Background and aims
Prevention measures for cardiovascular diseases (CVD) have shifted their focus from lipoproteins to the immune system. However, low-grade inflammation and dyslipidemia are tightly entangled. The objective of this study was to assess the relations between a broad panel of inflammatory biomarkers and lipoprotein subclass parameters.
Methods
We utilized data from the population-based Study of Health in Pomerania (SHIP-TREND, n = 403). Plasma concentrations of 37 inflammatory markers were measured by a bead-based assay. Furthermore, we employed nuclear magnetic resonance spectroscopy to measure total cholesterol, total triglycerides, total phospholipids as well as the fractional concentrations of cholesterol, triglycerides, phospholipids, ApoA1, ApoA2 and ApoB in all major lipoprotein subclasses. Associations between inflammatory biomarkers and lipoprotein subclasses were analyzed by adjusted linear regression models.
Results
APRIL, BAFF, TWEAK, sCD30, Pentraxin-3, sTNFR1, sTNFR2, Osteocalcin, Chitinase 3-like 1, IFN-alpha2, IFN-gamma, IL-11, IL-12p40, IL-29, IL-32, IL-35, TSLP, MMP1 and MMP2 were related with lipoprotein subclass components, forming two distinct clusters. APRIL had inverse relations to HDL-C (total and subclasses) and HDL Apo-A1 and Apo-A2 content. MMP-2 was inversely related to VLDL-C (total and subclasses), IDL-C as well as LDL5/6-C and VLDL-TG, IDL-TG, total triglycerides as well as LDL5/5-TG and HDL4-TG. Additionally, we identified a cluster of cytokines linked to the Th1-immune response, which were associated with an atherogenic lipoprotein profile.
Conclusion
Our findings expand the existing knowledge of inflammation-lipoprotein interactions, many of which are suggested to be involved in the pathogeneses of chronic non-communicable diseases. The results of our study support the use of immunomodulatory substances for the treatment and possibly prevention of CVD.
Sex-specific associations of cardiorespiratory fitness and galectin-3 in the general population
(2022)
Aims
Low cardiorespiratory fitness (CRF) is associated with greater mortality and morbidity. Galectin-3 (Gal-3) is a prognostic biomarker for fibrosis and heart failure. Gal-3 is also associated with a greater risk for cardiovascular mortality. Whether CRF is related with Gal-3 is unclear. The objective of this study was to assess the sex-specific associations of CRF and Gal-3 levels in the general population.
Methods
Gal-3 concentrations were determined using a sandwich enzyme immunoassay in the population-based Study of Health in Pomerania (SHIP-TREND-0). Sex-stratified linear regression models adjusted for age, current smoking status, and renal function were used. Individuals with left ventricular ejection fraction (LVEF) <40%, previous myocardial infarction, atrial fibrillation, chronic lung disease, severe renal disease (estimated glomerular filtration rate <30 mL/min/mm2), a history of cancer, and extreme values for Gal-3 (<1st percentile; >99th percentile) were excluded.
Results
A total of n = 1515 participants with a median age of 49 (IQR: 39–60 years, 48% males) were included. In men, a 1 L/min greater VO2peak was significantly related to 0.50 ng/mL (95% CI −0.8068 to −0.1938, P < 0.01) less Gal-3. In males, a 1 mL/min/kg higher VO2peak adjusted for body weight was associated with −0.0286 ng/mL (95% CI −0.0052 to −0.0005, P = 0.02) less Gal-3. When VO2peak was adjusted for lean mass 1 mL/kg/min more was correlated with a −0.0022 ng/mL (95% CI −0.0043 to -0.0007, P = 0.04) less Gal-3. In women, VO2peak (β −0.2046 95% CI −0.6541 to 0.2449, P = 0.37) and VO2peak adjusted for lean mass (β −0.0019 95% CI −0.0421 to –0.0050, P = 0.12) were not related with Gal-3, whereas a 1 mL/min/kg higher VO2peak adjusted for body weight was significantly associated with a −0.0064 ng/mL lower Gal-3 (95% CI −0.0092 to -0.0035, P < 0.01). There were no differences between pre-menopausal and post-menopausal women.
Conclusions
VO2peak was associated with Gal-3 only in men, but VO2peak adjusted for body weight in women and men. Our results suggest that the adverse consequences of low CRF may be mediated by Gal-3. Further research is needed to understand the sex-specific association between CRF and Gal-3 and whether they are clinically relevant.
Objectives
During the corona pandemic, dental practices temporarily closed their doors to patients except for emergency treatments. Due to the daily occupational exposure, the risk of SARS-CoV-2 transmission among dentists and their team is presumed to be higher than that in the general population. This study examined this issue among dental teams across Germany.
Materials and methods
In total, 2784 participants provided usable questionnaires and dry blood samples. Dry blood samples were used to detect IgG antibodies against SARS-CoV-2. The questionnaires were analyzed to investigate demographic data and working conditions during the pandemic. Multivariable logistic mixed-effects models were applied.
Results
We observed 146 participants with positive SARS-CoV-2 IgG antibodies (5.2%) and 30 subjects with a borderline finding (1.1%). Seventy-four out of the 146 participants with SARS-CoV-2 IgG antibodies did not report a positive SARS-CoV-2 PCR test (50.7%), while 27 participants without SARS-CoV-2 IgG antibodies reported a positive SARS-CoV-2 PCR test (1.1%). Combining the laboratory and self-reported information, the number of participants with a SARS-CoV-2 infection was 179 (6.5%). Though after adjustment for region, mixed-effects models indicated associations of use of rubber dams (OR 1.65; 95% CI: 1.01–2.72) and the number of protective measures (OR 1.16; 95% CI: 1.01–1.34) with increased risk for positive SARS-CoV-2 status, none of those variables was significantly associated with a SARS-CoV-2 status in fully adjusted models.
Conclusions
The risk of SARS-CoV-2 transmission was not higher among the dental team compared to the general population.
Clinical relevance.
Following hygienic regulations and infection control measures ensures the safety of the dental team and their patients.
Non-carious dental lesions such as developmental defects of enamel (DDE) and erosive tooth wear (ETW) are the subject of intensive research. This paper aims to give perspectives on both DDE, including dental fluorosis and molar incisor hypomineralization (MIH), and ETW, presenting epidemiological data from the Americas and associated diagnostic aspects. Besides, it is important to present evidence to guide the clinical assessment process, supporting the clinicians’ management decisions towards better oral health of their patients. The overall increase in the worldwide prevalence of non-carious lesions discussed in this this paper may reflect the need of perceptual changes. Although the number of publications related to these conditions has been increasing in the last years, there is still a need for clinical diagnostic and management awareness to include these conditions in routine dental practice. Besides, it is important to provide recommendations for standardized clinical assessment criteria, improving the process and helping clinicians’ adherence. In this sense, this paper discusses the most commonly implemented indices for each condition. Thus, despite the wide range of diagnostic indices, BEWE is proposed to be the index recommended for ETW assessment, Dean or Thylstrup & Fejerskov indices for fluorosis and preferably the EAPD criteria (or modified DDE index) for MIH. Overall, non-carious lesions are a growing concern, and it is important to implement preventive measures that control their severity and progression, and accurate diagnosis by the dental clinician.
Obesity and diabetes have reached epidemic proportions and have emerged as massive public health problems globally. The etiology of both obesity and diabetes are related, multifactorial, highly complex, and involves interplay of genetic, environmental, socio-economic and physiological factors, which calls for a more extensive research in understanding the risk factors and biological pathways. Hence, this dissertation contributed in part to understanding the role of iron markers in the development of type 2 diabetes mellitus and the role of intrauterine hyperglycemia in influencing the risk of offspring obesity along with investigating potential pathways.
In the first part of my dissertation, the associations of iron markers (ferritin and transferrin) with type 2 diabetes mellitus and metabolic syndrome were investigated using the population-based Study of Health in Pomerania. The present analyses were based on 3,232 participants aged 20-81 years with a follow-up time of nearly 11 years. The results suggest that serum ferritin concentrations were associated with a higher prevalence of type 2 diabetes mellitus and metabolic syndrome in the total population as well as in men. However, the effects of serum ferritin on incident type 2 diabetes mellitus were observed only in women, while the effects on incident metabolic syndrome were seen in the total population. Serum ferritin is also known to reflect systemic inflammation or hepatic dysfunction in addition to increased iron stores. Hence, upon further analyses, the associations were found to be attenuated after adjustment for hepatic enzymes but not after adjustment for inflammation. Transferrin was not associated with any of the outcomes. Thus, our study provides evidence for a link between the iron marker ferritin and type 2 diabetes mellitus and metabolic syndrome, although the association seemed to vary by sex. Moreover, hepatic dysfunction seems likely to be in the pathway between ferritin and type 2 diabetes mellitus and metabolic syndrome.
In the second part of my dissertation, the association between maternal hyperglycemia and the risk of offspring overweight and obesity were investigated using three different cohorts: TEDDY, TEENDIAB and BABYDIAB/BABYDIET. The present analyses were based on a total of 8,103 children who were followed until 6 years of age in TEDDY study and until 18 years of age in TEENDIAB and BABYDIAB/BABYDIET studies. The dissertation revealed that maternal hyperglycemia in general may be associated with increased risk for childhood overweight and obesity, and that the association gets stronger as children grow older, with the risk being clearly evident at late childhood and adolescence. Moreover, this dissertation adds that this association can be driven by different pathways based on the type of maternal diabetes to which the offspring was exposed. The association of maternal gestational diabetes mellitus with offspring overweight can be largely explained by the confounding influence of maternal BMI, whereas the association of maternal type 1 diabetes mellitus with offspring overweight can be substantially explained by birthweight in all three studies. In our attempt to understand biological pathways at a cellular level, we found that the offspring metabolome was unlikely to be in the causal pathway between maternal type 1 diabetes mellitus and overweight, because this association could not be explained by any of the potentially relevant metabolites.
To conclude, this dissertation acknowledges the fact that prevention and early intervention of obesity and diabetes is of paramount importance to lessen the impact of these public health problems. Thus, our findings of the role of ferritin in increasing the risk of type 2 diabetes mellitus/ metabolic syndrome and the role of intrauterine hyperglycemia in increasing the risk of offspring overweight helped to identify particular risk groups who may need closer attention with respect to prevention of obesity and diabetes.
Background: Iodine deficiency disorders (IDD) represent a global health threat to individuals and societies. IDD prevention programmes have been introduced in many parts of the world. However, challenges remain, particularly in Europe due to fragmentation and diversity of approaches that are not harmonized. Objectives: This review is dedicated to the public-health impact of IDD prevention programmes. It sums up experiences collected by the EUthyroid consortium so far and provides information on stakeholders that should be involved in actions directed to improve the impact of IDD prevention. Methods: A joint European database for combining registry-based outcome and monitoring data as well as tools for harmonizing study methods were established. Methods for analyzing thyroglobulin from a dried blood spot are available for assessing the iodine status in the general population and at-risk groups. Mother-child cohorts are used for in-depth analysis of the potential impact of mild-to-moderate iodine deficiency on the neurocognitive development of the offspring. A decision-analytic model has been developed to evaluate the long-term effectiveness and cost effectiveness of IDD prevention programmes. Results: EUthyroid has produced tools and infrastructure to improve the quality of IDD monitoring and follows a dissemination strategy targeting policymakers and the general public. There are tight connections to major stakeholders in the field of IDD monitoring and prevention. Conclusions: EUthyroid has taken steps towards achieving a euthyroid Europe. Our challenge is to inspire a greater sense of urgency in both policymakers and the wider public to address this remediable deficit caused by IDD.
Background: There is only limited data on the potential association between thyroid dysfunction and peripheral arterial disease (PAD). Objective: The aim of our study was to investigate the potential association of thyroid function, as defined by serum concentrations of the clinically used primary thyroid function marker thyrotropin [i.e. thyroid-stimulating hormone (TSH)] and 3,5-diiodothyronine (3,5-T2), with the ankle-brachial index (ABI) as a marker of PAD. Methods: We used data from 5,818 individuals from three cross-sectional population-based studies conducted in Northeast (SHIP-2 and SHIP-TREND) and Central Germany (CARLA). Measurement of serum TSH concentrations was conducted in one central laboratory for all three studies. In a randomly selected subpopulation of 750 individuals of SHIP-TREND, serum 3,5-T2 concentrations were measured with a recently developed immunoassay. ABI was measured either by a hand-held Doppler ultrasound using the Huntleigh Dopplex D900 or palpatorily by the OMRON HEM-705CP device. Results: Serum TSH concentrations were not significantly associated with ABI values in any of the three studies. Likewise, groups of individuals with a TSH <0.3 mIU/l or with a TSH ≥3.0 mIU/l had no significantly different ABI values in comparison with individuals with a TSH in the reference range. Analyses regarding TSH within the reference range or serum 3,5-T2 concentrations did not reveal consistent significant associations with the ABI. No sex-specific associations were detected. Conclusions: The results of our study do not substantiate evidence for an association between thyroid function and PAD, but further studies are needed to investigate the associations of overt forms of thyroid dysfunction with PAD.
