o.g. Faktoren auf ihre prognostische Relevanz überprüft. Material und Methoden: 69 Präparate in Formalin fixiert und in Paraffin eingebettet, immunhistochem. Untersuchungen nach der APAAP-Methode. Resultate: Es besteht kein statistisch signifikanter Zusammenhang zwischen MIB-1 Index, der 5-Jahres-Überlebensrate und o.g. Faktoren.
Über die Erfolge der modernen diagnostischen Verfahren (CT, MRT) und Therapien (Chirurgie, Chemotherapie, Radiotherapie) bei der Behandlung von Gliomen wird weiter intensiv diskutiert. Von Interesse ist deshalb der Vergleich von Patientenpopulationen, die vor und nach der Einführung der MRT und der Mikrochirurgie behandelt wurden. Deswegen wurde eine retrospektive statistische Analyse für alle Patienten mit einem Gliom durchgeführt, die zwischen den Jahren 1965 und 1974 (88 Patienten, Gruppe I) oder zwischen den Jahren 1986 und 1995 (249 Patienten, Gruppe II) an der Universitätsklinik Greifswald operiert wurden. Es erfolgte zwischen den beiden Gruppen eine deutliche Reduktion sowohl der perioperativen Morbidität und Mortalität als auch des Zeitintervalls vom Symptombeginn bis zur Behandlung. Es gab eine Verbesserung der postoperativen Überlebenszeit in der Gruppe II für Patienten mit low-grade Gliom. Jedoch hat sich die Prognose der high-grade Gliome von den 70er-Jahren bis zu den 90-er Jahren insgesamt kaum verändert.
