Die vorliegende qualitativ-hermeneutische Dissertation untersucht auf der Basis von 20 Interviews die Motivation von Patientinnen der Plastischen Alterschirurgie, die einen größeren Eingriff (Facelift) anvisieren oder bereits haben vornehmen lassen. Sie kommt zu dem Ergebnis, dass das bisher gängige Prozedere die psychologischen Risiken und Nebenwirkungen einer solchen Operation vernachlässigt. Nach Sichtung der bewussten Beweggründe der Patientinnen und des sozialen Kontextes ihrer Entscheidung für eine OP beleuchtet die Arbeit komplexe Fragen des persönlichen Selbst- und Weltbilds, der Emotionalität und neurotischer Fixierungen. Das häufige Vorliegen von maladaptivem Coping, narzisstischem Selbstwertmanagement, niedriger sozioemotionaler Kompetenz, fehlender Sinnressourcen, wie auch der Neigung zum Selbstbetrug wirft ein problematisches Licht auf die Indikation. Unter Abwägung der Chancen und Risiken einer OP aus psychotherapeutischer Sicht erscheint es zweifelhaft, ob ein verjüngender Eingriff die Probleme der Patientinnen beheben kann, ob er unter ungünstigen Umständen nicht sogar mehr schadet als nützt. Dieses grundsätzliche psychologische Dilemma könnte berücksichtigt und minimiert werden durch ein verbindliches psychologisches Beratungs- und Aufklärungsgespräch im Vorfeld der Operation. So schließt die Arbeit mit dem Vorschlag eines kombinierten chirurgisch-psychologischen Vorgehens zur ganzheitlich konzipierten Therapie der Altersängste der Patientinnen, eine Neuerung, die sowohl im Dienst der Patienten wie auch im Dienst des behandelnden Chirurgen stünde.
Decreased inflammatory profile in oral leukoplakia tissue exposed to cold physical plasma ex vivo

Background
Oral leukoplakia (OL) is an unfavorable oral disease often resistant to therapy. To this end, cold physical plasma technology was explored as a novel therapeutic agent in an experimental setup.
Methods
Biopsies with a diameter of 3 mm were obtained from non-diseased and OL tissues. Subsequently, cold atmospheric pressure plasma (CAP) exposure was performed ex vivo in the laboratory. After 20 h of incubation, biopsies were cryo-conserved, and tissue sections were quantified for lymphocyte infiltrates, discriminating between naïve and memory cytotoxic and T-helper cells. In addition, the secretion pattern related to inflammation was investigated in the tissue culture supernatants by quantifying 10 chemokines and cytokines.
Results
In CAP-treated OL tissue, significantly decreased overall lymphocyte numbers were observed. In addition, reduced levels were observed when discriminating for the T-cell subpopulations but did not reach statistical significance. Moreover, CAP treatment significantly reduced levels of C-X-C motif chemokine 10 (CXCL10) and granulocyte-macrophage colony-stimulating factor in the OL biopsies' supernatants. In idiopathically inflamed tissues, ex vivo CAP exposure reduced T-cells and CXCL10 as well but also led to markedly increased interleukin-1β secretion.
Conclusion
Our findings suggest CAP to have immuno-modulatory properties, which could be of therapeutic significance in the therapy of OL. Future studies should investigate the efficacy of CAP therapy in vivo in a larger cohort.
Due to a variety of plasma sources in terms of type of discharge, energy yield, working gas or geometric factors, it is recommended to standardize the study protocol by choosing a plasma source and easy access to rugged tumor surfaces as demonstrated by the CAP-plasma-jet. The intention of the trial shall be to optimize the plasma jet for tumor site capability and operating room implementation.
It makes sense to start clinical trials in plasma medicine with the treatment of head and neck squamous cell carcinoma patients of infected wounds and ulcerations.
CAP is able to reduce contamination of cancer ulcerations and the typical fetid odor that often accompanies head and neck cancer patients. The intention of the trial shall be to evaluate the efficiency of decontamination in head and neck cancer ulcerations in terms of pathogenic species, amount of reduction and reliability.
Standardize study protocol:
Phase I, clinical explorative single-arm, randomized, open, multicenter
Primary objective
Reduction of microbial burden of cancer ulcerations by application of CAP
Secondary objective:
Reduction of tumor following local CAP application
Inclusion:
20 Patients suffering from locally advanced oral cavity carcinoma with open tumor surfaces, treated with palliative intention and no more curative treatment options
Exclusion:
No wish for treatment, no compliance and understanding the protocol of the clinical study
Efficacy:
reduction of microbial burden; Documentation of visible changes by photography; Pathohistological and biochemical examination of specimen, taken from the tumor area and control areas
Procedure:
Plasma is applied for 1 minute per cm², spot area of 3 mm diameter distance between nozzle and tumor surface of 14 mm. 3 times/week with a break of 1 week followed by a repeated cycle for another week.
Conclusion:
The most important intention of the trial from the clinician’s point of view shall be to make CAP-treatment an effective and well-accepted addition to standard cancer therapy based upon EBM at least in palliative medicine.
Weisheitszähne, deren Behandlung und Extraktion gehören zur täglichen Arbeit von Zahnärzten, Kieferorthopäden, Oralchirurgen und Mund-, Kieferund Gesichtschirurgen. Es besteht jedoch noch immer ein Mangel an validen Daten zum Vorkommen von Weisheitszähnen, ihrer Retention, der Angulation im Unterkiefer und der Lage zur Kieferhöhle im Oberkiefer in der Allgemeinbevölkerung. Deshalb wurde diese Thematik im Rahmen der randomisierten und kontrollierten SHIP-MRT-Studie an einer sehr großen Kohorte aus Vorpommern untersucht. Da das bildgebende Verfahren der Magnetresonanztomographie in der Befunderhebung des Zahnstatus in der Praxis bisher kaum eine Rolle spielte, soll diese Studie auch dazu beitragen, weitere Anhaltspunkte über die Eignung der MRT für diese Diagnostik zu liefern.
Reactive oxygen species (ROS) have been subject of increasing interest in the pathophysiology and therapy of cancers in recent years. In skin cancer, ROS are involved in UV-induced tumorigenesis and its targeted treatment via, e.g., photodynamic therapy. Another recent technology for topical ROS generation is cold physical plasma, a partially ionized gas expelling dozens of reactive species onto its treatment target. Gas plasma technology is accredited for its wound-healing abilities in Europe, and current clinical evidence suggests that it may have beneficial effects against actinic keratosis. Since the concept of hormesis dictates that low ROS levels perform signaling functions, while high ROS levels cause damage, we investigated herein the antitumor activity of gas plasma in non-melanoma skin cancer. In vitro, gas plasma exposure diminished the metabolic activity, preferentially in squamous cell carcinoma cell (SCC) lines compared to non-malignant HaCaT cells. In patient-derived basal cell carcinoma (BCC) and SCC samples treated with gas plasma ex vivo, increased apoptosis was found in both cancer types. Moreover, the immunomodulatory actions of gas plasma treatment were found affecting, e.g., the expression of CD86 and the number of regulatory T-cells. The supernatants of these ex vivo cultured tumors were quantitatively screened for cytokines, chemokines, and growth factors, identifying CCL5 and GM-CSF, molecules associated with skin cancer metastasis, to be markedly decreased. These findings suggest gas plasma treatment to be an interesting future technology for non-melanoma skin cancer topical therapy.
Die therapeutische Anwendung kalter Atmosphärendruckplasmen (CAP) ist der zentrale Schwerpunkt des in einer hoch interdisziplinären Schnittstelle zwischen Natur- und Lebenswissenschaften angesiedelten Forschungsgebiets der Plasmamedizin. Trotz eines auf Basis grundlagentechnologischer und anwendungsorientierter Forschung zunehmenden Erkenntnisgewinns, wirft die Plasmamedizin sowohl bezüglich spezifischer Wirkeffekte als auch Risikopotentiale nach wie vor verschiedene Fragestellungen auf und zeigt, vor dem Hintergrund einer nicht ausschließlich auf Grundlage von in vitro Daten zu prognostizierenden unbedenklichen in vivo Anwendbarkeit, vor allem ein Defizit an klinischen Daten.
Im Rahmen der vorliegenden Arbeit erfolgte erstmals eine systematische Langzeituntersuchung eines unter standardisierten Bedingungen mittels kaltem atmosphärischen Plasma behandelten Probandenkollektivs. Die 5 Probanden waren 5 Jahre zuvor Teil einer Untersuchung zum CAP-Einfluss auf die Wundheilung CO2-Laser-induzierter Hautläsionen, im Zuge derer jeweils 3 der 4 Läsionen mit unterschiedlichen Applikationsschemata (1 x 10 s; 1 x 30 s; 3 x 10 s) mittels des kINPen® MED behandelt wurden.
Neben einer klinischen Evaluation wurde die Nachuntersuchung durch aufwendige, bildgebende Diagnostika wie Dermatoskopie, konfokale Laserscanmikroskopie (KLSM) und Hyperspectral Imaging (HSI) assistiert sowie fragebogengestützt hinsichtlich plasmavermittelter Hautveränderungen (POSAS) und einer möglichen therapieassoziierten Beeinträchtigung der Lebensqualität (DLQI) ergänzt.
Im Ergebnis konnten weder klinisch-makroskopisch, dermatoskopisch, noch mittels KLSM oder HSI malignitätssuspekte Veränderungen, Anzeichen chronischer Entzündungsreaktionen, pathologische Modifikationen der Zellarchitektur und Zellmorphologie oder mikrozirkulatorische Pathologien in den plasmabehandelten Arealen, gegenüber dem umliegenden, nicht mittels Kaltplasma therapierten Gewebe, belegt werden.
Die in dieser Komplexität im Rahmen einer Langzeituntersuchung am Menschen bisher einzigarten Daten tragen, insbesondere auf dem hochaktuellen Diskussionsfeld plasmaassoziierter Risikopotentiale, zu einem weiteren Erkenntnisgewinn bei. Zur Sicherung indikationsspezifischer Dosisempfehlungen und Behandlungsfenster bedarf es, aufgrund verschiedener allgemeiner und spezieller Limitationen, jedoch weiterer klinischer Studien.
Structured examination and treatment are essential in medicine. For dental students, a structured approach to the assessment of oral mucosal lesions is missing thus far. To validate an approach, a structured questionnaire was compared with the habitually used free description of oral lesions (white lesions, ulcers, hyperplasia). Thirty-three dental students were divided into two groups (Group 1 (n = 17) used the free description; Group 2 (n = 16) used a guided questionnaire) to characterize mucosal lesions in patients and make a tentative diagnosis. Although no difference was found between the groups regarding the suspected diagnosis or the histopathological findings, there was a significant advantage of the structured questionnaire in all aspects of the description compared to the free description (p = 0.000018). Thus, a structured description is an important aspect in the evaluation of oral mucosal changes, and a guided questionnaire should be implemented in the study of dentistry.
Die anatomischen Beziehungen der A. maxillaris in der Fossa infratemporalis wurden untersucht. Der mittlere vertikale Abstand zwischen Karotisgabel und dem Abgang der A. maxillaris betrug 63,6 mm. Der Mittelwert der extrakraniellen Länge der A. meningea betrug 15,1 mm. Eine Verlaufstypisierung der A. maxillaris wurde entwickelt. Die vertikalen und horizontalen Abstände der A. maxillaris zum Foramen mandibulae wurden ermittelt. Der kleinste gemessene vertikale Abstand war 4,8 mm, der kleinste horizontale 1,1 mm. In unserem Untersuchungsgut war ein proximaler Abgang der A. alveolaris inferior am häufigsten (59%), gefolgt von einem proximalen Abgang der A. meningea media (26%). Der Mittelwert des Durchmessers der A. maxillaris am Abgang betrug 3,8 mm. Der größte gemessene Durchmesser betrug 5,5 mm. Die A. maxillaris ist ein großlumiges, variabel verlaufendes Gefäß mit mehreren Arterienabgängen. Wir fanden die meisten Arterienabgänge kranial und dorsal des Foramen mandibulae. Dies ist insofern von Bedeutung, als bei der Lokalanästhesie am Foramen mandibulae oberhalb des vermuteten Foramens injiziert wird, um ein Absinken des Lokalanästhetikums an den Zielpunkt und eine sichere Schmerzausschaltung zu gewährleisten. Die oben genannten Ergebnisse legen eine direkte Punktion der A. maxillaris als Ursache positiver Aspirationsproben bei der Mandibularisanästhesie nahe. Gelangt ein Bolus eines Lokalanästhetikums proximal des Abgangs der A. meningea media in die A. maxillaris, so bewirkt der Vasokonstriktorzusatz eine Ischämie distal der Injektionsstelle. Bei Individuen, deren Retina über die A. meningea media versorgt wird, kann es hierbei zu ischämischen Netzhautläsionen kommen. Des weiteren kann ein Bolus eines fälschlich in die A. maxillaris injizierten Lokalanästhetikums schnell und in großer Menge über die A. meningea media in der A. centralis retinae anfluten, um dort toxische Wirkungen hervorzurufen.
Background: Gram-negative infections of the peritoneal cavity result in profound modifications of peritoneal B cell populations and induce the migration of peritoneal B cells to distant
secondary lymphoid organs. However, mechanisms controlling the egress of peritoneal B cells from
the peritoneal cavity and their subsequent trafficking remain incompletely understood. Sphingosine1-phosphate (S1P)-mediated signaling controls migratory processes in numerous immune cells. The
present work investigates the role of S1P-mediated signaling in peritoneal B cell trafficking under
inflammatory conditions. Methods: Differential S1P receptor expression after peritoneal B cell activation was assessed semi-quantitatively using RT-PCR in vitro. The functional implications of
differential S1P1 and S1P4 expression were assessed by transwell migration in vitro, by adoptive
peritoneal B cell transfer in a model of sterile lipopolysaccharide (LPS)-induced peritonitis and in
the polymicrobial colon ascendens stent peritonitis (CASP) model. Results: The two sphingosine-1-
phosphate receptors (S1PRs) expressed in peritoneal B cell subsets S1P1 and S1P4 are differentially
regulated upon stimulation with the TLR4 agonist LPS, but not upon PMA/ionomycin or B cell receptor (BCR) crosslinking. S1P4 deficiency affects both the trafficking of activated peritoneal B cells
to secondary lymphoid organs and the positioning of these cells within the functional compartments of the targeted organ. S1P4 deficiency in LPS-activated peritoneal B cells results in significantly reduced numbers of splenic innate response activator B cells. Conclusions: The S1P-S1PR system is implicated in the trafficking of LPS-activated peritoneal B cells. Given the protective role of peritoneal B1a B cells in peritoneal sepsis, further experiments to investigate the impact of S1P4 mediated signaling on the severity and mortality of peritoneal sepsis are warranted.
