Die Bedeutung der Visite in der Medizin bezieht sich auf eine gute Kommunikation zwischen Arzt und Patient. In dieser Studie wurden der Visitenablauf und -gestaltung quantitativ und qualitativ betrachtet. Der Arzt war direktiv steuernd und zeigte adressatenorientierendes Kommukationsverhalten. Inhaltlich war die unfallchirurgische Visite somatisch orientiert, aber es zeigte sich auch ein hohes Maß an Small talk/Konversation. Zudem wurde die Zufriedenheit von Arzt und Patient als Auswertungskriterien herangezogen. Beide, Arzt und Patient, waren meist zufrieden mit der Visite.
Objective
To identify and summarise evaluated interventions aiming to improve the communication of palliative care (PC) and end-of-life (EoL) issues in physicians caring for cancer patients. Such interventions are needed with regard to the aim of an earlier communication of those issues in oncology daily practice, which is associated with a range of benefits for patients and caregivers but is often impeded by physicians’ communication insecurities.
Design
Systematic review based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.Data sourcesRelevant publications were systematically searched in MEDLINE, PsycINFO, CINAHL and Web of Science databases in September 2020 with an update in July 2021.
Eligibility criteria
We included publications reporting a quantitative evaluation of a communication intervention on one or more PC/EoL issues with a communication-related main outcome. Target group had to be physicians caring for cancer patients non-specialist in PC.
Data extraction and synthesis
Two independent raters extracted intervention characteristics, publication characteristics and publication quality. Results were narratively synthesised.
Results
24 publications reporting 22 interventions were included. 13 publications reported randomised controlled trials. A majority of the interventions addressed one specific PC/EoL issue, most often breaking bad news. Teaching strategies mostly involved role-plays. Target group were mainly oncologists. In addition to self-reported outcome measurements for evaluation, most publications also reported the use of external rating data. All but one publication reported significant intervention effects on at least one outcome parameter. Publication quality was overall moderate.
Conclusions
The empirically tested communication interventions on PC/EoL issues seem to effectively improve physicians’ communication. Future interventions should focus on other issues than breaking bad news, such as preparing for the future. Target group should also be organ-specific oncologists, as all primary caring physicians are responsible for timely communication. Our risk-of-bias assessment revealed some weaknesses, indicating that more high-quality studies for evaluation are needed.

Die Arthrose des Hüft- und auch des Kniegelenkes ist eine weltweit häufige Gelenkerkrankung. Die konservativen Maßnahmen wie Analgesie, Physiotherapie und Gewichtsreduktion können die Schmerzen nicht ausreichend lindern. Ultima Ratio ist der operative Gelenkersatz durch eine Totalendoprothese, der bei Patient*innen großes Angsterleben auslösen kann.
Ziel dieser randomisiert-kontrollierten Studie war, durch eine patient*innenzentrierte präoperative Kurzintervention Angst vor der Operation zu reduzieren. Es wurden 68 Patient*innen (35 Frauen, 33 Männer) der Klinik für Orthopädie vor einer Hüft- oder Knie-TEP-Operation eingeschlossen. Präoperativ wurde das subjektive Angstempfinden (NAS, STAI) und der Grad der Schmerzchronifizierung (MPSS) erfasst.
Die innovative Kurzintervention wurde so konzipiert, dass sie einmalig präoperativ anwendbar und durch die kurze Dauer von 20 Minuten in den Stationsalltag integrierbar war. Zur Stärkung der Selbstwirksamkeit der Patient*innen wurden die metakognitiven Konzepte Mentales Kontrastieren nach Oettingen und Implementierungsintentionen nach Gollwitzer kombiniert. Zusätzlich beinhaltete die Kurzintervention Psychoedukation zum Angst- und Schmerzempfinden sowie zur Bedeutung der frühen postoperativen Mobilisation. Diese Form der präoperativen Kurzintervention kam erstmalig bei orthopädischen Patient*innen zur Anwendung.
Unsere Daten zeigen, dass eine deutliche Reduktion des postoperativen subjektiven Schmerzempfindens in der Interventionsgruppe zu erreichen war. Die ANOVA mit Messwiederholung ergab einen Haupteffekt in der Anwendung der Intervention mit einem Trend zu einem statistisch signifikanten Unterschied (p=.051). Die Kurzintervention konnte durch ihre modernen Strategien zur Angstreduktion bei den orthopädischen Patient*innen das direkte postoperative Schmerzempfinden und somit den wichtigsten Risikofaktor für die Entstehung von chronischen postoperativen Schmerzen reduzieren. Die Variablen präoperatives Angstempfinden, Mobilisation und Analgetikaeinnahme an den ersten drei postoperativen Tagen waren nicht beeinflusst.
Mit dieser Kurzintervention wurde zu einem verbesserten Outcome der Patient*innen beigetragen. Es bedarf jedoch weiterer Forschungsarbeiten, um die auf das Gesundheitssystem zukommende große Anzahl von Patient*innen vor einer Hüft- und Knie-TEP-Operation angemessen zu versorgen.
APOE ε4 in Depression-Associated Memory Impairment—Evidence from Genetic and MicroRNA Analyses

(1) Background: The aim of this study was to replicate a reported interaction between APOE ε4 status and depression on memory function in two independent, nondemented samples from the general population and to examine the potential role of circulating plasma miRNAs. (2) Methods: The impact of the APOE ε4 allele on verbal memory and the interaction with depression is investigated in two large general-population cohorts from the Study of Health in Pomerania (SHIP, total n = 6286). Additionally, biological insights are gained by examining the potential role of circulating plasma miRNAs as potential epigenetic regulators. Analyses are performed using linear regression models adjusted for relevant biological and environmental covariates. (3) Results: Current depression as well as carrying the APOE ε4 allele were associated with impaired memory performance, with increasing effect for subjects with both risk factors. In a subcohort with available miRNA data subjects with current depressive symptoms and carrying APOE e4 revealed reduced levels of hsa-miR-107, a prominent risk marker for early Alzheimer’s Disease. (4) Conclusions: Our results confirm the effect of depressive symptoms and APOE ε4 status on memory performance. Additionally, miRNA analysis identified hsa-miR-107 as a possible biological link between APOE ε4, depressive symptoms, and cognitive impairment.
Introduction
Privacy concerns are an important barrier to adoption and continued use of digital technologies, particularly in the health sector. With the introduction of mobile health applications (mHealth apps), the construct of app information privacy concerns has received increased attention. However, few validated measures exist to capture said concerns in population samples, although they can help to improve public health efforts.
Methods
Using a cross-sectional survey of German adults (mean age = 35.62; 63.5% female), this study examined psychometric properties of the app information privacy concerns scale (AIPC). Analyses comprised confirmatory factor analysis, factorial validity (exploratory factor analysis), internal consistency, convergent validity (i.e., correlations with privacy victimhood, and app privacy concerns), and discriminant validity (i.e., daily app use, adoption intentions, and attitudes toward COVID-19 contact tracing app use).
Results
The analysis did not support the proposed three-factor structure of the AIPC (i.e., anxiety, personal attitude, and requirements). Instead, a four-factor model was preferable that differentiated requirements regarding disclosure policies, and personal control. In addition, factors mirroring anxiety and personal attitude were extracted, but shared a significant overlap. However, these factors showed good reliability, convergent and discriminant validity.
Discussion
The findings underline the role of app information privacy concerns as a significant barrier to mHealth app use. In this context, anxiety and personal attitudes seemed particularly relevant, which has implications for health communication. Moreover, the observed differentiation of external (disclosure) and internal (control) requirements aligns with health behavior change models and thus is a promising area for future research.
Little is known about the (co-)occurrence of smoking, alcohol at-risk drinking, physical inactivity and overweight, and the motivation to change these behavioral health risk factors (HRFs) in older general hospital patients with cardiovascular disease. Between October and December 2016, all consecutively admitted patients aged 50 to 79 years were proactively recruited on 3 cardiology wards and asked to participate in a survey on HRFs and behavior change motivation. Of the eligible patients, 80.4% participated in the survey (n = 328). The mean age was 66.5 years (standard deviation 9.0), and 65.5% were male. At least 1 HRF was present in 91.8% (n = 280), at least 2 HRFs in 54.4% (n = 166), and 3 or 4 HRFs in 12.1% (n = 37) of participants. The proportion of older adults who contemplated or were changing or planning to change their behavior to meet health behavior recommendations ranged between 66.0% (smoking) and 93.2% (alcohol consumption). The results indicate a notable co-occurrence of behavioral HRFs in older patients with cardiovascular disease. The majority of older adults were at least considering changing the respective behavior. To prevent and treat diseases efficiently, hospitalization may be a suitable moment for systematic multiple HRF screening and intervention.
Introduction
Patients and families affected by paediatric cancer experience psychosocial burden not only during active treatment but also during follow-up care. Use of health services during follow-up treatment should be organised according to patients’ and family members’ needs with regard to their physical and mental situation. This study aims (1) at analysing healthcare use (medical and psychosocial) and associated factors in follow-up care of paediatric cancer patients and (2) at investigating the psychosocial situation and support needs of children and their families during follow-up care. Based on the results, recommendations for healthcare planning and for the development of new and the optimisation of existing support offers will be derived.
Methods and analysis
We will conduct a prospective observational study using a naturalistic explorative design with quantitative and qualitative methods. Paediatric cancer patients in follow-up care, their parents and siblings will be invited to fill out a questionnaire at three measurement points (baseline, 6 months follow-up, 12 months follow-up; target n=252 complete data sets over all measurement points). Additionally, parents will be interviewed using a semistructured interview guideline (target n=15–20) at baseline. Quantitative data will be analysed using descriptive statistics, linear mixed models and regression models. Moreover, explorative analyses will be conducted. Qualitative data will be analysed using qualitative content analyses.
Ethics and dissemination
The study was approved by the Local Psychological Ethics Committee (LPEK-0281). Our findings will be published in scientific, peer-reviewed journals and presented to clinicians and researchers on conferences. To assure that results will be available to affected patients and families, a lay summary will be written and disseminated using several ways (upload on the homepage of the research group, upload on the homepage of the psychosocial working group in the Society for Paediatric Oncology/Haematology in Germany, sending to relevant patient organisations).

Zielsetzung: Wiesmann, Niehörster, Hannich und Hartmann (2008) haben das generalisierte gesundheitsbezogene Selbstkonzept entwickelt. Gesundheit wird demnach als multidimensionales Konstrukt mit gesundheitsschützenden und -gefährdenden Faktoren verstanden. Es werden fünf Dimensionen des gesundheitsbezogenen Selbstkonzeptes differenziert: health-protective dispositions, health-protective motivation, vulnerability, health-risky habits, und external, avoidant motivation. Die vorliegende Arbeit überprüfte diesbezüglich die Eignung des Instrumentes zur Erfassung des gesundheitsbezogenen Selbstkonzeptes (HRSC-25). Zum einen fehlt es bisher an einer Replikation der multidimensionalen Struktur des gesundheitsbezogenen Selbstkonzeptes für eine berufstätige Erwachsenenstichprobe. Zum anderen stellt sich die Frage, ob die Dimensionen des gesundheitsbezogenen Selbstkonzeptes kulturspezifisch oder kulturübergreifend geprägt sind. Weiterhin sollte der Zusammenhang zwischen gesundheitsbezogenem Selbstkonzept und Stresserleben untersucht werden. Methode: Insgesamt nahmen 169 deutsche (75.7 % weiblich) sowie 154 indische (81.8 % weiblich) Lehrkräfte an der Befragung teil. In der deutschen Stichprobe ließ sich das theoretische Modell zur Erfassung des gesundheitsbezogenen Selbstkonzeptes (HRSC-25) mit Hilfe einer konfirmatorische Faktorenanalyse untersuchen. Da das Modell ausschließlich an westlich-orientierten Kulturen entwickelt sowie angewandt wurde, sollte die empirische Datenstruktur der indischen Erhebung durch eine explorative Faktorenanalyse überprüft werden. Weiterhin wurden multiple Regressionsanalysen zur Vorhersage des Stresserlebens durchgeführt. Ergebnisse: Die faktoranalytischen Ergebnisse der deutschen Stichprobe bestätigten die mehrdimensionale Struktur des gesundheitsbezogenen Selbstkonzeptes (HRSC-25). Für das Messmodell konnten weitestgehend akzeptable Fit-Indizes berechnet werden, wobei eine Modifizierung des Modells (HRSC-21) die Modellgüte verbesserte. In der indischen Stichprobe konnten die fünf gesundheitsbezogenen Dimensionen nicht repliziert werden. Allerdings ließen sich die ersten vier Hauptkomponenten interpretieren, indem die vier höchst geladenen Variablen auf jeder Komponente sinnvoll zusammenfasst wurden. Des Weiteren konnte das subjektive Stresserleben der deutschen sowie indischen Lehrkräfte durch die gesundheitsbezogenen Dimensionen, health-protective dispositions und extrinsic, avoidant motivation, vorhergesagt werden. Diskussion: Zum einen bestätigten die Befunde die multidimensionale Struktur des gesundheitsbezogenen Selbstkonzeptes, welche sowohl gesundheitsschützende als auch gefährdende Faktoren erfasst. Aufgrund der Nähe der verschiedenen Fit-Indizes zu den empfohlenen Grenzwerten wurde das modifizierte Messmodell (HRSC-21) als ein geeignetes Instrument bewertet. Zum anderen ließ sich ein Zusammenhang zwischen gesundheitsbezogenem Selbstkonzept und Stresserleben feststellen.
Das Kohärenzgefühl im höheren Lebensalter -Entwicklung und Validierung eines Messinstrumentes-

In unserer Gesellschaft vollzieht sich ein demografischer Wandel, der zu einer zunehmend alternden Bevölkerung führt. Dies erfordert, Menschen höheren Lebensalters vermehrt in den Fokus von Forschung und Medizin zu rücken, einem Anspruch, dem die vorliegende Arbeit gerecht werden möchte. Zielsetzung war die Entwicklung eines auf das von Antonovsky begründeten Konzeptes der Salutogenese ausgerichteten Messinstrumentes, zur Erfassung des Kohärenzgefühls im höheren Lebensalter. Die neue Skala sollte einerseits die testspezifischen Gütekriterien Objektivität, Reliabilität und Validität erfüllen, gleichzeitig aber auch forschungsökonomischen Prinzipien gerecht werden und das vor dem Hintergrund einer zielgruppenorientierten Ausrichtung.
Die Fragebogenentwicklung vollzog sich in mehreren Teilschritten. Als Einstieg diente eine explorative Vorstudie in Form von Gruppendiskussionen, die der Itemgenerierung diente. Hieraus entstand ein Pool von 90 Items, der im Anschluss einer Expertenbefragung hinsichtlich seiner Eignung für den Fragebogen unterzogen wurde. Als Resultat entstand eine 30 Items umfassende Sammlung, in der jede SOC-Komponente quantitativ gleich vertreten war. Diese wurde einer Gruppe von sechs Probanden zur Testung auf Verständlichkeit zugeführt. In der Folge wurde ein Item umformuliert, die anderen konnten unverändert in die vorläufige Fragebogenversion, Meine Lebensorientierung, übernommen werden. Diese wurde, eingebettet in eine umfangreiche Fragebogenbatterie, im Rahmen der Greifswalder Studie zur Lebensorientierung im Alter, im Oktober 2011 in drei Kleinstädten in Norddeutschland eingesetzt. Im Anschluss fand die statistische Datenauswertung statt. Mittels Itemanalyse wurden nicht geeignete Items identifiziert und aus der Sammlung eliminiert. Als Resultat entstand eine neue Skala, Das Kohärenzgefühl im höheren Lebensalter, mit 15 Items, jeweils fünf pro SOC-Komponente. In der durchgeführten Studie zeigte diese Skala eine gute interne Konsistenz. Hingegen ließen sich die theoriegeleiteten drei SOC-Komponenten faktorenanalytisch nicht reproduzieren, auch von dem Vorliegen eines Generalfaktors ist, als weitere wichtige Erkenntnis, nicht auszugehen. Die externe Validität wurde in der Untersuchung als mittelstark berechnet.
Abschließend lässt sich festhalten, dass die finale Skala, Das Kohärenzgefühl im höheren Lebensalter, ein reliables und valides Messinstrument darstellt, das zur ökonomischen Erfassung des Kohärenzgefühls, z. B. als Screening-Methode im Rahmen der hausärztlichen oder geriatrischen Betreuung, sinnvoll seinen Einsatz finden kann.
