Liu–II coal pit is a typical example of China’s deep coal mines which is seriously threatened by groundwater inrush from the underlying carboniferous Taiyuan limestone formation. An exhaustive data set of this confined aquifer exists. The aquifer lies 45 m∼ 60 m below the major coal seam. A traditional artesian aquifer test has been performed in order to assess the hydraulic properties, e.g. transmissivity (T) and storage coefficient (S). This artesian aquifer test is conducted with four simultaneously operating production wells while the discharge of each production well varied with time. The results of this test suggest that the aquifer is heterogeneous. Therefore, the according problems are: (1) how to analyze the artesian aquifer test with linearly declining discharge; (2) how to deal with multiple production wells in an aquifer test; (3) how to adequately consider aquifer heterogeneity. Thus, the objective of this thesis is to solve these problems. 1) As opposed to classical above-ground pumping tests, it is difficult to control the discharge rate of the production well in a deep mine artesian aquifer test since the hydraulic pressure is extraordinary high. Moreover the discharge rate won’t descend rapidly to zero, thus the analytical solution of Jacob and Lohman (1952) type curve for the artesian aquifer test will not be applicable. It is more reasonable to analyze the test as a pumping test with variable discharge. It is considered to rebuild a hydrogeological conceptual model which is similar with Theis (1935) model but with the variable discharge. A general equation for any discharge variability is given. Its application for the linearly declining discharge is presented subsequently, and a type curve of this equation with linearly declining discharge is given as well. After that, a simple numerical model is built by FEFLOW to simulate an artificial pumping test with the linearly declining discharge by assigning different parameter sets for transmissivity and storage coefficient. The type curve method is applied to evaluate transmissivity and storage coefficient for the linearly declining discharge well. The deviation between the given values of transmissivity and storage coefficient in FEFLOW and the values of those calculated by matching point are sufficiently small. Thus, when the discharge of production well declines linearly, a type curve method as an empirical method is reasonable and gives satisfactory values of these hydrogeological parameters. 2) In some cases, it is necessary to conduct a pumping test (or an artesian aquifer test) with several pumping wells (or production wells) which work simultaneously in order to discharge maximum quantity of groundwater. Normally, the superposition method or numerical simulation is applied to analyze the test result. However, a new approach called “Well Generalization Method” is defined and analyzed in this thesis. It is an easy–to–use approach for hydrogeologist to estimate the aquifer parameters while conducting an aquifer test. Since the key point of this approach is using a generalization well to substitute the pumping (or production) wells, it is obvious that this approach will generate the estimated error of parameters. Accordingly, several scenarios are analyzed and discussed based on the artificial type aquifer designed in FEFLOW. A homogeneous aquifer and a heterogeneous aquifer which is generated by geostatistical stochastic simulation technique (see 3)) are discussed separately. As a result, this approach is feasible and applicable under some conditions when the calculated observation well is arranged more than about 2.5 times the scale of the multi–pumping–wells field away from the center of the multi–pumping–wells field, furthermore, the maximum deviation of drawdown resulting from these observation wells will be less than 0.5 m, and the estimated value of transmissivity will be 0.44% smaller than real value. 3) Finally aquifer heterogeneity is addressed, in order to check the introduced method for applicability under realistic conditions. It has been described that aquifer heterogeneity plays a major role in hydrodynamic processes (e.g. de Marsily et al., 1998). Geostatistics which is considered as a useful tool for characterizing the spatial variability of transmissivity is applied to solve this problem. Based on the results of the artesian aquifer test conducted in Liu–II coal pit, a model of spatial variability of transmissivity is developed. Sequentially, the variogram model is applied in ordinary kriging to interpolate the transmissivity distribution, and in sequential Gaussian simulation to simulate a random field of transmissivity data in order to reflect its small scale variability. A comparison of the results of estimation and simulation of transmissivity indicates that the simulated values better reflect the spatial variability, reversely, the estimated values are much smoother.
MicroRNAs (miRNAs) and long non-coding RNAs (lncRNAs) are involved in the modulation of the DNA-damage response (DDR) and upon exposure to ionizing radiation (IR), their expression fluctuates. In this study, we propose a workflow that enables the creation of regulatory networks by integrating transcriptomics data as well as regulatory data in order to better understand the interplay between genes, transcription factors (TFs), miRNAs, and lncRNAs in the cellular response to IR. We preprocessed and analyzed publicly available gene expression profiles and then applied our consensus and integration approach using open source data and tools. To exemplify the benefits of our proposed workflow, we identified a total of 32 differentially expressed transcripts corresponding to 20 unique differentially expressed genes (DEGs) and using these DEGs, we constructed a regulatory network consisting of 106 interactions and 100 nodes (11 DEGs, 78 miRNAs, 1 DEG acting as a TF, and 10 lncRNAs). Overrepresentation analyses (ORAs) furthermore linked our DEGs and miRNAs to annotations pertaining to the DDR and to IR. Our results show that MDM2 and E2F7 function as network hubs, and E2F7, miR-25-3p, let-7a-5p, and miR-497-5p are the four nodes with the highest betweenness centrality. In brief, our workflow, that is based on open source data and tools, and that generates a regulatory network, provides novel insights into the regulatory mechanisms involving miRNAs and lncRNAs in the cellular response to IR.
Computational chemical physics can give important input to astrophysical modelling and other fields of physics, where molecular properties are of importance. Understanding of spectroscopic and reactive behaviour is crucial for many systems of astrophysical interests like stars, interstellar medium and comets. Especially stellar atmospheres are of interest, because the complex physics of stars are not yet completely understood. Stars are in an unstable balance of gravitation and radiation pressure and the atmospheric dynamics have been subject of extensive modelling. Complete and accurate spectroscopic information of the atoms and molecules in these atmospheres is necessary for this attempt. In addition, the only information we have about astrophysical systems is light which is emitted or absorbed by particles in these media. This is not only true for astrophysics. In plasma physics sometimes the usage of invasive diagnostics, like Langmuir probes, is not wanted because they disturb the system. In these cases some information of the system can be regained by passively measuring infrared spectra of the plasma or by active induction of electronic transition like the laser-induced fluorescence method. Another remote sensing application is the measurement of the atmospheric composition on earth. Here, larger particles in the atmosphere as well as greenhouse gases are of current interest. Unfortunately, the experimental spectroscopic data, which is needed for the understanding and interpretation of the measured spectra, is often incomplete. This gap can be, to some extend, filled by computational chemical physics. The aim of this work was to investigate the capabilities and limitations of ab initio based potential energy surfaces for spectroscopic and reactive studies and to apply these methods to problems of rovibrational and rovibronic spectroscopy and reaction dynamics. The choice of ab initio methods and the potential fitting methods is critical for the computational chemical physics, as all further quantities directly depend on their quality. In this work modified versions of the Braams polynomial potential energy surface were used. A high level coupled cluster ab initio method was used to build potentials for a series of small hydrocarbons. Hydrocarbons can be found almost everywhere on earth and in the universe. They exist in laboratory plasmas, stellar and planetary atmospheres and interstellar gases. In all these cases, light emitted or absorbed by the molecules is an important diagnostics of the system. The potential constructed in this work partly included a cluster expansion, which adds reactant configuration spaces to the fits. This could not be done for CH_3 and higher hydrocarbons, because of the limitations of the Coupled Cluster ab initio method, which is well suited for the potential wells, but not for the dissociation regions. The examples of methyl and methane show how the potentials can be used for rovibrational spectroscopy. Results of radiation transport simulations illustrate the importance of as complete-as-possible line lists for radiation transport calculations.\\ The rovibronic spectroscopy of diatomic molecules is another important aspect for the stellar atmospheric modelling. Metal hydrides and oxides add opacity to the atmosphere in the visible light and ultraviolet frequency regions, as well as do the hydrocarbons in the infrared one. In addition the spectra of metal hydrides/oxides can be used to gather information about metal and their isotope abundances. They are used as markers for the conditions in the atmospheres of stars. In this work a new code was developed, that efficiently calculates bound-bound transitions between electronic states and bound-continuum cross sections for diatomic molecules. It also offers an adequate treatment of quasi-bound rovibrational states. One important representative of the diatoms is magnesium hydride, MgH. Before this work, line lists and photodissociation cross section were available involving the three lowest doublet states of MgH. In this work new potential energy curves were calculated and adapted to updated experimental data. This causes changes in the relative energies between the electronic states and therefore shifts in the line lists. These are important, because accurate line positions are needed for the identification of spectral lines. In addition two further electronic states were included in the calculations. This expands the spectral range of MgH into the near ultraviolet region. Radiation transport models showed significant absorption by MgH from the newly added electronic states. A second usage of the diatomic potential energy curves are photodissociation cross sections. As interstellar environments are chemically active, such data is necessary for a complete picture of the ongoing processes. The photodissociation cross sections of MgH reveal a stronger dependence of the underlying potential than the bound-bound lines. In the case of MgH the cross sections are rather weak, besides occasional resonance lines which can be several orders of magnitude stronger. As mentioned, not only spectroscopic, but also reactive behaviour of molecules is important in astrophysics. A current problem connected with this is the abundance of CH^+ in interstellar clouds. Its measured abundances do not fit the predictions from theoretical models. In addition Gerlich and co-workers recently measured low temperature H + CH^+ -> C^+ + H_2 reaction rates, which diverge from the theoretical picture and which could not be explained. In this work a reactive potential energy surface was built for the CH_2^+ system, which was then used to perform extensive calculations with quasi-classical trajectory and quantum scattering methods. It was found out, that the potentials used in previous works are not accurate enough to allow low temperature calculations. Results from these potentials must be taken with care. Furthermore, the results from the new potential energy surface indicate significantly reduced reaction rates compared to previous numerical studies. This is in agreement with the new results of Gerlich and co-workers. Nevertheless, the large error bars in the low temperature range for experimental as well as numerical results strongly suggest refined methods to be developed for both, before a final conclusion can be made. This work demonstrated the possibility of modern computational chemical physics to supply consistent data for spectroscopy and reaction dynamics. These are necessary and important inputs for fields like astrophysics, plasma physics and chemistry.
Prüfungsangst stellt eine schwerwiegende und häufig auftretende psychische Störung dar. In der klinischen Praxis war die Abgrenzung klinisch relevanter Prüfungsangst von subklinischer Prüfungsaufregung lange Zeit schwierig und die psychische Störung wurde uneinheitlich als soziale oder als spezifische Phobie kodiert, weil es an eindeutigen Diagnosekriterien mangelte. In den vergangenen Jahrzehnten intensiver Beforschung des Themenkomplexes Prüfungsangst, insbesondere durch die Pädagogische Psychologie, wurden vielfältige Variablen mit Prüfungsangst in Verbindung gebracht und als direkte oder indirekte Prädiktoren diskutiert. Bislang fehlte es jedoch an der Integration dieser unterschiedlichen Erklärungsansätze in ein geeignetes Rahmenmodell. Zunächst wurde untersucht, ob sich das „Test Anxiety Inventory“ (TAI) eignet, klinisch unauffällige von klinisch relevanter Prüfungsangst abzugrenzen. Dazu wurden eine Stichprobe 47 prüfungsängstlicher Patienten einer Psychotherapieambulanz und eine Gruppe von 41 Studenten mit gesunden Ausmaßen an Prüfungsangst verglichen. Dabei wurde auch untersucht, mittels welcher Diagnose die Prüfungsangst der Patienten von den behandelnden Therapeuten kodiert wurde und ob sich objektivierbare Unterschiede zwischen unterschiedlich klassifizierten Patienten finden lassen. Im zweiten Schritt wurden in Anlehnung an das Prüfungsangstmodell von Zeidner und Matthews (2007) die wichtigsten Prüfungsangstprädiktoren hinsichtlich ihrer prädiktiven Validität für die Unterscheidung pathologischer und gesunder Prüfungsangstintensitäten analysiert. Im dritten und letzten Arbeitsschritt wurde eine Stichprobe von 22 Prüfungsangstpatienten im Längsschnittverlauf einer kognitiv-verhaltenstherapeutischen Behandlung unter realistischen Therapiebedingungen betrachtet. Ziel war es dabei zu überprüfen, ob die bisherigen Erkenntnisse auch für die Vorhersage des Therapieerfolgs bedeutsam sind. Ein Cut-Off-Wert von 80 Punkten im TAI scheint sich zur Unterscheidung klinischer und nicht-klinischer Ausmaße an Prüfungsangst zu eignen. Das Krankheitsbild der untersuchten Prüfungsangstpatienten zeigt sich sehr einheitlich und ist unabhängig von der vergebenen Störungsdiagnose des Therapeuten. Das Vorliegen einer komorbiden depressiven Erkrankung beeinflusst nicht die Schwere der Prüfungsangst. Selbst bei Beachtung des Einflusses der grundsätzlichen psychischen Belastung ist eine Unterscheidung pathologischer und nicht-pathologischer Prüfungsangst anhand der Konstrukte Lernzielorientierung, Fähigkeitsselbstkonzept, Selbstbeschuldigung, Elaboration im Lernen und Perfektionismus möglich. Diese Variablen mit der höchsten diskriminierenden Validität entspringen allen drei Erklärungsebenen des Prüfungsangstmodells von Zeidner und Matthews, welches sich offensichtlich zur Untersuchung der Bedeutung der unterschiedlichen Prüfungsangstprädiktoren eignet. Im Rahmen der psychotherapeutischen Behandlung der Prüfungsangst kam es zwar insgesamt zur Reduktion prüfungsängstlicher, depressiver und sozialängstlicher Symptome sowie der grundsätzlichen psychischen Belastung, jedoch haben die Patienten sehr unterschiedlich auf die Behandlung angesprochen. Nahezu 50 Prozent der behandelten Betroffenen weisen auch nach dem Therapieende noch immer klinisch relevante Werte an Prüfungsangst und nur unerhebliche Verbesserungen der anderen interessierenden Variablen auf. Die Bedeutung der Variablen Elaboration, Lernzielorientierung, Fähigkeitsselbstkonzept und Selbstbeschuldigung bestätigt sich auch in der Längsschnittanalyse. Der empfohlene Cut-Off-Wert im TAI sollte in repräsentativen Stichproben repliziert und das Instrument konventionell zur Diagnostik von Prüfungsangst verwendet werden um die Identifikation pathologischer Prüfungsangst zu erleichtern und dem Screening sowie der Differentialdiagnostik der Störung zu dienen. Schwere und Generalisierungsgrad sozialängstlicher Symptome sollten in der Prüfungsangstdiagnostik stärker beachtet werden. Die Möglichkeit, Prüfungsangst wie im DSM-5 als Sozialphobie mit dem Spezifikator „Nur in Leistungssituationen“ zu diagnostizieren, sollte zukünftig auch im ICD Anwendung finden um die Kodierung der Prüfungsangst zu vereinheitlichen. Parallel vorliegende psychische Erkrankungen sollten frühzeitig im Verlauf der Diagnostik in ihrer Bedeutung als Ursache oder Folge von Prüfungsangst identifiziert werden um entsprechende Ableitungen für den Behandlungsplan vornehmen zu können. Die klinische Forschung sollte sich stärker auf das Prüfungsangstmodell von Zeidner und Matthews und bei Replikation unserer Ergebnisse auf die zentralen Prüfungsangstprädiktoren Lernzielorientierung, Fähigkeitsselbstkonzept, Selbstbeschuldigung, Elaboration und Perfektionismus konzentrieren. Entsprechende Behandlungsansätze sollten gezielt auf ihren Therapieeffekt hin untersucht werden. Zudem sollte genau analysiert werden, welche weiteren Faktoren es gibt, die über das Therapieansprechen entscheiden.
Abbau von Phenylalkanen und weiteren alkylsubstituierten Aromaten durch Hefen und filamentöse Pilze

Gegenstand der vorliegenden Arbeit war es, den Abbau von Phenylalkanen durch eukaryotische Mikroorganismen, insbesondere Pilze, zu untersuchen. Im Focus der Dissertation lagen dabei Untersuchungen mit der Hefe Trichosporon asahii SBUG-Y 833. Des Weiteren erfolgten Analysen mit Candida maltosa SBUG Y 700, Trichosporon mucoides SBUG Y 801 und neun filamentösen Pilzen der Gattungen Cunninghamella, Fusarium, Lecanicillium, Mucor, Penicillium, Sporothrix und Umbelopsis. Als Substrate wurden Phenylalkane mit fünf bis zehn und zwölf Kohlenstoff-Atomen in der Alkylseitenkette eingesetzt. Zur Charakterisierung der Abbau- und Transformationsleistungen der Hefen, insbesondere von T. asahii, erfolgten darüber hinaus Biotransformationsexperimente mit Phenylalkan-Derivaten und aromatischen Säuren. Candida maltosa 1. Mit der Hefe C. maltosa, die zur Assimilation von n Alkanen befähigt ist, konnte ein Wachstum mit Phenylalkanen (0,5 % [v/v]), deren Alkylseitenkette mindestens 8 Kohlenstoff-Atome aufwiesen, ermittelt werden. 2. In Biotransformationsexperimenten mit ungeradzahligen Phenylalkanen (Phenylheptan und Phenylnonan) konnte eine kontinuierliche extrazelluläre Akkumulation von Benzoesäure nachgewiesen werden. Phenylalkane mit einer geraden Anzahl von Kohlenstoff-Atomen in der Alkylseitenkette (Phenylhexan, Phenyloctan, Phenyldecan und Phenyldodecan) werden via Phenylbuttersäure und 4 Phenyl 3-butensäure zu Phenylessigsäure abgebaut, die ebenso wie Benzoesäure extrazellulär angereichert wird. 3. C. maltosa ist nicht zur weiteren Oxidation von Benzoesäure und Phenylessigsäure befähigt und akkumuliert daher diese Säuren während des Phenylalkan-Abbaus als dead-end-Produkte. Trichosporon asahii 1. In Wachstumsexperimenten mit T. asahii konnte gezeigt werden, dass die Hefe n Alkane (n Dodecan, n Tetradecan, n Hexadecan) und Phenylalkane mit mindestens sieben Kohlenstoff-Atomen in der Alkylseitenkette assimilieren kann. 2. In Biotransformationsexperimenten mit ruhenden Zellen und Phenylheptan konnten anhand von HPLC-, GC-MS- und z. T. NMR-Analysen neun Produkte identifiziert werden: 7 Phenylheptansäure, 7-(2 Hydroxyphenyl)-heptansäure, 3 (2 Hydroxyphenyl) propionsäure, Benzoesäure, 3,4 Dihydroxybenzoesäure, Cumarin, 4 Hydroxycumarin, 4,6 Dihydroxycumarin und 4,8 Dihydroxy-cumarin. 3. Die Bildung der Metaboliten 2 Hydroxyphenylheptansäure und 2 Hydroxyphenylpropionsäure sowie der Cumarine konnte erstmals durch die Ergebnisse der vorliegenden Arbeit für den mikrobiellen Abbau von Phenylalkanen beschrieben werden. Die hydroxylierten Cumarine 4 Hydroxy-, 4,6 Dihydroxy- und 4,8 Dihydroxycumarin wurden bis Versuchende kontinuierlich im Inkubationsmedium akkumuliert, während die übrigen sechs Produkte nur zwischenzeitlich durch die Hefe ausgeschieden wurden. Die Inkubation von T. asahii mit Phenyloctan führte dagegen nur zum Nachweis der hydroxylierten Cumarine. In Biotransformationsexperimenten mit Phenylnonan, Phenyldecan und Phenyldodecan konnte als einziger Metabolit 4 Hydroxy-cumarin detektiert werden. Die für andere Hefen typischen Abbauprodukte wie Benzoesäure und Phenylessigsäure wurden durch diese Aromaten verwertende Hefe nicht akkumuliert. 4. Die Bildung von 4 Hydroxycumarin konnte auch in Biotransformationsexperimenten mit Phenylheptansäure, 2 Hydroxyphenyl-propionsäure, trans 2 Hydroxyzimtsäure sowie Cumarin nachgewiesen werden. Während die Transformation der zwei ortho-hydroxylierten Säuren in Ausbeuten von über 70 % 4 Hydroxycumarin innerhalb von 24 h resultierte, wurden nur 9,4 % der Phenylheptansäure und ca. 13 % des Cumarins in 4 Hydroxycumarin transformiert. 6. Im Hinblick auf die medizinische Bedeutung der Cumarine wurde die Bildung von Cumarinen aus den Präkursor-Stoffen 2,4 Dihydroxyphenylpropionsäure und 7 Hydroxycumarin durch T. asahii geprüft. Dabei konnte 4,7 Dihydroxycumarin während der Inkubation mit 2,4 Dihydroxyphenyl-propionsäure und 7 Hydroxycumarin nachgewiesen werden und zusätzlich 6,7 Dihydroxycumarin mit 7 Hydroxycumarin als Substrat. Eine 20-fache Steigerung der 6,7 Dihydroxycumarin-Konzentration wurde mit Zellen einer Phenol-Kultur im Vergleich zu Zellen, die mit Hefeextrakt kultiviert wurden, erreicht, was auf die Beteiligung einer induzierbaren Phenolhydroxylase hindeutet. 7. Unter Verwendung des Cytochrom P450-Inhibitors 1 Aminobenzotriazol konnte eine Beteiligung von Cytochrom-P450-Enzymen an der ortho-Hydroxylierung des Benzenrings von Phenylalkanen bzw. alkylsubstituierten aromatischen Säuren ermittelt werden. Diese Reaktion ist neben der Einführung einer Doppel-bindung in der Alkylseitenkette eine wesentliche Voraussetzung für die Bildung von Cumarinen. 8. Während der Inkubation von T. asahii mit dem Phenylheptan-Derivat Heptanophenon wurden primär Metaboliten detektiert, die am C1-Atom der Alkylseitenkette eine Hydroxy-Gruppe aufweisen und/oder subterminal am C4-, C5- und C6-Atom oxidiert sind. Aufgrund der Ergebnisse der vorliegenden Arbeit konnte für Hefen erstmals eine subterminale Oxidation von gesättigten Alkylketten nachgewiesen werden. Trichosporon mucoides 1. In den Untersuchungen mit T. mucoides konnte gezeigt werden, dass die Hefe nicht zur Assimilation von n Alkanen (n Dodecan, n Tetradecan, n Hexadecan) befähigt ist. Die Kultivierung mit Phenylnonan und Phenyldecan führte zwar nur zu einer geringen, dennoch signifikanten Zunahme der Biomasse. 2. Obwohl T. mucoides keine n Alkane verwerten kann, wurden in Biotransformationsexperimenten mit Phenylalkanen Metaboliten detektiert, die nicht nur aus terminalen und ß Oxidationsreaktionen an der Alkylseitenkette hervorgegangen sind, sondern auch subterminalen und am Ring stattfindenden Reaktionen zugeschrieben werden konnten. Das Metabolitenspektrum, das in den Untersuchungen mit Phenylalkanen und aromatischen Säuren ermittelt wurde, glich im Allgemeinen dem von T. asahii. Filamentöse Pilze 1. Mit Ausnahme von Penicillium chrysogenum zeigten alle Stämme der getesteten filamentösen Pilze die Fähigkeit zum Wachstum mit Phenyldodecan. Eine besonders starke Zunahme der Biomasse war dabei mit Sporothrix nivea SBUG M 35 und Umbelopsis isabellina SBUG M 1145 zu verzeichnen. Phenylalkane mit kürzeren Alkylseitenketten konnten von den meisten der untersuchten Pilze kaum bzw. nicht als Wachstumssubstrate genutzt werden. 2. In Biotransformationsexperimenten mit C. elegans, M. hiemalis und U. isabellina konnten 5 neuartige Metaboliten identifiziert werden: Zimtaldehyd, Zimtalkohol, Phenylpropanol und Benzylalkohol (deren Bildung wird auf reduktive Reaktionen der entsprechenden Carbonsäuren zurückgeführt) sowie ein Glycinamid der Zimtsäure, das eine Art Konjugat darstellt. 4. Während der Inkubation der filamentösen Pilze Sp. nivea SBUG-M 25 und SBUG M 242 sowie C. elegans und U. isabellina mit Phenylheptan wurde – analog zu Versuchen mit T. asahii - auch 4 Hydroxycumarin als Metabolit nachgewiesen.
Abomasal emptying rate of diarrhoeic and healthy suckling calves fed with oral rehydration solutions

Abstract
The aim of the study was to determine the abomasal emptying rate (AER) of calves suffering from naturally occurring diarrhoea compared with that of healthy calves. Furthermore, the effects of an oral rehydration solution (ORS) mixed into milk replacer on the AER were determined. Acetaminophen absorption test (APAT) was performed to estimate the AER. Sixty Holstein–Frisian calves (age < 14 days) were included in the study and divided into groups as follows: healthy calves (H; n = 16), healthy calves fed with ORS (HORS; n = 14), diarrhoeic calves (D; n = 15) and diarrhoeic calves fed with ORS (DORS; n = 15). For the APAT, the calves were fed 2 L of milk replacer containing 50 mg acetaminophen (AP)/kg body weight. Venous blood samples were collected before and after milk replacer and AP intake in 30–60 min intervals for 12 hr. During the APAT, no significant differences in median maximum acetaminophen concentration (Cmax) were observed among all groups. Time to reach maximum acetaminophen concentration (Tmax) in DORS (median 390 min, 25/75 quartiles: 300/480 min) was significantly higher compared with that in H (median: 270 min 25/75 quartiles: 210/315 min) and HORS (median: 300 min (25/75 quartiles: 240/360 min). Non‐linear regression revealed that the calculated abomasal half‐life (AP t1/2) tended to be delayed in DORS (median: 652 min, 25/75 quartiles: 445/795 min, p = .10). The area under the AP curve values (AUC) from 0 to 120 min and 0 to 240 min of the observation period were significantly higher in H than D and DORS. In conclusion, significant differences in the AER indices reflected delayed abomasal emptying in diarrhoeic calves. Furthermore, the hypertonic ORS tended to have an additive delaying impact on the AER, which needs attention for the feeding management of diarrhoeic calves.
The potential of several ion-sensitive electrodes responds to the incorporated cations and anions. This has led some authors to misinterpret the potential of metal salt membrane electrodes and of electrodes of the second kind. Neglecting the kinetics of potential establishment and interpreting the potentials solely based on thermodynamics produce completely irrelevant data and suggest that ion concentrations down to 10−45 mol L−1 are accessible by simple potentiometric measurements. The switching from cation to anion response mechanism cannot be derived from thermodynamic equations. It bears complete similarity to the switching of response in the case of foreign interfering ions.
Microbial cell factories have been largely exploited for the controlled production of recombinant proteins, including industrial enzymes and biopharmaceuticals. The advent of high-throughput ‘-omics’ techniques have boosted the design of these production systems due to their valuable contribution to the field of systems metabolic engineering, a discipline integrating metabolic engineering with systems and synthetic biology. In order to thrive, the field of systems metabolic engineering needs absolute proteomics data to be generated, as proteins are the central players in the complex metabolic and adaptational networks. Due to advent of mass spectrometry-based proteomics, a substantial amount of absolute proteomic data became available in the past decade. However, membrane proteins remained inaccessible to these efforts.
Nonetheless, comparative studies targeting the membrane proteome have been quite successful in characterizing physiological processes. Hence, label-free proteomics was used in a study (Quesada-Ganuza et al, 2019 – Article I) to identify and optimize PrsA in Bacillus subtilis, for improved yield of amylase. Amylase is one of the most relevant enzymes in the biotechnological sector. By employing a label-free mass spectrometry approach targeting the membrane proteome of this bacterium, relative changes in heterologous and native levels of PrsA could be quantified. The results of this study evidenced that each PrsA shows different relative abundancies, but with no relevant impact in the yield of amylase.
Even though relative protein quantification can already provide a good visualization of the physiological changes occurring between different conditions, they are not sufficient to understand how resources are allocated in the cell under certain physiological conditions. Therefore, a global method for absolute membrane protein quantification remains the biggest requirement for systems metabolic engineering.
Hence, with this work, we successfully developed a mass spectrometry-based approach enabling the absolute quantification of membrane proteins (Antelo-Varela et al, 2019 – Article II). This study was also performed in the Gram-positive model organism Bacillus subtilis, regarded as a prolific microbial cell factory. The method developed in this work combines the comprehensiveness of shotgun proteomics with the sensitivity and accuracy of targeted mass spectrometry. Fundamental to the method is that it relies on the application of a correction and an enrichment factor to calibrate absolute membrane protein abundances derived from shotgun mass spectrometry. This has permitted, for the first time reported, the calculation of absolute membrane protein abundances in a living organism.
The newly developed approach enabled to accurately quantify ~40% of the predicted proteome of this bacterium, offering a clear visualization of the physiological rearrangements occurring upon the onset of osmotic stress. In addition, this work also provides evidence for new membrane protein stoichiometries.
Overall, this study enabled the development of a straightforward methodology long-needed in the scientific and biotechnological community and, for the first time reported, providing absolute abundances of one of the most puzzling fractions of the cell – the membrane proteome.
The next step of the work summarized here was to implement the afore described method to a biotechnological relevant strain, as absolute membrane protein abundances are essential to understand the fundamental principles of protein secretion and production stress. Hence, this work was applied in a genome-reduced B. subtilis strain, ‘midiBacillus’, expressing the major staphylococcal antigen IsaA (Antelo-Varela et al, submitted – Article III). The employed absolute membrane protein quantification methodology enabled the analysis of physiological rearrangements occurring upon the induction of heterologous protein production. This work showed that, even though IsaA was successfully secreted into the growth medium, one of the main requirements for the biotechnological sector, it was still partly accumulated in the cell membrane of this bacterium. This led to an exacerbated physiological response where membrane proteins involved in the management of secretion stress were activated. In addition, this study also showed that a rearrangement of the cell’s translocation machinery occurs upon induction of production, where a ‘game’ of in- and decrease of transporters takes place.
Anticipating the impact of genetic and environmental insults, such as the ones caused by production stress, is essential for the field of systems metabolic engineering. Thus, the highly accurate and comprehensive dataset generated during this work can be implemented in predictive mathematical models, thereby contributing in the rational design of next-generation secretion systems.
In this work, spatial distributions for reactive stable and transient species that are involved
in the reaction cycle of H2O2, a key species for biomedical applications, were
determined directly in the effluent of a kINPen-sci plasma jet. The small diameter
of cold atmospheric pressure plasma jets and their operation at atmospheric pressure
that causes strong quenching reactions make diagnostics challenging. Here, various diagnostic
techniques have been employed and adapted for the use in the effluent of a
cold atmospheric pressure plasma jet, which were laser atomic absorption spectroscopy
(LAAS) at 811.5 nm for the detection of Ar(3P2), picosecond two-photon absorption
laser-induced fluorescence spectroscopy (ps-TALIF) at 225 nm and 205 nm for the
detection of O and H atoms, respectively, and continuous wave cavity ring-down spectroscopy
(cw-CRDS) at 1.506 µm for the detection of HO2, and cw-CRDS at 8000 µm
for the detection of H2O2. All these methods provide absolute number densities. In
this work, spatial distributions within the small diameter of the effluent of a CAPJ
were obtained, which have not been reported so far literature. In order to overcome the
line-of-sight limitations of CRDS, radial scans were performed and transformed into a
spatial distribution by using Abel inversion.
Based on the determined spatial density distributions for H atoms, O atoms, HO2
radicals, and H2O2 molecules, together with the investigated impact of humidity in the
feed gas on the excitation dynamics and the production of Ar(3P2), and finally on a
comparison of the experimental results to a plasma chemical and reacting flow model,
three different zones with varying reaction kinetics were identified. The densities close
to the nozzle of the kINPen-sci plasma jet were dominated by reactions within the
plasma zone including the dissociation of H2O added to the Ar feed gas and O2 that
was presumably transferred into the plasma zone by counter-propagating ionisation
waves. Notably, also the larger molecules, such as HO2 and H2O2 were mainly formed
within the plasma zone of the plasma jet. Between 1.5 mm and 5 mm below the nozzle,
the atomic species and molecular radicals generated in the plasma zone were consumed
by chemical reactions with the surrounding gas, whose composition was controlled by
applying a gas curtain. At further distances from the nozzle, where typically biological
samples are positioned, only H2O2 and HO2 were observed.
With this work, it is successfully demonstrated that even for the small diameters of
cold atmospheric pressure plasma jets the determination of spatial profiles for reactive
transient and stable species is possible within the effluent. By combining the experimental
results, important insights into the formation and consumption of H2O2 and its
precursors were gained, which are essential for the understanding of use of plasmas in
biomedical applications.
Tunable Diode Laser Absorption Spectroscopy in the mid InfraRed spectral range (IR-TDLAS) has been applied to investigate the behaviour of CF, CF2 and C2F4 species produced in pulsed CF4/H2 capacitively coupled radio frequency plasmas (13.56 MHz CCP). This experimental technique was shown to be suitable for temporally resolved measurements of the absolute number density of the target molecules in the studied fluorocarbon discharges. The temporal resolution of about 20…40 ms typically achieved in the standard data acquisition mode (“stream mode”) was sufficient for the real-time measurements of CF2 and C2F4, but not of CF whose kinetics was observed to be much faster. Therefore, a more sophisticated approach (“burst mode”) providing a temporal resolution of 0.94 ms was established and successfully applied to CF density measurements. In order to enable the TDLAS measurements of the target species, preliminary investigations on their spectroscopic data had been carried out. In particular, pure C2F4 has been produced in laboratory by means of vacuum thermal decomposition (pyrolysis) of polytetrafluoroethylene and used as a reference gas. Therefore, an absorption structure consisting of several overlapping C2F4 lines around 1337.11 cm-1 was selected and carefully calibrated, which provided the first absolute measurements of the species by means of the applied experimental technique. The absolute number density traces measured for CF, CF2 and C2F4 in the studied pulsed plasmas were then analysed, in which two differential balance equations were proposed for each of the species to describe their behaviour during both “plasma on” and “plasma off” phases. Analytical solutions of the balance equations were used to fit the experimental data and hence to deduce important information on the kinetics of the studied molecules. In particular, during the “plasma off” phase, the self-recombination of CF2 (CF2 + CF2 (+M) → C2F4 (+M)) was found to be dominant in the kinetics of the radical, but of minor importance for C2F4 production. A rapid consumption of CF observed within 7…25 ms after switching off the plasma was explained mainly by volume reaction with other species (most likely with CF3), whereas diffusion of the radical towards the reactor walls followed by sticking on the surfaces was found to contribute only at relatively low pressures (<10 Pa). Under certain discharge conditions, measured CF density traces exhibited significant overshoots in 50…150 ms after the plasma ignition, which had not been known from literature before. The electron impact fragmentation of C2F4 was shown to be essential for CF production at the beginning of the “plasma on” phase and therefore for formation of the observed CF density overshoots. Finally, the broad band FTIR spectroscopy was applied in order to better characterize the gas phase composition of the studied plasmas. Thus, absorption bands of CF4, C2F4, C2F6, C3F8, CHF3 and HF stable molecules were detected in the FTIR spectra recorded between 400 and 4000 cm-1. The spectra were then successfully deconvolved and the absolute concentration of the detected species was estimated. In particular, the absolute number density of C2F4 obtained from the FTIR measurements was in a good agreement with that achieved by means of the IR-TDLAS technique. Background: Robotic-assisted laparoscopy (RAL) is being widely accepted in the field of urology as a replacement for conventional laparoscopy (CL). Nevertheless, the process of its integration in clinical routines has been rather spontaneous. Objective: To determine the prevalence of robotic systems (RS) in urological clinics in Germany, Austria and Switzerland, the acceptance of RAL among urologists as a replacement for CL and its current use for 25 different urological indications. Materials and Methods: To elucidate the practice patterns of RAL, a survey at hospitals in Germany, Austria and Switzerland was conducted. All surgically active urology departments in Germany (303), Austria (37) and Switzerland (84) received a questionnaire with questions related to the one-year period prior to the survey. Results: The response rate was 63%. Among the participants, 43% were universities, 45% were tertiary care centres, and 8% were secondary care hospitals. A total of 60 RS (Germany 35, Austria 8, Switzerland 17) were available, and the majority (68%) were operated under public ownership. The perception of RAL and the anticipated superiority of RAL significantly differed between robotic and non-robotic surgeons. For only two urologic indications were more than 50% of the procedures performed using RAL: pyeloplasty (58%) and transperitoneal radical prostatectomy (75%). On average, 35% of robotic surgeons and only 14% of non-robotic surgeons anticipated RAL superiority in some of the 25 indications. Conclusions: This survey provides a detailed insight into RAL implementation in Germany, Austria and Switzerland. RAL is currently limited to a few urological indications with a small number of high-volume robotic centres. These results might suggest that a saturation of clinics using RS has been achieved but that the existing robotic capacities are being utilized ineffectively. The possible reasons for this finding are discussed, and certain strategies to solve these problems are offered.
Background: Uterine leiomyosarcoma (LMS) is a rare entity amongst malignant gynaecological tumours and is mostly diagnosed after surgery for benign leiomyoma (LM) of the uterus. As minimal invasive surgery is widely used, the morcellation of LM and the uterus is rather common. As there is little known about the impact of the morcellation of LMS on local and distant metastases, as well as overall survival, we carried out a large-scale retrospective study. Methods: A total of 301 LMS cases from the German Clinical Competence Centre for Genital Sarcomas and Mixed Tumours were analysed. We distinguished morcellated and non-morcellated LMS from pT1 and >pT1 tumours. Fine–Gray competing risks regressions and cumulative incidence rates were computed for the time to local recurrence, distant metastases, and patient death. Results: The recurrence free interval in pT1 LMS was significantly lower in the morcellation group with a 2-year cumulative incidence rate of 49% vs. 26% in non-morcellated LMS (p = 0.001). No differences were seen in >pT1 tumours. Distant metastases were more frequently found in non-morcellated pT1 LMS compared to the morcellated cases (5-year cumulative incidence: 54% vs. 29%, p < 0.001). There was no significant difference in time to death between both groups neither in the pT1 stages nor in >pT1 disease. Subdistribution hazard ratios estimated by multivariable competing risks regressions for the morcellation of pT1 LMS were 2.11 for local recurrence (95% CI 1.41–3.16, p < 0.001) and 0.52 for distant metastases (95% CI 0.32–0.84, p = 0.008). Conclusions: Tumour morcellation is not associated with OS for pT1 tumours. The morcellation of pT1 LMS seems to prolong the time to distant metastases whereas local recurrence is more likely to occur after the morcellation of pT1 LMS.
Abstract
Background
Heparin induced thrombocytopenia (HIT) is likely a misdirected bacterial host defense mechanism. Platelet factor 4 (PF4) binds to polyanions on bacterial surfaces exposing neo‐epitopes to which HIT antibodies bind. Platelets are activated by the resulting immune complexes via FcγRIIA, release bactericidal substances, and kill Gram‐negative Escherichia coli.
Objectives
To assess the role of PF4, anti‐PF4/H antibodies and FcγRIIa in killing of Gram‐positive bacteria by platelets.
Methods
Binding of PF4 to protein‐A deficient Staphylococcus aureus (SA113Δspa) and non‐encapsulated Streptococcus pneumoniae (D39Δcps) and its conformational change were assessed by flow cytometry using monoclonal (KKO,5B9) and patient derived anti‐PF4/H antibodies. Killing of bacteria was quantified by counting colony forming units (cfu) after incubation with platelets or platelet releasate. Using flow cytometry, platelet activation (CD62P‐expression, PAC‐1 binding) and phosphatidylserine (PS)‐exposure were analyzed.
Results
Monoclonal and patient‐derived anti‐PF4/H antibodies bound in the presence of PF4 to both S. aureus and S. pneumoniae (1.6‐fold increased fluorescence signal for human anti‐PF4/H antibodies to 24.0‐fold increase for KKO). Staphylococcus aureus (5.5 × 104cfu/mL) was efficiently killed by platelets (2.7 × 104cfu/mL) or their releasate (2.9 × 104cfu/mL). Killing was not further enhanced by PF4 or anti‐PF4/H antibodies. Blocking FcγRIIa had no impact on killing of S. aureus by platelets. In contrast, S. pneumoniae was not killed by platelets or releasate. Instead, after incubation with pneumococci platelets were unresponsive to TRAP‐6 stimulation and exposed high levels of PS.
Conclusions
Anti‐PF4/H antibodies seem to have only a minor role for direct killing of Gram‐positive bacteria by platelets. Staphylococcus aureus is killed by platelets or platelet releasate. In contrast, S. pneumoniae affects platelet viability.
Objective: In acute pancreatitis (AP), bacterial translocation and subsequent infection of pancreatic necrosis are the main risk factors for severe disease and late death. Understanding how immunological host defence mechanisms fail to protect the intestinal barrier is of great importance in reducing the mortality risk of the disease. Here, we studied the role of the Treg/Th17 balance for maintaining the intestinal barrier function in a mouse model of severe AP.
Design: AP was induced by partial duct ligation in C57Bl/6 or DEREG mice, in which regulatory T-cells (Treg) were depleted by intraperitoneal injection of diphtheria toxin. By flow cytometry, functional suppression assays and transcriptional profiling we analysed Treg activation and characterised T-cells of the lamina propria as well as intraepithelial lymphocytes (IELs) regarding their activation and differentiation. Microbiota composition was examined in intestinal samples as well as in murine and human pancreatic necrosis by 16S rRNA gene sequencing.
Results: The prophylactic Treg-depletion enhanced the proinflammatory response in an experimental mouse model of AP but stabilised the intestinal immunological barrier function of Th17 cells and CD8+/γδTCR+ IELs. Treg depleted animals developed less bacterial translocation to the pancreas. Duodenal overgrowth of the facultative pathogenic taxa Escherichia/Shigella which associates with severe disease and infected necrosis was diminished in Treg depleted animals.
Conclusion: Tregs play a crucial role in the counterbalance against systemic inflammatory response syndrome. In AP, Treg-activation disturbs the duodenal barrier function and permits translocation of commensal bacteria into pancreatic necrosis. Targeting Tregs in AP may help to ameliorate the disease course.

A N‐heterocyclic olefin (NHO), a terminal alkene selectively activates aromatic C−F bonds without the need of any additional catalyst. As a result, a straightforward methodology was developed for the formation of different fluoroaryl‐substituted alkenes in which the central carbon–carbon double bond is in a twisted geometry.
Abstract
Because isoenzymes of the experimentally and therapeutically extremely relevant sirtuin family show high similarity, addressing the unique selectivity pocket of sirtuin 2 is a promising strategy towards selective inhibitors. An unrelated approach towards selective inhibition of isoenzymes with varied tissue distribution is targeted drug delivery or spatiotemporal activation by photochemical activation. Azologization of two nicotinamide‐mimicking lead structures was undertaken to combine both approaches and yielded a set of 33 azobenzenes and azopyridines that have been evaluated for their photochemical behaviour and bioactivity. For some compounds, inhibitory activity reached the sub‐micromolar range in their thermodynamically favoured E form and could be decreased by photoisomerization to the metastable Z form. Besides, derivatization with long‐chain fatty acids yielded potent sirtuin 2 inhibitors, featuring another intriguing aspect of azo‐based photoswitches. In these compounds, switching to the Z isomer increased aqueous solubility and thereby enhanced biological activity by up to a factor of 21. The biological activity of two compounds was confirmed by hyperacetylation of sirtuin specific histone proteins in a cell‐based activity assay.
Indoleamine 2,3-dioxygenase (IDO) and tryptophan 2,3-dioxygenase (TDO2) are the key enzymes of tryptophan (TRP) metabolism in the kynurenine pathway (KP). Both enzymes function as indicators of immunosuppression and poor survival in cancer patients. Direct or indirect targeting of either of these substances seems thus reasonable to improve therapy options for patients. In this study, glioblastoma multiforme (GBM) as well as head and neck squamous cell carcinomas (HNSCC) were examined because of their different mechanisms of spontaneous and treatment-induced immune escape. Effects on gene expression and protein levels were examined. Accompanying assessment of TRP metabolites from treated GBM cell culture supernatants was conducted. Our results show a heterogeneous and inversely correlated expression profile of TRP-metabolizing genes among GBM and HNSCC cells, with low, but inducible IDO1 expression upon IFNγ treatment. TDO2 expression was higher in GBM cells, while genes encoding kynurenine aminotransferases were mainly confined to HNSCC cells. These data indicate that the KP is active in both entities, with however different enzymes involved in TRP catabolism. Upon treatment with Temozolomide, the standard of care for GBM patients, IDO1 was upregulated. Comparable, although less pronounced effects were seen in HNSCC upon Cetuximab and conventional drugs (i.e., 5-fluorouracil, Gemcitabine). Here, IDO1 and additional genes of the KP (KYAT1, KYAT2, and KMO) were induced. Vice versa, the novel yet experimental cyclin-dependent kinase inhibitor Dinaciclib suppressed KP in both entities. Our comprehensive data imply inhibition of the TRP catabolism by Dinaciclib, while conventional chemotherapeutics tend to activate this pathway. These data point to limitations of conventional therapy and highlight the potential of targeted therapies to interfere with the cells' metabolism more than anticipated.
RationaleThe ubiquitin–proteasome system (UPS) is responsible for skeletal muscle atrophy. We showed earlier that the transcription factor EB (TFEB) plays a role by increasing E3 ubiquitin ligase muscle really interesting new gene-finger 1(MuRF1)/tripartite motif-containing 63 (TRIM63) expression. MuRF 1 ubiquitinates structural proteins and mediates their UPS-dependent degradation. We now investigated how TFEB-mediated TRIM63 expression is regulated.
ObjectiveBecause protein kinase D1 (PKD1), histone deacetylase 5 (HDAC5), and TFEB belong to respective families with close structural, regulatory, and functional properties, we hypothesized that these families comprise a network regulating TRIM63 expression.
Methods and ResultsWe found that TFEB and transcription factor for immunoglobulin heavy-chain enhancer 3 (TFE3) activate TRIM63 expression. The class IIa HDACs HDAC4, HDAC5, and HDAC7 inhibited this activity. Furthermore, we could map the HDAC5 and TFE3 physical interaction. PKD1, PKD2, and PKD3 reversed the inhibitory effect of all tested class IIa HDACs toward TFEB and TFE3. PKD1 mediated nuclear export of all HDACs and lifted TFEB and TFE3 repression. We also mapped the PKD2 and HDAC5 interaction. We found that the inhibitory effect of PKD1 and PKD2 toward HDAC4, HDAC5, and HDAC7 was mediated by their phosphorylation and 14-3-3 mediated nuclear export.
ConclusionTFEB and TFE3 activate TRIM63 expression. Both transcription factors are controlled by HDAC4, HDAC5, HDAC7, and all PKD-family members. We propose that the multilevel PKD/HDAC/TFEB/TFE3 network tightly controls TRIM63 expression.
Turbulence is a state of a physical system characterized by a high degree of spatiotemporal disorder. Turbulent processes are driven by instabilities exhibiting complex nonlinear dynamics, which span over several spatial as well as temporal scales. Apart from fluids and gases, turbulence is observed in plasmas. While turbulent mixing of a system is sometimes a desired effect, often turbulence is an undesired state. In hot, magnetically confined plasmas, envisaged for energy generation by thermonuclear fusion, plasma turbulence is clearly a problem, since the magnetic confinement time is drastically deteriorated by turbulent transport. Hence, a control mechanism to influence and to suppress turbulence is of significance for future fusion power devices. An important area of plasma turbulence is drift wave turbulence. Drift waves are characterized by currents parallel to the ambient magnetic field, that are tightly coupled to a coherent mode structure rotating in the perpendicular plane. In the present work, the control of drift waves and drift wave turbulence is experimentally investigated in the linear magnetized helicon experiment VINETA. Two different open-loop control systems - electrostatic and electromagnetic - are used to drive dynamically parallel currents. It is observed that the dynamics of the drift waves can be significantly influenced by both control schemes. If the imposed mode number as well as the rotation direction match those of the drift waves, classical synchronization effects like, e.g., frequency locking, frequency pulling, and Arnold tongues are observed. These confirm the nonlinear interaction between the control signal and the drift wave dynamics. Finally, the broadband drift wave turbulence, and thereby turbulent transport, is considerably reduced if the applied control signal is sufficiently large in amplitude.
Active Middle Ear Implant Evoked Auditory Brainstem Response Intensity-Latency Characteristics

Objective
To analyze intensity-latency functions of intraoperative auditory evoked brainstem responses (ABRs) to stimulation by the Vibrant Soundbridge (VSB) active middle ear implant with respect to coupling efficiency, VSB evoked ABR thresholds, and coupling modality [oval window (OW) placement vs. Incus placement and vs. round window (RW) placement].

Bi-centric study at tertiary referral centers.
Patients
Twenty-four patients (10 female, 14 male, mean age: 58 years) who received a VSB.
Outcome Measures
Wave-V intensity-latency functions of intraoperative VSB evoked ABRs using a modified audio processor programmed to preoperative bone conduction thresholds for stimulation. Threshold level correction to coupling efficiency and ABR thresholds. Individual plots and exponential function fits.
Results
After ABR threshold level correction, the latency functions could be aligned. A large variance of latencies was observed at individual threshold level. Wave-V latency was longest in the Incus placement subgroup (9.73 ms, SD: 1.04) as compared to OW placement subgroup (9.47 ms, SD: 1.05), with the shortest latency in the RW placement subgroup (8.99 ms, SD: 0.68). For increasing stimulation levels, the variance decreased with intensity-latency function slopes converging toward a steady-state (saturation) latency caused by saturation of audio processor (stimulation) gain. Latency saturation was reached at a stimulation level of 50 dB nHL for the OW placement subgroup, 35 dB nHL for the Incus placement subgroup, and 30 dB nHL for the RW placement subgroup. The latency and saturation results indicated decreased dynamic range for RW placement, i.e., reverse stimulation.
Conclusions
VSB evoked ABR wave-V intensity-latency function slopes were similar to acoustic stimulation at high stimulation levels with a shift toward longer latencies caused by audio processor signal delay. Saturation of latencies occurred for higher stimulation levels due to saturation of audio processor gain. Thus, the analysis of VSB evoked intensity-latency functions appears to allow for the objective assessment of a patient's individual dynamic range. This can further improve diagnostics as well as intraoperative and postoperative quality control.
Abstract
Amine transaminases (ATAs) are biocatalysts for the synthesis of chiral amines and can be identified in sequence databases by specific sequence motifs. This study shows that the activity level towards the model substrate 1‐phenylethylamine can be predicted solely from the sequence. To demonstrate this, 15 putative ATAs with a different distribution of hydrophobic or hydrophilic amino acid side chains near the active site were characterized. Hydrophobic side chains were associated with a high activity level and were a better predictor of activity than global sequence identity to known ATAs with high or low activities. Enzyme stability investigations revealed that four out of the 15 ATAs showed a good operational stability.
Genetic variants in α-actinin-2 (ACTN2) are associated with several forms of (cardio)myopathy. We previously reported a heterozygous missense (c.740C>T) ACTN2 gene variant, associated with hypertrophic cardiomyopathy, and characterized by an electro-mechanical phenotype in human induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs). Here, we created with CRISPR/Cas9 genetic tools two heterozygous functional knock-out hiPSC lines with a second wild-type (ACTN2wt) and missense ACTN2 (ACTN2mut) allele, respectively. We evaluated their impact on cardiomyocyte structure and function, using a combination of different technologies, including immunofluorescence and live cell imaging, RNA-seq, and mass spectrometry. This study showed that ACTN2mut presents a higher percentage of multinucleation, protein aggregation, hypertrophy, myofibrillar disarray, and activation of both the ubiquitin-proteasome system and the autophagy-lysosomal pathway as compared to ACTN2wt in 2D-cultured hiPSC-CMs. Furthermore, the expression of ACTN2mut was associated with a marked reduction of sarcomere-associated protein levels in 2D-cultured hiPSC-CMs and force impairment in engineered heart tissues. In conclusion, our study highlights the activation of proteolytic systems in ACTN2mut hiPSC-CMs likely to cope with ACTN2 aggregation and therefore directs towards proteopathy as an additional cellular pathology caused by this ACTN2 variant, which may contribute to human ACTN2-associated cardiomyopathies.
Despite a plethora of therapeutic approaches, the injection of local anaesthetics itself remains one of the most painful and dreadful procedures among children. Stimulation of acupoint LI4 is associated with analgesic effects in dentistry. Goal of the study To investigate whether stimulation of LI4, added to standard therapy (ST), reduces pain and distress during injection of local anaesthetic (LA) in comparison with ST alone. Materials and Methods Children, scheduled for dental treatment in local anaesthesia on 2 separate days were enrolled in this trial, approved by local ethics commission. On one day each child received bilateral acupuncture of LI4 point, using indwelling fixed “New Pyonex” needles (0.2 x 1.5 mm; Seirin, Japan). The parents of the children were asked to stimulate the needles by massage. Standardized injection of LA was performed 5 min following acupuncture. The needles were withdrawn at the end of dental treatment. On the other day of treatment children received LA injection without acupuncture. The order of treatment days (acupuncture first or vice versa) was randomised. Primary endpoint was the pain intensity during LA injection reported by children on Visual Rating Scale from 0=no pain to 10=maximal pain imaginable (VRS-11). Secondary endpoints were parent- and dentist-assessed pain intensity (measured on Numeric Rating Scale 1-10), patients’ heart rate before and during dental treatment and satisfaction with received therapy (measured on Numerical Rating Scale 1-5.) Side effects of LI4 stimulation were also recorded. Results and Discussion The data of 49 children (22 females; age 10 ± 4 yrs; mean ± SD), who completed both visits, were analysed. Children reported less pain with than without acupuncture: 2.2 ± 2.5 vs. 3.9 ± 2.7; mean ± SD, p<0.001. Heart rate decreased after LI4 stimulation compared to ST alone throughout the dental treatment (p<0.05). LI4 stimulation was safe and raised better satisfaction with the treatment among children and parents, than ST alone (p<0.05). Other secondary endpoints were comparable between both sessions. Conclusion Stimulation of acupuncture point LI4 reduces pain and autonomous stress during injection of local anaesthetics in paediatric dentistry.
The establishment of a guideline for long-term noninvasive ventilation treatment (LTH-NIV) of acute hypercapnic exacerbations of chronic obstructive pulmonary disease (AECOPD) requiring acute ventilation has proven elusive. Most studies thus far have shown no mortality benefit of long-term noninvasive ventilation treatment. Using retrospective analysis of the data of our patients (n = 143) recruited from 2012 to 2019, we aimed to compare patients discharged with and without long-term noninvasive ventilation. The follow-up results showed no significant difference (p = 0.233) between the groups [LTH-NIV (n = 83); non-NIV (n = 60)] regarding readmission due to clinical worsening. However, the first- and second-year survival rates were 82% and 72%, respectively, in the LTH-NIV group and significantly different (p = 0.023) from 67 and 55% in the non-NIV group. The statistical models showed a significant mortality risk for the non-NIV group, with a hazard ratio (HR) of 2.82 (1.31; 6.03). To the best of our knowledge, this is the first study to demonstrate the mortality benefit of long-term NIV therapy for patients with AECOPD under real-world conditions.
BACKGROUND Acute disseminated encephalomyelitis (ADEM) is a rare, acquired demyelination syndrome that causes cognitive impairment and
focal neurological deficits and may be fatal. The potentially reversible disease mainly affects children, often after vaccination or viral infection, but may
be seen rarely in adults.
OBSERVATIONS A 50-year-old woman presented with loss of visual acuity of the left eye. Magnetic resonance imaging (MRI) revealed an intra- and
suprasellar mass, which was removed successfully. On postoperative day 1, MRI showed gross total resection of the lesion and no surgery-related
complications. On postoperative day 2, the patient presented with a progressive left-sided hemiparesis, hemineglect, and decline of cognitive
performance. MRI showed white matter edema in both hemispheres. Cerebrospinal fluid analysis revealed mixed pleocytosis (355/mL) without further
evidence of infection. In synopsis of the findings, ADEM was diagnosed and treated with intravenous immunoglobulins. Shortly thereafter, the patient
recovered, and no sensorimotor deficits were detected in the follow-up examination.
LESSONS Pituitary gland pathologies are commonly treated by transsphenoidal surgery, with only minor risks for complications. A case of ADEM after
craniopharyngioma resection has not been published before and should be considered in case of progressive neurological deterioration with multiple
white matter lesions.
Summary
The article discusses Adam Naruszewicz‘s famous Ode to Justice (1773) and the engagement of occasional poetry in contemporary discussions about the handling of justice in political trials. Looking at the trial of 1773 the Ode addresses the question of finding a just sentence for the abortive attempt two years earlier to abduct king Stanisław August. The article presents the pertinent aspects for such an analysis in three parts: 1) an introduction to the conceptualization of royal justice in European thought of the Enlightenment, 2) the known facts about the abduction and its historical contexts, 3) an overview of the occasional poetry written by Naruszewicz about the incident from 1771 to 1773 leading to an analysis of the Ode to Justice in regard to the political reasoning of its author.
Adaptation mechanisms within the B cell composition for successful human and murine pregnancies.

Introduction
A well-balanced immune maternal status is essential for favourable outcome of pregnancy. Due to their complexities, not all immune adaptations that promote tolerance during pregnancy are known. To understand the adaptation of the B cell compartment, we analysed and compared B cell lymphopoiesis in different lymphoid tissues in a number of murine models.
Furthermore, we focused on the humoral immune response during pregnancy. We analysed immunoglobulin profiles in human subjects and mice during pregnancy.
These cellular alterations are subject to the influence of chemokines, among others. Therefore, we assessed serum levels of B cell activation factor to clarify its effects during pregnancy.
Methods
For analysis of the human peripheral B cell compartment, peripheral blood samples from age-matched non-pregnant and pregnant women without pregnancy complications, immunological disease or acute/chronic inflammation were collected and sub-classified into four different groups: non-pregnant, and first, second, or third trimester of pregnancy. The experiments, based on a mouse model, were performed with 8-week-old female mice: clinically healthy non-pregnant (CBA/J (H2k)), pregnant mice with normal gestation (BALB/c (H2d) x CBA/J (H2k)), and mice with pregnancy loss (DBA/2J (H2d) x CBA/J (H2k)). Subsequently, peripheral blood mononuclear cells from blood and lymphatic organs were isolated following standard protocols. The B cell analysis was performed by flow cytometry. The immunoglobulin serum levels of the human and murine subgroups were quantitated using Bio-Plex isotyping assay and analysed by a Bio-Plex reader. To quantify B cell activating factor (BAFF) in serum of pregnant and non-pregnant mice a BAFF enzyme-linked immunosorbent assay was used. The concentrations were determined by using a FLUOstar OPTIMA microplate reader. All statistical analyses were performed using the Kruskal–Wallis test with Dunn’s post-test in GraphPad Prism software. P values of < 0.05 were considered statistically significant.
Results
We were able to demonstrate B cell lymphopenia in mice bone marrow downstream of pre-pro B cells, irrespective of pregnancy outcome. The mature bone marrow B cells did not show this adjustment mechanism during normal gestation.
Closer inspection of the splenic tissue revealed expansion and activation of marginal zone B cells in mice with a normal pregnancy. However, this was not observed in mice suffering from pregnancy disturbances. Natural antibodies secreted from marginal zone B cells were also present at higher concentrations in serum of pregnant mice, compared to non-pregnant animals.
We also found significantly higher levels of natural antibodies in serum of pregnant women compared to non-pregnant age-matched controls. Analysis showed significantly lower levels of BAFF in mice with normal pregnancy as compared to non-pregnant mice.
Conclusions
We are able to show mechanisms within the B cell compartment as well as the change within the natural antibodies that might be crucial for successful pregnancy in both humans and mice. Furthermore, BAFF seems to play a central role as a mediator of peripheral B cell compartment and B cell lymphopoiesis in the bone marrow for successful pregnancy.
The toluene-degrading and solvent-tolerant strain Pseudomonas putida DOT-T1E was investigated with respect to its suitability and economic efficiency as biocatalyst in aqueous-organic two-phase systems with aliphatic solvents as organic phase (Rojas et al. 2004, chapter 4 and 5) and to its adaptive responses to the solvent decanol. The adaptive changes on the level of cell morphology (chapter 2), membrane fatty acids and permeability (chapter 3), as well as energetics and surface properties (chapter 5) of P. putida DOT-T1E have been investigated in order to ascertain information about the strain's suitability for two-phase biotransformation systems (chapter 4). The morphological adaptation to the presence of solvents was observable in changes of the cell size of P. putida DOT-T1E. Those changes were dependent on the cellular activity and occurred only after addition of non-lethal solvent concentrations. The cells reacted to the presence of organic solvents by decreasing the ratio between surface and volume of the cells and therefore reducing their relative surfaces (chapter 2). The cell surface and especially the cytoplasmic membrane are the major targets for toxic effects of membrane-active compounds like solvents. The mechanism of the cis-trans isomerisation of unsaturated fatty acids counteracts the fluidizing effect of solvents by increase the ordering of the membrane and therefore its rigidity. By comparing the responses of the cells to a series of stress factors (like solvents), a direct correlation between the activation of this mechanism and the well investigated K+-uptake pumps was observed (chapter 3). Huertas et al. (1998) reported that this strain tolerated concentrations of heptane, propylbenzene, octanol, and toluene of at least 10 % (vol/vol). 1-decanol is, in comparison to toluene, less hazardous and volatile, and it possesses good extraction properties for the desired fine chemical products. In further investigations of possible biotechnological processes, it was discovered that decanol is also a more suitable solvent as organic phase (chapter 4). Although the cells of P. putida DOT-T1E needed additional energy for their adaptation to the presence of the solvent decanol, they were able to maintain or activate their electron transport phosphorylation allowing homeostasis of ATP level and energy charge in the presence of the solvent, at the price of a reduced growth yield. On the other hand, significantly enhanced cell hydrophobicities converging with more negative cell surface charges were observed in cells grown in the presence of 1-decanol (chapter 5). It is however important to note that all the cell’s properties observed are closely linked to each other since they are all part of the adaptive response of the cells. It can be concluded that the easy adaptability and good growth properties of Pseudomonas putida DOT-T1E in the presence of the organic solvent 1-decanol make this system an excellent candidate for two-phase fermentation processes. Moreover, the absence of differences in the energetics of the bacteria during exposure to 1-decanol as compared to bacteria that grew in the absence of 1-decanol, support that this organism can be used for the industrial production of fine chemicals in an economically sound manner.
Dementia is a leading cause of disability and dependency in older people worldwide. As the number of people affected increases, so does the need for innovative care models. Dementia care management (DCM) is an empirically validated approach for improving the care and quality of life for people with dementia (PwD) and caregivers. The aim of this study is to investigate the influencing factors and critical pathways for the implementation of a regionally adapted DCM standard in the existing primary care structures in the German region of Siegen-Wittgenstein (SW). Utilizing participatory research methods, five local health care experts as co-researchers conducted N = 13 semi-structured interviews with 22 local professionals and one caregiver as peer reviewers. Data collection and analysis were based on the Consolidated Framework for Implementation Research (CFIR). Our results show that among the most mentioned influencing factors, three CFIR constructs can be identified as both barriers and facilitators: Patients’ needs and resources, Relative advantage, and Cosmopolitanism. The insufficient involvement of relevant stakeholders is the major barrier and the comprehensive consideration of patient needs through dementia care managers is the strongest facilitating factor. The study underlines the vital role of barrier analysis in site-specific DCM implementation.
Objective
To identify and summarise evaluated interventions aiming to improve the communication of palliative care (PC) and end-of-life (EoL) issues in physicians caring for cancer patients. Such interventions are needed with regard to the aim of an earlier communication of those issues in oncology daily practice, which is associated with a range of benefits for patients and caregivers but is often impeded by physicians’ communication insecurities.
Design
Systematic review based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.Data sourcesRelevant publications were systematically searched in MEDLINE, PsycINFO, CINAHL and Web of Science databases in September 2020 with an update in July 2021.
Eligibility criteria
We included publications reporting a quantitative evaluation of a communication intervention on one or more PC/EoL issues with a communication-related main outcome. Target group had to be physicians caring for cancer patients non-specialist in PC.
Data extraction and synthesis
Two independent raters extracted intervention characteristics, publication characteristics and publication quality. Results were narratively synthesised.
Results
24 publications reporting 22 interventions were included. 13 publications reported randomised controlled trials. A majority of the interventions addressed one specific PC/EoL issue, most often breaking bad news. Teaching strategies mostly involved role-plays. Target group were mainly oncologists. In addition to self-reported outcome measurements for evaluation, most publications also reported the use of external rating data. All but one publication reported significant intervention effects on at least one outcome parameter. Publication quality was overall moderate.
Conclusions
The empirically tested communication interventions on PC/EoL issues seem to effectively improve physicians’ communication. Future interventions should focus on other issues than breaking bad news, such as preparing for the future. Target group should also be organ-specific oncologists, as all primary caring physicians are responsible for timely communication. Our risk-of-bias assessment revealed some weaknesses, indicating that more high-quality studies for evaluation are needed.

Rewetting is the most effective way to reduce greenhouse gas (GHG) emissions from drained peatlands and must significantly contribute to the implementation of the Paris Agreement on Climate within the land sector. In 2010–2013, more than 73 thousand hectares of fire-prone peatlands were rewetted in the Moscow Region (the hitherto largest rewetting program in the Northern Hemisphere). As the Russian Federation has no national accounting of rewetted areas yet, this paper presents an approach to detect them based on multispectral satellite data verified by ground truthing. We propose that effectively rewetted areas should minimally include areas with wet grasslands and those covered with water (cf. the IPCC categories “rewetted organic soils” and “flooded lands”). In 2020, these lands amounted in Moscow Region to more than 5.3 and 3.6 thousand hectares, respectively. Assuming that most rewetted areas were former peat extraction sites and using IPCC default GHG emission factors, an overall GHG emission reduction of over 36,000 tCO2-eq year−1 was calculated. We furthermore considered the uncertainty of calculations. With the example of a 1535 ha large rewetted peatland, we illustrate the estimation of GHG emission reductions for the period up to 2050. The approach presented can be used to estimate GHG emission reductions by peatland rewetting on the national, regional, and object level.
Abstract
Background
A growing proportion of older people in Germany receive long‐term care from informal and professional caregivers at home. Their personal assessment of the individual care situation is scarcely considered.
Aim
This study aimed to explore the subjective views of care recipients, informal and professional caregivers on the adequacy of care provision in long‐term home care arrangements.
Design and Methods
Qualitative semi‐structured face‐to‐face interviews were conducted with ten care recipients, ten professional caregivers and eight informal caregivers to capture their perspectives on the adequacy of the care received and delivered. Qualitative content analysis was applied using MAXQDA software.
Results
All groups highlighted that they perceive an underprovision of care, even though their explanations differed. The underprovision was mainly described regarding the quality rather than quantity of services. It occurs especially in interpersonal relationships and social inclusion, where the gap between the self‐perceived current situation and the desires of those affected is most prominent. The ambivalent impact of home care on social participation becomes apparent. Perceptions of an overprovision of care range from the view that it appears mainly with respect to informal care to the statement that it is currently non‐existent or generally impossible. Misprovision of care is experienced as serious whenever the interviewees face the challenge of preserving existing abilities or regaining certain skills.
Die zunehmende Prävalenz von Übergewicht und Adipositas ist mit einer Reihe von Begleiterkrankungen und sozioökonomischen Problemen verbunden. Daher besteht die Notwendigkeit die Folgen und Interaktionen dieser Entwicklung zu erforschen. Neben einer Reihe von Wechselwirkungen zwischen Adipositas und entzündlichen Erkrankungen, konnten aktuelle Studien auch Zusammenhänge mit Parodontalerkrankungen nachweisen. Grundlage dieser Arbeit ist die epidemiologische Longitudinalstudie Study of Health in Pomerania. Die Analyse und der Vergleich der Daten von SHIP 0 und SHIP 1 konnten Wechselwirkung zwischen Adipositas und Parodontitis nachweisen. Ziel dieser Arbeit ist es, die Tendenzen und Entwicklungen der Interaktionen zwischen den zentralen Parametern zu untersuchen. Es konnte gezeigt werden, dass ansteigende BMI- und WHR-Werte mit hohen Leukozyten, Fibrinogen und hs-CRP-Konzentrationen verbunden sind. Zu dem konnten mit steigender Konzentration der Akute-Phase-Proteine (hs-CRP, Fibrinogen) eine verstärkte Progression des Attachmentverlustes registriert werden. Es wurde keine deutliche Dosis-Wirkungsbeziehung zwischen der Progression parodontaler Erkrankungen (dargestellt durch Attachmentverlust, Plaque- und Blutungswerte) und erhöhtem BMI bzw. WHR festgestellt. Lediglich bei Probanden mit sehr starker Ausprägung der Parodontitis wiesen erhöhte BMI-Werte einen negativen Einfluss auf das Attachment auf. Zusätzlich ist der Einfluss sehr hoher BMI- bzw. WHR-Werte auf chronische Parodontalerkrankungen deutlich stärker als der Einfluss von leicht erhöhten Werten. Darüber hinaus zeigte sich, dass mit zunehmendem BMI das Zahnverlustrisiko signifikant steigt. Die Entzündungsmediatoren modifizieren dabei die Adipositas-Parodontitis-Beziehung. Hohe Konzentrationen von hs-CRP, Fibrinogen und Leukozyten scheinen ein wichtiges Bindeglied in den Wechselwirkungen von Fettleibigkeit und chronischen Zahnbetterkrankungen zu sein. Zusammenfassend konnte kein Beweis für Adipositas als unabhängigen Risikofaktor für Parodontitis erbracht werden. Die Ergebnisse zeigen jedoch deutlich, dass Interaktionen zwischen den Parametern bestehen, und diese durch proinflammatorische Entzündungsmediatoren modifiziert werden.
Ziel:
Diese populationsbasierte Studie untersucht sowohl die Prävalenz adipöser Schwangerer und deren Geburtsoutcome als auch den Einfluss der Adipositas auf das Outcome des Neugeborenen. Sie beschreibt die Bedeutung der Gewichtsentwicklung in der Schwangerschaft für Mutter und Kind.
Material/Methode:
Insgesamt wurden n=4593 Mütter und ihre Kinder in der populationsbasierten SNiP Studie, Survey of Neonates in Pomerania untersucht. Die Datenerhebung erfolgte im Zeitraum von März 2003 bis November 2008 in Universitäts- und Kreiskrankenhäusern im Nordosten von Mecklenburg-Vorpommern nach standardisierten Fragebögen, Erhebung von Laborparametern und klinischer Dokumentationen. Zur Beurteilung der Schwangerschaftskomplikationen und des Schwangerschaftsausganges wurden die Schwangeren in einzelne BMI Gruppen eingeteilt. Die individuelle Gewichtzunahme wurde ermittelt. Als Outcomeparameter wurden prä- und perinatale Erkrankungen, Pathologien und Risiken bei der Schwangeren und dem Neugeborenen ermittelt und ausgewertet. Hierbei wurden auch sozioökonomischen Faktoren erhoben und ausgewertet.
Ergebnis:
Adipositas ist eine Volkskrankheit. Die mit dieser Volkskrankheit im Zusammenhang stehenden gesundheitlichen Risiken treten nicht nur im Alter auf, sondern es entstehen auch zunehmend Gefährdungen junger Menschen. Hier sind besonders schwangere Frauen mit ihren Neugeborenen betroffen. Mehr als ¼ der schwangeren Frauen im Studiengebiet OVP sind präadipös (BMI 25-29,9) oder adipös (BMI ≥ 30).
Adipöse Schwangere finden sich dem weltweiten Trend entsprechend in der unteren sozialen Bevölkerungsschicht.
Eine Adipositas der Mutter beinhaltet Risiken für Mutter und Kind. Das Risiko einer adipösen Mutter (BMI ≥ 30) an einem Gestationsdiabetes zu erkranken gegenüber einer normalgewichtigen Mutter (BMI 19-24,9) steigt auf das 4,5fache. Das Risiko, eine Gestose auszubilden, steigt auf das 3fache.
Das Risiko des Auftretens mehr als einer Schwangerschaftskomplikation verdoppelt sich bei adipösen Müttern gegenüber normalgewichtigen Müttern. Dies kann unter der Geburt zu höheren Komplikationsraten führen. Häufiger ist bei adipösen Müttern eine primäre oder sekundäre Sectio indiziert. Für die Mütter beinhaltet eine Sectio die allgemeinen Risiken einer Operation (Thrombose/Embolie- Risiko, Blutungen, Wundinfektionen und Bildungen von Verwachsungen) bis hin zur Unfruchtbarkeit.
Zusätzlich stellt sich negativ heraus, dass die Fruchtwasserqualität mit zunehmender Adipositas schlechter wird. Es zeigt sich häufiger Mekonium im Fruchtwasser als Ausdruck einer Hypoxie mit Hyperperistaltik des kindlichen Darms. Durch die Sauerstoffunterernährung kann es vor oder während der Geburt zur Meconiumaspiration kommen, die Obstruktionen und chemische Schädigungen der Lunge verursachen können.
Bei der Betrachtung der Ergebnisse stellt sich allerdings immer wieder heraus, dass die Gewichtszunahme während der Schwangerschaft bei der Beurteilung des Geburtsoutcomes eine untergeordnete Rolle spielt. Allenfalls ist sie ein Parameter zur Abschätzung der Körpermaße des Kindes.
Der BMI eignet sich besser zur Abwägung von Risiken für Mutter und Kind.

Dennoch sollten untergewichtige Frauen auch nicht außer Acht gelassen werden.
Untergewichtige Frauen haben häufiger Fehlgeburten oder gebären häufiger Frühgeborene mit fehlenden Reifezeichen.
Schlussfolgerung:
Schwangerschaften von adipösen Schwangeren sind mit deutlich erhöhten prä- und perinatalen Schwangerschaftsrisiken für Mutter und Kind behaftet. Daher ist es sinnvoll, schon frühzeitig (am besten vor der Schwangerschaft bei Kinderwunsch) die werdende Mutter bei bestehender Disposition über die Folgen einer Adipositas aufzuklären und durch geeignete Maßnahmen (Diätberatung, Ernährungsumstellung) der Adipositas entgegen zu wirken. Die Beratung sollte jedoch nicht mit der Geburt des Kindes enden. So sollte nach der Geburt das Stillen des Kindes für einen gewissen Zeitraum empfohlen werden, um das spätere Adipositasrisiko des Neugeborenen zu verringern.
Der soziale Status spielt bei der Bekämpfung der Adipositas eine besondere Rolle.
Bildung kann zu einem großen Teil das Auftreten von Adipositas vermeiden.
Adjuvante Poetry-Therapie in der Psychiatrie : eine Analyse von 50 Poetry-Gruppentherapiesitzungen

Problem: In dieser Doktorarbeit werden 50 protokollierte Gruppentherapiestunden in der Psychiatrie aus den Jahren 1985 bis 1987 von SCHRÖDER (unveröff.), in denen Poetry-Therapie (PT) angewendet wird, in mehreren Ebenen analysiert. Methode: Die ersten 25 PT-Gruppenstunden werden in der geschlossenen Psychiatrie, die letzten 25 Stunden in der offenen Psychiatrie durchgeführt. Nach dem reihum Vorlesen eines vom Therapeutenteam ausgewählten dichterischen Textes folgt ein teils freies, teils vom Gruppentherapeuten strukturiertes Gruppengespräch über den zuvor gelesen Text. Währenddessen protokolliert der Gruppentherapeut anfangs schematisch, in späteren Stunden detailliert das PT-Gruppengespräch. Der PT-Gruppenstunde schließt sich eine ca. 15minütige Therapeutenteambesprechung ohne Anwesenheit der Patienten an. In den Gesprächsprotokollen lässt sich ein regelmäßiger PT-Gruppengesprächsverlauf beobachten, der in dieser Arbeit zu der Entwicklung einer Fünf-Phasen-Regel führt: Phase 1: Interpretation des Textes, Phase 2: Bewertung des Textes oder des Protagonisten, Phase 3: Einbringen von eigenen Erlebnissen in Bezug auf Inhalt des Textes, Phase 4: Gruppengespräch löst sich vom Text, Phase 5: Austausch von Erfahrungen, Gefühlen, Meinungen, Erinnerungen, Wünschen und Hoffnungen unabhängig vom Text. Des Weiteren ist bei der Bearbeitung der Gesprächsprotokolle eine Klassifizierung der ausgewählten Texte entstanden: in sehr gute, gute, mittelmäßige PT-Literatur. Die Einteilung erfolgt anhand von 10 Bewertungskriterien. Abschließend wird mithilfe der statistischen Tests von Kendalls Tau und Goodman und Kruskals Gamma überprüft, ob eine Korrelation zwischen der Literaturklassifizierung und der Fünf-Phasen-Regel existiert. Ergebnis: Es werden die einzelnen PT-Texte, die Gruppentherapieverläufe, die anschließende Teambesprechung sowie tabellarisch das Erreichen der verschiedenen Gesprächsphasen mit Gesprächsauszügen dargestellt. 75% der sehr gut geeigneten Literatur, 64% der gut geeigneten und 45% der mittelmäßig geeigneten Literatur führen zu einer guten Entwicklung des Gesprächs (Gesprächsphasen 4 und 5). 25% der sehr guten Literatur, 36% der guten und 54% der mittelmäßig geeigneten Literatur führen zu gering entwickelten Gesprächen (Gesprächsphasen 2 und 3). Es zeigt sich keine statistisch signifikante Korrelation zwischen angewendeter Literaturklassifizierung und Gesprächsentwicklung nach der Fünf- Phasen-Regel. Diskussion: Eine solche oder ähnliche Einteilung bei angewendeter dichterischer PT findet sich bisher nicht in der Literatur, wohl aber testpsychologische Untersuchungen, die einen Erfolg adjuvanter PT mit verhaltenstherapeutischen Texten in der Behandlung z.B. von Depression, von Panikstörungen und zur Suizidprophylaxe, etc. verzeichnen. Aufgrund von Effektivität und Kostenersparnis sollte über einen breiteren Gebrauch von adjuvanter Poetry-Therapie in Deutschland nachgedacht werden.
Abstract
Introduction
Using the situated expectancy–value theory, it becomes possible to explain motivational functioning across alternating learning conditions not only at a particular moment but also over time. The situated expectancy–value theory provides evidence for the critical role of context. The present research examines how adolescents' success expectancies, task values, and effort develop when the conditions of the academic environment change. It also evaluates whether adolescents adopt more adaptive expectancy–value profiles in response to the need‐based nature of self‐directed learning as an extension of regular instruction. Within the self‐directed learning approach under investigation, adolescents take responsibility for their own learning processes.
Methods
The present research offers insights into the expectancy–value profiles of 754 German adolescents (Mage = 13.56; SD = 1.2; 49.4% female). A four‐wave study was used to examine perceptions of self‐efficacy, intrinsic value, utility value, and effort. Latent profile analyses and latent transition analyses were employed.
Results
Notably, the results provide evidence that expanding instruction via self‐directed learning intervals that occur for 1 week per semester contributes to more favorable expectancy–value profiles within a student's favorite subject. A mixed profile (highly confident, hardly interested) disappeared. Instead, success expectations aligned more closely with adolescents' task values and effort. All profiles settled at a higher level.
Conclusions
The findings indicate that instruction that is expanded via self‐directed learning intervals has positive consequences for motivational profiles over time. An initial self‐directed learning episode led to a shift to more interested profile groups. The continuation of a positive trend even after a self‐directed learning interval indicates that adolescents are able to continually adapt their learning to their needs during teacher‐directed instruction. This study provides clues about how to design curricula in a way that counteracts the downward trend in students' motivation to learn.
Following the relational-developmental systems approach, this three-wave study examines whether acute stress (T2) mediates the relationship between the development of personality traits from the beginning of 8th grade (T1, Mage = 15.63, SD = 0.59; 22 girls) to the end of 9th grade (T3). Using the Montréal Imaging Stress Task, which is a task that provokes acute social stress by negative social feedback, this study combined the functional magnetic resonance imaging (fMRI), heart rate, and longitudinal survey data of 41 adolescents. Mediation analysis revealed that stress-induced left insula activation partially mediates the longitudinal stability of conscientiousness. These results highlight the impact of negative social feedback during stress on students’ personality development.
Agriculture in the populated islands of the Galapagos Archipelago, a protected area due to its unique biodiversity, has been detrimental to its conservation but highly required to meet food necessities. A potential solution to make agricultural farming more sustainable is adopting water-saving technologies (WSTs). Therefore, this study aimed to test the effectiveness of using WSTs such as Groasis Waterboxx® in three of the most valuable crops in the islands through participatory research with the involvement of a group of farmers from the Floreana and Santa Cruz islands and explore a possible transition to more sustainable agricultural practices. Capsicum annuum, Cucumis sativus and Solanum lycopersicum were cultivated using Groasis Waterboxx® and compared to conventional irrigation practices (drip-irrigated controls) to assess the variability of productivity, the number of fruits and individual fruit weight (IFW). In addition, differences in plant traits were analyzed by crop, and island. Results suggested that WSTs such as Groasis Waterboxx® may provide on-farm benefits regarding the yields of the studied traits. From this study, it is difficult to determine whether participation in such a research study will permanently change irrigation practices. However, the participant’s responses to the study suggest an increase in their understanding of the use and benefits of WST.
Abstract
Two decades after the discovery of adult‐born neurons in the brains of decapod crustaceans, the deutocerebral proliferative system (DPS) producing these neural lineages has become a model of adult neurogenesis in invertebrates. Studies on crayfish have provided substantial insights into the anatomy, cellular dynamics, and regulation of the DPS. Contrary to traditional thinking, recent evidence suggests that the neurogenic niche in the crayfish DPS lacks self‐renewing stem cells, its cell pool being instead sustained via integration of hemocytes generated by the innate immune system. Here, we investigated the origin, division and migration patterns of the adult‐born neural progenitor (NP) lineages in detail. We show that the niche cell pool is not only replenished by hemocyte integration but also by limited numbers of symmetric cell divisions with some characteristics reminiscent of interkinetic nuclear migration. Once specified in the niche, first generation NPs act as transit‐amplifying intermediate NPs that eventually exit and produce multicellular clones as they move along migratory streams toward target brain areas. Different clones may migrate simultaneously in the streams but occupy separate tracks and show spatio‐temporally flexible division patterns. Based on this, we propose an extended DPS model that emphasizes structural similarities to pseudostratified neuroepithelia in other arthropods and vertebrates. This model includes hemocyte integration and intrinsic cell proliferation to synergistically counteract niche cell pool depletion during the animal's lifespan. Further, we discuss parallels to recent findings on mammalian adult neurogenesis, as both systems seem to exhibit a similar decoupling of proliferative replenishment divisions and consuming neurogenic divisions.
Technological advances in light microscopy have always gone hand in hand with unprecedented biological insight. For microbiology, light microscopy even played a founding role in the conception of the entire discipline. The ability to observe pathogens that would otherwise evade human observation makes it a critical necessity and an indispensable tool to infectious disease research. Thus, the aim of this thesis was to optimize, extend, and functionally apply advanced light microscopy techniques to elucidate spatio-temporal and spatio-morphological components of bacterial and viral infection in vitro and in vivo.
Pathogens are in a constant arms race with the host’s immune system. By finding ways to circumvent host-mediated immune responses, they try to evade elimination and facilitate their own propagation. The first study (publication I) demonstrated that the obligate intracellular pathogen Coxiella burnetii is not just able to infect natural killer (NK) cells, but is actually capable of surviving the harsh degradative conditions in the cytotoxic lymphocyte’s granules. Using live-cell imaging of reporter-expressing Coxiella burnetii, the transient NK cell passage was closely monitored to provide detailed spatio-temporal information on this dynamic process in support of a range of static analyses. Bacterial release from NK cells was pinpointed to a time frame between 24 to 48 hours post-infection and the duration of release to about 15 minutes.
The second approach (publications II-V) aimed at shedding light on the greater spatio-morphological context of virus infection. Thus far, most studies investigating the distribution or tropism of viruses in vivo have used conventional immunohistochemistry in thin sections. Omitting the native spatial context of the infection site in vivo inherently bears the risk of incomplete description. While the microscopic tools and sample preparation protocols needed for volumetric 3D immunofluorescence imaging have recently been made available, they had not gained a foothold in virus research yet. An integral part of this thesis was concerned with the assessment and optimization of available tissue optical clearing protocols to develop an immunofluorescence-compatible 3D imaging pipeline for the investigation of virus infection inside its intact spatio-morphological environment (publication II). This formed the basis for all subsequent volumetric analyses of virus infection in vivo presented here. Consequently, this thesis provided a valuable proof of concept and blueprints for future virus research on the mesoscopic scale of host-pathogen interactions in vivo (publications II-V), using rabies virus (RABV; publications II-IV) and the newly-emerged severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2; publication V) as infection models for the nervous system and the respiratory tract, respectively.
Applying and further improving this volumetric 3D imaging workflow enabled unprecedented insights into the comprehensive in vivo cell tropism of RABV in the central (CNS) (publication III) and peripheral nervous system (PNS) (publication IV). Accordingly, differential infection of CNS-resident astrocytes by pathogenic and lab-attenuated RABV was demonstrated (publication III). While either virus variant showed equal capacity to infect neurons, as demonstrated by quantitative image analysis, only pathogenic field RABVs were able to establish non-abortive infection of astrocytes via the natural intramuscular inoculation route. A combined 3D LSFM-CLSM workflow further identified peripheral Schwann cells as a relevant target cell population of pathogenic RABV in the PNS (publication IV). This suggested that non-abortive infection of central and peripheral neuroglia by pathogenic RABV impairs their immunomodulatory function and thus represents a key step in RABV pathogenesis, which may contribute significantly to the establishment of lethal rabies disease.
Finally, utilizing the full volumetric acquisition power of LSFM, a further refined version of the established 3D imaging pipeline facilitated a detailed mesoscopic investigation of the distribution of SARS-CoV-2 in the respiratory tract of the ferret animal model (publication V). Particularly for this newly-emerged pathogen of global concern, in-depth knowledge of host-pathogen interactions is critical. By preserving the complete spatio-morphological context of virus infection in the ferret respiratory tract, this thesis provided the first specific 3D reconstruction of SARS-CoV-2 infection and the first report of 3D visualization of respiratory virus infection in nasal turbinates altogether. 3D object segmentation of SARS-CoV-2 infection in large tissue volumes identified and emphasized a distinct oligofocal infection pattern in the upper respiratory tract (URT) of ferrets. Furthermore, it corroborated a preferential replication of SARS-CoV-2 in the ferret URT, as only debris-associated virus antigen was detected in the lower respiratory tract of ferrets, thus providing crucial information on the spatial distribution of SARS-CoV-2.
Background
A redistribution of tasks between specialized nurses and primary care physicians, i.e., models of advanced nursing practice, has the potential to improve the treatment and care of the growing number of people with dementia (PwD). Especially in rural areas with limited access to primary care physicians and specialists, these models might improve PwD’s quality of life and well-being. However, such care models are not available in Germany in regular healthcare. This study examines the acceptance, safety, efficacy, and health economic efficiency of an advanced nursing practice model for PwD in the primary care setting in Germany.
Methods
InDePendent is a two-arm, multi-center, cluster-randomized controlled intervention study. Inclusion criteria are age ≥70 years, cognitively impaired (DemTect ≤8) or formally diagnosed with dementia, and living in the own home. Patients will be recruited by general practitioners or specialists. Randomization is carried out at the physicians’ level in a ratio of 1:2 (intervention vs. waiting-control group). After study inclusion, all participants will receive a baseline assessment and a follow-up assessment after 6 months. Patients of the intervention group will receive advanced dementia care management for 6 months, carried out by specialized nurses, who will conduct certain tasks, usually carried out by primary care physicians. This includes a standardized assessment of the patients’ unmet needs, the generation and implementation of an individualized care plan to address the patients’ needs in close coordination with the GP. PwD in the waiting-control group will receive routine care for 6 months and subsequently become part of the intervention group. The primary outcome is the number of unmet needs after 6 months measured by the Camberwell Assessment of Need for the Elderly (CANE). The primary analysis after 6 months is carried out using multilevel models and will be based on the intention-to-treat principle. Secondary outcomes are quality of life, caregiver burden, acceptance, and cost-effectiveness. In total, n=465 participants are needed to assess significant differences in the number of unmet needs between the intervention and control groups.
Discussion
The study will provide evidence about the acceptance, efficacy, and cost-effectiveness of an innovative interprofessional concept based on advanced nursing care. Results will contribute to the implementation of such models in the German healthcare system. The goal is to improve the current treatment and care situation for PwD and their caregivers and to expand nursing roles.
Hair follicles constitute important drug delivery targets for skin antisepsis since they contain ≈25% of the skin microbiome. Nanoparticles are known to penetrate deeply into hair follicles. By massaging the skin, the follicular penetration process is enhanced based on a ratchet effect. Subsequently, an intrafollicular drug release can be initiated by various trigger mechanisms. Here, we present novel ultraviolet A (UVA)-responsive nanocapsules (NCs) with a size between 400 and 600 nm containing hydroxyethyl starch (HES) functionalized by an o-nitrobenzyl linker. A phase transfer into phosphate-buffered saline (PBS) and ethanol was carried out, during which an aggregation of the particles was observed by means of dynamic light scattering (DLS). The highest stabilization for the target medium ethanol as well as UVA-dependent release of ethanol from the HES-NCs was achieved by adding 0.1% betaine monohydrate. Furthermore, sufficient cytocompatibility of the HES-NCs was demonstrated. On ex vivo porcine ear skin, a strong UVA-induced release of the model drug sulforhodamine 101 (SR101) could be demonstrated after application of the NCs in cyclohexane using laser scanning microscopy. In a final experiment, a microbial reduction comparable to that of an ethanol control was demonstrated on ex vivo porcine ear skin using a novel UVA-LED lamp for triggering the release of ethanol from HES-NCs. Our study provides first indications that an advanced skin antisepsis based on the eradication of intrafollicular microorganisms could be achieved by the topical application of UVA-responsive NCs.
Ion traps such as Paul traps and MR-ToF (multi-reflection time-of-flight) devices are indispensable tools at radioactive ion beam facilities for the preparation of high-quality radioactive ion beams for subsequent experiments or for precise measurements of the properties of radioactive ions, such as nuclear binding energies or nuclear charge radii.
Within the work of this thesis, Doppler- and sympathetic cooling is implemented in a linear Paul-trap cooler-buncher enabling a reduction of the longitudinal emittance of radioactive ion beams resulting in a significant improvement of the ion beam quality. Moreover, a next-generation MR-ToF device is conceptualized in order to achieve isobaric pure beams with a higher ion intensity than state-of-the-art MR-ToF devices can provide. Once fully constructed and commissioned, it will operate at an unprecedented ion beam energy of 30 keV. Both of these advances are expected to become important for a wide range of experimental programs pursued at low-energy branches of RIB facilities ranging from fundamental symmetry studies, nuclear structure, rare isotope studies with antimatter, searches of physics beyond the standard model to material science and the production of medical isotopes.
The next-generation MR-ToF mass separator is based on MIRACLS’ 30-keV MR-ToF device for highly sensitive and high-resolution collinear laser spectroscopy. By storing the ions in the Multi Ion Reflection Apparatus for Collinear Laser Spectroscopy (MIRACLS), the same ion bunch is probed by a spectroscopic laser for thousands of times compared to a single passage in traditional collinear laser spectroscopy (CLS). Dedicated simulation studies show that the accuracy and resolution will be close to traditional single-passage CLS while the sensitivity is significantly enhanced. Hence, measurements of nuclear properties via fluorescence-based CLS of very rare radionuclides as well as highly sensitive and high-precision measurements of electron affinities via laser-photodetachment-threshold spectroscopy of negatively-charged (radioactive) ions will become possible.
First measurement campaigns employing MIRACLS’ 1.5-keV MR-ToF device confirm the outstanding boost in signal sensitivity and provide confidence in the application of the MIRACLS technique for the measurement of scarcely produced radioactive ions that have been so far beyond the reach of conventional techniques. Furthermore, the electron affinity of 35Cl was measured, which is in perfect agreement with the literature value. These measurements will serve as important benchmarks for modern atomic and nuclear theory, especially in its description of nuclear charge radii.
In summary, the implementation of Doppler and sympathetic cooling at RIB facilities, the conceptualization of a 30-keV MR-ToF apparatus for highly selective and high-flux mass separation as well as for highly sensitive and high-resolution fluorescence-based laser spectroscopy and the expansion of the MIRACLS technique for the study of negatively-charged ions will enable unprecedented new measurement opportunities at RIB facilities.
Peatlands contribute to a wide range of ecosystem services. They play an important role as carbon sinks in their natural state, but when they are drained, they cause carbon emissions. Rewetting drained peatlands is required to reduce carbon emissions and create new carbon sinks. However, drained peatlands are commonly used as grassland or croplands; therefore, alternative agriculture schemes are required following rewetting. Paludiculture, i.e., agriculture on wet and rewetted peatlands, is an option in these areas after rewetting to produce biomass sustainably. Monitoring of peatland management is challenging, yet needed to ensure a successful rewetting and plantation of, e.g., Phragmites australis and Typha spp., two plants which are commonly used in paludiculture. Remote sensing is an excellent tool for monitoring the vegetation composition of vast rewetted peatland regions. However, because many peatland species have similar spectral characteristics, such monitoring is ideally based on high-spatial, high-temporal hyperspectral images. Data that complies with all these requirements does not exist on a regular basis. Therefore, we assessed the potential for mapping peatland vegetation communities in the Peene and Trebel river basins of the federal state of Mecklenburg-Western Pomerania, Germany, using multi-date hyperspectral (PRISMA) data. We used regression-based unmixing to map fractions of different peatland vegetation classes. Results were analyzed with regard to the contribution of multi-date observations and, in comparison, to multispectral datasets (Landsat-8/Sentinel-2). Our results showed that different classes are best mapped at different observation dates. The multi-date hyperspectral datasets produced less Mean Absolute Error (MAE = 16.4%) than the single-date hyperspectral images (ΔMAE + 1%), with high accuracies for all classes of interest. Compared to the results obtained with multispectral data from similar acquisition dates and annual spectral-temporal metrics (STM), the results from hyperspectral data were always clearly superior (ΔMAE + 4%). Besides the superior performance during comparisons, our results also indicate that information that can be derived from the hyperspectral data with the regression-based unmixing goes clearly beyond that of discrete classification. With more hyperspectral sensors coming up and an expected higher availability of multi-data hyperspectral imagery, these data can be expected to play a bigger role in the future monitoring of peatlands.
Advancing Radiation-Detected Resonance Ionization towards Heavier Elements and More Exotic Nuclides

RAdiation-Detected Resonance Ionization Spectroscopy (RADRIS) is a versatile method for highly sensitive laser spectroscopy studies of the heaviest actinides. Most of these nuclides need to be produced at accelerator facilities in fusion-evaporation reactions and are studied immediately after their production and separation from the primary beam due to their short half-lives and low production rates of only a few atoms per second or less. Only recently, the first laser spectroscopic investigation of nobelium (Z=102) was performed by applying the RADRIS technique in a buffer-gas-filled stopping cell at the GSI in Darmstadt, Germany. To expand this technique to other nobelium isotopes and for the search for atomic levels in the heaviest actinide element, lawrencium (Z=103), the sensitivity of the RADRIS setup needed to be further improved. Therefore, a new movable double-detector setup was developed, which enhances the overall efficiency by approximately 65% compared to the previously used single-detector setup. Further development work was performed to enable the study of longer-lived (t1/2>1 h) and shorter-lived nuclides (t1/2<1 s) with the RADRIS method. With a new rotatable multi-detector design, the long-lived isotope 254Fm (t1/2=3.2 h) becomes within reach for laser spectroscopy. Upcoming experiments will also tackle the short-lived isotope 251No (t1/2=0.8 s) by applying a newly implemented short RADRIS measurement cycle.
A paradigm was developed to experimentally investigate the dysregulation of affective reactivity in clinical depression. The literature so far reported evidence for three directions of dysregulation - negative potentiation, positive attenuation, and emotion context insensitivity. Therefore a paradigm was designed to allow to test all three hypotheses simultaneously. Furthermore, to enable generalization across the specific stimuli used in the experiment, stimuli of two sensory modalities were used - pictures and sounds. Because it was hypothesized, that the specificity of affective reactivity of depressed patients will be especially prominent in long lasting affective situations, a categorically blocked presentation mode was chosen. Regarding the dependent variables, a multimethod approach was conducted. Besides self-report ratings of the feeling state, startle responses, skin conductance responses, heart rate, and the electromyogram of the corrugator and zygomatic muscle were recorded. In a separate session, BOLD-responses during picture viewing were collected by functional magnetic resonance imaging (fMRI). Both sessions were conducted with three samples: a healthy student sample, a depressed outpatient sample, and a healthy age and gender matched control sample. The results of the patient sample support an integration of the emotion context insensitivity and the negative potentiation hypothesis. Patients reported generally to feel more unpleasant and more aroused than healthy controls. Skin conductance and startle responses were modulated by valence to a smaller degree in the patients than in the controls. No group differences were found in the facial muscle activity. BOLD-responses were potentiated during unpleasant compared to neutral pictures in the patient but not in the control group in the amygdala, the insular cortex and the orbito frontal cortex. A model to integrate these results is developed. Its central assumption is, that the inability to respond to affective stimuli is an aversive experience and therefore leads to a negativity bias in attention and cognition. Direction of further research and implications for psychotherapies are discussed.
Fragestellungen: In dieser Dissertation wurde unter Verwendung psychophysiologischer Parameter die affektive Dysregulation bei Patienten mit einer Borderline-Persönlichkeitsstörung (BPS) untersucht. Klinische Beobachtungen legen nahe, dass Personen mit einer BPS Defizite in der emotionalen Steuerung, eine sogenannte affektive Dysregulation mit einer hohen emotionalen Reaktivität, vor allem auf aversive affektive Reize, aufweisen. Die empirischen Befunde sind jedoch inkonsistent. Es wurde daher experimentell überprüft, ob sich bei Patienten mit BPS generell eine gesteigerte emotionale Reaktivität im Vergleich zu gesunden Kontrollprobanden nachweisen lässt oder ob sich die affektive Dysregulation vorrangig in Reaktion auf persönliche oder störungsspezifische emotionale Themen zeigt. Zusätzlich wurde der Einfluss einer, bei der BPS häufigen, komorbiden Posttraumatischen Belastungsstörung (PTBS) sowie einer dissoziativen Symptomatik auf die emotionale Reaktivität der BPS-Patienten untersucht. Methodik: Unter Verwendung eines Paradigmas zur Imagination emotionaler Skripte wurden die affektiven Reaktionen von 40 unmedizierten BPS-Patienten (37 weiblich) und 32 psychisch gesunden Kontrollprobanden (27 weiblich) untersucht. Neben standardisierten emotional unangenehmen, neutralen und angenehmen Skripten wurden persönliche (idiographisch aversive) Skripte verwendet, die ein extrem belastendes Lebensereignis beschrieben. Die persönlichen Skripte der BPS-Patienten beinhalteten zumeist Szenen traumatischer Erfahrungen. Außerdem wurden störungsspezifische Szenen zu Ablehnung und Verlassenwerden verwendet. Die Probanden waren instruiert, sich die Skripte nach dem Lesen so lebendig wie möglich vorzustellen. Als Maß der emotionalen Aktivierung während der Imagination der Skripte wurden psychophysiologische Parameter wie die emotionsinduzierte Modulation der Schreckreaktion und Indikatoren autonomer Erregung wie die Herzrate und die elektrodermale Aktivität gemessen. Weiterhin wurde die akute und generelle Dissoziation erfasst. Von den 40 Patienten mit einer BPS erfüllten 26 die Kriterien für eine komorbide aktuelle PTBS. Diese wurden bezüglich des Schweregrades in zwei Subgruppen unterteilt (moderate PTBS n = 13, schwere PTBS n = 13). Ergebnisse: Die vorliegenden Daten zeigen klar, dass eine generelle affektive Dysregulation bei der Imagination von emotionalen Skripten unterschiedlicher Valenz bei BPS-Patienten im Vergleich zu gesunden Kontrollpersonen nicht nachweisbar ist. Beide Gruppen wiesen eine vergleichbare Ausprägung und Modulation der Schreckreaktionen und der Indikatoren autonomer Erregung auf. Allerdings zeigten BPS-Patienten eine erhöhte defensive Reaktivität mit potenzierten Schreckreaktionen und einem Anstieg der autonomen Erregung während der Imagination der störungsspezifischen Skripte. Eine komorbide PTBS war mit der Beeinträchtigung defensiver Reaktionen assoziiert. BPS-Patienten mit aktueller PTBS zeigten im Vergleich zu BPS-Patienten ohne BPS während der Imagination aller Skripte generell verminderte Schreckreaktionen und eine eingeschränkte emotionale Modulation. Gerade BPS-Patienten mit schwerer PTBS wiesen während der Imagination idiographisch aversiver und störungsspezifischer Skripte eine fehlende Potenzierung der Schreckreaktionen bei einem gleichzeitig deutlich ausgeprägten Anstieg der Herzrate als Indikator autonomer Erregung auf. Des Weiteren scheint ein, in die gleiche Richtung weisender, Zusammenhang zwischen dissoziativen Symptomen und den emotionalen Reaktionen der BPS-Patienten zu bestehen. Ein höheres Ausmaß an akuter Dissoziation hing mit einer Verminderung der Schreckreaktionen während der Imagination idiographisch aversiver Skripte und gleichzeitig stärker ausgeprägter emotionaler und physiologischer Erregung zusammen. Mit zunehmendem Schweregrad der komorbiden PTBS erhöhte sich die aktuelle und generelle Dissoziationsneigung. Schlussfolgerungen: Diese Daten implizieren, dass die im klinischen Kontext zu beobachtende affektive Dysregulation bei Patienten mit BPS kein generelles Phänomen darstellt, sondern eher durch Aktivierung spezifischer Schemata ausgelöst wird. Eine komorbide PTBS moduliert die emotionalen Reaktionen der BPS-Patienten während der Imagination emotionaler Skripte in substantieller Weise. Durch die Ergebnisse der vorliegenden Arbeit wird auf die Bedeutung therapeutischer Interventionen im Hinblick auf die manifesten Annahmen über Ablehnung und Verlassenwerden bei BPS-Patienten hingewiesen. Die Stärkung der Fähigkeiten, bei Aktivierung dieser Annahmen, Erfahrungen klar zu differenzieren und damit einhergehende unangenehme Gefühle in Beziehungen zu regulieren, stellen ein zentrales Ziel in der Therapie der BPS dar. Dabei ist es in der klinischen Arbeit von immenser Bedeutung, neben der dissoziativen Symptomatik, das Ausmaß der posttraumatischen Belastung zu beachten, um neue Lernerfahrungen im therapeutischen Kontext zu ermöglichen.
Es gibt Hinweise darauf, dass das Kleinhirn an affektiven und kognitiven Verarbeitungsprozessen und an Arbeitsgedächtnisleistungen beteiligt ist. In dieser Arbeit wurden 8 Patienten mit Kleinhirninsulten (Durchschnittsalter 61,25 Jahre), die in der neurologischen Klinik der Universitätsmedizin Greifswald behandelt wurden und 7 Patienten mit peripher neurologischen Erkrankungen (Durchschnittsalter 56,71 Jahre), bei denen eine Kleinhirnläsion ausgeschlossen worden war, untersucht. Zur Beurteilung veränderter neuronaler Aktivitäten wurde eine 129-Kanal-Elektroenzephalographie-Studie (EEG) verwendet und mithilfe der Interpretation ereigniskorrelierter Potentiale (EKP) verschiedene affektive und kognitive Verarbeitungsprozesse analysiert. In der Teilstudie 1 wurde die frühe Verarbeitung visuell-affektiver Stimuli, in der Teilstudie 2 affektive und kognitive Verarbeitungsprozesse während der Präsentation visueller Stimuli, in der Teilstudie 3 affektive und kognitive Verarbeitungsprozesse während der Präsentation visueller und akustischer Stimuli und in der Teilstudie 4 die späte Verarbeitung visuell-affektiver Stimuli untersucht. Zur Untersuchung der affektiven Verarbeitungsprozesse wurden Bilder verschiedenen emotionalen Inhaltes (angenehm, neutral, unangenehm) und Erregungsstufe (schwach bis stark erregend) aus dem Katalog des International Affective Picture System (IAPS) verwendet. Es wurden Bilder in schneller 333ms (Teilstudien 1 bis 3) oder in langsamer Abfolge von 1000ms (Teilstudie 4) präsentiert. Zur Untersuchung kognitiver Verarbeitungsprozesse wurden die IAPS-Bilder bearbeitet. Für die Teilstudie 2 wurden sie mit Linien (horizontal/vertikal) überlagert und für die Teilstudie 3 mit Tönen (hoch/tief) synchronisiert. Linien und Töne unterschieden sich in ihrer Wahrscheinlichkeit des Auftretens, wobei die seltenen Reize als Zielreize dienten, welche von den Probanden mitgezählt werden mussten. Es wurden durch dieses Studiendesign folgende ereigniskorrelierte Potentiale gemessen: Die EPN, die visuelle P200 und P300, die akustische P300 und das LPP. Bezüglich der frühen und späten Verarbeitung visuell-affektiver Stimuli konnten folgende Daten erhoben werden. In der Teilstudie 1 lösten in der Läsionsgruppe nur stark erregend angenehme vs. neutrale Bilder eine EPN aus. Ein signifikanter Gruppeneffekt bestand jedoch nicht. In der Teilstudie 2 war weder für schwach noch für starke erregend affektive vs. neutrale Bilder eine EPN in der Läsions- und Kontrollgruppe nachweisbar. In der Teilstudie 3 konnte zwar nur in der Kontrollgruppe für stark erregend angenehme vs. neutrale Bilder eine EPN nachgewiesen werden, die Gruppen unterschieden sich jedoch nicht signifikant voneinander. In der Teilstudie 4 lösten weder schwach noch stark erregend affektive Bilder ein LPP in der Läsionsgruppe aus. Ein signifikanter Gruppeneffekt bestand nicht, trotz nachweisbaren LPPs in der Kontrollgruppe für schwach erregend angenehme und stark erregend affektive vs. neutrale Bilder. Bezogen auf kognitive Verarbeitungsprozesse konnte in beiden Gruppen in der Teilstudie 2 eine visuelle P300 nach der Präsentation seltener Zielreize nachgewiesen werden. Die Läsionsgruppe wies dagegen eine signifikante visuelle P200 nach Präsentation von Zielreizen gegenüber der Kontrollgruppe auf. Eine akustische P300 (P3b) war in der Teilstudie 3 nach der Präsentation akustischer Zielreize in keiner Gruppe nachweisbar. Dagegen bestand in der Kontrollgruppe eine signifikant stärkere P3a. Die Ergebnisse zeigen, dass Patienten mit einer Kleinhirnläsion keine Beeinträchtigung in der frühen oder späten Verarbeitung visuell-affektiver Stimuli aufweisen. Sie sind in der Lage, eine Bottom-up-Prozessierung visuell-affektiver Stimuli durchzuführen und sie nach ihrer Motivationsrelevanz einzuordnen. Patienten mit einer Kleinhirnläsion unterscheiden sich nicht signifikant in ihrer neuronalen Aktivität gegenüber der Kontrollgruppe während intra- und crossmodaler Verarbeitungsprozesse von visuell-affektiven Stimuli während visueller oder akustischer Aufgaben. Die in vielen Studien beobachteten affektiven Auffälligkeiten bei Patienten mit einer Kleinhirnischämie sind daher auf spätere Verarbeitungs- und Ausführungsprozesse von Emotionen zurückzuführen, welche einer kognitiven und somit Top-down-Kontrolle unterliegen. Patienten mit einer Kleinhirnläsion benötigen allerdings mehr Arbeitsgedächtnisleistung, um die gestellte visuell-kognitive Aufgabe zu absolvieren. Des Weiteren weisen sie Beeinträchtigungen in supramodalen kognitiven Verarbeitungsprozessen auf. Je schwieriger die kognitiven Anforderungen sind, umso mehr weisen Patienten mit einer Kleinhirnläsion Beeinträchtigungen in Form veränderter neuronaler Aktivität auf. Die Ergebnisse dieser Arbeit weisen darauf hin, dass das Kleinhirn vor allem an kognitiven und weniger an affektiven Verarbeitungsprozessen beteiligt ist.
We consider Iterated Function Systems (IFS) on the real line and on the complex plane. Every IFS defines a self-similar measure supported on a self-similar set. We study the transfer operator (which acts on the space of continuous functions on the self-similar set) and the Hutchinson operator (which acts on the space of Borel regular measures on the self-similar set). We show that the transfer operator has an infinitely countable set of polynomial eigenfunctions. These eigenfunctions can be regarded as generalized Bernoulli polynomials. The polynomial eigenfuctions define a polynomial approximation of the self-similar measure. We also study the moments of the self-similar measure and give recursions for computing them. Further, we develop a numerical method based on Markov chains to study the spectrum of the Hutchinson and transfer operators. This method provides numerical approximations of the invariant measure for which we give error bounds in terms of the Wasserstein-distance. The standard example in this thesis is the parametric family of Bernoulli convolutions.
African swine fever virus (ASFV) is one of the most threatening animal viruses which has dramatically expanded its distribution range within the last years. ASFV was first described and is endemic in sub-Saharan Africa where it is transmitted in a sylvatic cycle between indigenous suids and Ornithodoros soft ticks. Therefore, ASFV is the only known DNA-arbovirus and, in addition to that, the only member of the genus Asfivirus within the family Asfarviridae. Being highly infectious to domestic pigs and wild boar, the virus was introduced into Georgia in 2007 and has subsequently spread throughout eastern Europe reaching the European Union in 2014. Despite almost 100 years of intensive research and the occurrence of African swine fever (ASF) on four continents including Europe, many aspects of its epidemiology, vector dynamics and virus evolution are unknown. In our study, first evidence is presented on endogenous ASFV-like (EASFL)- elements which are integrated into the genome of ASFV natural vectors, O. moubata soft ticks. Through a series of experiments including next-generation sequencing, infection experiments, phylogenetic and BEAST analyses as well as PCR-screening, evidence is provided that these elements belong to an ancestral ASFV strain that might have existed 50,000 to 30,000 years BCE. Further results suggest that the EASFL-elements are involved in protecting ticks against ASFV infection and might belong to a generalised tick defence mechanism. In order to evaluate factors influencing ASFV epidemiology in eastern Europe, experiments were conducted on possible indigenous vector species and circulating virus isolates. In the absence of the natural tick vector, blow fly larvae were considered as possible mechanical vectors involved in ASFV transmission and persistence. Results are presented that even after feeding on highly infectious wild boar tissue, fly larvae and pupae showed no contamination with infectious virus. On the contrary, the maggots appeared to have inactivated the virus in the organ tissue through their salivary secretions. Further experiments conducted on an ASFV-strain isolated from northeastern Estonia resulted in the first report of an ASFV-strain with attenuated phenotype isolated in Eastern Europe. Results from NGS-analyses provided evidence for a major genome reorganisation in that strain that included a large deletion and a duplication of multiple ASFV genes.
Taken together, this study provides novel insights into the epidemiology of ASF and evolution of ASFV one of the major threats to animal health worldwide and therefore does not only contribute significantly to basic research but possibly also to specific knowledge necessary for future disease management.
Abstract
Caveolae position CaV3.2 (T‐type Ca2+ channel encoded by the α‐3.2 subunit) sufficiently close to RyR (ryanodine receptors) for extracellular Ca2+ influx to trigger Ca2+ sparks and large‐conductance Ca2+‐activated K+ channel feedback in vascular smooth muscle. We hypothesize that this mechanism of Ca2+ spark generation is affected by age. Using smooth muscle cells (VSMCs) from mouse mesenteric arteries, we found that both Cav3.2 channel inhibition by Ni2+ (50 µM) and caveolae disruption by methyl‐ß‐cyclodextrin or genetic abolition of Eps15 homology domain‐containing protein (EHD2) inhibited Ca2+ sparks in cells from young (4 months) but not old (12 months) mice. In accordance, expression of Cav3.2 channel was higher in mesenteric arteries from young than old mice. Similar effects were observed for caveolae density. Using SMAKO Cav1.2−/− mice, caffeine (RyR activator) and thapsigargin (Ca2+ transport ATPase inhibitor), we found that sufficient SR Ca2+ load is a prerequisite for the CaV3.2‐RyR axis to generate Ca2+ sparks. We identified a fraction of Ca2+ sparks in aged VSMCs, which is sensitive to the TRP channel blocker Gd3+ (100 µM), but insensitive to CaV1.2 and CaV3.2 channel blockade. Our data demonstrate that the VSMC CaV3.2‐RyR axis is down‐regulated by aging. This defective CaV3.2‐RyR coupling is counterbalanced by a Gd3+ sensitive Ca2+ pathway providing compensatory Ca2+ influx for triggering Ca2+ sparks in aged VSMCs.
Endothelial dysfunction (ED) comes with age, even without overt vessel damage such as that which occurs in atherosclerosis and diabetic vasculopathy. We hypothesized that aging would affect the downstream signalling of the endothelial nitric oxide (NO) system in the vascular smooth muscle (VSM). With this in mind, resistance mesenteric arteries were isolated from 13-week (juvenile) and 40-week-old (aged) mice and tested under isometric conditions using wire myography. Acetylcholine (ACh)-induced relaxation was reduced in aged as compared to juvenile vessels. Pretreatment with L-NAME, which inhibits nitrix oxide synthases (NOS), decreased ACh-mediated vasorelaxation, whereby differences in vasorelaxation between groups disappeared. Endothelium-independent vasorelaxation by the NO donor sodium nitroprusside (SNP) was similar in both groups; however, SNP bolus application (10−6 mol L−1) as well as soluble guanylyl cyclase (sGC) activation by runcaciguat (10−6 mol L−1) caused faster responses in juvenile vessels. This was accompanied by higher cGMP concentrations and a stronger response to the PDE5 inhibitor sildenafil in juvenile vessels. Mesenteric arteries and aortas did not reveal apparent histological differences between groups (van Gieson staining). The mRNA expression of the α1 and α2 subunits of sGC was lower in aged animals, as was PDE5 mRNA expression. In conclusion, vasorelaxation is compromised at an early age in mice even in the absence of histopathological alterations. Vascular smooth muscle sGC is a key element in aged vessel dysfunction.
Abstract: Ischemic stroke is an aging disease and causes high mortality or long-term disability. The reduced neurological recovery in aging is possibly associated with impairment of angiogenesis and non-specific enhancement of inflammatory reaction. To check this hypotheses, those events were compared within young and elder animals brain at day 14 following focal ischemic stroke. Moreover, it is of importance to investigate also the potential therapies of indomethacin for prolonging the therapeutic window using aged animal models. The focus of present study was on neurobiological and neurological differences between young and old rats modulated by indomethacin daily treatment beginning at four hours post-ischemic episode. The effectiveness of indomethacin treatment in young and elder rats was probed using immunohistochemistry, oligonucleotide microarray, Real Time PCR and neurological evaluation. Our results provide insight of several age-independent positive consequences of Cox non-specific inhibition by indomethacin including increased NeuN positive surviving neurons, reduced infarct volume and enhanced neuroprotective response of innate immune system evidenced by increased Iba1 and Anx3 immunoreactivities in moderately activated microglia in periinfarction. From gene level we observed in both age groups downregulation of Mdk and Cxcl1 chemokines, and Id3 transcription factor which might modulate inflammatory response and facilitate repair. Other several findings showed age-dependent drug effect. Indomethacin had reduced efficacy in aged ischemic brain. From a total of 34 genes differential regulated, we observed 43% in young and only 28% of genes in aged have tendency toward age-matched sham expression level. In aged rats, indomethacin is ineffective in inhibiting phagocytic activity which is probably due to no expression changes of several cytokines like Tnfá and Cxcl4. Also, at protein level we observed no change of lysosomal ED1 immunoreactivity under treatment. On the other hand aging is characterized by no expression changes of Plau, Timp1, Timp2 and Col18a1 after treatment resulting in no improvement of angiogenesis. In young rats, conversely, drug administration decreased phagocytic activity by downregulating several cytotoxic cytokines such as Tnfá and Cxcl4. Moreover, the observable decrease of proteases like MMP10, Plau and MMP inhibitor Timp2 employed in matrix remodeling together with downregulation of Col18a1 expression after treatment might sustain angiogenesis in young rat ischemic brain. Indomethacin improves the motor-sensory performance in ischemic stroke rats as compared with age-matched untreated animals. Young rats fully recovered while aged showed important recuperation but did not achieve the preoperative level. In view of all this, indomethacin treatment might be consider as adjuvant therapy following ischemic stroke, even if aging blunts the positive effect of indomethacin on altered angiogenic-related gene expression. Because of the small number of rats, the results obtained from this study show only a tendency to significance and that further studies with more animals need to be statistically validated before firmly conclusions can be drawn. External effects of agglomeration and human capital is a more than a century old research topic. Their theory and empirics have evolved over time to interact with more economic factors and to dig more deeply into heterogeneous effects, whose empirical evidence remains scarce, especially for developing countries. Furthermore, local human capital and agglomeration are often introduced in separate models rather than a single model in explaining productivity. These limitations motivate the implementation of this thesis. To achieve this, firm-level panel data from Vietnam is employed. This developing country provides an interesting case because its level of agglomeration and human capital is still low but its socio-economic conditions are highly dynamic with the high growth rates of urbanization and university-educated labor force (production inputs) as well as income (production output), in comparison with developed countries. The heart of this thesis lies on its chapter 2 and chapter 3, which are summarized as follows.
Chapter 2 aims at finding which agglomeration forces play the dominant role in affecting firms’ productivity and how agglomeration induces unequal influences across various firm characteristics. To achieve this, a six-year panel data set is employed, and the estimation is based on a production function that the left-hand side is firm’s total factor productivity while the righthand side is local technology which contains the agglomeration terms. In the first step of regression, consistent values of productivity are obtained following a strategy that combines the control function approach with the instrumental variables technique to tackle endogeneity caused by a possibility that firms choose their production inputs based on their productivity. In the second step, log of productivity is regressed on agglomeration proxies and controls, using multiple fixedeffects terms to control for unobserved factors and local shocks. Estimated results show that urbanization rather than specialization has a positive impact on productivity. Besides, the agglomeration effects are stronger for foreign-owned, small-sized, or young firms.
Chapter 3 shows attempts to find evidence of human capital externalities along with urbanization economies, given that the two external terms are rarely placed together in a single specification in literature. The estimation is implemented based on a production function whose context is an unique spatial equilibrium resulting from migration behaviors of entrepreneurs and workers. In this function, the externalities play the role of a region-specific productivity shifter. The model is regressed primarily with the instrumental variables technique to tackle possible identification problems. Between the two external terms, the resulting estimates confirm only the existence of urbanization economies. However, human capital externalities are found to be strong and significant in high-tech industries, implying that the effects of local human capital are very heterogeneous across different technological levels.
Aging is an independent risk factor for hypertension, cardiovascular morbidity, and mortality. However, detailed mechanisms linking aging to cardiovascular disease are unclear. We studied the aging effects on the role of perivascular adipose tissue and downstream vasoconstriction targets, voltage-dependent KV7 channels, and their pharmacological modulators (flupirtine, retigabine, QO58, and QO58-lysine) in a murine model. We assessed vascular function of young and old mesenteric arteries in vitro using wire myography and membrane potential measurements with sharp electrodes. We also performed bulk RNA sequencing and quantitative reverse transcription-polymerase chain reaction tests in mesenteric arteries and perivascular adipose tissue to elucidate molecular underpinnings of age-related phenotypes. Results revealed impaired perivascular adipose tissue-mediated control of vascular tone particularly via KV7.3–5 channels with increased age through metabolic and inflammatory processes and release of perivascular adipose tissue-derived relaxation factors. Moreover, QO58 was identified as novel pharmacological vasodilator to activate XE991-sensitive KCNQ channels in old mesenteric arteries. Our data suggest that targeting inflammation and metabolism in perivascular adipose tissue could represent novel approaches to restore vascular function during aging. Furthermore, KV7.3–5 channels represent a promising target in cardiovascular aging.
AGnES in der Regelversorgung - Umsetzung des § 87 Abs. 2b S. 5 SGB V im Bundesmantelvertrag AGnES (AGnES: Arztentlastende, Gemeinde-nahe, E-Healthgestützte, Systemische Intervention) ist ein vom Institut für Community Medicine der Univeritätsmedizin Greifswald entwickeltes Modell zur Entlastung von Hausärzten, bei dem Hausbesuche auf speziell fortgebildete nicht-akademische Fachkräfte delegiert werden. Die Hausbesuche werden ohne Anwesenheit des Arztes durchgeführt und sollen gerade im ländlichen Raum Ärzte entlasten und dem Ärztemangel entgegenwirken. AgnES wurde von 2005 bis 2008 in mehreren Bundesländern erprobt. Im Jahr 2008 wurde im Rahmen einer Pflegereform (Pflege-Weiterentwicklungsgesetz) auch § 87 Abs. 2b S. 5 SGB V ergänzt. In dieser Norm werden die Behörden der Selbstverwaltung im Gesundheitswesen (in diesem Fall die Kassenärztliche Bundesvereinigung, der Spitzenverband Bund der Krankenkassen und der Bewertungsausschuss) verpflichtet, Regelungen zur Vergütung von Hausbesuchen nicht ärztlicher Fachkräfte zu erlassen. Der Bundesgesetzgeber verfolgte mit der Schaffung dieser Norm gerade das Ziel, das AGnES-Projekt oder ein inhaltlich gleichwertiges, wissenschaftlich evaluiertes und breit erprobtes Delegationsmodell in der Regelversorgung der Gesetzlichen Krankenversicherung zu verankern. Die Selbstverwaltung im Gesundheitswesen hat hierfür 2009 einerseits den Bundesmantelvertrag durch eine Anlage ergänzt, die sog. Delegationsvereinbarung. Außerdem wurde der Einheitliche Bewertungsmaßstab als Leistungs- und Vergütungskatalog der Gesetzlichen Krankenversicherung um Abrechnungspositionen für nicht-ärztlich durchgeführte Hausbesuche erweitert. Delegationsvereinbarung und EBM bleiben weit hinter dem vom Gesetzgeber gemeinten AgnESProjekt zurück. Die Delegationsvereinbarung verstößt deshalb in vielfältiger Hinsicht gegen den Vorrang des Gesetzes. Sie lässt sich unter anderem hinsichtlich der bis Ende 2014 vorgenommenen Beschränkung auf unterversorgte Gebiete (§ 2 Abs. 2), der Einschränkung der infrage kommenden Patientengruppen (§ 3 lit. a) und b)), des Anstellungszwanges der Praxisassistentinnen bei den Praxen (§ 4 Abs. 2), der Notwendigkeit der ärztlichen Einzelfallanordnung (§ 5 Abs. 1 S. 1, Abs. 2 S. 2), den zugelassenen Ausgangsqualifikationen der Fachkräfte (§ 6 S. 2 lit.a)) und vor allem hinsichtlich der Dauer der Fortbildung und ihrer Inhalte u.a. (§ 7 Abs. 2) nicht mit dem von § 87 Abs. 2b S. 5 SGB V vorgegeben Anforderungen vereinbaren. Die Regelungen der Delegationsvereinbarung verfehlen nicht nur die vom parlamentarischen Gesetzgeber gestellten Anforderungen, sondern werden auch den anderen Beteiligten nicht gerecht: Durch die zu niedrig festgelegten Qualifikationsanforderungen wird die Sicherheit der Patienten möglicherweise gefährdet. Zudem werden durch die zu restriktiven Voraussetzungen, welche Patientengruppen überhaupt von Praxisassistentinnen versorgt werden dürfen, unnötig viele Patienten ausgeschlossen. Hinsichtlich der Praxisassistentinnen wird die Chance vertan, einen großen Schritt hin zur Steigerung der Attraktivität der Heilhilfsberufe zu unternehmen und diesen den Stellenwert im Gesundheitssystem einzuräumen, den sie in den anderen europäischen Ländern bereits haben und wo sowohl finanzielle als auch personelle Probleme mit Hilfe der Praxisassistentinnen abgefedert werden können. Schließlich werden die Regelungen der Delegationsvereinbarung und des EBM auch den Vertragsärzten nicht gerecht. Ihnen steht die dringend notwendige Entlastung bis Ende 2014 nur in unterversorgten Gebieten zur Verfügung, die notwendige Attraktivitätssteigerung des Hausarztberufes bleibt auf der Strecke. Schließlich sind Zahl und Schwierigkeitsgrad der delegierbaren Leistungen aufgrund der potentiell ungenügenden Qualifikation der Praxisassistentinnen nach der Delegationsvereinbarung und damit ihre Entlastungswirkung deutlich in Frage gestellt, wollen nicht Hausärzte und Praxisassistentinnen Gefahr laufen, deshalb haftungsträchtige Behandlungsfehler zu begehen.
We are currently facing an antimicrobial resistance crisis, which means that a lot of bacterial pathogens have developed resistance to common antibiotics. Hence, novel and innovative solutions are urgently needed to combat resistant human pathogens. A new source of antimicrobial compounds could be bacterial volatiles. Volatiles are ubiquitous produced, chemically divers and playing essential roles in intra- and interspecies interactions like communication and antimicrobial defense. In the last years, an increasing number of studies showed bioactivities of bacterial volatiles, including antibacterial, antifungal and anti-oomycete activities, indicating bacterial volatiles as an exciting source for novel antimicrobial compounds. In this review we introduce the chemical diversity of bacterial volatiles, their antimicrobial activities and methods for testing this activity. Concluding, we discuss the possibility of using antimicrobial volatiles to antagonize the antimicrobial resistance crisis.
Bei moderaten sinusförmigen Betriebsspannungen tritt in reinem Stickstoff der diffuse Townsend-Modus (APTD) auf. Das elektrische Feld ist hier über den Entladungsspalt annähernd konstant, weshalb ein anodengerichteter exponentieller Anstieg der Intensität der Emission beobachtet wird. Dementsprechend ist das Intensitätsmaximum direkt vor der Anode lokalisiert. Überraschenderweise lässt sich die APTD unter den gegebenen experimentellen Bedingungen (Breite des Entladungsspalts d_Spalt=1 mm und sinusförmige Betriebsspannung) ebenfalls in einer Helium-BE genieren. Für gewöhnlich wird jedoch in einer Helium-Entladung der diffuse Glimmentladungs-Modus (APGD) beobachtet, wobei der Entladungsspalt zwischen 2-5 mm breit ist. Das Emissionsmaximum einer solchen Entladung befindet sich durch die Ausbildung eines Kathodenfallgebiets vor der Kathode. Die geringe Breite des Entladungsspalts verhindert hier jedoch die Ausbildung der APGD. Entsprechend kann sich das Kathodenfallgebiet nicht entwickeln, wodurch die Spaltspannung nur schwach einbricht. Das Intensitätsmaximum der Emissionsentwicklung befindet sich wie bei der diffusen Stickstoff-BE direkt vor der Anode. Die Zünd- und Brennspannung ist in Stickstoff größer als in Helium, da die Vibrationszustände des Stickstoffs effizient durch Elektronen angeregt werden und diesen dabei Energie entzogen wird. Helium hat jedoch keine Vibrationszustände, weshalb die Elektronentemperatur ansteigt und die Zünd- und Brennspannung deutlich geringer ist. Eine Erhöhung des Spannungsanstiegs dU/dt beeinflusst signifikant die Entladungsentwicklung in der diffusen Helium-Entladung. So führt eine Variation von der Sinus- zur Rechteckspannung zu einem Wechsel des Entladungsmodus, nämlich von der APTD zur APGD. Die Ursache hierfür ist der deutlich höhere Energieeintrag, was sich auf die Ionisationsprozesse auswirkt. Die Verwendung einer Sägezahnspannung stellt in Bezug auf den Spannungsanstieg dU/dt eine Kombination aus der Sinus- und der Rechteckspannung dar. Mit dieser Betriebsspannung war es erstmals möglich, in einer Entladungsperiode entsprechend der Spannungsgradienten beide Entladungsformen (APTD und APGD) zu beobachten und zu studieren. Durch die Oberflächenladungsmessung konnte nachgewiesen werden, dass die während eines elektrischen Durchbruchs im Entladungsvolumen transferierte Ladung vollständig auf den Dielektrika akkumuliert wird. Der Vergleich der phasenaufgelösten Oberflächenladungsdichtemessung mit der zeitlichen Integration der Stromdichte zeigt, dass die Akkumulation von Oberflächenladungen instantan mit dem Auftreten eines Strompulses stattfindet. Nach einem Entladungsstrompuls bleiben die Oberflächenladungen unabhängig vom Entladungsmodus auf dem Dielektrikum konstant, bis die Entladung in der nächsten Halbwelle erneut zündet. In der filamentierten Entladung markieren die Oberflächenladungen den Auftreffpunkt der einzelnen Mikroentladungen. Die Oberflächenladungen sind an diesen Stellen stark lokalisiert. Die gemittelten radialen Oberflächenladungsdichteprofile haben gezeigt, dass diese sowohl für die negativen als auch für die positiven Oberflächenladungen einer Gauß-Verteilung folgen. Die volle Halbwertebreite der entsprechenden Oberflächenladungsdichteprofile unterscheidet sich. Die negativen Oberflächenladungen nehmen eine größere Fläche ein als die positiven Oberflächenladungen. Es konnte erstmals gezeigt werden, dass Mikroentladungen über viele Entladungsperioden immer wieder an der gleichen Stelle zünden, wo sich aus einer vorhergehenden Entladung ein lokalisierter Oberflächenladungsfleck entgegengesetzter Polarität befand. Dieses Phänomen wird als Memory-Effekt bezeichnet. Durch zeitlich definiertes Abschalten der Entladung konnten die Lebensdauern von Oberflächenladungen beider Polaritäten auf dem BSO-Kristall gemessen werden. Es konnte gezeigt werden, dass der Abbau der Oberflächenladungen in zwei Zerfallsprozesse k_1 und k_2 unterteilt ist. Während des Prozesses k_1 nimmt die Oberflächenladungsdichte innerhalb einiger weniger Sekunden deutlich ab. Die Zeitkonstante k_1 ist trotz der photoleitenden Eigenschaft des BSO-Kristalls unabhängig von der Beleuchtungsfrequenz des Kristalls ist. Der zweite deutlich langsamer ablaufende Prozess zeigte hingegen eine starke Abhängigkeit von der Beleuchtungsfrequenz der BSO-Kristalls. Wurde der Kristall kontinuierlich beleuchtet, verschwanden die Oberflächenladungen unabhängig von ihrer Polarität nach wenigen Sekunden vollständig. Je kleiner die Beleuchtungsrate des Kristalls ist, desto länger waren die Oberflächenladungen nachweisbar. Der Zerfallsprozess k_2 beruht auf intrinsischen Transportprozessen. Hierbei wird davon ausgegangen, dass die negativen Oberflächenladungen durch Elektronen nahe der Oberfläche gebildet werden. Die positiven Oberflächenladungen sind Löcher im Valenzband, die durch Elektronen-Ionen-Rekombination entstehen.
Aktive Immunisierung mit GD2-Peptidmimotopen und anti-Idiotypen zur Immuntherapie beim Neuroblastom

Das Neuroblastom ist der häufigste solide, extrakranielle Tumor des Kindesalters. Die schlechte Prognose von Patienten mit fortgeschrittener Erkrankung macht die Entwicklung neuer Therapiemodalitäten zu einem der wichtigsten Forschungsziele auf dem Gebiet der pädiatrischen Onkologie. Durch die passive Immuntherapie mit Antikörpern gegen das Glykolipidantigen Disialogangliosid GD2, welches von Neuroblastomen hoch exprimiert wird, konnte die Überlebensrate von Hochrisiko-Neuroblastompatienten zuletzt deutlich verbessert werden. Dennoch ist die passive Immuntherapie mit Nachteilen verbunden: Das Ausbleiben einer langfristigen Immunität erfordert repetitive Antikörpergaben mit akuten antikörperbezogenen Nebenwirkungen und dem Risiko, eine humorale Immunreaktion gegen den applizierten Antikörper zu entwickeln. Eine aktive GD2-gerichtete Immunisierung erscheint daher vorteilhaft. Die schwache Immunogenität des Glykolipids ist hierbei ein wesentliches Hindernis bei der Induktion einer effektiven GD2-gerichteten Immunität. Dieses Problem kann durch die Verwendung von Proteinantigenen wie GD2-Peptidmimotopen oder anti-Idiotyp-Antikörpern umgangen werden. Zunächst beschreibt diese Arbeit die Charakterisierung von zwei GD2-Peptidmimotopen („MA“ und „MD“), welche durch Screening von Phagenbibliotheken identifiziert wurden, sowie den Nachweis der erfolgreichen Induktion einer neuroblastomspezifischen Immunität im syngenen Mausmodell. In einem zweiten Schritt wurden die Peptidmimotope durch Austausch einzelner Aminosäuren in ihrer Affinität zu GD2-Antikörpern optimiert und das so geschaffene neue Peptidmimotop („C3“) im Hinblick auf die Induktion einer humoralen GD2-spezifischen Immunität erfolgreich im Mausmodell getestet. Zudem gelang es, einen neuen monoklonalen GD2-anti-Idiotyp-Antikörper („Ganglidiomab“) zu erzeugen. Ganglidiomab weist typische Eigenschaften eines anti-Idiotypen wie die kompetitive Inhibition der Bindung von GD2 an GD2-Antikörper auf und erwies sich im Mausmodell als wirksam bei der Induktion einer GD2-spezifischen humoralen Immunantwort. Das optimierte GD2-Peptidmimotop C3 und der neue monoklonale GD2-Anti-Idiotyp Ganglidiomab bilden somit eine Basis zur weiteren Entwicklung einer wirksamen und sicheren Vakzine zur Behandlung von Hochrisiko-Neuroblastompatienten.
Im Rahmen dieser Arbeit standen Kulturüberstände von je 20 S. aureus-Stämmen von gesunden Probanden und von an einer Sepsis erkrankten Patienten zur Verfügung. Die verwendeten Kulturüberstände waren in Vorarbeiten unter anderem hinsichtlich ihrer Superantigeneigenschaften gut charakterisiert worden. Mittels eines Assays zur Bestimmung der procoagulatorischen Aktivität von Monozyten konnte nachgewiesen werden, dass S. aureus-Überstände konzentrationsabhängig eine TF-Aktivierung auf Monozyten induzieren. Dieser Effekt war nicht davon abhängig, ob es sich um ein Isolat gesunder Spender oder erkrankter Patienten handelte, auch die Menge an produziertem FXa durch einen Stamm aus Rachenabstrichen oder Blutkulturen unterschied sich nicht. Monozyten verschiedener Spender reagierten unterschiedlich auf den gleichen Kulturüberstand. Die Superantigen-Eigenschaften der Kulturüberstände nahmen keinen Einfluss auf die prokoagulatorische Aktivität von Monozyten. Sechs Kulturüberstände waren nicht in der Lage eine PCA zu induzieren, daher erfolgten verschiedene Untersuchungen zu Ermittlung der Zellvitalität. Im MTT-Test zeigte sich ein konzentrationsabhängiger zytotoxischer Effekt der Überstände, allerdings betrug der Anteil vitaler Zellen stets über 60 %. Ergänzende durchflusszytometrische Messungen konnten jedoch zeigen, dass Monozyten teilweise nur noch sporadisch nachweisbar waren. Um die prokoagulatorischen Eigenschaften der Sekretionsprodukte von S. aureus genauer zu charakterisieren, kamen der Laborstamm RN6390 und seine isogenetischen agr(-)- und sar(-)-Mutanten zum Einsatz. Der Wildtyp RN6390 sowie dessen Mutanten induzierten in hohen Konzentrationen eine geringere Faktor Xa-Generierung auf Monozyten als nur mit Medium behandelte Zellen. Während sich durch den Wildtyp und die agr(-)-Mutante auch in höheren Verdünnungsstufen keine PCA von Monozyten induzieren ließ, war dies mit der sar(-)-Mutante möglich. Dieses Aktivierungsmuster ließ sich auf ausgeprägte zytotoxische Eigenschaften zurückführen, sodass es nicht möglich war, den für die Faktor Xa-Generierung verantwortlichen Faktor genauer zu charakterisieren. Als eine mögliche Monozyten aktivierende Komponente untersuchten wir Peptidoglykan im Faktor Xa-Assay. Bereits geringste Konzentrationen von Peptidoglykan konnten eine PCA induzieren. In den Kulturüberständen selbst konnten wir PG semiquantitativ nachweisen. Ob Peptidoglykane allein oder in Synergie mit anderen Sekretionsfaktoren von S. aureus die TF-Aktivierung auslösen, ließ sich mit dieser Arbeit nicht abschließend klären.
Aktivierung von humanen Thrombozyten durch Staphylococcus aureus und seine Sekretionsprodukte

Die Ansichten über die Funktion der Thrombozyten unterliegen derzeit einem Wandel. Neben dem Verschluss von Endothelläsionen spielen sie eine Rolle in der Pathologie der Artherosklerose, der Endokarditis und weiterer Erkrankungen und es existieren Hinweise auf eine Beteiligung bei der Immunantwort über die Expression von CD40L (Elzey et al., 2003). Ihre übermäßige Aktivierung und damit ihr Verbrauch bei der DIC sind noch nicht endgültig aufgeklärt. Zu dieser schwerwiegenden Komplikation im Gerinnungssystem kann es unter anderem durch eine Bakteriämie bzw. Sepsis mit dem grampositiven Erreger Staphylococcus aureus kommen, der in jüngster Zeit durch eine weit reichende Resistenzentwicklung in den Mittelpunkt der Forschung gerückt ist. Die Sepsis ist mit einer Letalität von 30 – 50% trotz modernster Intensivmedizin weiter ein ernstes Problem und grampositive Erreger wie S. aureus verursachen einen großen Anteil der Fälle (Moerer et al., 2004). Die Interaktion von Thrombozyten und S. aureus wurde bisher immer unter dem Aspekt des direkten Kontakts dieser Zellen untersucht, jedoch scheint auch eine Betrachtung der Sekretionsprodukte und ihrer Auswirkungen auf Thrombozyten wegweisend, denn nicht immer ist bei Auftreten von septischen Gerinnungskomplikationen eine Bakteriämie nachweisbar. In dieser Arbeit sollte daher die thrombozytenaktivierende Wirkung von Sekretionsprodukten von S. aureus untersucht werden, es sollte eine Annäherung an die Identität dieser Substanzen erfolgen und es sollte geklärt werden, ob der menschliche Organismus Abwehrmechanismen gegen diesen spezifischen Angriffsweg besitzt. Dazu wurden 20 kommensale und 20 invasive (aus Blutkulturen gewonnene) Isolate von S. aureus sowie die Laborstämme RN6390, RN6911 (RN6390Δagr) und ALC136 (RN6390ΔsarA) bis zur stationären Wuchsphase kultiviert und ihr zellfreier Kulturüberstand untersucht. Das Sekretom von RN6390 und seiner regulatordefizienten Mutanten ist bekannt (Ziebandt et al., 2001). Die Thrombozytenaggregation wurde größtenteils in einem einfachen Funktionstest mit gewaschenen Thrombozyten gemessen. Weitere Tests wurden mit gewaschenen Erythrozyten durchgeführt. 60% der Isolate sezernieren thrombozytenaggregationsauslösende Substanzen, dabei ist die Herkunft des Isolats (kommensal oder invasiv) und die im Kulturüberstand enthaltene Gesamtproteinmenge nicht entscheidend. 35% der Isolate sezernieren ausreichend hämolysierende Substanzen, dies ist ebenfalls unabhängig von ihrer Herkunft. Die zellaktivierenden Substanzen waren hitzeempfindlich, daher konnten Zellwandbestandteile wie Lipoteichonsäure oder Peptidoglykane als Verursacher ausgeschlossen werden. Mittels PCR wurden die genetischen Anlagen der verschiedenen Hämolysine der Isolate untersucht. Die thrombozytenaggregierende und hämolysierende Wirkung von alpha-Toxin konnte in dieser Arbeit nachvollzogen werden (Bhakdi et al., 1991). Weiterhin konnte nachgewiesen werden, dass verschiedene Protease-Typen von S. aureus an der Thrombozytenaggregation beteiligt sind. Diese Ergebnisse konnten bei der Untersuchung von RN6390 und seiner Mutanten bestätigt werden. Die aggregationsauslösenden Substanzen sind positiv durch den globalen Genregulator agr reguliert. Eine Koinkubation mit Serum, aus Serum gewonnenen IgG und IgG-Präparationen führte zu einer konzentrationsabhängigen Hemmung der Aggregation ausgelöst durch bakterielle Kulturüberstände. Insgesamt handelt es sich bei der durch Sekretionsprodukte von S. aureus ausgelösten Thrombozytenaggregation um ein multifaktorielles Geschehen, welches zur Pathogenese der Gerinnungsstörungen bei einer Sepsis oder Infektion beitragen kann. Die Aggregation kann durch im Serum enthaltene IgG in vitro gehemmt werden. In weiteren Untersuchungen muss die genaue Identität dieser Antikörper gefunden werden. Dies kann als neuer Ansatzpunkt in der Behandlung der Gerinnungskomplikationen während eine S. aureus- Sepsis dienen, zusätzlich zu bisherigen Strategien der Substitution und medikamentösen Intensivtherapie.
Als Folge eines akuten Myokardinfarktes kommt es zu einem Anstieg der intrazellulären Ca2+-Konzentration assoziiert mit einer Aktivierung der kardialen Cysteinproteasen Calpain I und II. Diese scheinen in der Pathogenese von Gewebeschäden und Nekrose nach Myokardinfarkt eine wesentliche Rolle zu spielen. Darüber hinaus gibt es Hinweise auf eine Beteiligung von Calpain I bei kardialen Umbauprozessen nach Infarkt. Calpaininhibitoren könnten daher in der Therapie des Myokardinfarkts erfolgreich eingesetzt werden. Unter den Bedingungen einer kardialen Ischämie konnte bei einigen dieser Substanzen eine kardioprotektive Wirkung demonstriert werden. Im Rahmen der vorliegenden Dissertation wurde ein Aktivitätsprofil der beiden Calpainformen im infarzierten und nicht-infarzierten Myokard erstellt und die Auswirkungen des Calpaininhibitors CAL 9961 in vitro und in vivo untersucht. Hierzu wurde an Ratten durch permanente Ligation der linken Koronararterie ein akuter Myokardinfarkt induziert. Ein, 3, 7 und 14 Tage nach Infarkt wurden Calpain I und II aus Gewebehomogenaten von interventrikulärem Septum (IS) und linker, freier Ventrikelwand (LVFW) mittels Chromatographie auf DEAE-Sepharose getrennt. Die Aktivität wurde in scheinoperierten und Infarkt-induzierten Tieren mit chronischer Placebo- oder CAL 9961-Therapie mithilfe eines Enzymassays mit synthetischem Substrat gemessen. Die Therapie wurde 3 Tage vor Infarktinduktion begonnen. Calpain I-Aktivität erreichte 14 Tage nach Infarkt höchste Werte im nicht-infarzierten Myokard (IS), während die maximale Calpain II-Aktivität 3 Tage nach Infarktereignis im infarzierten Herzgewebe (LVFW) gemessen wurde. In Experimenten in vitro hemmte CAL 9961 beide Calpainformen vollständig. In vivo verhinderte eine chronische Therapie mit CAL 9961 bei Tieren mit Myokardinfarkt teilweise den Anstieg der Calpain I-Aktivität im nicht-infarzierten Gewebe (IS) und reduzierte die Calpain II-Aktivität im infarzierten Myokard (LVFW) auf das Level scheinoperierter Tiere. Die in dieser Dissertation erbrachten Ergebnisse zeigen, dass die kardialen Calpaine I und II nach einem akuten Myokardinfarkt aktiviert werden, sich ihre Aktivierung jedoch innerhalb des Myokards zeitlich und regional unterscheidet. Die chronische Inhibition dieser Enzyme könnte die Calpain-vermittelten Gewebeschäden limitieren und zur Erhaltung der strukturellen Integrität des Myokards nach Infarkt beitragen.
Zusammenfassung
Die Verordnung einer Stressulkusprophylaxe im Rahmen stationärer Aufenthalte ist im medizinischen Alltag weit verbreitet. Die Verschreibungspraxis einer SUP wurde bisher vor allem im Bereich der Intensivmedizin und der Inneren Medizin untersucht [33, 40, 42, 45, 46]. Allerdings konnte gezeigt werden, dass eine stressulkusbedingte gastrointestinale Blutung unter stationären Bedingungen ein sehr seltenes Ereignis darstellt [12,17]. Im Bereich chirurgischer, nicht kritisch erkrankter Patienten gibt es bislang nur wenige Studien zur aktuellen Praxis der Verschreibung einer SUP [16, 46].
Um die aktuelle Verschreibungspraxis einer SUP in einem chirurgischen Patientenkollektiv beschreiben und bezüglich des Vorliegens einer Indikation nach aktuellen Leitlinien charakterisieren zu können, wurden retrospektiv von Januar bis Juni 2016 Patientendaten an der Universitätsmedizin Greifswald erhoben und ausgewertet [49]. Des Weiteren erfolgte eine Untersuchung der landesweiten Verschreibungspraxis einer SUP auf der Basis einer Befragung der Abteilungsleiter und Mitarbeiter chirurgischer Abteilungen der Akutkliniken in Mecklenburg-Vorpommern [50].
Die Auswertung der retrospektiv erhobenen Patientendaten zeigte eindeutig, dass im weit überwiegenden Anteil der Patienten mit neu verordneter SUP keine Risikofaktoren für die Ausbildung von Stressulzera und stressulkusbedingten Blutungen vorlagen [49]. In 85.7-99.6 % der Patienten mit SUP konnte die Indikation für die Gabe nicht nachvollzogen werden [49].
Die Befragung chirurgisch tätiger Ärzte in Mecklenburg-Vorpommern zeigte, dass die Verschreibung der SUP nur in einer Minderzahl der Kliniken durch eine SOP geregelt war [50]. In den Kliniken mit vorhandener SUP war deren Inhalt nur einer Minderheit der klinisch tätigen Ärzte inhaltlich bekannt [50]. Eine kritische Reevaluation der Indikation einer SUP erfolgte nicht regelhaft bei Verlegung von Intensiv- auf die Normalstation oder bei Entlassung [50].
Die vorliegende Untersuchung zeigt, dass bei chirurgischen Patienten auch heute noch eine massive Überverschreibung einer medikamentösen Stressulkusprophylaxe zur medizinischen Routine gehört [49, 50]. Dabei scheint von den behandelnden Ärzten das Risiko eines Auftretens stressulkusbedingter Blutungen massiv überschätzt beziehungsweise den Patientengruppen nicht regelrecht zugeordnet werden zu können [49, 50]. Angesichts der Tatsache, dass den zur SUP verwendeten Medikamente eine zunehmend größere Zahl unerwünschter Nebenwirkungen (bakterielle Gastroenteritis, akute interstitielle Nephritis, Vitamin-B12-Mangel, ein erhöhtes Risiko für eine Covid-19-Infektion, ambulant und nosokomial erworbene Pneumonien, Demenzerkrankungen, Osteoporose sowie Elektrolytstörungen) zugeordnet werden kann, sollte die Verschreibung der SUP nach einem festen Indikationsschemas erfolgen und im Laufe des stationären Aufenthaltes wiederholt kritisch hinterfragt werden [23-27].
Die Entwicklung einheitlicher Empfehlungen und Richtlinien zur Handhabung von SUP und die Sensibilisierung chirurgisch tätiger Ärzte im Rahmen von Fort- und Weiterbildungen erscheint somit, angesichts der erarbeiteten Daten, dringend notwendig zu sein.
Ziel dieser Arbeit war es, den prognostischen Wert neonatal abgeleiteter FAEP für das Outcome im korrigierten Alter von 12-15 Monaten bei Früh- und Neugeborenen mit perinatalen Risikofaktoren zu ermitteln. Im Weiteren sollte die Beobachtung "besserer" neonataler FAEP bei Frühgeborenen unter 35 SSW mit späteren Entwicklungsstörungen überprüft werden. Von den 226 untersuchten Kindern zeigten 179 (79,2%) ein normales neuromotorisches Outcome mit 12 Monaten, 31 (13,7%) eine leichte bis mittelschwere ZKS und 16 (7,1%) eine schwere ZKS bis hin zu einer spastischen Bedrohung. Der Chiquadrat-Test zeigt für bestimmte klinische Parameter einen hoch signifikanten Zusammenhang (p<0,001) in Bezug auf die motorische Entwicklung mit 12 Monaten, anhand der Kontingenzkoeffizienten kann man in dieser Gruppe eine Wertung vornehmen: Die Entwicklung eines Hydrozephalus (0,533) hat die größte Aussagekraft auf die spätere motorische Entwicklung, vor dem motorischen Status mit 3 Monaten (0,451), den intracraniellen Hämorrhagien (0,408), dem Microcephalus (0,371), der Schwangerschaftsdauer (0,369), der periventrikulären Leukomalazie (0,337), dem Geburtsgewicht (0,325) und dem Auftreten cerebraler Krampfanfälle (0,287). Für die neuromotorische Prognose mit Hilfe der FAEPs läßt sich bei den reifen Neugeborenen ein grenzwertig signifikanter Zusammenhang (p=0,05) erkennen, bei den Frühgeborenen unter 35 SSW dagegen kann keine Prognose mit Hilfe der FAEPs gemacht werden. Abschließend wurde anhand von matched pairs die Interpeaklatenzen I-III und I-V verglichen. Dabei wurde jedem Kind aus der Gruppe mit pathologischer motorischer Entwicklung (MOT12-2) ein Partner mit gleicher Schwangerschaftsdauer, gleichem Alter bei FAEP-Ableitung und normaler Entwicklung (MOT12=0) zugeordnet. Hierbei trat zwar wiederum ein minimaler Trend zu kürzeren IPL auf, er erreichte jedoch keine statistische Signifikanz.
Bei der chronisch obstruktiven Lungenerkrankung handelt es sich um eine pulmonale Erkrankung mit persistierender respiratorischer Symptomatik. Für die Erkrankung pathognomonisch ist eine Atemflussminderung aufgrund einer progressiven Atemwegsobstruktion. Trotz individueller Krankheitsverläufe und verschiedenen klinischen Phänotypen resultiert häufig eine chronisch respiratorische Insuffizienz mit obstruktiver Ventilationsstörung, welche sich limitierend auf die Überlebenszeit auswirkt und die Wahrscheinlichkeit für wiederholte Hospitalisierungen erhöht. Die nichtinvasive Beatmung stellt seit Jahren eine etablierte und anerkannte Therapieoption der akuten respiratorischen Insuffizienz bei COPD-Patienten da. Weniger eindeutig verhält sich jedoch die Studienlage bezüglich der Etablierung einer langfristigen außerklinischen NIV-Therapie bei COPD-Patienten mit chronisch respiratorischer Insuffizienz. Kurz- und Langzeiteffekte dieser Therapieoption bei chronisch respiratorischer Insuffizienz wurden in den letzten 20 Jahren nur in wenigen Publikationen untersucht und es konnte kein Vorteil hinsichtlich des Überlebens von Patienten mit Überleitung auf eine LTH-NIV-Therapie gezeigt werden. Ziel dieser retrospektiven Studie war es, die Auswirkungen der NIV-Therapie auf das Langzeitüberleben und die Rehospitalisierung zu untersuchen und mögliche Einflussfaktoren bzw. Prädiktoren auf diese zu identifizieren. Zur Beantwortung der Fragestellung wurden 151 Patienten in die Studie eingeschlossen, welche sich in dem Zeitraum vom 01.01.2012 bis 30.06.2019 aufgrund einer akuten nicht-invasiven Beatmungsnotwenigkeit bei AECOPD in den Abteilungen für innere Medizin der UMG in Behandlung befanden. Vom Gesamtkollektiv wurden 83 Patienten im Anschluss an den Indexkrankenhausaufenthalt direkt auf eine LTH-NIV-Therapie eingestellt. Die Non-NIV-Gruppe bildeten 68 Patienten, welche ohne Überleitung auf eine LTH-NIV-Therapie nach dem Indexaufenthalt aus dem Krankenhaus entlassen wurden. Über ein Follow-up von zwei Jahren wurden Blutgasanalysen, Beatmungsparameter, Rehospitalisierungen und die Mortalität betrachtet. Es wurden eine Überlebenszeitanalyse nach der Kaplan-Meier-Methode sowie eine Cox-Regression zur Analyse mehrerer Einflussgrößen auf das Überleben im Gruppenvergleich durchgeführt.
In der vorliegenden retrospektiven Datenauswertung von COPD-Patienten mit akuter nicht-invasiver Beatmungsnotwendigkeit im Rahmen einer AECOPD, konnte gezeigt werden, dass die Einstellung auf eine LTH-NIV-Therapie das Überleben in einem zweijährigen Beobachtungszeitraum signifikant (p=0,027) verbessert. Das 24-Monats-Risiko einer Rehospitalisierung lag in der LTH-NIV-Gruppe bei 41% gegenüber 55,9% in der Non-NIV-Gruppe. Dieses Ergebnis verfehlte nur knapp die statistische Signifikanz (p=0,053). In der statistischen Testung ergab sich ein verringertes Mortalitätsrisiko bei den Patienten, die im direkten Anschluss an eine AECOPD auf eine LTH-NIV-Therapie übergeleitet wurden [HR: 0,37 (0,17; 0,79)].
In dieser Dissertation wurden Transient Severe Motion (TSM) Artefakte in der mit Gadoxetat kontrastierten Magnetresonanztomographie (MRT) der Leber untersucht. Dieses Phänomen beeinträchtigt die Diagnostik erheblich. Erforscht wurde die Häufigkeit der TSM-Artefakte, der Einfluss des genauen Zeitabschnitts der bildlichen Erfassung der arteriellen Kontrastierungsphase auf ihr Auftreten sowie Risikofaktoren dafür. 354 Patienten, die von 2013 bis 2016 am Institut für Diagnostische Radiologie und Neuroradiologie der Universität Greifswald untersucht wurden, gingen in die retrospektive Studie ein. 69 dieser Patienten erhielten eine Folgeuntersuchung.
Aufnahmen der arteriellen Phase wurden hinsichtlich der TSM-Artefakte auf einer vierstufigen Skala nach Schweregrad eingeteilt (0=keine TSM, 1=leichte TSM, 2=mäßige TSM, 3=schwere TSM). Das Auftreten von TSM-Artefakten war erheblich vom genauen Zeitabschnitt der bildlichen Erfassung der arteriellen Phase (früharteriell, vollarteriell, spätarteriell) sowie eventueller TSM-Artefakte in Voruntersuchungen und verschiedenen Risikofaktoren abhängig.
TSM-Artefakte traten bei 48,59% der untersuchten Patienten auf (172/354). Bei 22,88% aller Patienten (n=81) traten TSM-Artefakte des Grades 1 auf. Bei 18,36% (n=65) wurden TSM-Artefakte des Grades 2 festgestellt. Bei 7,34% (n=26) kam es zu TSM-Artefakten des Grades 3.
In der vollarteriellen Phase kam es mit 58,8% der Untersuchungen am häufigsten zu TSM-Artefakten. In der früharteriellen Phase (51,6%) sowie der spätarteriellen Phase (42,1%) traten die Artefakte signifikant seltener auf (p=0,031). Das Auftreten von TSM-Artefakten korrelierte signifikant mit dem BMI (p=0,001) sowie grenzwertig mit Leberzirrhose (p=0,05). TSM-Artefakte der verschiedenen Schweregrade korrelierten signifikant mit Gewicht (p=0,03), Größe (p=0,033), BMI (p=0,003) sowie γ-GT (p=0,029). Bei Folgeuntersuchungen zeigte sich ein signifikanter Zusammenhang zwischen TSM-Artefakten in der Erstuntersuchung und TSM-Artefakten in Folgeuntersuchungen (p=0,041).
In dieser Studie konnte gezeigt werden, dass die geringste Wahrscheinlichkeit für TSM-Artefakte in der spätarteriellen Phase besteht. Daher ist bei der Diagnostik der mit Gadoxetat kontrastierten Leber-MRT die spätarterielle Phase zu empfehlen.
Die Herzinsuffizienz mit eingeschränkter LVEF führt zu einer Reduktion des aortalen AIx in Ruhe. Wie sich die zentralen Parameter bei diesen Patienten unter Belastung verändern, ist bisher wenig untersucht. Probanden mit einer linksventrikulären Pumpfunktion ≤ 40 % und NYHA ≥ 2 und ohne Vorhofflimmern (CHF) wurden mit Kontrollprobanden ohne Herzinsuffizienz (Con) verglichen, die für Alter, Geschlecht und Nierenfunktion individuell gematcht wurden. Eine oszillometrische Pulswellenanalyse (Mobil-O-Graph®) wurde im Liegen, im Sitzen und über die Dauer von zehn Minuten nach einer spiroergometrischen Ausbelastung durchgeführt. In Einklang mit der Studienlage sahen wir einen niedrigeren AIx bei der CHF-Gruppe vs. der Con-Gruppe im Liegen in Ruhe (28,4 ± 3,4 % vs. 19,9 % ± 2,8; p < 0,05). In sitzender Position ließ sich dies jedoch nicht reproduzieren. Unmittelbar nach der Belastung war die Pulswellengeschwindigkeit in der Con-Gruppe signifikant höher als in der CHF-Gruppe (9,09 ± 0,25 m/s vs. 8,45 ± 0,29 m/s, p < 0,05). Der AIx stieg gegenüber dem Ruhewert bei der CHF-Gruppe signifikant an und war im Vergleich zur Con-Gruppe höher. Ursächlich waren Unterschiede in der Amplitude der reflektierten Welle, welche in der Kontrollgruppe nach der Belastung im Vergleich zur Ruhemessung abnahmen, in der CHF-Gruppe jedoch nicht. Die Belastungsintoleranz bei herzinsuffizienten Patienten konnte möglicherweise durch den erhöhten AIx erklärt werden. Ursächlich hierfür war ein Anstieg der Amplitude der reflektierten Welle. Die vorliegende Arbeit liefert eine Einsicht in die komplexen hämodynamischen Auswirkungen von körperlicher Belastung bei Patienten mit Herzinsuffizienz
Mit weltweit potenziell 5,3 Millionen Todesopfern pro Jahr ist die Sepsis eine der häufigs-ten Todesursachen3. Ursprung dieser ist eine Dysregulation einer immunologischen Reak-tion mit Inflammation und daraus folgenden Organschäden9,10. Der Nutzen von Albumin in der supportiven Therapie dieser lebensbedrohenden Erkrankung wird seit längerem disku-tiert. Aus diesem Grund befasst sich die hier vorliegende Arbeit mit dem Zusammenhang zwischen der Albuminsubstitution und Letalität der schweren Sepsis und des septischen Schocks. Als Grundlage dienten die Patient*innendaten aus der intensivmedizinischen Sepsisdatenbank der Universitätsmedizin Greifswald, im Zeitraum von 2010 bis 2015. Zu-nächst wurden die Patient*innen in zwei Gruppen eingeteilt. Die Patientin*innen der „Gruppe A“ erhielten kein Albumin und die Patient*innen der „Gruppe B“ bekamen Albumin substituiert. Anschließend erfolgte die Unterteilung je nach Ausprägung der Hypalbuminä-mie, bemessen am niedrigsten Serumalbuminwert, in vier Subgruppen (Gruppe 1 A/B bis 4 A/B). In einer weiteren Unterteilung, anhand einer messbaren Erhöhung des Serumal-bumins nach Substitution, wurden die Patient*innen der „Gruppe B“ in „Responder“ und „Nonresponder“ gruppiert.
Insgesamt konnten dadurch 701 Patient*innen in die Studie eingeschlossen werden. Von diesen waren 258 weiblich (36,8 %) und 443 männlich (63,20 %). Entsprechend lag das akkumulierte mittlere Erkrankungsalter aller Patient*innen bei 67,93 ± 12,6 (MW ± SD) Jahren. In der Gesamtheit betrachtet war der septische Schock mit 76,03 % (n = 533) häufiger vertreten als die schwere Sepsis mit 23,97 % (n = 168). Der „APACHE II Score“ der Gesamtpopulation lag im Mittel bei 20,19, was einem Mortalitätsrisiko von rund 40 % entspricht. Die Patient*innen, die kein Albumin substituiert bekamen, wiesen ein 30 % höheres Risiko auf innerhalb der ersten 28 Tage nach Sepsisdiagnose zu versterben (Fisher-Exact-Test: p = 0,0279; KI: 1.019 - 1.257). Die statistische Betrachtung der Pati-ent*innen mit besonders niedrigen Albuminwerten (≤ 15 g/l) zeigte, dass Patient*innen ohne Substitution ein 56 % höheres Risiko hatten in den ersten 28 Tagen zu versterben. Im Vergleich der Gruppen bezüglich ihrer 90-Tage-, Intensiv- und Krankenhaussterblich-keit ergab sich keinen signifikanten Unterschied. Bei den Gruppen B 2-4 mit Albuminkon-zentrationen über 15 g/l konnte ebenfalls, im Vergleich der Letalität, keine Unterschiede ausgemacht werden.
Grundsätzlich wurde anhand der vorliegenden Daten aufgezeigt, dass die Substitution von Albumin mit einer reduzierten Letalität in den ersten 28 Tagen assoziiert ist. Dies gilt für alle Patient*innen mit einer Albumintherapie und insbesondere für Patient*innen mit einem Albuminwert unter 15 g/l. Fraglich bleibt jedoch, ob eine alleinige Albuminsubstitution eine Letalitätsreduktion bedingt. Allerdings konnte die Hypalbuminämie als unabhängiger Risi-kofaktor für eine erhöhte Letalität in der Sepsis bestätigt werden.
Abschließend kann gesagt werden, dass die durch diese Studie gewonnenen Ergebnisse die aktuellen Leitlinien zur Diagnostik und Therapie der Sepsis unterstützen7. Um aller-dings genauere Aussagen über den Einfluss von Albumin in Bezug auf die Letalität in der Sepsis machen zu können, sollten Studien unter kontrollierten Bedingungen und unter der Einbeziehung einer größeren Studienpolulation durchgeführt werden.
Background: Alcohol consumption accounts for a high burden of disease. The general population of West Pomerania has been characterized as a population at risk with a high prevalence of behavioural risk factors such as alcohol risk drinking. This is reflected by the high proportion of patients being admitted to general hospitals due to alcohol-attributable diseases. The aims of the present dissertation were (a) to analyze dose-response relations between volume of alcohol drinking and the risk of diseases with different alcohol-attributable fractions (AAF) in general hospital inpatients (study 1); (b) to assess motivation to change drinking behaviour and motivation to seek help for alcohol problems during their hospital stay as well as changes in motivation to change drinking behaviour, motivation to seek help and changes in daily alcohol consumption across time according to diseases with different AAFs (study 2); and (c) to investigate the association of fatty liver disease (FLD) with blood pressure and hypertension in a general population sample and to test for the specific contribution of alcohol consumption to this association (study 3). Methods: For studies 1 and 2, data from 'Early Intervention at General Hospitals', a randomized controlled trial to test the effectiveness of brief intervention for alcohol problem drinking in general hospitals, were used. Study 1 comprised data from 846 inpatients, study 2 comprised data from 294 inpatients aged 18 to 64 years with alcohol problem drinking and alcohol-attributable diseases from four general hospitals in West Pomerania. Hospital diagnoses were classified according to their AAF: (1) diseases wholly attributable to alcohol consumption by definition (AAF=1), (2) diseases partially attributable to alcohol consumption (AAF<1), and (3) diseases with no relation to alcohol consumption or where alcohol consumption has been found to be a protective factor (AAF=0). Study 3 encompassed data from the 'Study of Health in Pomerania', a general population sample of 3191 adults aged 20-79 years. FLD was defined using ultrasound in combination with increased serum alanine aminotransferase levels. Results: Analyses showed that 46.8% of the general hospital inpatients had a disease attributable to alcohol consumption. There was a dose-response relationship between volume of alcohol drinking and the risk of diseases with different AAFs. Inpatients consuming >120 g and inpatients consuming 61-120 g of pure alcohol per day revealed significantly higher odds for diseases with AAF=1 compared to inpatients consuming 31-60 g of pure alcohol per day with odds ratios (OR) of 6.3 (95% CI 3.6-11.3) and 2.9 (95% CI 1.6-5.1), respectively. Regarding diseases with AAF<1, inpatients consuming >120 g of pure alcohol per day had significantly higher odds compared to inpatients consuming 31-60 g of pure alcohol per day (OR 2.0, CI 1.2-3.4). Analyses on motivation to change drinking behaviour and on motivation to seek help at hospitalization revealed that motivation to change drinking behaviour was higher among inpatients with alcohol-attributable diseases than among inpatients without alcohol-attributable diseases (p<.001). Among inpatients with AAF=1, motivation to seek help was higher than among inpatients with AAF<1 and AAF=0 (p<.001). While motivation to change drinking behaviour remained stable within one year after hospitalization in all three AAF groups, motivation to seek help decreased in this time period. The volume of alcohol consumed decreased in all three AAF groups within one year after hospitalization. Data from the general population study revealed that FLD was associated with blood pressure and hypertension at baseline and at five-year examination follow-up. For example, the chance of hypertension at both time points was threefold higher in individuals with FLD (OR 2.8, CI 1.3-6.2; OR 3.1, CI 1.7-5.8, respectively) compared to individuals without FLD. Analyses further revealed that the association of FLD with blood pressure and hypertension was independent of alcohol consumption. Conclusion: The results of the present dissertation provide relevant implications for public health. In view of the high proportion of general hospital inpatients with alcohol-attributable diseases, a screening procedure for problem drinking is needed. Furthermore, appropriate interventions considering the inpatient’s motivational level have to be implemented. The concept of AAFs to classify disease conditions according to their causal relationship with alcohol consumption might be a tool to detect inpatients with problem drinking. The results regarding FLD and its association with blood pressure and hypertension demonstrate that it is important to pay attention to alcohol-attributable diseases in the general population and that alcohol-attributable diseases are associated with subsequent serious sequelae. The results of the present work further indicate that the concept to distinguish between alcoholic and non-alcoholic origin of FLD might be obsolete and should be replaced by a concept that regards FLD as a multifactorial disease condition.
Alcohol dehydrogenases as biocatalysts for the production of enantiomerically pure chiral alcohols

Summary Enantiomerically pure chiral alcohols are key compounds in the production of certain chemicals including pharmaceuticals. Chemical synthesis allows to obtain maximal yield of 50% for one enantiomer ( >50% yield is achievable with chiral catalysts used in chemical synthesis), whereas biosynthesis leads to nearly 100% yield. Hence, expensive and time consuming resolution of racemic mixture can be avoided. Alcohol dehydrogenases are the most popular enzymes used in the chiral alcohols synthesis due to high activity with appropriate aldehydes or ketones. ADHs require a cofactor which has to be regenerated after the conversion of aldehyde/ketone to the respective alcohol. Thereby, different regeneration methods were used in the practical work to compare and choose the better one. R. erythropolis and C. hydrogenoformans alcohol dehydrogenases were chosen based on the literature screening. Each gene was cloned into Xplor2 vector and pFPMT vector. Xplor2 vector was used for the transformation of A. adeninivorans and pFPMT vector was used for the transformation of H. polymorpha. Chemically synthesized alcohol dehydrogenase sequences from R. erythropolis (ReADH) and C. hydrogenoformans (ChADH) were cloned between TEF1 promoter and PHO5 terminator which are components of Xplor2 vector or between FMD promoter and MOX terminator which are genetic elements of pFPMT vector. Moreover, ChADH and ReADH sequences with His-tag encoding sequence at the 5’ or 3’ end were constructed and the most active form of the protein was selected for further studies. ReADH-6H was used for the synthesis of 1-(S)-phenylethanol and ethyl (R)-4-chloro-3-hydroxybutanoate whereas ChADH-6H was used for the production of ethyl (R)-mandelate. ReADH-6H synthesized in A. adeninivorans and H. polymorpha was fully biochemically characterized. The enzymes from the two yeast species showed some differences in their pH and temperature optima, thermostability and activity levels. A-ReADH (A. adeninivorans) and H-ReADH (H. polymorpha) were highly active with the same substrates which were: acetophenone, 4-hydroxy-3-butanone and ethyl 4-chloroacetoacetate for reduction reaction along with 1-phenylethanol and 1,6-hexanediol for oxidation reaction. Recombinant A-ReADH-6H and H-ReADH-6H were synthesized in A. adeninivorans and H. polymorpha, respectively. Both enzymes were used for the synthesis of 1-(S)-phenylethanol and ethyl (R)-4-chloro-3-hydroxybutanoate with the use of substrate-coupled cofactor regeneration system. The enantiopurity of the products was >99%. Moreover, A. adeninivorans whole cell catalyst was also used for the synthesis of both chiral alcohols. BmGDH (Bacillus megaterium glucose dehydrogenase) was co-expressed with ReADH-6H for NADH cofactor regeneration. Comparison between isolated enzymes and permeabilized whole cell catalysts indicate that cell biocatalysts are more suitable for the production of 1-(S)-phenylethanol with 92% of acetophenone being converted in 60 min. However, cells did not show any significant advantage over isolated enzymes in the synthesis of ethyl (R)-4-chloro-3-hydroxybutanoate although the velocity of the synthesis of ethyl (R)-4-chloro-3-hydroxybutanoate was slightly improved using whole-cell catalysts, giving an 80% substrate conversion in 120 min. Recombinant C. hydrogenoformans alcohol dehydrogenase was synthesized in A. adeninivorans and biochemically characterized. Enzyme showed high activity only with one substrate, ethyl benzoylformate. The A. adeninivorans and H. polymorpha cell catalysts synthesizing ChADH and BmGDH (Bacillus megaterium glucose dehydrogenase) were constructed and used in the synthesis of ethyl (R)-mandelate (reduction product of ethyl benzoylformate) with the enantiopurity of the reaction product being >98%. H. polymorpha catalysts were more effective in the synthesis than A. adeninivorans cells. The first were able to convert 93% of ethyl benzoylformate within 180 min and the latter were converting 94% of the substrate within 360 min. Re-use of non-immobilized cells and catalysts entrapped in Lentikat® was performed and the improvement of the stability of immobilized catalysts was reported. Space time yield of 3.07 mmol l-1 h-1 and 6.07 mmol l-1 h-1 was achieved with A. adeninivorans and H. polymorpha cell catalysts, respectively. Alcohol dehydrogenase 1 from A. adeninivorans was analyzed concerning the synthesis of enantiomerically pure chiral alcohols. The enzyme did not synthesize industrially attractive products. However, based on biochemical characterization enzyme plays a role in the synthesis of 1-butanol or ethanol and thereby it is of biotechnological interest.
Alcohol-related somatic disorders are highly prevalent among general hospital inpatients. Alcohol problem drinking can be differentiated into alcohol use disorders (alcohol dependence and alcohol abuse) and three subtypes of drinking above recommended levels (at-risk drinking only [AR], heavy episodic drinking only [HE], at-risk and heavy episodic drinking [ARHE]). The aims of this study were threefold. First, proportions of alcohol problem drinking among general hospital inpatients in a region of north-eastern Germany were estimated (study 1). Second, among individuals with alcohol problem drinking the association between beverage preference and alcohol-related diseases was tested (study 2). Third, subtype differences regarding demographics, alcohol-related variables, motivation to change drinking behaviors, and the risk to develop short-term alcohol dependence among the three subtypes of drinking above recommended levels were analyzed (study 3). The data presented in this dissertation are based on the randomized controlled trial “Early Intervention at General Hospitals”, which is part of the Research Collaboration Early Substance Use Intervention (EARLINT). Study 1 includes a sample of consecutively admitted general hospital inpatients between 18 and 64 years old (n = 14,332). The study adopted a two-stage-sampling approach: (a) screening and (b) ascertainment of alcohol problem drinking. Those who were identified with alcohol problem drinking were asked for written consent for further study participation. This included further baseline assessment, the consent to use routine treatment diagnoses and participation in a follow-up interview 12 months after hospitalization. For study 2, routine treatment diagnoses were provided by hospital physicians for a total of 1,011 men with problem drinking. These diagnoses were classified into three categories according to their alcohol-attributable fractions (AAF): diseases totally attributable to alcohol by definition (AAF=1), diseases partially attributable to alcohol (AAF<1) and diseases with no empirical relationship to alcohol or with a possibly protective effect associated with alcohol (AAF=0). Study 3 was restricted to study participants with drinking above recommended levels (n=425). Study 1: Among all general hospital inpatients, 8.9% were identified with current problem drinking in the following descending order: 5.3% exhibited alcohol use disorders and 3.6% drinking above recommended levels. Higher proportions of problem drinking were found at rural sites compared to urban sites (13.7 vs. 7.5%, p<.001). Study 2: Because of the low proportion of women with alcohol problem drinking the following analyses were restricted to males. Multinomial regression analyses revealed different risks for alcohol-related diseases in relation to beverage preference while controlling for alcohol-associated and demographic confounders. Compared to all other groups, spirits only drinkers had the highest risk for having a disease with AAF>0; e.g., beer only drinkers had lower odds of having a disease with AAF<1 (odds ratio, OR=0.50, 95% confidence interval, CI: 0.27-0.92). Study 3: Men with alcohol use disorder were excluded from the following analysis. At baseline, multinomial logistic regression revealed differences between individuals with AR, HE and ARHE while controlling for age. ARHE was associated with higher odds of having a more severe alcohol problem (OR=2.06, CI: 1.23-3.45), using formal help (OR=2.21, CI: 1.02-4.79), and having a disease with AAF=1 (OR=3.43, CI: 1.58-7.43), compared with AR. In addition, individuals with ARHE had higher odds of taking action to change drinking behaviors (i.e., beginning to implement change) than individuals with HE (OR=2.29, CI: 1.21-4.34) or AR (OR=2.11, CI: 1.15-3.86). At follow-up, individuals with ARHE had higher odds of having alcohol dependence according to the DSM-IV (OR=4.73, CI: 1.01–22.20) compared to individuals with AR. In addition to alcohol use disorders, drinking above recommended levels is a common problem among general hospital inpatients. Thus, the implementation of systematic alcohol screening and brief interventions should be considered. These data suggest an association between beverage preference and alcohol-related diseases. Among hospitalized problem drinkers, spirits only drinkers had the greatest risk of having diseases with AAF>0. Of the three subtypes of drinking above recommended levels, ARHE seems to be particularly problematic because there appears to be an indication of a subclinical diagnosis. To provide adequate intervention, clinical practice should distinguish between the three groups of drinking above recommended levels. Brief alcohol intervention should be tailored to the individual’s motivation to change and to the type of alcohol problem drinking. The effectiveness of such a procedure remains to be evaluated in further studies.
Hohe Aldosteron-Konzentrationen haben einen nicht zu unterschätzenden Einfluss auf kardiovaskuläre Risikofaktoren wie arterielle Hypertonie, Störungen des Glukosemetabolismus, eingeschränkte Nierenfunktion und Fettstoffwechselstörung. In vorausgehenden Studien an speziellen Patientenkollektiven aber auch in klinischepidemiologischen Studien wurde eine Assoziation zwischen der Plasma AldosteronKonzentration (PAC) und bestimmten Komponenten des Lipidmetabolismus beschrieben. Ob dieser Zusammenhang auch in der allgemeinen Bevölkerung, und innerhalb des Referenzbereiches für die PAC besteht ist unklar. Um dies zu beantworten, beschäftigte sich die vorliegende Arbeit mit der Assoziation zwischen der PAC und ausgewählten Lipoproteinen (High density lipoprotein cholesterol (HDL-C), Low density lipoprotein cholesterol (LDL-C), Triacylglyceride, gesamt Cholesterol, non-HDL-C) in der allgemeinen Bevölkerung. Hierfür wurden Daten von 793 Männern und 938 Frauen zwischen 25-86 Jahren genutzt die am ersten Follow-up der Study of Health in Pomerania (SHIP-1) teilnahmen. Die Assoziation zwischen der PAC und den oben genannten Lipoproteinen wurde anhand von multivariablen, linearen Regressionsmodellen erfasst. Diese wurden adjustiert für Geschlecht, Alter, BMI, geschätzter glomerulärer Filtrationsrate (eGFR) und HbA1c. Es zeigte sich, eine statistisch signifikante positive Assoziation von PAC mit LDL-C und non-HDL-C sowie eine inverse Assoziation mit HDL-C.
Erstmalig konnte in dieser Arbeit somit gezeigt werden, dass die PAC im physiologischen Bereich im engen Zusammenhang mit dem Lipidmetabolismus steht. Die beobachteten und bereits aus vorherigen Studien bekannten Assoziationen sind daher nicht auf ein bestimmtes Patientenkollektiv beschränkt, sondern auch in der allgemeinen Bevölkerung vorhanden. Weiterhin bestätigen die Ergebnisse aus SHIP-1 die bereits bekannte inverse Assoziation zwischen der PAC und HDL-C. Die hier beschriebene Assoziation zwischen der PAC und LDL-C sowie dem non-HDL-C wurde bis Dato noch nicht beschrieben.
Background: Alexithymia is a personality trait characterized by difficulties in identifying and describing emotions and associated with various psychiatric disorders. Neuroimaging studies found evidence for morphological and functional brain alterations in alexithymic subjects. However, the neurobiological mechanisms underlying alexithymia remain incompletely understood. Methods: We study the association of alexithymia with cortical correlation networks in a large community-dwelling sample of the Study of Health in Pomerania. Our analysis includes data of n = 2,199 individuals (49.4% females, age = 52.1 ± 13.6 years) which were divided into a low and high alexithymic group by a median split of the Toronto Alexithymia Scale. Cortical correlation networks were constructed based on the mean thicknesses of 68 regions, and differences in centralities were investigated. Results: We found a significantly increased centrality of the right paracentral lobule in the high alexithymia network after correction for multiple testing. Several other regions with motoric and sensory functions showed altered centrality on a nominally significant level. Conclusions: Finding increased centrality of the paracentral lobule, a brain area with sensory as well as motoric features and involvement in bowel and bladder voiding, may contribute to explain the association of alexithymia with functional somatic disorders and chronic pain syndromes.
Hintergrund: Die vorliegende Arbeit setzt sich mit dem Auftreten von Alexithymie bei Zwangspatienten und deren erstgradigen Angehörigen unter Berücksichtigung komorbider Erkrankungen auseinander. Einige Studien beschreiben bereits eine Assoziation zwischen Alexithymie und Zwangsstörung. Unbeantwortet bleibt aber bisher die Frage, ob und in welchem Maße alexithyme Charakterzüge bei Zwangspatienten auf ein familiäres Defizit im kognitiven Verarbeiten und Ausdrücken von Gefühlen zurückzuführen sind. Folgende Hypothesen liegen den Untersuchungen zugrunde: Zwangspatienten zeigen im Vergleich zu gesunden Kontrollprobanden verstärkt alexithyme Züge, ungeachtet vorhandener komorbider Erkrankungen. Auch ihre erstgradigen Angehörigen sind in höherem Maße alexithym als die Angehörigen von Kontrollprobanden. Die TAS-20-Werte sind innerhalb der Familien korreliert. Methode: Aus dem Projekt „German Epidemiologic Network for OCD-Studies“ (GENOS) wurden 82 Zwangsprobanden mit 169 erstgradigen Angehörigen 76 Kontrollprobanden mit 144 erstgradigen Angehörigen gegenübergestellt. Von diesen Probanden lagen die Toronto-Alexithymie-Skala (TAS-20) und das PADUA-Inventory (PI-WSUR) vor. Die direkten Interviews wurden mit der „Schedule for Affective Disorders and Schizophrenia - Lifetime Anxiety for the assessment of DSM-IV diagnoses“ (SADS-LA-IV) und die Fremdbefragungen mit dem „Instrument Family Informant Schedule and Criteria“ (FISC) in den jeweiligen deutschen Übersetzungen durchgeführt. Ergebnisse: Es zeigten sich signifikant erhöhte Alexithymiewerte bei den Zwangsprobanden gegenüber den Kontrollprobanden. Die TAS-20-Werte der erstgradigen Zwangsangehörigen, der Kontrollprobanden und deren erstgradigen Angehörigen wiesen keine signifikanten Unterschiede auf. Das Vorhandensein komorbider Erkrankungen hatte keinen Einfluss auf diese Ergebnisse. In linearen Regressionsanalysen konnte eine signifikante intrafamiliäre Assoziation der TAS-20-Werte in den Kontrollfamilien, nicht aber in den Familien der Zwangsprobanden gefunden werden. Diskussion: Die Zwangsstörung ist eine psychische Erkrankung, die –unabhängig von anderen komorbiden Störungen- mit Schwierigkeiten in der Emotionswahrnehmung und im Emotionsausdruck assoziiert ist. Diese Schwierigkeiten treten lediglich bei den Zwangsprobanden, nicht jedoch bei ihren Angehörigen auf. Hinsichtlich der Alexithymieentwicklung bei Zwangspatienten kann neben psychoanalytischen Ansätzen auch ein lerntheoretischer Ansatz angeführt werden, der die Ursache der Alexithymieentwicklung in familiären Gegebenheiten, nämlich in einem Defizit der Mutter-Kind-Beziehung sieht, was zu einer mangelnden Entwicklung des affektregulierenden Systems führt.
In their idealized forms, enzymes can facilitate complex reactions with extreme specificity and selectivity. Additionally, in this imaginative form, they only require mild reaction conditions, resulting in low energy consumption, and they are biodegradable, efficient, reusable, and sustainable. Unfortunately, this idealized form often deviates significantly from reality, where enzymes are more likely to be associated with marginal stability and low reaction rates, leaving them less than desirable for many industrial applications. As such, if we could master the process of engineering the configuration of a protein towards a given task, the implications could be staggering.
This thesis aims to contribute to the process of protein engineering, mainly how computational tools can be used to make the protein engineering process more efficient and accessible.
Article I explores the current state of the art in machine learning-guided directed evolution and serves as a foundation for Article II, which is a concrete application of these techniques to an engineering campaign. Despite successfully improving overall activity and selectivity, we also observe limitations and constraints within the methodology. Article III then delves into these drawbacks and attempts to lay the foundation for a more generalizable and, more importantly, efficient engineering workflow, balancing the strengths and weaknesses of computational techniques with advances in gene synthesis. We then validated this novel pipeline in Article IV, where we show the potential of this methodology. Article V describes a more standard protein engineering campaign on squalene-hopene cyclases for potentially interesting products in the flavor and fragrance industry. Lastly, Article VI outlines a PyMol plugin for molecular docking.
Die Pflanze Mitragyna speciosa Korth. ist ein Rötegewächs, das vorwiegend in den Sumpfgebieten Thailands und Malaysias vorkommt. Die Pflanze wie auch die Zubereitungen aus den Blättern der Pflanze werden in Thailand als Kratom bezeichnet. Die Blätter dieses Baumes werden traditionell in der Naturheilkunde, aber auch als Rauschmittel verwendet. Die Wirkung der darin enthaltenen Alkaloide, die sich wie der Neurotransmitter Serotonin von der Aminosäure Tryptophan ableiten, werden als einerseits stimulierend und andererseits sedierend/analgesierend beschrieben, was in zahlreichen Studien auch belegt wurde. In der vorliegenden Arbeit werden Methoden zum Auszug von Alkaloiden aus Blattmaterial, zur Extraktion dieser Alkaloide aus diversen Lösungen und zu deren Detektion in einem HPLC/DAD- sowie einem GC/MS-System beschrieben. Weiterhin werden Möglichkeiten zur Identifizierung von Mitragyna-Alkaloiden ohne das Vorliegen von Vergleichssubstanzen gezeigt. Für die Indol-Alkaloide Mitragynin, Mitraciliatin, Speciogynin, Speciociliatin waren für die vier erstgenannten Substanzen Spektren in einer elektronischen Vergleichsbibliothek (NIST05) vorhanden. Das Paynanthein konnte aufgrund seines Fragmentierungsmusters identifiziert werden. Im Falle der übrigen ausgewählten Alkaloide konnten Vorschläge zur Grundstruktur erarbeitet werden. Für diese Substanzen waren keine Bibliotheksspektren als Vergleich verfügbar und auch die Literaturangaben erlaubten keine genaue Identifizierung. Wir schlagen aber aufgrund der Fragmentierungsmuster folgende Zuordnung vor: Speciofolin oder Isomer, Isorhynchophyllin oder Isomer, Mitragynin Oxindol A/B und (Iso-) Corynantheidin. Der Gehalt der identifizierten Alkaloide in 11 verschiedenen Kratomprodukten wurde ermittelt und der so erhaltene Alkaloid-Fingerprint zu den Herkunftsangaben dieser Produkte ins Verhältnis gesetzt. Zwei unbekannte Proben wurden mit diesen Daten verglichen. Als effektivstes Auszugsmittel erwies sich ein 80 %iges Methanol/Wasser-Gemisch, das zur Bestimmung der Alkaloidgehalte verwendet wurde. Die Mitragynin-Gehalte der untersuchten Kratomprodukte lagen zwischen 0,6 und 1,2 %; in einem als „10x-Extrakt“ angebotenen Produkt bei 3,9 %. Die benutzte Extraktionsmethode zeigt eine nahezu vollständige Wiederfindung der betrachteten Alkaloide und erlaubt eine Quantifizierung von Mitragynin in einer Konzentration, die weit unterhalb der von uns angestrebten Nachweisgrenze in Kratomprodukten (0,001 % des Blattmaterials) liegt. Es sollten jedoch auch prinzipiell für jedermann zugängliche Auszugsmethoden zur Anwendung gebracht werden, so dass darüber hinaus Teezubereitungen und Auszüge mit Trinkalkohol gefertigt und untersucht wurden. Der Auszug mit einem 80 %igen Ethanol/Wasser-Gemisch zeigte vergleichbare Ergebnisse zum methanolischen Auszug, wohingegen in den Teezubereitungen eine Ausbeuteverschlechterung und Verschiebung im Alkaloidmuster zu erkennen war. Die hier verwendete Standard-Methode wurde durch die ermittelten Parameter (Spektren, Retentionszeiten, Kalibration) so erweitert, dass eine Quantifizierung von Mitragynin im Routinebetrieb möglich ist. Eine Zuordnung der Produkte zu den angegebenen Herkunftsgebieten anhand des Alkaloid-Fingerprints erwies sich als nur begrenzt möglich. Eine GC/MS-SIM-Methode zur qualitativen Erfassung von Alkaloiden wurde erstellt und kann für weitere Untersuchungen (Quantifizierung) an Körperflüssigkeiten erweitert werden.
Hintergrund: Alkoholassoziierte Morbidität und Mortalität ist ein bedeutender Kostenfaktor im Gesundheitswesen. Daher sind genaue Kenntnisse über den Zusammenhang zwischen Alkoholkonsum und der Inanspruchnahme medizinischer Leistungen von Relevanz für alle Entscheidungsträger im Gesundheitswesen. Studien aus Japan, Kanada, Spanien und den USA zeigen, dass Personen mit riskantem Alkoholkonsum und Personen, die derzeit abstinent leben, seltener medizinische Hilfe in Anspruch nehmen als leichte Alkoholkonsumenten. Ziel dieser Arbeit ist es, diesen beschriebenen U-förmigen oder umgekehrt linearen Zusammenhang in der bundesdeutschen Allgemeinbevölkerung zu bestätigen. Außerdem werden zwei Erklärungsansätze geprüft, welche dem Befund einer höheren Inanspruchnahme medizinischer Leistungen bei abstinent lebenden Personen zugrunde liegen könnten. Methode: Die Daten wurden im Rahmen der Study of Health in Pomerania (SHIP) und des Bundesgesundheitssurvey 1998 (BGS) erhoben und basieren auf Zufallsstichproben der erwachsenen Allgemeinbevölkerung. Die Studienregion der SHIP umfasst die Region Nordost-Vorpommern und erreichte mit N = 4.310 eine Ausschöpfungsquote von 69%. Der BGS umfasste das gesamte Bundesgebiet; N = 7.124 nahmen an der Untersuchung teil (Ausschöpfung 60%). In beiden Querschnittsstudien wurden mittels Fragebögen, computergestützten Interviews und medizinischen Untersuchungen Daten erhoben, welche mit Hilfe multivariabler statistischer Verfahren ausgewertet wurden. Ergebnisse: In der erwachsenen Allgemeinbevölkerung bestand eine höhere Nachfrage medizinischer Leistungen bei abstinent lebenden Personen als bei Konsumenten mit moderatem Alkoholkonsum. Darüber hinaus fand sich ein umgekehrter Zusammenhang zwischen Alkoholkonsum und der Inanspruchnahme medizinischer Leistungen. Zwei Hypothesen für die höhere Inanspruchnahme abstinent lebender Personen wurden untersucht. (1) Ehemalige Risikokonsumenten, die derzeit keinen Alkohol trinken, nahmen mehr ambulante Leistungen wahr als andere Abstinente. (2) Abstinent lebende Personen und Alkoholkonsumenten unterschieden sich hinsichtlich sozialer, lebensstil- und gesundheitsbezogener Risikofaktoren, welche mit einem schlechteren Gesundheitszustand und höherer Inanspruchnahme in Verbindung stehen. Die Berücksichtigung dieser Merkmale als konfundierende Variablen im statistischen Modell verdeutlichte, dass insbesondere alkoholassoziierte Erkrankungen, welche bei Abstinenten häufiger auftraten, für den Befund einer höheren Inanspruchnahme abstinenter Personen verantwortlich sind. Diskussion: Angesichts der vorliegenden Ergebnisse muss davon ausgegangen werden, dass die in internationalen Studien gefundene höhere Inanspruchnahme medizinischer Leistungen auf unberücksichtigte konfundierende Variablen sowie auf eine Fehlklassifikation der ehemaligen Risikokonsumenten zurückzuführen ist. Dieser Befund ist auch für die wissenschaftliche Evidenz zum protektiven Effekte des moderaten Alkoholkonsum von Relevanz, weil in den meisten Studien Konfundierung und Fehlklassifikation bei der Datenanalyse nicht ausreichen Berücksichtigung gefunden haben.
Hintergrund: Das in Händedesinfektionsmitteln enthaltende Ethanol wird während der Anwendung dermal und pulmonal resorbiert. Obwohl sich hieraus keine Gesundheitsgefährdung ableiten lässt, kann eine sich durch die Resorption ergebende grenzwertüberschreitende Konzentration von Ethylglucuronid (EtG) (>100 ng/ml) im Rahmen der gutachterlichen Beurteilung der Einhaltung der Abstinenz rechtlich schwerwiegende Folgen haben. Methode: 33 freiwillige Teilnehmer aus Klinik und Labor führten in einer Arbeitsschicht wiederholt praxisübliche Händedesinfektionen (HD) bei zusätzlicher Alkoholabstinenz (Studienphase 1) und bei erlaubtem Alkoholkonsum (Studienphase 2) durch. Der bei jedem natürlichen Toilettengang gewonnene Urin wurde auf EtG mittels Immunoassay und Flüssigchromatographie mit Massenspektrometrie-Kopplung (LC-MS/MS) sowie auf Ethanol und Acetaldehyd mittels Gaschromatographie (GC-FID) untersucht. Ergebnisse: Während der ersten Studienphase lag die Maximalkonzentration für EtG bei 958 ng/ml (MW 281,4 ng/ml), für Ethanol bei 9,2 mg/l (MW 1,7 mg/l) und für Acetaldehyd bei 0,81 mg/l (MW 0,16 mg/l). Drei Teilnehmer überschritten noch am folgenden Morgen den EtG-Grenzwert mit 149 ng/ml, 131 ng/ml und 120 ng/ml. In Studienphase 2 erreichte die Maximalkonzentration für EtG einen Wert von 5043,0 ng/ml (MW 2.328,8 ng/ml), für Ethanol von 1.020,1 mg/l (MW 110,4 mg/l) und für Acetaldehyd von 1,83 mg/l (MW 0,33 mg/l). Schlussfolgerung: In der Praxis durchgeführte ethanolische HD können grenzwertüberschreitende EtG-Konzentrationen im Urin verursachen, die u. U. noch nach 24 h nachweisbar sind. Daher müssen die Ergebnisse einer positiven EtG-Testung bei Teilnahme an einem Abstinenzprogramm stets im individuellen Kontext betrachtet und die HD ggf. auf ein Propanol basiertes Händedesinfektionsmittel umgestellt werden. Verglichen mit der als gesundheitlich verträglich geltenden oralen Aufnahme von 20 bzw. 10 g Ethanol/d für das männliche bzw. weibliche Geschlecht ist die Aufnahme von Ethanol durch wiederholte HD als unbedenklich zu bewerten.
Die Händedesinfektion als keimzahlreduzierendes Verfahren wird im medizinischen Bereich von einer großen Personenezahl genutzt, um die Übertragung von nosokomialen Infektionen per Handkontakt zu minimieren. In diesem Zusammenhang stellt sich die Frage, inwiefern bei der Durchführung der alkoholischen Händesdesinfektion die Möglichkeit einer dermalen Resorption der Wirkstoffe besteht, und ob bei einer Permeation durch die Haut ein gesundheitliches Risiko zu erwarten ist. Da bisher nur wenige realitätsnahe in-vivo Untersuchungen zur Resorption der Wirkstoffe aus alkoholischen Desinfektionsmitteln durchgeführt worden sind, haben wir in einer Studie mit 12 Probanden und fünf verschiedenen Desinfektionsmitteln die Bedingungen der hygieneischen und der chirurgischen Händedesinfektion simuliert, um anschließend mit geeigneten Blutuntersuchungen die Resorption und den Metabolismus der Alkohole Ethanol, Propan-1-ol und Propan-2-ol darzustellen. Bei jedem Probanden und für alle fünf Präparate ist die Resoprtion der Wirkstoffe reproduzierbar gezeigt worden. Die median erreichten Ethanolkonzentrationen liegen für die hygienische Händedesinfektion je nach verwendetem Präparat zwischen 6,9 mg/l und 20,9 mg/l und für die chirurgische Händedesinfektion zwischen 8,8 mg/l und 30,1 mg/l. Propan-1-ol erreicht bei der hygienischen Händedesinfektion Medianwerte von 6,5 mg/l bzw. 9,2 mg/l. Bei der chirurgischen Händedesinfektion erreicht Propan-1-ol mediane Konzentrationen von 3,2 mg/l bzw. 18,0 mg/l. Die medianen Propan-2-ol-Konzentrationen erreichen bei der hygienischen Händedesinfektion 4,9 mg/l bzw. 5,3 mg/l und bei der chirurgischen Händedesinfektion 5,8 mg/l bzw. 10,0 mg/l. Dabei ist anteilig inkeinem der Fälle mehr als ¼ der auf die Haut aufgetragenen Alkoholmenge resorbiert worden. Im Ergebnis eines Risk Assessment läßt sich schlußfolgern, dass bei bedachtem Einsatz alkoholischer Händedesinfektionsmittel zu keinem Zeitpunkt eine toxische Gefährdung besteht und die Gefahr gesundheitlicher Schäden bei vorschriftsmäßiger anwendung durch den gesunden Nutzer gerin ist.
Mit dieser Arbeit wird eine populationsgenetische Studie für die Y-STR-Systeme DYS 19, DYS385, DYS390, DYS391, DYS392, DYS393, DYS389I, DYS389II und YCAII in der Region Vorpommern vorgelegt. In den 208 Proben aus dem Einzugsgebiet des Greifswalder Institutes für Rechtsmedizin fanden sich im extended haplotype 193 verschiedene Haplotypen. Davon kamen 181 singulär vor, 12 Haplotypen konnten bei mehr als einer Person (bis zu vier) nachgewiesen werden. Im minimal haplotype dagegen wurden 183 verschiedene Haplotypen gefunden, wovon 15 bei mehr als einer Person (bis zu vier) auftraten. Die ermittelte PD (Power of Discrimination) der Haplotypen des minimal haplotype betrug 0,993, die des extended haplotype 0,994. Die PD-Werte der einzelnen STRs betrugen 0,66 in DYS19, 0,73 in DYS390, 0,51 in DYS391, 0,57 in DYS392, 0,33 in DYS393, 0,80 in DYS385, 0,59 in DYS389I, 0,76 in DYS389II, 0,83 in DYS389I/II und 0,74 in YCAII. Diese Ergebnisse zeigen eine weitgehende Übereinstimmung der vorpommernschen Daten mit den Daten anderer mitteleuropäischer Studien.
Humans are exposed to a plethora of microorganisms that reside on outer and inner body surfaces. These are collectively referred to as the human microbiome. The evolutionary relationship between humans and their microbiome is very complex. It is now widely accepted that these microorganisms are not just passive spectators but play an important role in health. The presence or absence of certain microbes is also linked to various diseases, including inflammatory bowel disease, cardiovascular disease, obesity, cancer, and allergies.
Allergies are several conditions caused by a misguided immune response to foreign antigens that are typically harmless. Common allergic diseases include atopic dermatitis (AD), allergic asthma, hay fever, and anaphylaxis. The incidences of allergic diseases are continuously rising, with up to 40% of the human population thought to be sensitised to environmental antigens. This increased incidence is not simply the result of societies becoming more aware and better at diagnosing these diseases. It is believed that the increases in allergies and sensitisation have environmental causes and are related to Western lifestyles. It is known that the rate of allergies is less frequent in developing countries. They are also more likely to occur in urban than rural areas. The prevailing view of the involvement of bacteria in allergies is described by the hygiene hypothesis. The hypothesis claims that decreased exposure to diverse microbial communities early in life increases the risk of developing allergic diseases. There are numerous examples to support this claim. For example, children born and raised in close contact to farm animals or in the presence of pets, and who are thus in direct and constant contact with a complex microbial environment, are protected from allergic diseases. On the other hand, colonisation or infection with certain bacteria increases allergic disease risks. This seems to contradict the hygiene hypothesis.
It appears that the members of the microbiome have different effects on allergy, and the hygiene hypothesis may not apply to every player in the complex microbial diversity that humans are in contact with. Therefore, a better understanding of the host bacterial interaction is required on the level of bacterial species.
This work studies the interplay between bacteria and the immune system to identify and characterise bacterial components with allergenic properties. In this quest, Staphylococcus aureus (S. aureus) and Staphylococcus epidermidis (S. epidermidis) were investigated for their allergenic properties and involvement in different allergic diseases. In the case of S. aureus, evidence is presented on allergic implications for two different components; serine protease-like proteins (Spls) and superantigens (SAg). Furthermore, experimental support is provided on the allergenic properties of the extracellular serine protease (Esp) from S. epidermidis. We argue that stimulating allergic reactions by staphylococci is an immune evasion mechanism that increases the survival chances of the bacteria within the host.
In chapter 1, an introduction is given to both S. aureus and S. epidermidis and their interactions with the immune system. Also, the bacterial components with allergenic properties and allergic diseases with known bacterial involvement are presented. Finally, the question of why bacteria cause allergy is discussed.
Chapter 2 describes allergic reactions to the Spls of S. aureus in a cohort of cystic fibrosis patients. Chapter 3 focuses on the SAgs of S. aureus. SAgs were discovered more than 30 years ago, but their physiological function is still under discussion. In this chapter, the allergenic properties of SAgs and their possible immunological mechanisms are reviewed, and a possible link between SAgs and allergic diseases is discussed. In chapter 4, the focus shifts to S. epidermidis and its involvement in AD. The human immune response to the Esp from S. epidermidis is characterised in healthy and AD individuals. The allergenic properties of Esp imply a detrimental role of S. epidermidis in AD. Finally, chapter 5 summarises and discusses the results of this thesis. In this section, the pieces are put together, and attention is brought back to the question of why bacteria cause allergies.
In cystic fibrosis (CF) infectious and allergic airway inflammation cause pulmonary exacerbations that destroy the lungs. Staphylococcus aureus is a common long-term colonizer and cause of recurrent airway infections in CF. The pathogen is also associated with respiratory allergy; especially the staphylococcal serine protease-like proteins (Spls) can induce type 2 immune responses in humans and mice. We measured the serum IgE levels specific to 7 proteases of S. aureus by ELISA, targeting 5 Spls (76 CF patients and 46 controls) and the staphopains A and B (16 CF patients and 46 controls). Then we compared cytokine release and phenotype of T cells that had been stimulated with Spls between 5 CF patients and 5 controls. CF patients had strongly increased serum IgE binding to all Spls but not to the staphopains. Compared to healthy controls, their Spl-stimulated T cells released more type 2 cytokines (IL-4, IL-5, IL-13) and more IL-6 with no difference in the secretion of type 1- or type 3 cytokines (IFNγ, IL-17A, IL-17F). IL-10 production was low in CF T cells. The phenotype of the Spl-exposed T cells shifted towards a Th2 or Th17 profile in CF but to a Th1 profile in controls. Sensitization to S. aureus Spls is common in CF. This discovery could explain episodes of allergic inflammation of hitherto unknown causation in CF and extend the diagnostic and therapeutic portfolio.
Staphylococcus aureussuperantigens (SAgs) are among the most potent T cell mitogensknown.They stimulate large fractions of T cells by cross-linking their T cell receptor withmajor histocompatibility complex class-II molecules on antigen presenting cells, resulting in Tcell proliferation and massive cytokine release. To date, 26 different SAgs have been described in thespeciesS. aureus; they comprise the toxic shock syndrome toxin (TSST-1), as well as 25 staphylococcalenterotoxins (SEs) or enterotoxin-like proteins (SEls). SAgs can cause staphylococcal food poisoningand toxic shock syndrome and contribute to the clinical symptoms of staphylococcal infection. Inaddition, there is growing evidence that SAgs are involved in allergic diseases. This review providesan overview on recent epidemiological data on the involvement ofS. aureusSAgs and anti-SAg-IgEin allergy, demonstrating that being sensitized to SEs—in contrast to inhalant allergens—is associatedwith a severe disease course in patients with chronic airway inflammation. The mechanisms by whichSAgs trigger or amplify allergic immune responses, however, are not yet fully understood. Viele hämatologische Neoplasien treten vornehmlich in einem Lebensalter deutlich jenseits des 50. Lebensjahres auf. Gleichzeitig sind viele dieser Erkrankungen derzeit nur durch eine allogene Stammzelltransplantation mit kurativem Ansatz behandelbar. Durch die Einführung von intensitätsreduzierten Konditionierungen und der Verbesserung der supportiven Therapie, besonders der antibiotischen und immunsuppressiven Behandlung, wird diese Behandlungsoption für ältere Patienten zunehmend eingesetzt. Im Zeitraum von 1999 und 2011 wurden an der Klinik für Innere Medizin C der Universitätsmedizin Greifswald 91 allogene Stammzelltransplantationen an 86 Patienten durchgeführt, die zum Zeitpunkt der Transplantation 55 Jahre oder älter waren. 85 Patienten bzw. 90 Transplantationen konnten in die Auswertung eingehen. Das mediane Alter lag bei 61 Lebensjahren. Die nach Sorror et al eingestufte Komorbidität erbrachte einen medianen Wert von 3, wobei 50,6% einen Wert von 3 oder höher aufwiesen und damit in die Hochrisikogruppe einzuordnen waren. Das Spektrum der Grunderkrankungen war, einem unselektierten Patientengut entsprechend, heterogen. In 91,1% wurden im Rahmen des Engraftments Leukozyten über 1 Gpt/l und Thrombozyten über 20 Gpt/l erreicht. Eine akute GvHD wurde in einem Drittel der Fälle beobachtet, und zwar in 27,8% nach der Transplantation und 5,6% nach einer DLI-Gabe. Eine chronische GvHD ließ sich in 21,1% der Fälle diagnostizieren, wobei mehr als die Hälfte im Stadium der Limited Disease blieben. Im Beobachtungszeitraum sind 58 Patienten [68,2%] verstorben. 26 dieser Patienten [44,8% der Todesfälle] verstarben aufgrund der Wiederkehr oder Progression der malignen Grunderkrankung. 32 Todesfälle [55,2% der Todesfälle] ereigneten sich in einem erkrankungsfreien Intervall [NRM]. Die Analyse der erhobenen Daten zeigte, dass eine genaue Erhebung des Sorror-Scores über einen Wert von 3 hinaus sinnvoll ist, da auch in diesen Bereichen statistisch signifikante Unterschiede bei Gesamtüberleben und erkrankungs- bzw. progressionsfreiem Überleben zu erhalten sind. Auch bei einem Punktewert von über 3 hinaus sank das Gesamtüberleben und das erkrankungs- bzw. progressionsfreie Überleben statistisch signifikant mit ansteigendem Komorbiditäts-Score. Darüber hinaus konnte ein signifikant längeres Gesamt- und erkrankungs- bzw. progressionsfreies Überleben in Verbindung mit einer chronischen GvHD gezeigt werden. Dies belegt eindrucksvoll die Rolle eines Graft-versus-tumor-Effektes in der Kontrolle der Grunderkrankung. Die Daten zeigen, dass die allogene Stammzelltransplantation für ältere Patienten einsetzbar ist.
Im Rahmen dieser Arbeit wurden neuartige alpha-Phosphanylaminosäuren untersucht. Die Verbindungen wurden durch eine Dreikomponenten-Eintopfreaktion bei Raumtemperatur aus Diphenylphosphan, einem primären Amin und Glyoxylsäure hergestellt. Alle Verbindungen sind luftempfindlich und bilden in Lösung langsam Zersetzungsprodukte. Es wurden P-Sulfide, P-Oxide und P-Pentacarbonylmetall(0)komplexe hergestellt, Versuche zur Synthese von BH3-Addukten in Molverhältnis 1:1 und 1:3 durchgeführt. Das enantiomerenreine 1-(p-methoxyphenyl)ethyl-substituierte Phosphanylglycin wurde als Ligand auf Eignung in enantioselektiven, katalytischen Hydrierungen verschiedener alpha-, beta-ungesättigter Ketoverbindungen untersucht. Sieben Verbindungen aus verschiedenen Gruppen N-substituierter Phosphanylglycine wurden als Liganden mit Ni(COD)2 zu in situ in Katalysatoren umgesetzt und damit die Poly/Oligomerisation von Ethylen untersucht. Die meisten untersuchten Liganden bewirkten hohe katalytische Umsätze von Ethylen und zeigten somit gute Eignung als Liganden zur Stabilisierung aktiver Ni-Oligomerisationskatalysatoren.
Alter und Überlebenswahrscheinlichkeit nach Polytrauma - Local Tailoring des DGU Prognosemodells

Hintergrund: Alter stellt einen von 5 auf der Basis von Daten des Traumaregisters der DGU als unabhängig herausgearbeiteten Prognosefaktoren dar. Wir stellten die Frage, ob das vorgeschlagene Prognosemodell eine ähnlich gute Vorhersagekraft im eigenen Patientengut besitzt. Es wurde zudem untersucht, ob sich das Alter oder vielmehr die altersbedingte Komorbidität prognostisch ungünstig auswirkt. Methoden: Als Datenbasis dienten die in unserem Zentrum im Rahmen des DGU-Traumaregisters prospektiv erfassten Daten von 103 polytraumatisierten Patienten (67 Männer, 36 Frauen, mittleres Alter 35,4±SD 19,0 Jahre, ISS 36,8±10,9). Anhand der miterfassten Nebenerkrankungen erfolgte eine Risikoabschätzung mit der ASA-Klassifikation. Die Rangkorrelation zwischen Alter und ASA wurde nach Spearman ermittelt. Mittels logistischer Regression wurden die prognostische Vorhersagekraft des Originalmodells im eigenen Patientengut mit und ohne ASA-Klassifikation, eventuelle Interaktionen und diskriminatorische Modellfähigkeiten überprüft. Ergebnisse: Die beobachtete Mortalität lag bei 31,7% (95% KI 22,7–41,7%). Es wurden Alter, ISS, GCS und ASA in das finale logistische Modell aufgenommen. Die Odds-Ratios des Originalmodells waren im eigenen Krankengut nahezu identisch zu reproduzieren (OR: Alter 1,048; ISS 1,066; GCS 0,822). Wir fanden eine hochsignifikante Korrelation zwischen Alter und ASA-Schweregrad (rho=0,60, p<0,0001), jedoch keine prognostische Bedeutung der Co-Morbidität. Schlussfolgerungen: Das vorgeschlagene Prognosemodell auf der Basis multizentrisch gewonnener Daten lässt sich mit geringen diskriminatorischen Einbußen auf die Ebene des einzelnen Zentrums übertragen. Hierbei scheint das Alter unabhängig von der Altersmorbidität prognostische Bedeutung zu besitzen.
The experience of abuse in the period of childhood and youth is a key stressor that has con-sequences on the developing brain and is associated with the genesis of mental disorders. Childhood abuse and depression often cooccur together and have both been associated with cortical thickness resulting in a difficulty to detangle the influence of each factor. In prior studies, childhood abuse and depression were inconsistently related to whole-brain cortical thickness. Thus, this thesis aims to investigate the link between childhood abuse, depres-sive symptoms, and alterations of the cortex.
Therefore, this study analyses 1,551 individuals of the general population. A significant in-teraction effect of childhood abuse and depressive symptoms is observed for whole-brain cortical thickness. Yet, the results indicate no influence of childhood abuse or depression alone. A thinner cortex was associated with more severe depressive symptoms in the abused, but not in the non-abused group. In non-depressed participants, an increased whole-brain cortex was found in the abused, compared to the non-abused group. Similar interaction effects were observed in 12 out of 34 cortical regions.
The results suggest, in line with prior findings, that depressed individuals with a history of childhood abuse are a specific ecophenotype which is also reflected in specific brain altera-tions. Cortical regions that are distinct associated with the interaction of depressive symp-toms and childhood abuse are involved in various fields such as sensory processing, self-conception, and memory. Greater cortical thickness in subjects with childhood abuse and without depressive symptoms might act compensatory and thus reflect resilience against depressive symptoms.
Practical implications concern the treatment and diagnostic system as well as the im-portance of early prevention programs. An individualised treatment is necessary as various studies found a less favourable outcome in depressive patients with a history of maltreat-ment. Therefore, it seems urgent to assess experiences of childhood abuse at the beginning of psychiatric and psychotherapeutic treatment. In addition, early prevention programs are in need to support vulnerable family systems and thereby strengthening the economic, health and social system.
Less invasive caries management techniques for treating cavitated carious primary teeth, which involve the concept of caries control by managing the activity of the biofilm, are becoming common. This study aimed to compare the clinical efficacy (minor/major failures) and survival rates (successful cases without any failures) of 3 carious lesion treatment approaches, the Hall Technique (HT), non-restorative caries treatment (NRCT), and conventional restorations (CR), for the management of occlusoproximal caries lesions (ICDAS 3-5) in primary molars. Results at 2.5 years are presented. A total of 169 children (3- to 8-year-olds) were enrolled in this secondary care-based, 3-arm parallel-group, randomised controlled trial. Participants were allocated to: HT (n = 52; sealing caries with stainless-steel crowns without caries removal), NRCT (n = 52; opening up the cavity and applying fluoride varnish), CR (n = 65; control arm, complete caries removal and compomer restoration). Statistical analyses were: non-parametric Kruskal-Wallis analysis of variance, Mann-Whitney U test and Kaplan-Meier survival analyses. One hundred and forty-two participants (84%; HT = 40/52; NRCT = 44/52; CR = 58/65) had follow-up data of 1-33 months (mean = 26). Overall, 25 (HT = 2, NRCT = 9, CR = 14) of 142 participants (17.6%) presented with at least 1 minor failure (reversible pulpitis, caries progression, or secondary caries; p = 0.013, CI = 0.012-0.018; Mann-Whitney U test). Ten (HT = 1, NRCT = 4, CR = 5) of 142 participants (7.04%) experienced at least 1 major failure (irreversible pulpitis, abscess, unrestorable tooth; p = 0.043, CI = 0.034-0.045). Independent comparisons between 2 samples found that NRCT-CR had no statistically significant difference in failures (p > 0.05), but for CR-HT (p = 0.037, CI = 0.030-0.040) and for NRCT-HT (p = 0.011, CI = 0.010-0.016; Kruskal-Wallis test) significant differences were observed. Cumulative survival rates were HT = 92.5%, NRCT = 70.5%, and CR = 67.2% (p = 0.012). NRCT and CR outcomes were comparable. HT performed better than NRCT and CR for all outcomes. In dieser Arbeit wird die Auswirkung des p75NTR-Knockouts auf die Dichte cholinerger Fasern und die Morphologie dendritischer Dornen im Hippocampus altersabhängig untersucht. Der p75-Knockout zeigt eine gesteigerte cholinerge hippocampale Faserdichte bei unveränderte Spinemorphologie. Dabei wird in einer getrennten Knockoutlinie nachgewiesen, dass die Expression des S-p75NTR, einer trunkierten Isoform des p75NTR, diese procholinergen Knockouteffekte altersabhängig verschwinden lässt.
Hintergrund: In dieser Arbeit wurde die Altersabhängigkeit der Gefäßreaktion bei der dynamischen Gefäßuntersuchung (DVA) retinaler Arterien und Venen in einem gesunden Probandenkollektiv untersucht. Die bereits erprobte statische Gefäßuntersuchung (SVA) diente dabei als Vergleichsmethode. Zukünftig sollen die in dieser Studie gewonnenen Daten als Referenz-Normal-Werte für die Bewertung hinsichtlich pathologischer Veränderungen der DVA-Reaktionsmuster gelten. Methode: Die dynamischen und statischen Gefäßanalysen wurden mit dem "Retinal vessel analyzer" (RVA) der Firma IMEDOS (Jena/ Germany) an insgesamt 52 gesunden Probanden zwischen 20-78 Jahren (44,28 ± 16,42) durchgeführt. Die Stimulation bei der DVA-Messung erfolgte durch dreimaliges Flickerlicht (12,5 Hz) über je 20 Sekunden. Ergebnis: Für die statische Gefäßuntersuchung ergab sich für alle drei Messgrößen eine signifikante Altersabhängigkeit mit "Central retinal arterial equivalent" (CRAE) (227,98 ±16,77) (p=0,017), "Central retinal venous equivalent" (CRVE) (265,41 ±16,034) (p=0,049) und einem unkorrigierten AV-Quotient (AVR) (0,90 ±0,05) (p=0,005). Für die DVA Untersuchung zeigte sich eine statistisch signifikante Altersabhängigkeit für die arterielle Konstriktion (-2,63 ±1,66) (p<0,001), die arterielle Gesamtreaktionsstrecke (6,17 ±2,49) (p=0,002), die arterielle Konstriktionsgeschwindigkeit (0,17 ±0,12) (p<0,001) und die venöse Dilatationsgeschwindigkeit (0,24 ±0,09) (p=0,046). Bei der arteriellen Dilatationsgeschwindigkeit (p=0,104) und der venösen Dilatationsstrecke (p=0,199) war nur eine tendenzielle Einflussnahme des Alters zu erkennen. Bedingt durch die repetitiv durchgeführten Blutdruckmessungen konnte als Nebeneffekt ein signifikanter Blutdruckabfall im Sinne einer Normalisierung hin zum Ruhewert über die ca. zweistündige Voruntersuchung von Ankunft (94,23 ±10,46) bis zum Beginn der DVA Messungen (89,03 ±7,40) (p<0,001) gezeigt werden. Schlussfolgerung: Sowohl bei der statischen als auch bei der dynamischen Gefäßanalyse muß die Altersabhängigkeit der gewonnenen Ergebnisse berücksichtigt werden. Die Einflussnahme aktueller kardiovaskulärer Stellgrößen wie z.B. dem Blutdruck auf die Ergebnisse der Messung ist anerkannte Tatsache. Die signifikante Veränderung der gemessenen Blutdruckwerte während der Probandenuntersuchung belegt somit die Notwendigkeit einer Miterfassung dieser Werte. Die in dieser Arbeit gewonnen Daten können nun für weitere Studien bezüglich pathologischer Veränderung als Referenzwerte dienen.
In Deutschland leben derzeitig mehr als eine Million Menschen mit demenziellen Erkrankungen. Rund 60% dieser Patienten leben in Privathaushalten und werden dort überwiegend von Familienmitgliedern betreut. Das Fortschreiten der Erkrankung geht mit einem steigenden Pflegebedarf einher, so dass der Pflege elementare Aufgaben innerhalb der Demenzversorgung zukommen. Im Rahmen der Ausbildung in der Gesundheits- und Krankenpflege bzw. Altenpflege gewinnt die Thematik Demenz zunehmend an Bedeutung. Für eine qualitätsgesicherte und evidenzbasierte Koordination der Behandlung und Betreuung ist jedoch eine Weiterqualifizierung der Pflegefachkraft notwendig. Eine Analyse der bestehenden Fort- und Weiterbildungsoptionen zeigt dabei, dass bisher keine Qualifizierung auf das spezifische Handlungsfeld der ambulanten, netzwerkbezogenen Demenzversorgung fokussiert. Ein neuer Lösungsansatz ist die Qualifizierung und der Einsatz von Pflegefachkräften nach dem Konzept des Dementia Care Managements, wie es im Centre for Integrated Dementia Care Research (CIDC) in einer Kooperation zwischen den Universitäten Rostock und Greifswald und dem Deutschen Zentrum für Neurodegenerative Erkrankungen (DZNE) aktuell entwickelt und evaluiert wird. Dazu ist es zunächst erforderlich, das Aufgaben- und Tätigkeitsfeld einer Pflegefachkraft im Dementia Care Management zu identifizieren und darauf basierend einen Qualifizierungsbedarf abzuleiten. Unter Berücksichtigung der Primärausbildung Gesundheits- und Krankenpflege bzw. Altenpflege sind in einem zweiten Schritt hieraus entsprechende Qualifizierungsinhalte zu entwickeln, was den Inhalt der vorliegenden Dissertationsarbeit darstellt. Diese leistet somit eine Vorarbeit für die Entwicklung eines Curriculums für die zukünftigen Dementia Care Manager. Die Bearbeitung der Fragestellung erfolgte in einem dreistufigen Forschungsprozess unter Anwendung einer Datentriangulation in Form eines Mixed Methodology Designs aus qualitativen und quantitativen Forschungsansätzen. Datentriangulation meint die Einbeziehung von verschiedenen Datenquellen. In Bezug auf das Arbeitsfeld eines Dementia Care Managers konnten insgesamt 27 Aufgaben und Tätigkeiten identifiziert werden. Die Vorstellungen zum Aufgabenfeld des Dementia Care Managers stimmen in 18 der 27 Tätigkeiten zwischen Pflegefachkräften und Hausärzten überein. Bezüglich der neun Tätigkeiten, in denen sich die Vorstellungen bei den Pflegefachkräften und Hausärzten deutlich unterscheiden, sollte im Rahmen des praktischen Einsatzes der Dementia Care Manager besonderes Augenmerk gelegt werden. Aus dem so entwickelten Aufgabenfeld des Dementia Care Managers lassen sich notwendige Kompetenzen ableiten, die eine Pflegefachkraft aufweisen sollte, um die Durchführung und Umsetzung der Tätigkeit als Dementia Care Manager in hoher Qualität zu gewährleisten. Die Thematik Demenz in der regulären Ausbildung zur Gesundheits- und Krankenpflege bzw. Altenpflege wird in den verschiedenen Ausbildungseinrichtungen in einer unterschiedlichen Stundenanzahl gelehrt. So beträgt der Stundenumfang an der beruflichen Schule am Dietrich-Bonhoeffer-Klinikum in Neubrandenburg 20 Stunden in beiden Ausbildungsgängen. Diese Arbeit zeigt, dass zur Übernahme der Aufgaben im Dementia Care Management durch Pflegefachkräfte eine Weiterqualifizierung der pflegerischen Profession erforderlich ist. Auch die befragten Pflegefachkräfte (79,3%), Hausärzte (90,3%) und pflegende Angehörige (63,3%) sehen für die Pflegefachkraft, die als Dementia Care Manager tätig werden wollen, einen Qualifizierungsbedarf, der das gegenwärtige Qualifizierungsangebot erheblich übersteigt. Im Rahmen der Qualifizierung sollten vor allem Grundlagen der Demenzerkrankung, medizinische Inhalte, Pflege, Kommunikation und Beratung sowie Themen der netzwerkbezogenen Demenzversorgung vermittelt werden. Limitationen dieser Arbeit betreffen unter anderem die geringe Grundgesamtheit der befragten Gruppen von Hausärzten (N=32), Pflegefachkräften (N=145) und pflegende Angehörige (N=108). Die getroffenen Aussagen der drei Zielgruppen sind damit beschränkt. Die Resultate dieser Arbeit lassen wichtige Hinweise zum Aufgaben- und Tätigkeitsfeld eines Dementia Care Managers ableiten. Durch die Identifizierung wichtiger Qualifizierungsinhalte kann ein wertvoller Beitrag zur Konkretisierung des Dementia Care Management Konzeptes geleistet werden.
Im deutschen Gesundheitswesen stellt MRSA nicht nur im stationären, sondern auch im ambulanten Bereich eine Behandlungserschwernis dar. Patienten mit chronischen Wunden sind bei MRSA-Infektionen besonders gefährdet. Deshalb wurden als Ausgangslage für die vorliegende Arbeit die Forschungsfragen formuliert, ob a) eine ambulante MRSA-Sanierung bei Patienten mit chronischen Wunden durchführbar ist und b) die damit erzielbaren Fallerlöse die Praxiskosten decken. Zur Beantwortung der Frage wurden a) Berichtsdaten der Kassenärztlichen Bundesvereinigung an das Bundesministerium für Gesundheit b) Abrechnungsdaten der Kassenärztlichen Vereinigung Mecklenburg-Vorpommern und c) die Daten der MRSA-fokussierten MEracL-Studie (MRSA eradication for chronic lesions) analysiert. Auf Grund der Ergebnisse konnten fünf Kernaussagen formuliert werden: 1) Die Durchführung einer ambulanten Sanierungsbehandlung ist möglich. 2) Das Vorhandensein einer chronischen Wunde ist ein MRSA-Risikofaktor, aber kein Hemmnis für die Durchführung einer ambulanten Sanierungsbehandlung. 3) Die Akzeptanz der Ärzte hinsichtlich der Durchführung einer ambulanten Sanierungsbehandlung ist regional unterschiedlich. 4) Die errechneten Fallerlöse decken nicht die realen Praxiskosten. 5) Das Budget für Leistungen im Zusammenhang mit MRSA sollte sektorenübergreifend geplant und eingesetzt werden. Die vorliegende Arbeit unterstreicht die Wichtigkeit einer gesundheitsökonomischen Analyse bei der Einführung neuer Behandlungsmethoden.
