Research has already shown that the maternal anthropometry affects birth weight. The impact on birth weight, body length and head circumference percentiles was not yet investigated. The aim of our observational studies was to develop individualized percentile charts (birth weight, body length and head circumference) for neonates based on maternal weight and height. To achieve this, we analyzed data of 2.2 million newborns stratified by maternal height and weight from the German Perinatal Survey.
The percentiles based on 18 groups stratified by maternal anthropometry for both sexes showed significant differences between identical original percentiles. The differences were up to almost 800 g between identical percentiles for petite and grande mothers. Birth length differed by several centimeters for the same percentiles between groups of short and tall stature mothers, whereas birth head circumference differed up to 1.2 cm.
Our analysis showed that maternal anthropometry has a significant effect on the classification of newborns as LGA (large for gestational age), AGA (appropriate for gestational age) and SGA (small for gestational age). Individualized charts show higher specificity than percentile charts that do not include those data and provide more individual prediction of perinatal risks.
Background: Native breast milk composition displays significant inter- and intra-individual variation which persists after standard fortification with fixed doses and challenges target fortification. This study aims to analyze the macronutrient composition of different commercially available fortifiers and the effect of different fortification strategies on nutritional intake of preterm infants.
Methods: In 103 preterm infants, native breast milk samples were collected from 24-h feeding batches (n = 3,338) and fat, protein and carbohydrate contents were analyzed. Nutrient content was compared for breast milk that had undergone either (i) standard fortification, (ii) targeted fortification, (iii) selective batching according to breast milk composition, or (iv) partial lyophilization. For (i) eight commercially available standard fortifiers were tested. Targeted fortification (ii) involved the addition of single component modulars of either protein, fat or carbohydrates to standard fortified breast milk. Using a mathematical growth model, the combined effect of protein, fat and carbohydrate intake on growth was assessed. The best composition of standard fortifiers as the initial step for target fortification was explored assuming three clinical scenarios for milk analysis.
Results: Macronutrient content was highly variable between native breast milk samples, and this variation was still present after standard fortification, however at elevated macronutrient levels. Standard fortification, breast milk batching, as well as partial lyophilization of human milk resulted in deficient and imbalanced enteral intakes in a significant proportion of infants. Target fortification reduced this variation in a, respectively, higher percentage of samples. The effect size was dependent on the number of measurements per week. The optimum composition of standard fortifiers was dependent on the clinical scenario (measurement frequency) for target fortification.
Conclusions: To provide precise and accurate intakes of macronutrients, breast milk should be target fortified. Standard fortified breast milk can result in excess above recommended intakes of some macronutrients which limits the efficiency of target fortification. Standard fortifiers with improved composition are needed for target fortification.
Inflammasome activation and formation of ASC specks in patients with juvenile idiopathic arthritis

Objective
The formation of large intracellular protein aggregates of the inflammasome adaptor ASC is a hallmark of inflammasome activation and characteristic of autoinflammation. Inflammasome activated cells release the highly proinflammatory cytokine IL-1β in addition to ASC specks into the extracellular space. Autoinflammatory activity has been demonstrated in systemic JIA, however minimal data exist on the role of inflammasomes in other JIA subtypes. We therefore investigated, if pyroptotic cells are present in the circulation of oligo- and poly-articular JIA.
Methods
Peripheral blood of JIA patients (n = 46) was investigated for ASC speck formation, a key step in inflammasome activation, by flow cytometry and immunofluorescence. Free ASC and proinflammatory cytokine levels were determined by ELISA and multiplex assay.
Results
Oligo-articular JIA patients showed a significantly increased proportion of ASC speck+ monocytes compared to poly-articular JIA patients. In serum free ASC alone is not sufficient to assess inflammasome activity and does not correlate with ASC speck+ monocytes. Compared to control several cytokines were significantly elevated in samples of JIA patients. JIA serum containing antinuclear antibodies, incubated with ASC specks boosts a secondary inflammation by IL-1β production in macrophages.
Conclusion
For the first time, we detect ex vivo inflammasome activation by ASC speck formation in oligo- and poly-articular JIA patients. Most notably, inflammasome activation was significantly higher in oligo- compared to poly-articular JIA patients. This data suggests that inflammasome derived autoinflammation may have a greater influence in the previously thought autoimmune oligo-articular JIA patients.
Um die Iodversorgung der Neugeborenen im Raum Vorpommern zu bewerten, wurde die Iodausscheidung von 418 gesunden Neugeborenen, die zwischen Mai 2005 und April 2007 in der Greifswalder Universitätsfrauenklinik geboren wurden, analysiert. 87 % der untersuchten Neugeborenen schieden > 10 µg Iod/dl Urin aus. Die mittlere Iodurie unterschied sich bei Kindern von Müttern, die während der Schwangerschaft zusätzlich Iod supplementiert hatten nur geringfügig von der Iodurie bei Kindern, deren Mütter während der Schwangerschaft kein zusätzliches Iod eingenommen hatten. Die Mediane lagen bei 20,1 µg/dl und 17,8 µg Iod/dl Urin. Das ist ein Zeichen dafür, dass sich die Iodausscheidung in den letzten Jahren deutlich verbessert hat und Neugeborene in der Region Vorpommern nach den WHO-Kriterien heute ausreichend mit Iod versorgt sind. Allerdings schieden bereits 54 % der untersuchten Neugeborenen > 20 µg Iod/dl Urin aus, was bereits die Gefahr einer Iodüberdosierung kennzeichnet.
Die vorliegende Arbeit beschäftigt sich mit dem Erregerdirektnachweis von Chlamydophila pneumoniae als Erreger akuter respiratorischer Erkrankungen bei Kindern und Jugendlichen. Der Erregerdirektnachweis erfolgte über die Polymerasekettenreaktion (PCR). Chlamydophila pneumoniae ist ein obligat intrazelluläres Bakterium der Ordnung Chlamydiales. Der Erreger kann intrazellulär in der Rachenschleimhaut persistieren. Die Übertragung des Erregers erfolgt über einen Wirt und wird mittels Tröpfcheninfektion von einer infizierten Person zur nächsten übertragen, so dass es leicht zu kleineren Epidemien kommen kann. Über einen Zeitraum von 31 Monaten wurden bei hospitalisierten Kindern mit respiratorischen Erkrankungen Rachenabstriche entnommen und prospektiv auf eine Infektion mit Chlamydophila pneumoniae untersucht. Hierzu wurden mikrobiologische, klinische und meteorologische Verlaufsparameter erhoben. Bei den 983 untersuchten Kindern wurde bei 9,0% Chlamydophila pneumoniae mittels PCR im Rachenabstrich nachgewiesen. Eine Altersabhängigkeit konnte nicht nachgewiesen werden. Die Zirkulation des Erregers fand bei den untersuchten Kindern mit respiratorischen Erkrankungen vor allem in der kalten Jahreszeit statt. Optimale Bedingungen, welche das Auftreten einer Chlamydophila pneumoniae Infektion erleichtern, liegen im Herbst bei einer Durchschnittstemperatur von 5,7°C, einer durchschnittlichen Luftfeuchtigkeit von 83,9% sowie einem mittleren Dampfdruckwert von 8,9 hPa vor. Schlussfolgern kann man aus den Ergebnissen, das die Ursache der Infektionshäufung in der kälteren Jahreszeit in der Schleimhautbarriere bzw. der reduzierten Abwehrlage / Immunsituation zu suchen ist. Dies zeigt, dass der Erreger bei reduzierter Abwehrlage seine volle Virulenz entfalten kann. Unsere Studie bestätigt, dass die Serologie als diagnostische Methode zum Nachweis einer akuten Chlamydophila pneumoniae Infektion nicht geeignet ist. Bei nur 18,2 % der PCR - pos. getesteten Kinder, konnte auch ein positives serologisches Ergebnis nachgewiesen werden Der direkte Chlamydophila pneumoniae Nachweis, z.B. mittels PCR, und der indirekte Infektionsnachweis mittels Serologie sollten zumindest im Kindesalter nicht gleichgesetzt, sondern differenziert verwendet werden. Das klinische Bild einer akuten Chlamydophila pneumoniae Infektion zeigt sich im Verlauf der Erkrankung relativ milde. Die häufigste Diagnose, welche mit dem Erreger zusammen auftritt ist die Pneumonie in einem milden Verlauf mit schwacher Signifikanz. Der fehlende Auskultationsbefund spricht nicht gegen eine Infektion. 32,2% der untersuchten Kinder hatten einen trockenen, unproduktiven Husten. Der trockene Reizhusten ohne Fieber ist häufig anzutreffen. Die Laborparameter sind mit erhöhter BSG und nur mäßig erhöhtem CRP eher unspezifisch. Bei Erkrankung der unteren Atemwege betrug die Prävalenz 54,5 %. Bezüglich des Rachens zeigt sich kein vermehrter pathologischer Befund. Dagegen zeigte sich in der Gruppe der PCR - pos. Kinder mit 36,8 % ein hoch signifikanter Anteil an Kindern mit pathologischem Tonsillenbefund. Empfehlen lässt sich, dass der Tonsillenabstrich prinzipiell besser zum Infektionsnachweis geeignet scheint, als der Rachenabstrich. Dies sollte man bei der Probenentnahme beachten.
Problemstellung: Ziel der aktuellen Studie war es Normwerte für den Lipidstatus von Neonaten verschiedener Gewichtsklassen unter Muttermilchernährung zu bestimmen. Dies ist nötig, da bisherige Empfehlung zu Lipidreferenzwerten auf der unkritischen Übernahme von Erfahrungen bei Kindern und Erwachsenen beruhen. Methoden: Es wurde eine prospektive Beobachtungsstudie auf der Neonatologie der Klinik für Kinder und Jugendmedizin der Universität Greifswald durchgeführt an gesunden,percentilengerecht wachsenden Neonaten, welche für mindenstens 48 Stunden vollständig oral mit einer Mindestmenge von 150 ml/kg/KG mit Muttermilch ernährt wurden. Das Einverständnis der Erziehungsberechtigten war vorausgesetzt. Ausgeschlossen wurden Neonaten, welche beatmet waren, Medikamente wie Steroide, Antibiotika oder Diuretika bekamen und Neonaten mit gastrointestinalen Erkrankung oder Sepsis. Es wurden der prä- und postprandialen Lipidstatus (Triglyzeride, Cholesterin, VLDL, HDL, LDL und die Acylcarnitine), als auch der Lipidgehalt der Muttermilch bestimmt. Die Neonaten wurden nach ihrem Gewicht am Studientag in Gruppen eingeteilt. Hypothesen: - Muttermilchernährte Neonaten haben wegen der höheren Fettzufuhr verglichen mit der parenteralen Ernährung höhere Lipidspiegel. -Die Lipidwerte hängen von dem Gewicht und dem Gestationsalter der Neonaten ab. - Small for gestational Age (SGA) - Neonaten haben höhere Lipidwerte. Ergebnisse: Es wurden insgesamt 65 Frühgeborene untersucht. Bei einer oralen Lipidzufuhr von 7 g/kg/KG wurden folgende präprandiale Lidspiegel gemessen . Triglyzeride 0.9 (0.6–1.1), Cholesterin 3.1 (2.6–3.5), VLDL 0.5 (0.3–0.6), LDL 1.3 (1.1–1.6), HDL 1.1(0.8–1.4) mmol/L und tAC 15,3 (10,2;20,0) µmmol/L. Für die Trigylzerid-, Cholesterin- und HDL- Werte konnten signifikante Unterschiede in den verschiedenen Gewichtsklassen nachgewiesen werden. Es wurden Korrelation zwischen den Lipidwerten und dem Geburtsgewicht (TG, Cholesterin, HDL, LDL), dem Gewicht am Studientag ( TG, postpransialer TG-Anstieg, Cholesterin, HDL) und dem Gestationsalter (TG, Cholesterin, HDL) nachgewiesen werden. SGA-Kinder hatten höhere präprandiale Trigylzeridspiegel (p < 0,005). Diskussion: Die Hypothese, dass es unter Muttermilchernährung zu höheren Lipidwerten kommt, wurde nicht erfüllt. Ursächlich könnte die Zusammensetzung der Muttermilch sein, welche einen lipidsenkenden Effekt auf den Fettmetabolismus hat. Außerdem könnte eine verminderte Fettresorption im Darm ursächlich sein, allerdings ist zu beachten, dass die Neoanten ausreichend gewachsen sind und aus diesem Grund eine unzureichende Fettresoprtion nicht anzunehmen ist. Die Hypothese, dass Lipidwerte vom Gewicht und Gestationsalter abhängen wurde erfüllt. Je jünger und je geringer das Gewicht (Geburt und Studientag) desto niedriger waren die Lipidwerte. Die Gründe sind nicht vollständig verstanden. Es könnte mit der Ernährung über eine Magensonde und der geringeren Fettzufuhr pro Mahlzeit zusammenhängen. Erneut ist eine verminderte Resorption zu diskutieren, da die Sekretion von Gallensäuren und Lipasen bei unreifen Frühgeborenen erniedrigt ist. Die Hypothese, dass SGA-klassifizierte Neonaten erhöhte Lipidwerte haben, wurde für die Triglyzeride bewiesen. Als ursächlich wird eine eine verminderte Aktivität der Lipoproteinlipase und eine Hyperinsulinämie angesehen. Fazit: Hohe Lipidwerte sind bei Frühgeborenen nicht physiologisch. Es sollte weiter an der Zusammensetzung von Lipidlösung zur parenteralen Ernährung geforscht werden, um auch bei dieser Form der Ernährung niedrige physiologische Lipidwerte zu erreichen.
Background
Short-term infusions of dinutuximab beta plus isotretinoin and cytokines administered in previous immunotherapy studies in neuroblastoma were associated with severe pain. Here, long-term, continuous infusion of single-agent dinutuximab beta was evaluated in patients with relapsed/refractory neuroblastoma.
Methods
In this open-label, single-arm, Phase 2 study, patients with either refractory or relapsed high-risk neuroblastoma received dinutuximab beta by continuous infusion over 10 days of each cycle, for up to five cycles. The primary endpoint was objective response rate 24 weeks after the end of cycle 5. Secondary endpoints included adverse events, intravenous morphine use, best response, duration of response, and three-year progression-free and overall survival.
Results
Of the 40 patients included, 38 had evaluable response. Objective response rate was 26% and best response rate 37%. Median duration of response was 238 days (IQR 108–290). Three-year progression-free and overall survival rates were 31% (95% CI 17–47) and 66% (95% CI 47–79), respectively. Prophylactic intravenous morphine use and duration of use decreased with increasing cycles. The most common grade 3 treatment-related adverse events were pain, diarrhea, and hypokalemia.
Conclusion
Long-term continuous infusion of single-agent dinutuximab beta is tolerable and associated with clinically meaningful responses in patients with relapsed/refractory high-risk neuroblastoma.

In der neonatologischen Intensivstation des Zentrums für Kinder- und Jugendmedizin in Greifswald war nach Einführen des Muttermilchsupplements FMS (Milupa®) für frühgeborene Kinder ein vermehrtes Auftreten metabolischer Azidosen aufgefallen. Dieser Eindruck sollte durch eine klinische Studie verifiziert oder falsifiziert werden. Durch die vorliegende prospektive, doppelblinde, randomisierte Studie konnte die Hypothese, dass das Muttermilchsupplement FMS für eine signifikant erhöhte Inzidenz von metabolischen Azidosen verantwortlich ist, verifiziert werden. Daraufhin wurde nach möglichen Erklärungen für diese metabolischen Azidosen gesucht. Die wahrscheinliche Ursache wurde in einer Imbalanz der Elektrolytzusammensetzung des Muttermilchsupplements gesehen. Um diese Ursächlichkeit zu beweisen und die erhöhte Inzidenz von metabolischen Azidosen zu beseitigen, wurde in Zusammenarbeit mit der Firma Milupa eine neue Rezeptur für das Muttermilchsupplement FMS entwickelt. Eine Folgestudie (nicht mehr Gegenstand dieser Dissertation) mit der neuen Rezeptur des Muttermilchsupplements FMS (FMS*) hatte zum Ergebnis, dass die Inzidenzen von metabolischen Azidosen unter Muttermilchsupplementierung mit FM 85 und FMS* keinen signifikanten Unterschied mehr aufwiesen. Den gewonnenen Ergebnissen unserer Studien zum Trotz werden noch viele Forschungsergebnisse notwendig sein, um die Ursachen für metabolische Azidosen von frühgeborenen Kindern unter der Supplementierung mit Fortifiern gänzlich zu erklären.
Rhinoviruses (RV) account for a significant number of asthma exacerbations, and RV species C may be associated with a severe course in vulnerable patient groups. Despite important evidence on the role of RV reported by clinicians and life scientists, there are still unanswered questions regarding their influence on asthma exacerbation in young patients. Thus, we measured the RVspecies-specific IgG titers in our German pediatric exacerbation cohort using a microarray-based technology. For this approach, human sera of patients with exacerbated asthma and wheeze, as well as healthy control subjects (n = 136) were included, and correlation analyses were performed. Concordantly with previously published results, we observed significantly higher cumulative levels of RV species A-specific IgG (p = 0.011) and RV-C-specific IgG (p = 0.051) in exacerbated asthma group compared to age-matched controls. Moreover, atopic wheezers had increased RV-specific IgG levels for species A (p = 0.0011) and species C (p = 0.0009) compared to non-atopic wheezers. Hypothesizing that bacterial infection positively correlates with immune memory against RV, we included nasopharyngeal swab results in our analyses and detected limited correlations. Interestingly, the eosinophil blood titer positively correlated with RV-specific IgG levels. With these observations, we add important observations to the existing data regarding exacerbation in pediatric and adolescent medicine. We propose that scientists and clinicians should pay more attention to the relevance of RV species in susceptible pediatric patients.
