Schilddrüsenhormone haben einen vielfältigen Einfluss auf den Stoffwechselmetabolismus. Die genauen molekularen Mechanismen, die mit Veränderungen der Schilddrüsenhormon-spiegel einhergehen, sind jedoch in vielen Bereichen noch unbekannt. Das Ziel der Arbeit bestand deshalb darin, auf Basis von Daten einer repräsentativen Stichprobe der Allgemein-bevölkerung die Zusammenhänge zwischen den Schilddrüsenhormonen und einem umfang-reichen Metabolitenpanel zu untersuchen, um so weitere Einblicke in die zugrunde-liegenden Ätiologien und Signal-wege zu erhalten.
Von 952 Teilnehmenden der SHIP-Trend-Studie wurden aus Plasma- und Urinproben in einem kombinierten Messverfahren (LC-MS sowie 1H-NMR) sowohl mit ungerichtetem als auch mit gerichtetem Ansatz das Metabolom quantifiziert. Um den Zusammenhang zwischen Thyroxin (fT4), Triiodthyronin (fT3) und Thyreotropin (TSH) mit den identifizierten Metaboliten zu untersuchen, wurden lineare Regressionsmodelle herangezogen. Bei Vorliegen eines signi-fikanten Interaktionseffekts zwischen fT4, fT3 bzw. TSH und dem Geschlecht wurden diese Analysen für Männer und Frauen getrennt durchgeführt. Eine Random-Forest-Analyse wurde angewendet, um die Relevanz phänotypischer Charakteristiken für die individuelle fT4-Konzentration vorherzusagen und die beobachteten Geschlechterunterschiede in den Asso-ziationen zu erklären.
Zwischen TSH und den Metaboliten im Plasma und Urin ließen sich keine signifikanten Assoziationen identifizieren. 106 von 613 Plasmametaboliten waren signifikant mit fT4 assoziiert. Assoziationen zwischen fT4 und den Urinmetaboliten ließen sich hingegen deutlich seltener beobachten (12 von 587). Die Mehrheit der mit fT4 assoziierten Plasmametabolite (n = 84) konnte den Klassen der Lipide und Lipidabkömmlingen zugeordnet werden. Die Analyse unterschiedlicher, mittels 1H-NMR-Spektroskopie quantifizierter Lipoproteinsub-fraktionen zeigte zudem in-verse Assoziationen zwischen der fT4-Konzentration und einer Reihe von großen sowie kleinen LDL- und HDL-Unterklassen auf. Dabei ließen sich auch einige geschlechtsspezifische Assoziationen beobachten. So konnten beispielsweise die Assoziationen zwischen fT4 und den HDL3-Subfraktionen nur bei Männern beobachtet werden. Assoziationen zwischen fT4 und verschiedenen mehrfach ungesättigten sowie gesättigten Fettsäuren waren hingegen nur bei Frauen sichtbar. Die identifizierten Geschlechterunterschiede in den Assoziationen lassen sich vermutlich bei Männern auf einen höheren Alkoholkonsum und bei Frauen auf Vorerkrankungen der Schilddrüse sowie den menopausalen Status zurückführen, denn die genannten phänotypischen Charakteristiken zeigten sich in den Random-Forest-Analysen als wichtige geschlechtsspezifische Faktoren für den individuellen fT4-Spiegel. Assoziationen zwischen fT3 und den Metaboliten im Plasma ließen sich im Vergleich zu fT4 deutlich seltener beobachten (55 von 613). Zudem waren 13 Urinmetabolite signifikant mit fT3 assoziiert. Die mittels 1H-NMR-Spektroskopie durch-geführte Analyse der Lipoproteinsubfraktionen zeigte positive Assoziationen zwischen fT3 und kleinen LDL-Unterklassen auf.
Die vorliegende Studie zeigte eine umfangreiche Signatur von Metaboliten auf, deren Konzentrationen im Plasma mit Serumspiegeln von fT4 und fT3, aber nicht mit TSH assoziiert waren. Somit deutet sie auf die besondere Relevanz von fT4 Spiegeln in der Therapie von Schilddrüsenerkrankungen hin, um eine ausreichende Versorgung peripherer Gewebe mit Schilddrüsen-hormonen zu gewährleisten.
Thiol or sulfhydryl groups are highly reactive functional groups in cellular systems. Molecules carrying thiol groups are mostly derivatives of the amino acid cysteine and are grouped as low molecular weight (LMW)-thiols: coenzyme A (CoA), glutathione (GSH) or bacillithiol (BSH). LMW-thiols can help in the maintenance of the reduced cellular environment as so called redox-buffers. Additionally, they act as co-factors in enzyme reactions or help in the detoxification of reactive oxygen or nitrogen species, electrophilic compounds or thiophilic metalloids (arsenite, tellurite). In proteins from different organisms cysteine is underrepresented compared to other amino acids, but still overtakes diverse roles. It is an important determinant in the tertiary and quaternary structure of proteins. The nucleophilic character of the thiol or thiolate group, respectively, makes cysteine the catalytically active amino acids of different enzymes. As a precursor cysteine participates in the formation of Fe-S clusters and coordinates different co-factors like heme, iron or zinc. The main goal of this study was the investigation of the different cellular thiol pools, now defined as the thiolome. The thiolome is the entity of the cellular thiol pools, i.e. LMW-thiols and protein thiols, and the dynamics between these pools. In Bacillus subtilis and Staphylococcus aureus mixed disulfides between protein thiols and free LMW-thiols, so called S-thiolations, were identified in different proteins in response to the thiol specific reagent diamide. Some of these S-thiolations were located at catalytically active cysteine residues. Subsequent analysis of metabolites supports this: the S-thiolation of the cobalamine-independent methionine-synthase MetE led to a decrease of the cellular methionine content. Additionally, the conversion of threonine to different branched-chain amino acids (BCAAs) was disrupted by the S-thiolation of the branched-chain amino acid aminotransferase YwaA, thereby probably inducing the synthesis of ppGpp, the alarmon of the stringent response. In addition to the identification of S-thiolations a technique was established which allowed the discrimination between intra- and intermolecular disulfides. The non-reducing/ reducing diagonal gel electrophoresis was applied to B. subtilis and S. aureus and confirmed known existing disulfide bonds, e.g. in alkyl hydroperoxide reductase AhpC or the thiol peroxidase Tpx. In response to diamide an increase of specific disulfide bonds in different proteins was observed. The analysis of the LMW-thiol content by an HPLC-approach allowed the observation of the dynamics of the thiolome. In response to diamide the reduced LMW-thiol content decreased by 75%, reduced protein thiols by 60%. Collaborations with other working groups allowed the identification of BSH in this approach. Additionally, an unknown thiol was found that is likely a derivative of BSH. Screening of the LMW-thiol content of different S. aureus-strains under various growth conditions revealed that strains 8325-4 and SH1000 lack BSH. The lack of BSH was attributed to an 8 bp-duplication in the bshC-gene that encodes the last enzyme of the BSH-synthesis. BSH-production was restored by transducing plasmid-borne functional BshC from strain Newman into strains 8325-4 and SH1000. The reconstitution of the BSH-synthesis aided in the resistance to the antibiotic fosfomycin but did not increase the resistance to different oxidants (diamide, sodium hypochlorite, hydrogen peroxide). The production of BSH had also positive effects on the survival of S. aureus inside human bronchial epithelial cells and murine macrophages in phagocytosis assays. Additionally, a GSH-uptake was observed into S. aureus which has before been known as a GSH-free bacterium. Taken together, this thesis provides the first insights into both, the LMW-thiol- and protein thiol pool of low GC, Gram-positive bacteria under different conditions. A plethora of different methodologies was used to describe the thiolome. The bacterial thiolome is a sophisticated system which is tightly regulated, but also flexible enough to not rely on determined molecules like BSH. The influences of the thiolome are not restricted to its own system and regulation, but also affect different branches of cellular physiology like the metabolism of BCAAs.
Die gestörte Glukosetoleranz (engl. impaired glucose tolerance, IGT) ist eines der präsymptomatischen Zustände des Typ-2-Diabetes mellitus (T2DM) und erfordert zur Diagnose einen oralen Glukosetoleranztest (oGTT). Ziele dieser Arbeit waren:
1) die Identifikation einer Metabolitsignatur zur Vorhersage einer IGT,
2) die pathophysiologische Einordnung der bedeutendsten Assoziationen und
3) die Identifizierung von Untergruppen innerhalb der IGT-Population, um so erste
Hinweise auf eine Patientenstratifizierung anhand objektivierbarer Metabolitmarker im
Blut zu erlangen.
Dazu dienten Metabolomdaten basierend auf Massenspektrometrie und Kernspinresonanzspektroskopie von 827 Teilnehmern der Study of Health in Pomerania (SHIP)-TREND. Durch einen targeted und non-targeted Anlayseansatz konnte ein breites Spektrum an Metaboliten erfasst und mithilfe statistischer und bioinformatischer Methoden die Assoziationen zum oGTT-Outcome, zur 2-Stunden Insulinkonzentration und daraus abgeleitet zur IGT, ausgewertet werden. Aus über 100 assoziierten Molekülen konnten beteiligte Stoffwechselwege, wie etwa der Katabolismus von verzweigtkettigen Aminosäuren und Veränderungen im Lipoprotein- und Fettstoffwechsel herausgearbeitet werden. Zudem konnte eine integrative Signatur zur Vorhersage einer IGT abgeleitet werden. Es wurde ein verbessertes IGT-Prädiktionsmodell etabliert, welches auf einer Kombination von Metaboliten und klinischen Merkmalen basiert. Diese Befunde sind jedoch in klinischen Studien zu prüfen. Über Verfahren des maschinellen Lernens konnten zwei Cluster innerhalb der IGT-Population identifiziert werden. Die IGT-Cluster spiegeln möglicherweise unterschiedlich fortgeschrittene IGT-Stadien oder verschiedene Ätiopathogenesen wider. Metabolitsignaturen könnten demnach gegebenenfalls eine frühzeitige Patientenstratifizierung ermöglichen. Somit wären bereits IGT-Patienten einem Risikoprofil zuzuordnen, was eine zuverlässigere T2DM- Risikoprognose erlauben würde. Durch rechtzeitige Präventivmaßnahmen bei IGT-betroffenen Personen könnte die Manifestation eines T2DM unter Umständen verhindert werden.
Die hier vorgestellten Befunde sind jedoch als vorläufig zu betrachten und bedürfen einer Replikation in unabhängigen Studien. Die Subtypen-Klassifizierung sollte idealerweise in einer längsschnittlichen Follow-up-Studie examiniert werden, um eine tatsächliche klinische Relevanz zu belegen. Von besonderer Bedeutung wäre dabei die Untersuchung der IGT-Cluster im Follow-up hinsichtlich eines Ansprechens auf allgemeine Interventionsmaßnahmen wie z.B. Lebensstiländerungen aber auch hinsichtlich einer spontanen IGT-Regredienz oder Progression zu einem T2DM und damit einhergehenden Komplikationen.
Orale Kontrazeptiva wurden 1960 zugelassen und erfreuen sich seitdem einer großen Beliebtheit. So gelten sie als eine der sichersten Verhütungsmethoden und haben auch weitere positive Effekte wie zum Beispiel eine Zyklusregulation und Abschwächen von Menstruationsbeschwerden. Nichtsdestotrotz leiden auch viele Nutzerinnen an Nebenwirkungen. In unserer und weiteren Studien konnte gezeigt werden, dass die Einnahme von oralen Kontrazeptiva die Serumcortisolkonzentration erhöht. Cortisol ist ein Steroidhormon, welches in Stresssituationen nötig ist, um die Homöostase beizubehalten und hat somit einen Einfluss auf den Stoffwechsel. Metabolite sind messbare Bestandteile des Stoffwechsels, da sie als Substrate und Produkte von Stoffwechselwegen Auskunft über die Prozesse des Körpers geben können. Sie unterliegen den vielfältigsten Einflussgrößen. Daher behandelt diese Dissertation den Einfluss von oralen Kontrazeptiva auf die Serumcortisolkonzentration und die Veränderung des Metaboloms. In einem weiteren Schritt wurde die Beeinflussung des Effekts von oralen Kontrazeptiva auf das Metabolom durch Cortisol berechnet.
Die Teilnehmerinnen der Studie waren prämenopausale Frauen, die im Rahmen einer Datenerhebung in der Durchschnittsbevölkerung im Nordosten Deutschlands rekrutiert wurden und zwei unabhängigen Stichproben angehörten (SHIP-TREND und SHIP-2).
Auch hier zeigten sich erhöhte Serumcortisolkonzentrationen bei Frauen, die orale Kontrazeptiva einnahmen. Des Weiteren konnten Metabolitveränderungen festgestellt werden, wie zum Beispiel 12 erhöhte Phosphatidylcholine, fünf erniedrigte und ein erhöhtes Lysophosphatidylcholine, fünf erniedrigte Aminosäuren und ein Carnitin, sowie ein erhöhtes Sphingomyelin. Auf den Großteil dieser Veränderungen hatte Cortisol einen signifikanten Einfluss in der Mediationsanalyse, wogegen die Menstruation nicht als Einflussfaktor nachgewiesen werden konnte.
Diese Studie stellt einen ersten Schritt zur Charakterisierung des Metaboloms bei der Einnahme von oralen Kontrazeptiva dar. Orale Kontrazeptiva haben einen systemischen Effekt und das Profil des Metaboloms könnte bei einer immer weiter individualisierten Medizin dazu beitragen, Frauen mit einem erhöhten Risiko für starke Nebenwirkungen der oralen Kontrazeptiva zu identifizieren, was zu einer Einnahme von besser geeigneten alternativen Verhütungsmethoden führen könnte.
The metabolomic approach is one part of the "-omics" cascade further comprising genomic, transcriptomic, and proteomic investigations. Since information about the metabolome of the important human pathogenic bacterium Staphylococcus aureus is scarce, the aim of this thesis is the characterization of the exo- and endometabolome of this bacterium on a most global scale. For this, the metabolomic platform consisting of the analytical instruments used for 1H-NMR spectroscopy, HPLC-MS, and GC-MS analysis was applied. First, the requirements for an accurate sampling procedure for the analysis of intracellular metabolites are presented, explaining important pitfalls during the sampling and the subsequent metabolome analysis via HPLC-MS and GC-MS (book chapter I). The challenging task of the metabolite identification is demonstrated, as well as the requirements for absolute quantification of intracellular metabolites. In order to enhance the knowledge about the staphylococcal physiology and the biochemical network, the impact of different stresses and varying cultivation media on the bacterial metabolite pool was investigated in several studies. In article I, a first description of the primary metabolism of growing S. aureus COL cells cultivated aerobically in CDM is provided. This study also monitored the adaptation to glucose starvation on the level of metabolites and proteins. The uptake of all amino acids and the secretion and reuse of overflow metabolites were analyzed in a time-dependent manner. During the switch to a non-growing state, a drastic rearrangement of the amino acid pool in the bacterial cells was detected, and intracellular amounts of glycolytic intermediates were found to decrease in parallel to extracellular glucose exhaustion. During infection processes, S. aureus has to cope with varying levels of oxygen supply, including anaerobic conditions. A global metabolomic approach investigated the adaptation of S. aureus COL to strict anaerobic conditions using CDM as the culture medium. Thereby only linear growth was possible despite the higher uptake rate of glucose compared to aerobically, logarithmically growing cells. In an anoxic environment, S. aureus mainly switched on the less reliable lactic acid fermentation. Only serine and threonine but no alanine were significantly taken up. Subsequent glucose limitation led to energy starvation indicated by a drop in the adenylate energy charge. This was accompanied with an arrest of the fermentative metabolism and declining numbers of colony-forming units without taking advantage of the energy supplying arginine deiminase pathway. Compared to the established CDM, the eukaryotic cell culture medium RPMI 1640 provides more in vivo-like growth conditions. In article II, the growth behavior and the metabolic footprint of the S. aureus strains COL and HG001 were investigated during the aerobic cultivation in RPMI 1640 medium. Both strains are commonly used in laboratory research. The observed uptake and secretion pattern of extracellular metabolites provides important information for infection studies in which this medium is used for the precultivation of S. aureus. The extracellular accumulation of the noncanonical D-amino acid D-isoleucine was an interesting outcome. The strain specific metabolic footprint points to noteworthy differences in the biochemical system of both strains. Moreover, this study demonstrates the impact of the cultivation medium on the metabolic status of bacterial cells. Due to increasing resistance against a large number of antibiotics, community- and hospital- acquired infections with S. aureus are of major concern in medical therapy. Thus, greater knowledge about adaptive mechanisms after antibiotic treatment is required. In article III, the response of S. aureus HG001 to antibiotics with varying target sides, such as ciprofloxacin, erythromycin, fosfomycin, vancomycin, and ampicillin, was investigated on the metabolite level. Thereby, the abundances of 176 intracellular metabolites were observed in a time-dependent manner, thus providing the most comprehensive experimental metabolite dataset so far available for S. aureus. None of the antibiotic compounds led to alterations of single metabolite amounts, but mostly entire metabolic pathways were affected. The intermediates of the cell wall biosynthesis were affected by each antibiotic, confirming this pathway as the most potential target for new antibacterial compounds. The metabolite composition of human nasal secretions and human sweat was analyzed, since such secretions present natural habitats of S. aureus during the colonization of typical host sides. The results confirm that the bacteria has to cope with low concentrations of most of the amino acids but large amounts of urea and lactate during host colonization. Considering the supply of amino acids, the results support the usage of the RPMI 1640 medium as a step to more in vivo-like cultivation experiments. Moreover, essential information for future studies about the adaptation of S. aureus to more in vivo growth conditions is provided. Altogether, the metabolomic approach was proven to be an important tool for helping unravel the complex bacterial metabolism and the environmental factors that also play a role in the virulence of Staphylococcus aureus.
Adipositas ist medizinisch und sozioökonomisch ein weltweit an Bedeutung gewinnendes Problem. Bariatrische Chirurgie hat sich als effektivste Möglichkeit zur Behandlung morbider Adipositas erwiesen. Dabei ergeben sich deutliche Verbesserungen des diabetischen Stoffwechsels bereits kurz nach dem Eingriff, bevor ein signifikanter Gewichtsverlust eingetreten ist. Die Mechanismen, die dazu führen, sind dabei noch nicht vollständig aufgeklärt. Ziel der Arbeit war es, mit Hilfe des Metabolomikansatzes herauszufinden, ob bariatrische Chirurgie einen Einfluss auf das Metabolom des Urins hat. Dazu wurden Urinproben von 50 Patienten jeweils prä-operativ und bis zu 13 Tage post-operativ mittels 1H-NMR untersucht und mit Hilfe von multivariaten statistischen Methoden analysiert. Dabei konnte deutlich zwischen prä- und post-operativen Proben unterschieden werden. PLS-DA und OPLS-DA Modelle waren in der Lage, 95 % der Spektren richtig in prä- und post-operativ zu klassifizieren. Zur Unterscheidung trugen in erster Linie die Buckets b20, b49 und b50 bei. Bei Betrachtung der gemittelten Spektren fielen eine Heraufregulation in den ppm-Bereichen 1,20-1,24, 2,1-2,5, 3,2-3,6, 4,1-4,2, 7,40-7,45 und 7,6-7,7 sowie eine Herabregulation in den ppm-Bereichen 7,5-7,6 und 7,8-7,9 jeweils post-operativ auf. Bariatrische Chirurgie verändert somit das Metabolom des Urins. Den Variationen im Spektrum liegen Metabolite zu Grunde, deren Identifikation Rückschlüsse auf Stoffwechselprozesse erlauben. Diese können wiederum Erklärungsansätze für den Gewichtsverlust und die Stoffwechselbeeinflussung in Folge einer bariatrischen Chirurgie liefern. Dieses bessere Verständnis der pathophysiologischen Vorgänge könnte weiterhin zur Entwicklung weniger invasiver chirurgischer Eingriffe oder spezieller, individueller pharmakologischer Therapien führen, zielgerichtet auf Gewichtsverlust und Remission des Diabetes mellitus. Weiterhin könnte Metabolomik bei der Entscheidung über die OP-Methode helfen. Dazu müsste es gelingen, aus einem großen Patientenkollektiv mit mehreren OP-Methoden im Urin z.B. einen Prädiktor zu finden, welcher Gewichtsverlust und Resolution von Komorbidität für einen individuellen Patienten vorhersagt. Insgesamt befindet sich die Metabolomikforschung noch in den Anfängen. Im Besonderen gilt dies für die Dokumentation des Einflusses chirurgischer Eingriffe auf das Metabolom des Urins. Weitere Studien mit einem größeren Patientenkollektiv und alternativen Fragestellungen könnten hier zu einem Erkenntnisgewinn führen.
Periodontitis is one of the most prevalent oral diseases worldwide caused by multifactorial interactions between host and oral bacteria. Altered cellular metabolism of host and microbes releases a number of intermediary end-products known as metabolites. Recently, there is an increasing interest in identifying metabolites from oral fluids like saliva to widen the understanding of the complex pathogenesis of periodontitis. It is believed, that some metabolites might serve as indicators toward early detection and screening of periodontitis and perhaps even for monitoring its prognosis in the future. Because contemporary periodontal screening methods are deficient, there is an urgent need for novel approaches in periodontal screening procedures. To this end we associated oral parameters (clinical attachment level, periodontal probing depth, supragingival plaque, supragingival calculus, number of missing teeth, and removable denture) with a large set of salivary metabolites (n=383) obtained by mass spectrometry among a subsample (n=909) of non-diabetic participants of the Study of Health in Pomerania (SHIP-Trend-0). Linear regression analyses were performed in age-stratified groups and adjusted for potential confounders. A multifaceted image of associated metabolites (n=107) with considerable differences according to age groups was revealed. In the young (20-39 years) and middle-aged groups (40-59 years), we found metabolites predominantly associated with periodontal variables; whereas among the older subjects (60 + years), tooth loss was strongly associated with metabolite levels. Metabolites associated with periodontal variables were clearly linked to tissue destruction, host- defence mechanisms and bacterial metabolism. Across all age groups, the bacterial metabolite phenylacetate was significantly associated with periodontal variables. Our results revealed alterations of the salivary metabolome in association with age and oral health status. Among our comprehensive panel of metabolites, periodontitis was significantly associated with the bacterial metabolite phenylacetate, a promising substance for further biomarker research.
